PERFORMANCE WORK STATEMENT (PWS)

Chief Joseph Dam Elevator Maintenance and Service

1. GENERAL: This is a non-personal services contract to provide routine and periodic testing and maintenance services for quarterly preventive maintenance, emergency service calls, Category 1 tests and inspections, and Category 5 tests (as outlined by table N-1, ASME 17.1) to be conducted by a contractor on the hydraulic and traction elevators located at Chief Joseph Dam. The Government shall not exercise any supervision or control over the contract service providers performing the services herein, however all work will be approved by CJD personnel prior to conducting any corrective repairs. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.

1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform quarterly preventive maintenance, emergency service calls, Category 1 tests and inspections and Category 5 tests as defined in this Performance Work Statement. The contractor shall perform to the standards in this contract and use trained personnel directly employed by a reputable elevator company.

CJD personnel will accompany all inspections, and reserves the right to perform/observe any corrective maintenance necessary to any system. The first inspection shall occur within 30 days of NTP.

1.2 Scope: Provide all labor, equipment, materials and supplies necessary to perform quarterly preventive maintenance, emergency service calls, Category 1 tests and inspections and Category 5 tests as defined in this Performance Work Statement on four (4) government owned elevators at Chief Joseph Dam Project, Bridgeport, Washington in accordance with State of Washington, applicable Elevator Safety Codes and Standards and the attached specifications. An option item for each elevator will be included to allow for emergency repairs. Each option item is not to exceed $7,000 in cost.

NOTE: All inspections will comply with ASME 17.1, and include but are not limited to the inspections outlined in this section.

1.2.1 Monthly Elevator Preventive Maintenance – Once a month, the contractor shall inspect the following:

Control and landing positioning systems Signal fixtures Machines Drive Motors Governors Sheaves and Ropes Power Units Pumps Jacks and Valves Car and hoistway door operating devices and door protection equipment Load weighters Car frames and platforms Counterweights

Quarterly Elevator Preventive Maintenance: Each quarter the Contractor will lubricate, adjust, and inspect the following equipment for satisfactory condition:

Controller parts, selectors and dispatching equipment, fuses, wiring, relays, solid-state components, transducers, resistors, condensers, power amplifiers, transformers, contacts, leads, dashpots, timing devices, computer and microcomputer devices, steel selector tapes, mechanical and electrical driving equipment, signal lamps, and position indicating equipment.

Door operators, car door hangers, car door contacts, door protective devices, load weighing equipment, car frames, car safety mechanisms, platforms, car and counterweight guide shoes including rollers and gibs, and emergency car lighting.

Hoistway door interlocks and hangers, bottom door guides, and auxiliary door closing devices.

Machines, worms, gears, thrust bearings, drive sheaves, drive sheave shaft bearings, brake pulleys, brake coils, contacts, linings, and component parts.

Motors, brushes, brush holders, and bearings.

Governor components, governor sheaves and shaft assemblies, bearings, contacts, governor jaws, deflector or secondary sheaves, car and counterweight buffers, car and counterweight guide rails, car and counterweight sheave assemblies, top and bottom limit switches, governor tension sheave assemblies, and compensating sheave assemblies.

Pumps, pump motors, operating valves, valve motors, leveling valves, plunger packings, exposed piping, above ground plungers and cylinders, and hydraulic fluid tanks.

1.2.2 Annual Elevator Preventive Maintenance – Once a year the contractor will perform the following:

Ride elevator car from top to bottom and back looking, listening and feeling for any unusual sounds or vibration that would indicate abnormal operation. Look for proper positioning at each stop. Test all of the following:

Emergency bell and stop switches Elevator communications system and fire fighter mode operations Adjust stop switches, as needed Check door and gate safety interlocks for proper operations, adjust as needed Inspect control cabinet interior for signs of overheating and relay contacts for burning

1.2.3 Annual Safety Test and Inspection: Contractor will provide all labor, equipment, materials and supplies necessary to perform annual and Category 1 testing and inspections on all elevators listed in section “1.2 Scope” in accordance with Washington States applicable Elevator Safety Codes and Standards, the National Code and Supplements pertaining to the type of conveyance listed in section 1.2, and the attached specifications. Inspectors shall meet minimum qualifications set forth in the Standard for the Qualification of Elevator Inspectors ASME QEI-1. Inspectors and inspection supervisors shall be certified by an organization accredited by The American Society of Mechanical Engineers Qualification of Elevator Inspectors Committee, in accordance with the requirements set forth in the Standard for the Qualification of Elevators Inspectors, ASME QEI-1.

