1. Is there an incumbent and who is it? What is the current contract number and annual cost of the contract?
· W9128F-14-P-0068 Awarded to FEI BEHAVIORAL HEALTH, INC. - BASE YEAR AWARDED for $18,450.00
How many face to face counseling sessions do you require per year per employee? (generally, customers choose either 3, 6, or 8 sessions per employee per annum) What is the current Utilization rate?
· Actual counts of face-to-face counseling are unpredictable and therefore historical information will not be provide. Please use information provided in Solicitation.
2. Can the Orientation meeting (contract kick-off) for contract award take place via telecon or is a face to face meeting required?
· Contractors are encouraged to provide methodologies and techniques proposed to meet the requirement of the Performance Work Statement. All Quotes will be evaluated IAW the solicitation.
3. What format is required for the annual training sessions at each location, live on-site or webcast/webinar?

4. Are there any health fair events that will require live-onsite presence? Where and how often?
