Dining Facility Attendant Services Performance Work Statement

(PWS)

FY 2012

STATEMENT OF WORK (PWS)

I. DESCRIPTION OF SERVICES

1. Scope of Work. The contractor shall, except as specified in this Performance Work Statement (PWS) as government furnished property or services, provide all personnel, supervision, and any items and services necessary to perform food service attendant services at the Maine Air National Guard Base, Bangor, ME. The contractor shall perform the requirements in this PWS, conform to the professional standards identified in this contract, and shall follow all applicable instructions and directives as identified by this PWS. Contractor tasks include the following:

cleaning facilities, equipment, furniture and utensils in dining areas; handling equipment and maintaining quality control. The estimated essential services are listed in Technical Exhibit 2, Workload Estimates. The contractor shall submit reports and documentation as identified throughout this PWS. Requests for such reports and documentation and responses shall be coordinated through the Contracting Officer Representative (COR).

2. Program Overview. The Maine Air National Guard food service operation consists of one dining facility operated part-time, predominately during Unit Training Assembly (UTA) weekends 11 months out of the year, and once per year, two weekdays prior to an UTA weekend. The facility is dedicated to providing a full lunch meal, meeting the nutritional needs of Department of Defense (DOD) personnel who frequent the facilities.

3. Sanitation Requirements. The contractor shall comply with all federal and Air Force mandated sanitation requirements. AFI 48-116 Food Safety Program all chapters available at www.e-publishing.af.mil

4. Operational Requirements and Specific Tasks. The contractor shall cleanup those areas that immediately affect customer service, to include appearance of the dining facility.

a. Table Cleaning. Clean and sanitize dining room tables and chairs as needed. Replenish dining table items (salt, pepper, napkins) so that they are available to all patrons without waiting.

b. Floor Cleaning. Clean floors so that no visible dirt or dust remains on floors, floor mats or runners, in corners, behind doors, or under furniture and equipment at the end of each meal period.

c. Spot Floor Cleaning. Spot clean floors to remove food spills and debris within 5 minutes of identifying or being notified of a spill.

d. Clean and sanitize dishes, pots, pans, utensils, and kitchen and dining areas in accordance with the FDA Food Code. They must be clean, free of food particles, film, and grease.

e. Beverage Dispensers and Ice Machine. Carbonated beverages, milk, coffee urns and ice machine shall be monitored regularly so as not to run out of product during the meal service. Surfaces and internal product compartments and lines shall be kept clean. Drains will have hot water poured down them after each meal service.

f. Kitchen and Serving Line Exhaust Hoods, (including light fixtures) shall be thoroughly cleaned on Sunday at the end of the meal service with a grease cutting solution.

g. Lavatories. Clean all toilets, urinals, sinks, and floors in the two bathrooms in the dining facility and the two located in the kitchen. All paper towels, toilet paper, and hand soap dispensers shall be adequately supplied at all times. Mops, brooms, and brushes used in cleaning of lavatories shall not be used for cleaning any other area.

h. Glass Cleaning - Contractor will clean all glass doors and entrances with glass in dining facility and kitchen.

5. Waste Management. The contractor shall assist the government by complying with the base recycling program.

This may require segregating reusable/recyclable waste materials by type. The government shall retain ownership of all reusable waste materials. Non-reusable waste materials (trash and garbage) will be removed from the facility by a government-approved method.

a. All HAZMAT materials brought on the base must be processed and preapproved through HAZMAT Pharmacy.

b. All garbage and trash containers will be removed by the contractor from the dining facility and transported to the dumpster when within 3 inches of the top of the container and at the end of each meal service, regardless of the amount of material in the container.

c. Garbage and trash containers should be thoroughly cleaned and sanitized before returning to point of use and clean plastic trash bags shall be replaced each time.

6. Contractor Personnel.

a. Management. The contractor shall provide a contract manager on site when contract employees are present and shall be responsible for the performance of the work. The name of this person, and an alternate or alternates who shall act for the contractor when the manager is absent, shall be designated in writing to the Contracting Officer prior to the start of the contract. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager or alternate shall be available at all times to meet at the Government facility with government personnel designated by the Contracting Officer to discuss problem areas. After normal duty hours, the manager or designated alternate shall be available within 24 hour. The contract manager and alternate or alternates must be able to read, write, speak, and understand English. The positions shall at no time be vacant. They shall comply with installation rules and regulations for motor vehicle use on the Government property.