1.2.4 Coordination: The Contractor will:

Have the maintenance technician contact the PROJECT to schedule the maintenance visit Upon arriving at the PROJECT, the maintenance technician will check in with one of the designated persons on section 1.4.11 of this PWS.

Upon completion of work, the maintenance technician will brief the designated individual, describing the work performed, and any outstanding work that is required, and any material that is to be purchased.

Maintenance technician will document all work performed on Maintenance Tasks and Records Logs provided with each controller.

1.2.5 Full Coverage Parts Replacement:

CONTRACTOR will repair or replace any components that are showing wear from normal operation During regularly scheduled visits, CONTRACTOR will replace any signal lamps that have failed.

1.2.6 Service Requests: Service requests are defined as minor adjustments and emergency entrapments that can be accomplished in two hours or less (excluding travel time). For service requests outside of normal business hours, the CONTRACTOR will cover all labor expenses. Labor expense is defined as travel time, travel expense, and time spent on the job. Response time for emergency services shall be within one hour of notification. Response may be characterized as a telephone call or email from the contractor.

1.2.7 Items Not Covered

Cosmetic, construction, or ancillary components of the elevator, including the cab enclosure, ceiling frames, panels, and/or fixtures, hoistway door panels, door frames, sills, car flooring, floor covering, lighting fixtures, main power line switches, circuit breakers, controller feeders, buried piping, alignment of elevator guide rails, smoke and fire sensors, fire service reports, batteries for emergency lighting and lowering, and ventilation fans.

1.3 Period of Performance: The contractor shall complete all phases of work on a recurring quarterly basis.

Maintenance shall be performed within the first 15 calendar days of the month.

1.4 General Information

1.4.1 Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract. After acceptance of the quality control plan the contractor shall receive the Contracting Officer’s acceptance in writing of any proposed change to his QC system.

1.4.2 Quality Assurance: The Government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

1.4.3 Government Holidays:

Contractor shall not be permitted to work on these days (unless an emergency option is exercised) New Year’s Day (First day in January) Martin Luther King Jr. Day (Third Monday in January) President’s Day (Third Monday in February) Memorial Day (Final Monday in May) Independence Day (4th day of July) Labor Day (First Monday in September) Columbus Day (Second Monday in October) Veteran’s Day (11th day of November) Thanksgiving Day (Fourth Thursday of November) Christmas Day (25th day of December)

1.4.4 Hours of Operation: Contractors shall be permitted to be onsite between the hours of 0700 and

1700 Monday thru Thursday. Extensions or changes to these timeframes may be requested in written format and provided to the Contracting Officer.

1.4.5 Place of Performance: The work to be performed under this contract will be performed at Chief Joseph Dam Project, Bridgeport, Washington.

1.4.6 Type of Contract: The government will award a services contract.

1.4.7 Security Requirements: Contractor personnel performing work under this contract must have an

FBI background check prior to working unescorted at the project sites. All personnel that will be working at USACE-NWS operating projects will be required to fill out the attached background check forms (See Technical Exhibits 3 and 4).

1.4.7.1 Physical Security: The contractor shall be responsible for safeguarding all Government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.

1.4.7.2 Access and General Protection/Security Policy and Procedures: All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation Provost Marshal Office, Director of Emergency Services or security office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the Government may require changes in contractor security matters or processes.

1.4.7.3 All contract employees, including subcontractor employees who are not in possession of the appropriate security clearance or access privileges, will be escorted in areas where they may be exposed to classified and/or sensitive materials and/or sensitive or restricted areas.

