NOTES:

1. Prices must be entered for all items of the schedule. Total amounts submitted without prices being entered on individual items will be rejected. Additions will be subject to verification by the Government. In case of variation between the Job (lump-sum) prices and the total amount, the lump-sum prices will be considered the bid.

2. A modification to a price which provides for a single adjustment to the total amount, should state the application of the adjustment to each respective lump-sum price affected. If the modification is not so apportioned, the single adjustment will be applied to Item No. 0004.

Basic Best Value

The Government will issue a purchase order resulting from this solicitation to the responsible offeror whose quote conforms to the solicitation and is determined to be the Best Value for the Government.

The following factors shall be used to evaluate quotes:

1. Past Performance – The offeror shall provide two (2) project examples that they have completed within the past five (5) years that are equal to or greater than this requested work in size and are of similar scope. Experience will be based on projects they have completed as a prime contractor or as a subcontractor. The offeror shall provide details of the project and pertinent information to demonstrate their role in the project and how the project is comparable to this requirement in size, scope and complexity. The offeror shall include points of contact, titles, telephone numbers, and email addresses (if available) for all references.

Additionally, past performance may be based on the contracting’s officer’s knowledge of and previous experience with similar projects acquired and any information contained in the Contract Performance Assessment Reporting System (CPARS), contained at www.cpars.gov.

• A rating of exceptional will be given for submitted past performance that demonstrates their successful completion of two projects and additional information retrieved from CPARS and from the Contracting Officer is favorable on both of those projects.

• A rating of satisfactory will be given for submitted past performance that demonstrates their successful completion of two projects and one of the submitted projects has reference that demonstrates successful completion and the information from CPARS and the Contracting Officer is favorable.

• A rating of unsuccessful will be given for submitted past performance that does not demonstrate any successfully completed similar projects and/or the additional information from CPARS and the Contracting Officer is not favorable.

2. Price – The quote shall be inclusive of all costs to provide the construction services specified in the scope of work and attachments.

SUPPLEMENTARY CONDITIONS (SPECIAL CONTRACT REQUIREMENTS)FOR PURCHASE ORDERS)

PART 1 GENERAL

1.1 SCOPE

The scope of work includes providing the work necessary to install an approximately two foot long, 5/16" steel liner inside of the existing 36" diameter steel lower level intake pipe and testing three reinforced concrete pipe segment joints at Bear Creek Dam intake structure.

Installation of the inner liner will require base metal preparation, fabrication and welded installation of liner, painting and grouting of the annular space between existing pipe and liner. Access to the lower level intake pipe will require the installation of a temporary inflatable plug at the upstream intake and the fabrication and installation of a temporary splash guard in the intake structure wet well, removal and reinstallation of the lower level intake pipe butterfly valve, (2) flexible couplings and

(2) pipe segments. Butterfly valve seals to be tested to ensure they are leak-proof. Three existing joints of the lower level RCP intake pipe segments will be tested for leaks in accordance with ASTMs C1103 and C969.

The dewatering, removal of material, cleaning, inspection activities, and all incidental construction must take place over a 30-day period between Wednesday, October 30, 2019 and Friday, November 29th, 2019 to minimize the impact on downstream water users and allowing for the coordination of travel plans with USACE inspectors. Bear Creek Lake will be lowered 6 inches prior to the start of the contract work and the 30 day window.

Flows will be redirected through the mid-level intake after dewatering of the lower intake. USACE inspection activities will take place between Wednesday, November 27, 2019 and Friday, November 29, 2019 (3 days). Any proposed change to the dates for this 30-day window shall be submitted to, and approved by, the Contracting Officer Representative (COR) prior to the start of the field work.

1.2 COMMENCEMENT, PROSECUTION, AND COMPLETION OF WORK.

The Contractor shall commence work under this contract within ten (10) calendar days after the date of receipt by him of Notice to Proceed, prosecute said work diligently, and complete the entire work except seeding ready for use not later than 120 calendar days after receipt of Notice to Proceed. The time stated for completion shall include final cleanup of the premises. (FAR 52.211-10)

1.3 CONTRACT DRAWING AND SPECIFICATIONS

The Contractor shall be responsible for making his own copies of specifications, drawings and any amendments, as needed for the duration of the project, from the electronic files provided.