7. Conduct. Contractor employees shall not loiter in any working or patron area. Upon completion of their assigned shifts, employees shall promptly depart the food service facilities and Guard Base.

8. Hours of Operation

a. Hours of Operation. Duty hours during UTA Weekends is 7:00am to 3:30pm; the contractor shall not arrive prior to the beginning of the duty day and will have all tasks completed prior to the end of the duty day. Meal service is from 11:00am to 12:30pm, as listed in Technical Exhibit 1. The contractor shall maintain a staff capable of meeting the service requirements of this PWS within the duty hours. Prior to the start of the contract, the contractor will submit their work hours to the Contracting Officer.

b. Unit Training Assembly (UTA) schedule. The current UTA schedule is listed in Technical Exhibit 1 through September 2012. New UTA schedules are release in the July to August time frame each year and will be made available to the contractor once published. The new schedules shall be incorporated into the contract without modification. The UTA schedule is subject to change with a 72-hour notice.

c. Emergency Operation During a Crisis or Special Event Services. Special and emergency situations (for example, accident and rescue operations, civil disturbances, weather warnings, base crisis) may necessitate a dining facility to operate on a fulltime extended basis of up to 24 hours per day for short periods of time lasting less than a 90 days. The contractor shall provide these services when required by the Contracting Officer. The Contracting Officer will verbally advise the contractor of the effort required. Pricing shall be negotiated and followed with a contract modification. At any time, based on the circumstances of the emergency, the Contracting Officer may make a determination that it is in the best interest of the Government to resolicit for fulltime services and Terminate for Convenience the remainder of the existing contract period.

d. Contingency Services. Operations which may increase workload, to include Performance of Services during Crisis:

(1) Provide service during military emergencies, natural disasters and other contingency operations. The numbers of operations vary, but can be reasonably predicted from history. Currently, there have been zero emergencies declared in the past 10 years that would warrant such contractor support.

9. Quality Control/Quality Assurance Program

a. Quality Control: The Contractor is required to establish and maintain a quality control program to assure the requirements of the contract are provided. The program shall include as a minimum;

(1) An inspection system covering all the services to be performed under the contract.

(2)The method of identifying deficiencies in the quality of services performed before the level of service becomes unacceptable.

(3)Maintain a file of any corrective actions taken. This file shall be made available to the

Government during the term of the contract.

b. Quality Assurance: The Government shall monitor the Contractors performance under this contract using the following procedures:

(1) The Government shall appoint a Quality Assurance Evaluator. The QAE shall inspect for compliance of the contract and shall record all surveillance observations. Those observations that indicate defective performance shall be brought to the attention of the Contractor’s attendant supervisor for immediate correction. The QAE shall initiate contract discrepancy reports on all uncorrected matters which exhibit deficient performance.

(2) When the Contractor’s performance is unsatisfactory, a Contract Discrepancy Report (CDR) shall be executed by the QAE and provided concurrently to the Contracting Officer and the Contractor’s Attendant Supervisor. The Contractor shall be required to reply in writing, giving the reasons for the unsatisfactory condition, the corrective actions taken or to be taken, and procedures to prevent recurrence.

Unsatisfactory performance may result in a payment deduction.

II. SERVICE DELIVERY SUMMARY

Performance Requirement PWS para. Performance Standard Comply with sanitation requirements of the food code and state and local laws/regulations, which results in a satisfactory or better rating in Health Inspections.

III. GOVERNMENT-FURNISHED PROPERTY AND SERVICES. The government shall make available the facilities, fixtures, equipment, materials and services listed here or in Technical Exhibit 4.

1. Property. The government shall provide necessary equipment for the contractor to perform its work under the contract. Such equipment is identified as:

- Garbage Disposal

- Dish washing machine

- Floor cleaning machine

2. Supplies. The government shall provide all cleaning supplies necessary for the contractor to perform the work required under the contract. Contractor is responsible to inventory supplies at the end of each service period and identify to the COR what needs to be replenished for the next service period.

3. Services. The government is responsible for:

a. Utilities. The government will provide gas, electricity, sewage, and water.

b. Mail Distribution. On-base only.

c. Telephone. "Class C" (on-base) telephone service.

d. Refuse Collection. Dumpsters are provided for waste.

e. Recyclable Materials Collection if applicable.

f. Security Forces and Fire Protection.