1.4.7.4 The Contractor must pre-screen Candidates using the E-verify Program

(http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the Contracting Officer no later than 3 business days after the initial contract award.

1.4.7.5 Key Control: The Contractor shall establish and implement methods of making sure all keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. NOTE: All references to keys include key cards. No keys issued to the Contractor by the Government shall be duplicated. The Contractor shall develop procedures covering key control that shall be included in the Quality Control Plan. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas. The Contractor shall immediately report any occurrences of lost or duplicate keys/key cards to the Contracting Officer.

1.4.7.5.1 The Contractor shall prohibit the use of Government issued keys/key cards by any persons other than the Contractor’s employees. The Contractor shall prohibit the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Contracting Officer.

1.4.8 Special Qualifications: All contractor personnel performing maintenance or inspection services on the elevators at the various USACE projects shall have the applicable licenses, certifications, or permits issued by the respective local state. Any technician not possessing the proper credentials shall not be permitted to work on the elevators.

1.4.9 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The Contracting Officer, and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor’s performance. At these meetings the Contracting Officer will apprise the contractor of how the Government views the contractor’s performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the Government.

1.4.10 Government Key Personnel: The following personnel are considered key personnel by the

Government:

1.4.11 Contractor Key Personnel:

The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the Contracting Officer. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.

1.4.12 Identification of Contractor Employees:

“Identification of Contractor Employees in the Federal Workplace (Aug 2008):

a) Contractor personnel shall clearly identify themselves to all attendees as a contractor employee before the commencement of meetings with Government or other contractor personnel. Contractor personnel shall clearly and immediately identify themselves as a contractor employee when placing, answering or participating in telephone/VTC conversations with Government or other contractor personnel. When onsite, Contractor personnel shall wear and display the appropriate “visitor badges” that will be issued daily by the Administrative Section at Chief Joseph Dam. Contractor personnel will be required to sign in to the Visitor Log Book upon entry to the project and sign out of the Visitor Log Book prior to leaving.

b) When contractor personnel send e-mail messages to a Government-owned computer, they shall include a signature block that includes their employer’s name and the employee’s full name and e-mail address.

1.4.13 Contractor Travel: All travel costs and coordination will be covered by the contractor and included in the final quote. EXCEPTION: In the event of trouble calls between regular examinations, additional funding shall be approved for travel time and expenses, at regular billing rates, from the dispatching point to the building under contract and return.

1.4.14 Government Rights: The Government has unlimited rights to all documents/material produced under this contract. All documents and materials, to include the source codes of any software, produced under this contract shall be Government owned and are the property of the Government with all rights and privileges of ownership/copyright belonging exclusively to the Government.

These documents and materials may not be used or sold by the contractor without written permission from the Contracting Officer. All materials supplied to the Government shall be the sole property of the Government and may not be used for any other purpose. This right does not abrogate any other Government rights. Additionally the Government maintains the right to service the elevators at the discretion of the project sites Chief of O&M. This contract shall not prevent or hinder the Government from servicing the elevators.

1.5 GOVERNMENT FURNISHED ITEMS AND SERVICES:

1.5.1 Facilities: The Government will provide all necessary workspace for the contractor to complete the services outlined in the PWS. Facilities include: desk space, limited telephone use, and use of the restroom facilities.

1.5.2 Hazardous energy control: Lock out tagout of equipment under service shall be coordinated with the PROJECT and controlled by PROJECT personnel in accordance with PROJECT Procedures.

1.5.3 Equipment: The Government will provide access to fax machines and copy machines.

1.6 CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES:

1.6.1 General: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract that are not listed under Paragraph 1.5 of this PWS.

1.6.2 Equipment: The Contractor shall mobilize and come to the project site with all equipment necessary to complete the task outlined in the PWS. This equipment may include but is not limited to ladders, fall protection equipment, scaffolding, Personal Protective Equipment, and all (hand and specialized) tools necessary.

1.6.3 Materials: The Contractor shall furnish all supplies necessary to meet the requirements under this

PWS.