1.4 SUBMITTALS

Government approval is required for submittals with a "G" designation;

submittals not having a "G" designation are for information only. When used, a designation following the "G" designation identifies the office that will review the submittal for the Government. The following shall be submitted in accordance with Section 01 33 00 SUBMITTAL PROCEDURES:

1.5 CONTRACTOR QUALITY CONTROL (CQC).

1.5.1 Responsibility of the Contractor

Except for isolated tests or other items of work specified to be performed by the Government, the quality of all work shall be the responsibility of the Contractor. Sufficient inspections and tests of all items of work, including that of subcontractors, to ensure conformance to applicable specifications and drawings with respect to the quality of materials, workmanship, construction, finish, functional performance, and identification shall be performed on a continuing basis.

1.6 GUARANTEE

The equipment and supplies to be furnished under this contract shall be guaranteed for a period of one (1) year from the date of acceptance thereof, either for beneficial use or final acceptance, whichever is earlier, against defective materials, design, and workmanship. Upon receipt of notice from the Government of failure of any of the guaranteed equipment during the guarantee period, new replacement parts shall be furnished and installed promptly by the Contractor at no additional cost to the Government. The Contractor shall also obtain and deliver to the Contracting Officer any warranties of equipment or supplies which the

DOCUMENT 00 73 00 Page 3 subcontractors, manufacturers, or suppliers would give normal commercial practice.

1.7 ACCIDENT PREVENTION

Adherence to 30-Nov-2015 EM 385-1-1, specifically Contractor-developed Activity Hazard Analysis (AHA), Accident Prevention Plan (APP), Confined Space Plan and Load Handling Equipment Plan if a crane is used. For additional supplementary safety submittal requirements refer to 09 97 02 PAINTING: HYDRAULIC STRUCTURES. The Contractor shall provide for protecting of the lives and health of employees and other persons;

preventing of damage to property; and avoiding work interruptions.

1.8 PROTECTION OF EXISTING FACILITIES AND WORKS

The Contractor shall be responsible for the protection of work area from damage and upon completion of the work shall leave existing works in a condition equal to that which existed when the work started.

1.9 CONTRACT DRAWINGS AND SPECIFICATIONS

1.9.1 NOTIFICATION OF DISCREPANCIES

The Contractor shall check all drawings furnished him immediately upon their receipt and shall promptly notify the Contracting Officer of any discrepancies. Dimensions marked on drawings shall be followed in lieu of scale measurements. Enlarged plans and details shall govern where the same work is shown at smaller scales. The Contractor shall compare all drawings and verify the figures before laying out the work and will be responsible for any errors which might have been avoided thereby.

1.9.2 OMISSIONS

Omissions from the drawings or specifications or the misdescription of details of work which are manifestly necessary to carry out the intent of the drawings and specifications, or which are customarily performed, shall not relieve the Contractor from performing such omitted or misdescribed details of the work but they shall be performed as if fully and correctly set forth and described in the drawings and specifications.

1.10 PAYMENT

1.10.1 PROMPT PAYMENT ACT

Pay requests authorized in CONTRACT CLAUSES clause: "Payments Under Fixed Price Construction Contracts", will be paid pursuant to the clause, "Prompt Payment for Construction Contracts". Pay requests will be submitted on ENG Form 93 and 93a, "Payment Estimate Contract Performance" and "Continuation". All information and substantiation required by the identified contract clauses will be submitted with the ENG Form 93, and the required certification will be included on the last page of the ENG Form 93a, signed by an authorized contractor official and dated when signed.

The designated billing office is the Office of the Area Engineer.

1.10.2 PAYMENTS FOR MODIFICATIONS

Payments may be made for cost bearing change orders within the scope of the contract only to the extent funds are authorized in the order on a two part modification. Contractor pricing proposed must be submitted at the

DOCUMENT 00 73 00 Page 4 earliest possible time after the change order is issued, or at a specific time as directed by the Contracting Officer. At the discretion of the Contracting Officer, any and all payments may be withheld on the modification until the Contractor has submitted a qualifying price proposal, in as much detail as required by the Contracting Officer, and the final price has been agreed.

