W912GY-16-T-0030- Optometry services

Q: On Page 1 & 2 of the solicitation what does a contractor fill out?

A: The contractor fills out part 17a and 30a-c on page 1. Nothing is filled out on Page 2.

Q: On the SOW, page 11 states “A copy of the Contractor’s license, and all other required licenses, permits and certifications, must be submitted with the Contractor’s Bid Proposal and remain valid throughout the term of the contract. There must be no current or prospective limitations on said licenses, permits or certifications.” Is a Nevada license ok and how do I show that my license is valid?

A: Yes, the license needs to be in the state where you are performing the work. Upon expiration of your license, if awarded, the contractor provides the new copy of the license.

Q: On the SOW, page 11 states “Upon request, the Contractor shall provide a copy of the examination to the Safety Office who may provide a copy to the employee for their records.” Do I submit the exam once given to the Depot?

A: No, upon request Sierra may ask for them. But contractor must maintain the exam on file for the duration of the contract.

Q: On CLIN 10001 and 1002, do I put in a NET amount.

A: No, just submit the unit cost for each category. Per clause 52.212-2 page 28 the unit costs will be evaluated based on those historical quantities.

Q: Past Performance Questionnaires- if we cannot meet the minimum requirement of three past performers can we submit a letter.

A: yes, a cover letter can be submitted to explain why at least three past performance questionnaires are NOT available. Just need something in writing in your proposal.

Q: Need clarification: “Contractor shall provide examination to obtain prescription information for safety glasses only.”

A: Only Safety glasses are authorized. Nothing is authorized outside of what is being individually bid on.

Q: Does the contractor provide employee with hand written prescription after examination? In the past, we were told that is not allowable that the prescription must be filled by the contract.

A: No, no hand written prescription is authorized. All prescriptions authorized will be filled by the contract.

Q: “RECHECK: If an error in the prescription is found the contractor will redo refraction at no additional charge.” Who pays for the glasses or lens if a re-check is needed?

A: The contractor is liable for the re-check and any prescription found in error as well as the replacement. Examination and prescription will only be filled by the contractor therefore, if any error is found, the contractor will be liable.

** AMENDMENT TO THE SOW: REMOVING THE BELOW SECTION IN MINIMUM REQUIREMENTS

G. Contractor shall accept prescription from outside source if employee already was issued one to be able to request contracted eyeglasses/tint/frames.