1.6.4 Training: The Contractor shall be responsible for the following training requirements:

1.6.4.1 AT Level I Training: All contractor employees, to include subcontractor employees, requiring access to Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the Contracting Officer, within five (5) calendar days after completion of training by all employees and subcontractor personnel. 1.6.4.2 Access and General Protection/Security Policy and Procedures: All contractor and all associated subcontractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of emergency Services or Security Office.

Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

1.6.4.3 iWATCH and/or CorpsWatch Training: The contractor and all associated sub-contractors shall brief all employees on the local iWATCH, Corps Watch, or See Something, Say Something program (training standards provided by the requiring activity ATO). This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the KO. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the KO NLT 5 calendar days after contract award.

1.6.4.4 All new contractor employees will complete Level 1 OPSEC Training within 30 calendar days of their reporting for duty. Additionally, all contractor employees must complete annual OPSEC Awareness Training. The contractor shall submit certificates of completion for each affected contractor and subcontractor employee, to the KO or to the contracting officer, within 5 calendar days after completion of the training. OPSEC Awareness training is available at the following websites: https://www.iad.gov/ioss or http://www.cdse.edu/catalof/operations-security.html; or it can be provided by the RA OPSEC Officer in presentation form which will be documented via memorandum.

1.6.4.5 The Contractor must pre-screen Candidates using the E-verify Program

The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure that correct information is entered into the E-verify system. An initial list of verified/eligible candidates must be provided to the KO no later than 3 business days after the initial contract award.

PART 2

DEFINITIONS & ACRONYMS

2. DEFINITIONS AND ACRONYMS:

2.1. DEFINITIONS:

2.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the Government. The term used in this contract refers to the prime.

2.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

2.1.3. DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.

2.1.4. DELIVERABLE. Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.

2.1.5. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

2.1.6. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

2.1.7. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

2.1.8. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.

2.1.9. QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.

2.1.10. SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.

2.1.11. WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.

2.1.12. WORK WEEK. Monday through Thursday, unless specified otherwise.

PART 3

GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES

3. GOVERNMENT FURNISHED ITEMS AND SERVICES:

3.1. Services: The Government will provide access, technical specification, and applicable technical drawings (when available) to the contractor. Assistance will also be provided with any questions or concerns relating to proprietary knowledge of the elevators or its subsystems.

3.2 Facilities: The Government will provide access to a restroom located in the control structure. Failure of the Contractor’s personnel to keep Government restrooms neat and orderly will result in denial of continued use.

Sanitation facilities furnished by the Contractor shall be in accordance with EM 385-1-1. These facilities will only be permitted during when Contractor crews are actively working on site. CJD personnel will approve and authorize location of sanitation facilities.

3.3 Utilities: The Government will provide power service via plug terminals in and around the elevators and maintenance rooms. It is recommended that drinking water be brought and staged for Contractor crews on site, however drinking fountains are available upon request throughout the facility. The Contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish the purpose of use.

PART 4

CONTRACTOR FURNISHED ITEMS AND SERVICES

4. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES:

4.1 General: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract that are listed in this PWS.

4.2 Materials: The Contractor shall furnish all consumable materials necessary to perform preventative maintenance of the systems. Any materials or parts needed for corrective maintenance will be procured outside of this contract.

4.3. Equipment: The Contractor shall furnish and supply all necessary equipment to perform the duties outlined in this PWS.

PART 5

APPLICABLE PUBLICATIONS

5. APPLICABLE PUBLICATIONS (CURRENT EDITIONS)

5.1. The Contractor must abide by all applicable regulations, publications, manuals, and local policies and procedures. This includes but is not limited to EM 385-1-1 (most recent version).

5.2. The Contractor shall provide all related publications for any equipment installed/replaced. These publications include but are not limited to manuals (owner’s, installation, etc.), drawings, schematics, and warranty information.

QUALITY ASSURANCE SURVEILLANCE PLAN

For Elevator Maintenance, Service, Testing, and Inspection U.S. Army Corps of Engineers Seattle District (USACE-NWS) Chief Joseph Dam

1. PURPOSE

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:

What will be monitored How monitoring will take place Who will conduct the monitoring How monitoring efforts and results will be documented

This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. In addition, the QASP should recognize that unforeseen and uncontrollable situations may occur.