1.11 AVAILABILITY OF UTILITY SERVICES

Use of public and private utilities will be as found available. The Contractor shall make his own arrangements for use of public and private utilities.

1.12 PROFIT

a. Weighted guidelines method of determining profit shall be used on any equitable adjustment change order or modification issued under this contract. The profit factors shall be as follows:

b. Based on the circumstances of each procurement action, each of the above factors shall be weighted from .03 to .12 as indicated below. The value shall be obtained by multiplying the rate by the weight. The value column when totaled indicates the fair and reasonable profit percentage under the circumstances of the particular procurement.

(1) Degree of Risk. Where the work involves no risk or the degree of risk is very small, the weighting should be .03; as the degree of risk increases, the weighting should be increased up to a maximum of .12. Lump sum items will have, generally, a higher weighted value than the unit price items for which quantities are provided. Other things to consider: the portion of the work to be done by subcontractors, nature of work, where work is to be performed, reasonableness of negotiated costs, amount of labor included in costs, and whether the negotiation is before or after performance of work.

(2) Relative Difficulty of Work. If the work is most difficult and complex, the weighting should be .12 and should be proportionately reduced to .03 on the simplest of jobs. This factor is tied in to some extent with the degree of risk. Some things to consider: the nature of the work, by whom it is to be done, where, and what is the time schedule.

(3) Size of Job. All work not in excess of $100,000 shall be weighted at .12. Work estimated between $100,000 and $5,000,000 shall be proportionately weighted from .12 to .05.

(4) Periods of Performance. Jobs in excess of 24 months are to be weighted at .12. Jobs of lesser duration are to be proportionately weighted to a minimum of .03 for jobs not to exceed 30 days. No weight where additional time not required.

(5) Contractor's Investment. To be weighted from .03 to .12 on the basis of below average, average, and above average. Things to consider: amount of subcontracting, mobilization payment item, Government furnished property, equipment and facilities, and expediting assistance.

(6) Assistance by Government. To be weighted from .12 to .03 on the basis of average to above average. Things to consider: use of Government-owned property, equipment and facilities, and expediting assistance.

(7) Subcontracting. To be weighted inversely proportional to the amount of subcontracting. Where 80 percent or more of the work is to be subcontracted, the weighting is to be .03 and such weighting proportionately increased to .12 where all the work is performed by the Contractor's own forces.

1.13 AS-BUILT DRAWINGS

The Contractor shall provide a marked-up half size drawing to show actual work performed to include necessary sketches, modification drawings, shop drawings and notes. Red ink is to be used for additions and deviations from the contract. Green ink is to be used to indicate work deleted from the contract. The drawings shall be maintained by the contractor as the work progresses and available for review by the government representatives. The as-built drawings shall be submitted for approval at the completion of the project. The completed set shall be available at time of final inspection.

1.14 PORTABLE TOILETS

Toilet facilities will not be available for Contractor's use. The Contractor shall provide and maintain portable toilets for use by the Contractor's staff and Government staff. Toilet(s) shall be placed at the location directed by the Contracting Officer. To prevent overturning by high winds, all portable toilets shall be anchored down.

1.15 FEDERAL HOLIDAYS AND WORKING HOURS

Work hours: The Contractor can work Monday through Friday 0700 to 1700.

Working outside these work hours will need written permission from the COR.

The Contractor will not be allowed to work on Federal holidays.

The following Federal legal holidays are observed by this installation:

1.16 ANTITERRORISM (AT)/OPERATIONS SECURITY (OPSEC) PROVISIONS

1. AT Level I Training

All Contractor employees, to include subcontractor employees, requiring access to Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days prior to mobilization. The Contractor shall submit certificates of completion and memo listing names of each affected Contractor employee and subcontractor employee who took the training on the date listed on the submitted certificate, to the COR or to the contracting officer, if a COR is not assigned, within 5 calendar days after completion of training by all employees and subcontractor personnel (submittal: AT Level I Training Sign In Sheets). Any following trainings documented via memo shall require at least one formal certificate matching the dates listed on the memo. AT Level I awareness training is available at the following website:

2. Access and General Protection/Security Policy and Procedures All Contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The Contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in Contractor security matters or processes.

