SUMMARY OF CHANGES

SECTION SF 30 - BLOCK 14 CONTINUATION PAGE

The following have been added by full text:

AMENDMENT 0002

The purpose of this amendment is to provide answers to vendor questions as follows:

1. Q: What specifically has to be included within the 15 page limit? The excerpts for your instruction below are confusing to me as whether the resume for the proposed CPF is or is not part of the 15 pages?

A: As noted under number 8 of the “Instructions to Quoters” section, the 15 one-sided page limit applies to all information aside from the SF 1449 (solicitation form). Within the 15 page limit, please provide all of the following:

a. Past performance information regarding recent (within the last 6 years) and relevant (the same or similar scope and complexity) contracts for the same or similar scope and complexity of this RFQ (see section 3.1 contracting organization qualifications and experience). For each past project, the information shall include the contract number (if applicable), dollar amount/value, dates of service and one point of contact with a valid telephone number, email address, and other relevant information as appropriate.

b. Experience: Provide information regarding recent and relevant contracts for the same or similar scope and complexity of this RFQ (see section 3.1 contracting organization qualifications and experience) detailing your experience in leadership development training and facilitation services. NOTE: Experience refers to work you have done and Past Performance refers to how well you did the work, and is generally evaluated in terms of (1) quality,

(2) schedule / timeliness of performance, (3) customer satisfaction, (4) management / personnel / labor, (5) cost / financial management, (6) safety / security, and (7) general. Experience information comes from the offeror, and past performance information is gathered by the government using references provided by the offeror, along with any and all other sources including but not limited to PPIRS.

c. Qualifications of Key Personnel: Provide the resume detailing the professional qualifications for the same or similar scope and complexity (see section 3.2 Contract Program Facilitator) for the Contract Program Facilitator that will be providing the training in person.

Other than the above requested information, it is up to you to decide what you want to include within the constraints of 15 pages.

2. Q: Instructions to Quoters 8.a (p 17) - Should Contractor include Past Performance Questionnaires or only provide the point of contact information for past contracts?

A: Please see the answer to number 1 above.

3. Q: 1.1 - RFQ states "Only one program will be run per year." Does this mean that 1) either Level 2 or Level 3 will be offered each year but not both or 2) one each of Level 2 and Level 3 will be offered each year?

A: Either level 2 or level 3 will be run in one year, not both.

4. Q: 3.3.3 - Is there a minimum number of diagnostic tools that USACE would like to be included in the LDP? If so, what is the minimum number of diagnostic tools to be used for Level 2? Level 3?

A: Sec 3.3.3 of the PWS was amended (see amendment 0001) to indicate that at least 3 diagnostic tests are to be used for each level. A 360-deg feedback tool is required for both program levels. The Tier 3 program is also required to have an organizational climate survey conducted for each participant's work unit.

5. Q: On page 23 of the SF 1449 it reads, "At a minimum, the Contractor shall provide certification of CEUs or contact hours for each class session." - Does this require the CPF to be certified to provide CEUs or will the Contractor have this certification? If "contact hours" will suffice, can you please distinguish from CEUs?

W912ES-15-T-0147

A: For the purposes of this program, certification of the contractor or the CPF to issue CEUs is not required. Each LDP participant is to receive written certification indicating the number of contact hours (hours spent in formal instruction) he or she experienced during the program duration. It's assumed that 10 contact hours equates to 1

CEU.

6. Q: On page 22 of the SF 1449 it reads, "At this review meeting, the Contractor shall provide the proposed diagnostic tools, agenda for contractor facilitated sessions ..." - Is the use of the word "Contractor" accurate there, or is "CPF" more accurate?

A: Either the Contractor or the CPF can participate in the review meeting, as long as the CPF has the authority to act on behalf of the Contractor and make decisions regarding program content.

All other terms and conditions remain unchanged.