IV. GENERAL INFORMATION

1. Contractor Personnel Meals. Under the terms and conditions of the contract, the contractor is authorized to eat lunch prior to the designated lunch time of members of the Maine Air National Guard, but the contractor shall pay the current Air Force meal price; currently $4.25. Price is subject to change without notice.

2. Security Requirements. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation, shall abide by all security regulations of the installation. The contractor shall sign an agreement stipulating the security requirements of this contract and certify to best of their knowledge that personnel performing under this contract do not pose a security risk to the United States. All personnel are subject to a security background check.

a. Base Access. The contractor shall obtain personal identification passes for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto the base. The contractor shall provide a Contractor/Vendor Access List, and submit it through the Contracting Officer to the Security Forces and Pass and Registration. This list will be used to conduct background checks on employees of the contractor. Vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles to be registered.

b. Physical Security. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the end of each work shift, all government facilities, equipment, and materials shall be secured.

3. Contractor Furnished Items and Services. Except for those items or services specifically stated as government-furnished, the contractor shall furnish everything required to perform this contract, to include:

a. Uniform Requirement. The uniform will consist of Shirts/Blouses and Long Pants. Tank tops, halter tops, shorts and other types of sleeveless tops are unauthorized and are considered a sanitation/safety issue. All contract individuals shall wear safety toe footwear during the duty hours. Open-toe footwear is unauthorized. The Contracting Officer will approve the standards of dress before the contract start date.

4. Performance of Services During Crisis. Reference NGFARS 37.9 Performance of services during crisis declared by the National Command Authority (September 2002). The services provided by this contract have been designated essential contractor provided services and must be continued during a crisis. The Contractor shall continue providing service/product to the Maine Air National Guard as required, 24 hours a day, until the crisis is over. The contractor shall ensure enough skilled personnel are available during a crisis for any operational emergency due to utility failure, damage control, and repair.

The list below is a tentative list of these Unit Training Assembly weekends, which is subject to change with a 72 hour notice. The contractor shall provide sufficient personnel at the Dining Facility to handle monthly Unit Training Assembly (UTA) requirements. (Estimated 1 weekend per month).

APPLICABLE PUBLICATIONS AND FORMS

Publications and forms applicable to this Statement of Work (PWS) are listed below. The contractor has coded publications as mandatory (M) or advisory (A). The contractor shall follow those publications coded as mandatory to the extent (that is, the specific procedure in a paragraph, section, chapter or volume) specified in the PWS. The contractor shall be guided by those coded advisory to the extent necessary to meet requirements in this PWS. At the start of the contract, the Government provides all publications and forms listed. The Government may issue supplements or amendments to listed publications from any organizational level during the life of the contract. The contractor shall keep all issued publications up-to-date. The contractor shall immediately implement those changes in publications that result in a decrease or no change in the contract price. Before implementing any such revision, supplement, or amendment that will result in an increase in contract price, the contractor shall submit to the Contracting Officer (CO) a price proposal for approval. Price proposals shall be submitted within 30 calendar days from the date the contractor receives notice of the revision, supplement, or amendment giving rise to the increase in cost of performance. The Government will consider changes in the contract price due to supplements and amendments shall be considered under the "Contract Terms and Conditions-Commercial Items" clause. The Government will continue to supply the Government forms needed for daily operations. Upon completion of the contract, the contractor shall return to the Government all issued publications and unused forms.

DEFINITIONS

Director of Military Public Health (MPH) – The installation medical authority who defines and monitors sanitation standards and procedures for food service activities. Representatives of the MPH inspect sanitary conditions and apply ratings based upon their professional qualitative judgment.

Equipment – Items used to store, prepare, cook, transport, and serve food. Equipment also includes items used in cleaning and sanitizing, as well as those used to transport and store supplies.

Food Service Manager (FSM) – An officer, noncommissioned officer, or civilian responsible to the base Chief of Services for matters relating to food service. Advises the Contracting Officer on matters relating to the contract.

Subsistence – Food items, including beverages and condiments.

Subsistence-in-Kind (SIK) – Food furnished to enlisted personnel at Government expense, instead of a monetary allowance.

Unit Training Assembly (UTA) – An authorized and scheduled period of unit inactive duty training of a prescribed length of time for Guard and Reserve components.