2.1 Submit a complete, updated and signed, list of all Contractor and subcontractor personnel, including their titles and intended working hours, who will be working on site prior to start of work. This listing shall be revised and resubmitted when personnel changes occur. (submittal: Security Personnel List)

2.2 Company Registration

All USACE Prime Contractors shall register their company, with USACE's HSPD-12 compliance system, information at https://www.nwd.usace.army.mil/Business-With-Us/Contractor-Vetting/ within ten (10) working days after award of any contract, task order, or notice to proceed. Cost to register company is $399 if more than 20 individuals will be accessing USACE sites, or $299 if less than 20 employees will be accessing USACE sites. Registration is good for 365 days. If the contract period is longer than 365, company must renew their registration.

2.3 Personnel Registration

All contractor and subcontractor personnel that will be onsite for more than 10 days must be registered via USACE's HSPC-12 Compliance system, information at https://www.nwd.usace.army.mil/Business-With-Us/Contractor-Vetting/.

Thirty days prior to mobilization, registration of employees must occur.

Cost to register an employee is $159. Registration is good for one year.

Employee may work on multiple contracts or for multiple companies and the

DOCUMENT 00 73 00 Page 7 registration will remain valid for 365 days. If the contract extends beyond that period a renewal fee of $159 is required.

a. Identity Verification - Once a contractor is cleared to work and access a USACE facility, the individual contractor employees shall be required to show an identification compliant with the REAL ID Act of 2005 (https://www.dhs.gov/real-id) to access the Jobsite. Acceptable forms of identity verification are documents issued by a federal Government agency that include: the individual's photograph, name, and date of birth, such as a passport or military identification (ID) card.

b. Waiver - If a denial occurs there is a waiver process on the contractor's page. If there are any difficulties with the waiver process please contact your COR.

c. Escort Requirements - Contractor personnel not cleared for authorized access to the Jobsite may be escorted by Government or Contractor personnel that have authorized unescorted access to the Jobsite. All costs related to the escorting of non-cleared personnel shall be at the expense of the Contractor. Additional burden shall not be placed upon the Government to provide these escorts. Prior to access, coordination with the Project Security Officer is required, including but not limited to:

(1) Verification of identity with photo identification

(2) Name of escorting individual and verification of unescorted status

(3) Time of entry into the Jobsite

(4) Time exiting the Jobsite.

4. iWATCH and/or CorpsWatch Training

The Contractor and all associated subcontractors shall brief all employees on the local iWATCH, Corps Watch, or See Something, Say Something program (training standards provided by the requiring activity ATO). This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 10 days prior to mobilization and within 30 calendar days of new employees commencing performance with the results reported to the COR. There is NO classified material at this project location.

14. Contractor Company to Obtain a Facility Clearance and Individual Clearances at the Appropriate Level

The Prime Contractor Company must have (or will have) a Facility Clearance (FCL) at the appropriate level (IAW the NISPOM DOD 5220.22-M and AR 380-49) prior to the start of the contract awarded period of performance.

Contractor personnel performing work under this contract must have the required security clearance, per AR 380-67, at the appropriate level at the start of the period of performance. Security Clearances and FCL requirements are required to be maintained for the life of the contract IAW the DD254 attached to the contract. If no FCL, the supporting Government Contracting Activity will sponsor the prime contract company in obtaining the FCL. This location does NOT require a Facility Clearance/FCL.

15. Pre-Screen Candidates using E-Verify Program

The Contractor must pre-screen Candidates using the E-verify Program (http://www.dhs.gov/E-Verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award.

PART 1 GENERAL

1.1 APPLICABLE REGULATIONS AND PUBLICATIONS

All diving operations shall be performed in accordance with the following regulations and publications. Where there is a difference in standards that exist, the more stringent regulation or publication shall apply. Where the specifications are more stringent than the regulations and publications, the specifications shall apply.

1.1.1 US Army Corps of Engineers EM 385-1-1(30 NOV 2014) Safety and

1.1.2 OSHA 29 CFR 1910 Subpart T (OSHA

1.2 GENERAL REQUIREMENTS

The Contractor shall provide all necessary personnel and equipment to perform the work described herein, to place and remove temporary plug(s) as required to perform repair work on the lower level intake pipe at the Bear Creek Dam Intake Structure. Diving work will be accomplished using surface-supplied air (SSA).

