Statement of Work
The following items must be available from the vendor:
· New steel size D cylinders tested and filled with medical grade oxygen
· Fill customer owned size D cylinder (13CF)
· Hydrostatic test customer owned steel size D cylinder
· Refurbished steel size D cylinder, hydrostatic tested and filled with medical grade oxygen
Delivery shall occur during normal work hours, 7:00 AM to 3:30 PM Central Standard Time Monday through Friday. Performance outside normal hours will be coordinated with the Government. If additional supplies are necessary due to usage, weekend work may be required. The vendor shall not perform deliveries on Federal Holidays. The following are recognized US holidays. The contractor shall not perform services on these days:
· New Year’s Day: January 1st
· Martin Luther King, Jr.’s Birthday
· President’s Day
· Memorial Day
· Independence Day: July 4th
· Labor Day
· Columbus Day
· Veteran’s Day: November 11th
· Thanksgiving Day
· Christmas Day
Vendor shall remove all empty containers and resupply full containers to the delivery site.
Vendor shall provide a regular scheduled route to the MEDLOG Warehouse on a monthly basis, and will have no minimum delivery amount. The facilities will place an order if the product is needed prior to the scheduled route. Vendor will have 5 Business days to deliver, once the order is placed. Any product shortages or delays must be communicated immediately to the warehouse.
Vendor will unload their own truck and place cylinders in appropriate storage area at the facility.
Any onsite HAZMAT issues must be reported to the facility manager immediately while at the delivery address.
Delivery will be verified by facility personnel. Bill of Lading or paperwork required per delivery will be signed by facility personnel. Vendor will submit for payment as described in this contract.
This contract will be set up as a base contract plus 4 option years. Period of performance will be:
01 June 2018 – 31 May 2019 Base year 01 June 2019 – 31 May 2020 Option Year 1 01 June 2020 – 31 May 2021 Option Year 2 01 June 2021 – 31 May 2022 Option Year 3 01 June 2022 – 31 May 2023 Option Year 4
