No activity shall commence until this QC plan is approved.
Must organize a weekly, biweekly or as requested by owner meeting. QCM will redact a minute for record.
Must keep record on every submittal and will be responsible for the record of all material and equipment installed in a log.
Must submit a weekly report with percentages and photos of the development of the project.
At the beginning of the project QCM will submit a progress schedule with a critical path management for QA approval. This progress schedule will be updated every month with the breakdown for payment. In the absence of the schedule the application for payment won’t be processed.
QA will require the QCM a two weeks ahead with all tasks defined.
At the beginning of every task the QCM will perform a meeting among the stakeholders and contracted personnel to establish the rules and the requirements for the task.
Example: concrete pouring time, pipe routes and beddings, equipment tests, excavation permits and coordination, electrical or water shutdowns, etc. This meeting will be coordinated with QA National Guard Personnel. QA personnel can request the QCM and the CONTRACTOR for additional task meetings when they understand there is not enough information or the task requirements are not clear.
QCM will be responsible to coordinate with every specialist their visits and their written certifications.
QCM will be responsible of files and record keeping. At the end of the project files will be delivered to the National Guard.
B-1.3 SPECIALISTS
The QC plan must include a list of the specialists. Every Specialist must have at least (5) five years of experience in related projects. The professional must prove proficiency in the area of specialty. A license does not constitute prove of proficiency it must be accompanied by proof of experience.
Every specialty in construction (i.e. electrical, mechanical, structural, surveying, OSHA, etc.)
must be certified by a professional engineer in that discipline. A copy of resume with the experience and certifications must be included for every professional.
These specialists must visit the project and verify that the progress of work is according to codes and drawings.
Each Specialist will certify every task. For example: a Professional engineer with structural background will certify the rebar is installed according to drawings before concrete is poured, a professional engineer with civil works background will certify the bedding for a pipeline before it is installed. Until that written certification is presented no concrete pouring will be allowed.
At the end of the project every one of them will submit a written certification that the project has been constructed according to codes, drawings and specifications.
Qualifications of the staff will match the control requirements of the plan and an individual’s qualifications must be adequate for the duties assigned.
B-1.4 TESTING OF MATERIALS
At least two laboratories for the testing of materials must be submitted. These laboratories must be certified by a federal agency.
The laboratory personnel must be present in every soil compaction, must collect concrete samples for every concrete pouring, environmental test and clearances. Cost for additional testing must be itemized in the QC Plan case the QA personnel request additional testing.
Slump test equipment and qualified personnel for its use will be always in the project.
B-1.5 DESIGN BUILD
When a portion of the project is design/build qualifications for the personnel in charge of that activity must be submitted. Professionals with no experience in the area will not be allowed. A list of previous projects must be submitted in a Resume. Progress meetings for the design will be made at 35% and 70%. No design will be accepted without KO approval.
B-1.6 OSHA REQUIREMENTS
A weekly safety meeting will be conducted. Proof of that meetings must be submitted.
Safety meetings will be prepared by an OSHA certified professional. The contents of that meetings will be prepared according to the specific task involved in the project. QCM will be responsible for enforcing the compliance of this requirements. All liability for accidents in the project site will be the CONTRACTOR’s responsibility.
The CONTRACTOR is responsible for the training, medical surveillance, monitoring, and necessary personal protective equipment (PPE) for their personnel. Ensure full compliance with title 29 Code Federal Regulation (CFR) Part 1910.1025, 29 CFR Subpart I- Personal Protective Equipment (29 CFR 1910.132/133/134), 29 CFR 1910.1200, EQB Regulations for the Control of Mitigation Activities of Lead-Based Paint (Rule 59701570) and any other OSHA and Environmental Protection Agency (EPA) regulation as applicable.
A written certification of having complied with the training and medical requirements specified above will be submitted to the contracting officer including copy of the medical clearance for respirator users. Medical records of the CONTRACTOR's employees will not be submitted.