1.2.1 DRAWINGS

See drawings for locations of intake structure and low level intake at bottom of reservoir. Larger and more detailed drawings may be available and at the U.S. Army Corps of Engineers, Tri-Lakes Office.

1.2.2 WORK AND DIVE CREW

The work will be accomplished using surface-supplied air (SSA) and a dive crew manned as prescribed in EM 385-1-1 (30 NOV 2014).

1.2.3 EQUIPMENT REQUIRED

The Contractor shall provide primary and emergency air supply, dive suits, hoses, lines, and all other related diving and safety equipment necessary for the work to be performed.

1.2.4 DIVER COMMUNICATIONS

(1) Hardwire communication with the divers is required at all times.

Electronic communication systems with an external speaker shall be incorporated so the entire dive team can monitor communications.

(2) Communications devices shall be tested prior to each dive, maintained in an operable condition, and protected from damage during use and storage IAW the manufacturer’s recommendations.

(3) All dive operations will be terminated in a safe, orderly fashion using line-pull signals if voice communications are lost.

(4) Defective electronic communication equipment shall not prevent a standby diver from deploying in an emergency if the dive supervisor determines it is safe for the diver to deploy and line-pull signals are used.

(5) All communications with the divers shall either be recorded or complete notes of the conversations shall be taken. Any other inspection data shall be recorded for government information.

1.3 PROJECT REQUIREMENTS

The dive work associated with this project will involve locating the lower level intake pipe. Once the pipe inlet is located, it shall be closed as required in the contract documents with inflatable devices inside the opening. The existing steel fabricated bulkhead that was intended for use in dewatering the lower intake pipe is no longer functional, and cannot be used for the purposes of this contract. After completing pipe repairs, a dive will be performed to release water into the tunnel by partially deflating the inflatable plug to introduce water to the pipe and check the butterfly valve, flexible couplings and seal for integrity. Secure the inflatable plug in the tunnel to prevent displacement during deflating.

This portion of the work will be repeated until the acceptable level of leakage has been attained as stated in the Statement of Work. After the leakage rate has been determined to be acceptable to the Government, a final dive will be performed to remove the inflatable device(s).

1.3.1 DEPTH OF DIVE

The depth of the water where the work is to be performed will be approximately a maximum of 31 feet to the lowest level of intake opening.

The projected pool elevation during the dive is expected to be 1558 feet

MSL.

1.3.2 LOCATION OF THE DIVE

The location of the work is at the northeast end of the reservoir, directly over and around the submerged low level intake opening of the 36 inch diameter pipe that feeds the lower butterfly valve located in the intake structure building. The approximate GPS coordinate for locating the inlet opening at the bottom of the reservoir is 39.6533337 degrees N and -

105.14020 degrees W.

1.3.3 WORK SCHEDULE

The work associated with this contract will be accomplished during the month of November, 2019. The dates and daily working hours of diving activities shall be coordinated with the Prime Contractor for this project, the Omaha District personnel at the Tri-Lakes Project Office, and the Omaha District Diving Coordinator. The information required to be submitted for approval prior to the dive shall be received by the Ompowerhousaha District Diving Coordinator a minimum of two weeks prior to the dive. See paragraph:

Submittals for information to be submitted by the Contractor prior to any diving operations.

1.4 PRE AND POST DIVE MEETINGS

PRE-DIVE BRIEFING (Prior to work commencing)

The contractor shall schedule and hold a pre-dive briefing at the Bear Creek Dam Project Site prior to the commencement of each type of diving operation in accordance with EM 385-1-1 30.A.19. This meeting is held to ensure that all submittals have been approved and to acquaint all personnel with the schedule, the Dive Operations Plan, AHA, Emergency Management Plan, Inspections, and any modifications needed.

1.4.1 PRE-DIVE TEAM BRIEFING (Prior to each dive)

The contractor shall hold a pre-dive briefing on site with the dive team prior to each dive in accordance with EM 385-1-1 30.A.20. All of the Contractors employees involved in the dive operation shall attend that meeting without exception. At the meeting the Contractor's AHA will be reviewed and discussed by all personnel that will be involved with the dive operation. Additional input shall be encouraged from all of those present. Equipment checklists, diving logs, diving conditions, and diving procedures shall also be addressed. Lock out Tag out (LOTO) Procedure for the entire Bear Creek Dam Operations Infrastructure shall be reviewed.