This QASP is a “living document” and the Government may review and revise it on a regular basis.

However, the Government shall coordinate changes with the contractor. Updates shall ensure that the QASP remains a valid, useful, and enforceable document. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.

The following FAR clauses may apply depending on contract type:

2. GOVERNMENT ROLES AND RESPONSIBILITIES

The following personnel shall oversee and coordinate surveillance activities.

a) Contracting Officer (KO) – The KO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The KO shall also assure that the contractor receives impartial, fair, and equitable treatment under this contract. The KO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

b) Acquisition Manager (AM) – The AM acts as an acquisition consultant and serves as a liaison between the USACE and the requesting program office, as well as liaison between the USACE and the supporting contracting office.

c) Quality Assurance Representative (QAR) – The QAR monitors all technical aspects of the contract and assists in contract administration. The QAR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of

Contractor personnel. The QAR is not authorized to change any of the terms and conditions of the contract.

4. PERFORMANCE STANDARDS

The Performance Requirements Summary Matrix, Technical Exhibit 1 in the Performance Work Statement includes performance standards. The Government shall use these standards to determine contractor performance and shall compare contractor performance to the Acceptable Quality Level (AQL).

6. METHODS OF QA SURVEILLANCE

Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the administration of this QASP.

Regardless of the surveillance method, the COR shall always contact the contractor’s task manager or on-site representative when a defect is identified and inform the manager of the specifics of the problem. The COR, with assistance from the AM, shall be responsible for monitoring the contractor’s performance in meeting a specific performance standard/AQL.

a) PERIODIC INSPECTION: Periodic Inspections of the quality and the status of the project may be scheduled daily or unscheduled, as required.

b) 100% INSPECTION: Upon completion of the work the COR will inspect the elevators, machinery rooms, and the area around the elevators to ensure that the contractor has adequately cleaned the area.

c) VALIDATED COMPLAINTS: Upon notification of a valid compliant the COR will complete an inspection.

Surveillance results may be used as the basis for actions (to include payment deductions) against the contractor. In such cases, the Inspection of Services clause in the Contract becomes the basis for the KO’s actions.

7. RATINGS

Metrics and methods are designed to determine if performance exceeds, meets, or does not meet a given standard and acceptable quality level. A rating scale shall be used to determine a positive, neutral, or negative outcome. The following ratings shall be used:

EXCEPTIONAL: Performance significantly exceeds contract requirements to the Government’s benefit.

SATISFACTORY: Performance meets contractual requirements.

UNSATISFACTORY: Performance does not meet contractual requirements.

8. DOCUMENTING PERFORMANCE

a) ACCEPTABLE PERFORMANCE

The Government shall document positive performance. A report template is attached. Any report may become a part of the supporting documentation for fixed fee payments, award fee payments, or other actions.

b) UNACCEPTABLE PERFORMANCE

When unacceptable performance occurs, the COR shall inform the contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file.

When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR), and present it to the contractor’s task manager or on-site representative. A CDR template is attached to this QASP.

The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government shall review the contractor’s corrective action plan to determine acceptability.

Any CDRs may become a part of the supporting documentation for contract payment deductions, fixed fee deductions, award fee nonpayment, or other actions deemed necessary by the KO.

9. FREQUENCY OF MEASUREMENT

a) Frequency of Measurement. Quarterly for Quarterly Maintenance Services

During contract/order performance, the COR shall review the report upon completion of the quarterly contract work or upon a valid compliant as specified in the Performance Standards Summary Matrix.

b) Frequency of Measurement. One Time for Annual Safety Tests

During contract/order performance, the COR shall make one report upon completion of the contract work or upon a valid compliant as specified in the Performance Standards Summary Matrix.

c) Frequency of Measurement. Monthly for Monthly Maintenance Services

During contract/order performance, the COR shall make one report upon completion of the monthly contract work or upon a valid compliant as specified in the Performance Standards Summary Matrix.
