SECTION 01 14 00 Page 6

1.3.4.1 Contractor Access Procedures

All Contractor personnel requiring access to Mountain Home AFB will be required to process in through the prime Contractor on an approved 366th SFS FORM 30. A separate 366th SFS FORM 30 shall be submitted for the personnel of each company working on this project. The Mountain Home Project Office will provide an electronic copy of this document upon request. Information required is full name (first, middle, last), SSN, DOB, State Issued Driver's License number (ie... ID 12345678), and duration/times of access required. An identification card issued by a Government agency may be used if the individual does not possess a driver's license and the identification number provided must be verifiable (i.e. SSN or Passport Number). Take special care to ensure all information on the 366th SFS FORM 30's has been verified to be accurate prior to submitting for approval. The Government will not be responsible for delays caused by disapproval of Contractor access requests due to inaccurate information provided by the Contractor.

The prime Contractor shall forward via email a completed copy of the 366th SFS FORM 30's to the designated COE representative at the Mountain Home Project Office. A background check will be done on ALL individuals seeking entry onto Mountain Home AFB. This will take 14 business days.

Unfavorable results from the background check will result in individuals being denied access to the installation, unfavorable results include, but are not limited to, individuals that are determined to fall into one or more of the following categories: those having outstanding wants or warrants, sex offenders, violent offenders, those who are on probation, and those who are in a pre-release program. Individuals with a prior felony conviction or who currently have a warrant for their arrest will not be given entry to Mountain Home AFB. This check must be completed annually.

Once the pre-screened individual(s) have been cleared they will then proceed to the Main Gate Visitor's Center (located right outside the Main Gate), to obtain a base pass (AF Form 75). They must provide a state/government issued picture identification to be verified against the 366th SFS FORM 30. Those employees, who clear the background check process, will be issued passes/DBIDS cards for up to one year. The contractor must repeat the above stated process on an annual basis. The 366th SFS FORM 30 will need to be resubmitted in its entirety any time personnel need to be added. Advance planning of personnel working on the contract will be required to meet this requirement.

Contractors are required to notify the Mountain Home Project Office, within 48 hours after the loss, if an employee on a 366th SFS FORM 30 terminates his or her employment with the Contractor or is involved in an incident resulting in an unfavorable result, including, but not limited to, those identified above. If a Contractor terminates an employee, it is incumbent upon the contractor to ensure that the employee's access badge is returned to the Mountain Home Project Office. If they are unable to retrieve the pass, they will contact the Mountain Home Project Office with the employee's information. The Mountain Home Project Office will then inform Pass and Registration who will then contact the installation entry control points and have them retrieve the pass if the person attempts to re-enter the base. If a contractor has a DBIDS issued card, it will be invalid for installation entry immediately upon notification to Pass and Registration.

The card must still be returned to the Mountain Home Project Office for destruction. The passes shall not be worn or displayed off the military base.

Retired military members and dependents of active duty or retired military

SECTION 01 14 00 Page 7 members who are Contractor employees may use their DoD identification when entering MHAFB for the performance of contract services. Under no circumstance will retired military members or dependents of active duty or retired military members vouch contractor personnel onto the installation utilizing their DoD identification card.

1.3.4.2 Vendor/Delivery/Distributor Access Procedure

Vendor/delivery/distributor are defined as those commercial businesses that have singular or multiple stops on base (i.e. COCESS deliveries, UPS, FedEx, food and beverage stocking trucks). When a vendor/delivery/distributor requires access to the base, they will be stopped prior to entering the base. To determine if a delivery is valid;

documentation, visual recognition, or confirmation from a base organization expecting delivery is required. If verified, the vehicle will be escorted with a military troop for the entire duration the vehicle is on base. As the vehicle departs, the escort will be dropped off at the gate. If an escort is not available, attempts will be made to contact a base functional representative. The functional representative will provide an escort for the entire time the vehicle is on the installation. If a functional representative is not contacted and an escort is not available, the vehicle will be denied access.

All commercial and private vehicles will be searched. Drivers should anticipate delays.

Procedures for commercial and private vehicle access to Mt. Home AFB are subject to change without prior notice.

1.4 COORDINATION, SAFETY AND REGULATORY REQUIREMENTS

1.4.1 Construction Traffic

Construction traffic entering the base via Grandview gate entrance shall be under control of a flagman in radio contact with base traffic control tower. When conflicts arise between construction activities, aircraft operations and safety, aircraft operation and safety will take precedence and govern. Construction sequencing shall be coordinated with airfield management through the Contracting Officer (CO). Continuous sweeping shall be required for pavement to be opened to traffic or areas open to air traffic within construction limits as directed by CO.

Vehicles and equipment shall not be allowed within areas 150 feet from the centerline of active taxiways, and 200 feet from centerline of active runway without permission from the CO.

This project involves work on an active military airfield. Aircraft will be operating near and directly over the project site. Following are special requirements of working in this environment.

1.6.1 Equipment Height

Do not use any equipment or erect any structure greater than 20 feet high in the flightline project area without prior approval from the Contracting Officer.

1.6.2 Project Limits

Do not operate equipment or permit personnel to work outside the project limits without approval from the Contracting Officer.

1.7 DUST CONTROL

Maintain strict dust control to prevent blowing dust at all times.

SECTION 01 14 00 Page 9

1.8 ACCESS AND ACCESS ROADS

Access to or around Mountain Home Air Force Base shall be coordinated with and require approval of the Contracting Officer a minimum of 24 hours in advance. The Contractor, his suppliers and subcontractors shall conduct operations in a manner that adheres to the requirements of this section, the Project Safety Plan and the project drawings.

1.8.1 Construction Access

The designated access to the project site is from Idaho State Highway 67.