1.4.2 POST-DIVE MEETINGS

A post-dive debriefing will be held at the end of each work day in accordance with EM 385-1-1 30.A.21 at the Bear Creek Project site within allowed contractor hours. The meeting will also be used to review progress and for the contractor to give additional details about what was found during the work.

1.5 SITE CONDITIONS

1.5.1 DIVE LOCATION

The Bear Creek Dam is located southwest of Denver, Colorado just northeast of the intersection of highway 470 and US Highway 285. Refer to Statement of Work, Appendix A, Reference Drawing PB19-62E 2, Sht. M-2 for access to site and boat ramp locations.

1.5.2 ANTICIPATED CONDITION AT THE DIVE SITE

1.5.2.1 Weather

Typical fall weather for Denver, Colorado during the month of November.

1.5.2.2 Water Condition

Water elevation will be approximately 1558 feet (M.S.L.).

1.5.2.3 Visibility

Varies between 2 and 10 feet depending on weather conditions and water turbulence.

1.5.2.4 Type of Bottom

Concrete bottom on structure inlet to pipe opening with some silt cover over concrete. Area outside of concrete inlet structure can be expected to either silt or rock covered.

1.5.2.5 Depth

Not to exceed 31 feet. NOTE: Due to the altitude of the dive (Over 1000 ft msl), the contractor shall use appropriate high altitude decompression tables that compensate for the increased elevation.

1.5.2.6 Currents/Turbulence

There should be little natural currents at the working depth. Waves on the reservoir could potentially occur during the installation. See the project drawings for site location.

1.5.2.7 Approximate Water Temperature

Water temperatures are projected to be 44-69 degrees Fahrenheit, depending upon weather conditions.

1.5.2.8 Underwater Hazards/Obstructions

Negligible trash/tree limbs on bottom.

1.5.3 AVAILABLE POWER

Electrical power is not available at the dive location and would have to be provided by means of generator at the expense of the diving Contractor.

1.5.4 Precautions Taken by the Corps Project

The low level intake valves shall be closed and placed under Hazardous Energy Controls during the dive. Refer to Paragraph 3 of the Statement of Work for the step-by-step sequence of work and implementation of steps in the Hazardous Energy Control policy. A contractor’s staging area will be established.

1.6 SUBMITTALS

1.6.1 Submittal Process

Government approval/acceptance is required for submittals with a "GA" designation; submittals not having a "G" designation are for information only or as otherwise designated. When used, a designation following the "GA" designation identifies the office that will review the submittal for the Government. Examples of designations which may follow the “GA” designation include:

No on-site work will be allowed until all SD-05 and SD-07 contractor submittals listed in the following section have been accepted or approved by the USACE. All items shall be submitted to the COR.

A USACE provided 4025-R form must be completed by the contractor and used as a cover page for all submittals. The USACE can provide an example of a correctly completed 4025-R form for contractor use. The USACE will use the contractor submitted 4025-R form to accept/approve/reject contractor submittals and will return the USACE signed 4025-R form to the contractor with associated acceptance/approval/rejection code as follows:

1.6.2 Contractor Required Submittals

Type SD-01 and SD-07 submittals shall be submitted for approval minimum twenty (20) business days prior to planned diving operations.

SD-01 Preconstruction Submittals

A Safe Practices Manual; See EM 385-1-1 (30 Nov 14) Section 30.A.15; GA –

DDC/ADC/DSR

• EM 385-1-1 30.A.15 Safe Practices Manual. Contractors and USACE Districts/Labs with in-house dive teams shall develop and maintain a safe practices manual that encompasses their entire diving program. The safe practices manual shall be available at all times to the Government representative and all dive team members at each diving location. The safe practices manual shall include, as a minimum, the following:

a. Dive safety procedures and checklists;

b. Assignments and responsibilities of dive team members;

c. Equipment certifications, procedures, and inspection checklists;

d. Emergency procedures for fire, equipment failure, adverse weather conditions, and medical illness or injury and specific procedures for:

1. Entrapped or fouled diver including fouled umbilical (suction and entanglement/debris);

2. Actions upon loss of vital support equipment;

3. Actions upon loss of gas supply;

4. Action upon loss of communication;

5. Lost diver plan (SCUBA Operations only);

6. Injured diver plan;

7. Actions upon discovery of fire;

8. Diver blow up/over rapid ascent to surface

9. Diver loss of consciousness

10. Injury/illness of member of surface crew with diver in the water.

e. Procedures for internal safety inspections (frequency, checklists, etc.);

f. A complete copy of OSHA, 29 CFR 1910, Subpart T, and a statement of the employer's policy for ensuring compliance with the standard;

g. The appropriate U.S. Navy Table(s), including as a minimum:

1. U.S. Navy Table of No-Decompression Limits and

Repetitive Group Designation for No-Decompression Air Dives;

2. U.S. Navy Residual Nitrogen Timetables for Repetitive Air Dives;

3. U.S. Navy Standard Air Decompression Table.

h. A sample of the diving log sheets to be used;

i. A sample of the repetitive dive worksheets or equivalent

(dive profile method) to be used;

j. An outline of the fitness for duty (including medical) requirements for dive team members, and

k. An outline of administrative and record-keeping procedures.

B Dive Operations Plan; (Location & Job Specific, each type of diving operation for each location shall have a specific Dive Operations Plan); See EM 385-1-1 (30 Nov 14) Section 30.A.16; GA –

DDC/ADC/DSR

• EM 385-1-1 30.A.16 Dive Operations Plan. This plan is a general overview of all tasks to be performed, dive modes and equipment, site access, etc. Complex projects involving more than one work task, location, and/or dive team require task-specific dive plans as part of the overall Dive Operations Plan. As a minimum, the Dive Operations Plan will contain the following:

a. Date of dive plan submission;

b. Name and contact information for diving supervisor preparing the dive plan;

c. Names and duties of on-site dive team members, including diving supervisor;

d. List of diving equipment to be used;

e. Type of diving platform to be used;

f. Detailed description of the mission; Identify how/if work will be divided into separate tasks or phases of work;

g. Date(s), time(s), duration, and location of operation;

h. Diving mode used (SSA) including a description of the back-up air supply, as required;

i. Nature of work to be performed by the divers, including tools used and materials to be handled or installed;

j. Anticipated surface and underwater conditions, to include visibility, temperature, currents, etc. Thermal protection will be considered as appropriate;

k. Maximum single dive bottom time for the planned depth of dive for each diver. Altitude adjustments to dive tables will be calculated for dives made at altitudes of 1000 ft(304.8 m) or more above sea level;

l. Identification of topside assistance/support to the dive team (i.e., crane operator, lock operator, etc.);

m. Means of direct communication between the dive site and the DDC, SECTION 01 08 00 Page 7 project office, the USACE project manager, and the contracting officer (if applicable);

n. Plans submitted for Contractor operations shall also include the name of Contractor (and diving subcontractor if applicable), Contract number, and names and contact information for key personnel.

**NOTE: The dive plan will include the following statement: "If for any reason the dive plan is altered in mission, depth, personnel, or equipment, the DDC will be contacted in order to review and accept the alteration prior to actual operation.

C Activity Hazard Analysis; Site & Job Specific, each type of diving operation & location shall have a specific AHA document developed while working on site. USACE will provide AHA format to be used.

Contractor to provide submittal acknowledging intent to use USACE AHA Format; See EM 385-1-1 (30 Nov 14) Section 30.A.17; GA – DDC/ADC/DSR

D Emergency Management Plan; (Site & Job Specific, each type of diving operation shall have a specific Emergency Management Plan); See EM 385- 1-1 (30 Nov 14) Section 30.A.18; GA – DDC/ADC/DSR

• EM 385-1-1 30.A.18 Emergency management plan. An emergency management plan will be prepared for each dive operation.

The minimum content of the plan will be as follows:

a. Location and phone number of nearest operational recompression chamber if not located at the dive site and the Divers Alert Network (DAN) phone number (919-684-9111);

b. Location, directions to and phone number(s) of nearest hospital(s) or available physicians capable of treating dive injuries;

c. Location and phone number of nearest USCG Rescue Coordination Center, where appropriate;

d. Description of an emergency victim transport plan including phone numbers of appropriate emergency transport services;

e. Procedures and phone numbers or other means of communications to activate emergency services at the facility where the work is being performed;

f. Diver rescue procedures conducted by the dive team, including responsibilities of team members, best location(s) where injured divers may be removed from the water, and best location(s) for performing first aid/stabilization prior to emergency medical assistance arrival.

SD-07 Qualifications & Certificates

• EM 385-1-1 30.A.05 Training documentation shall be in compliance with the OSHA Diving Standards 29 CFR 1910.410 and shall show that dive team members, including dive tenders, have successfully completed training to the appropriate level (e.g., SSA diver’s certificate, surface supplied mixed-gas diver certificate).

Copies of all dive-related training certificates are required.

Such training shall be provided by:

a. A commercial diving school, military school, Federal school

(e.g., USACE), or an Association of Commercial Diving Educators (ACDE) accredited school;

b. An in-house training program that meets the requirements contained in ANSI/ACDE- 01, or in the Association of Diving Contractors International (ADCI) Consensus Standards;

c. Training for Scientific Divers using compressed air (SCUBA or SSA), shall be in compliance with 29 CFR 1910.410 and shall meet the above requirements or the training guidelines in the Standards for Scientific Diving published by the American Academy of Underwater Scientists (AAUS).

2. Proof of Certification; See EM 385-1-1 30.A.06; GA – DDC/ADC/DSR

• EM385-1-1 30.A.06 Proof of certification (a diploma and/or official transcript) as a commercial working diver from an accredited commercial dive school and other dive-related training certificates (e.g. chamber operator, saturation diver, etc.) are required as proof of a dive team member’s certification and/or experience. An ADCI card or similar certification from an internationally recognized commercial diving organization may be substituted as proof of training for divers demonstrating more than five (5) years of diving experience within the six (6) years preceding beginning of dive operations.

3. Dive Log Evidence; See EM 385-1-1 30.A.07; GA – DDC/ADC/DSR

• EM 385-1-1 30.A.07 Contractors shall provide dive-log evidence that each dive team member has training and experience consistent with the performance requirements of the scope of work.

a. As a minimum, each diver, back-up diver, and dive team supervisor shall have at least 1 year of commercial experience in the applicable position.

b. Divers shall have completed at least 4 working dives with similar decompression techniques as in the dive plan, using the particular diving techniques and equipment. Divers shall demonstrate that at least 1 of the 4 qualification dives was performed in the last 9 months prior to the start of dive operations.

c. Dive tenders must have previous experience and training as a dive tender.

4. First Aid & CPR Certification; See EM 385-1-1 30.A.08; GA – DDC/ADC/DSR

• EM 385-1-1 30.A.08 Each dive team member shall have current certification in first aid and CPR from the American Red Cross (ARC), the American Heart Association (AHA), or from an organization whose training adheres to the standards of the International Liaison Committee on Resuscitation, or from a Licensed Physician (LP).

a. Additionally, each dive team member shall have current certification in the use of emergency oxygen systems, and, if provided on the dive site, the use of Automated External Defibrillators (AEDs).

b. All classes shall contain a hands-on component and cannot be taken online. Evidence of this will be a photocopy of the certificates. The certificate(s) shall state the date of issue and length of validity.

**Note: Training in the use of emergency oxygen systems shall be specific to underwater diving and shall meet the requirements of a nationally recognized training organization such as Divers Alert Network (DAN), Professional Association of Diving Instructors (PADI), National Association of Underwater Instructor s (NAUI), the YMCA or other recognized sources.

5. Diving Physical Approval; See EM 385-1-1 30.A.09; GA – DDC/ADC/DSR A. EM 385-1-1 30.A.09 Divers shall receive an annual diving physical according to ADCI or similar standards preferably by a hyperbaric physician (MD or DO) or other licensed physician knowledgeable in the physiology of diving.

a. A “Fit to Dive” statement that each diver has been medically examined within the previous 12 months and has been determined fit and approved to dive shall be signed and signature stamped by the examining physician.

b. The DDC will maintain a file of physician “Fit to Dive” statement for all USACE qualified divers.

c. Contractors shall submit physician’s “Fit to Dive” statement to the DDC in accordance with Section 30.A.13.

d. After any serious diving-related injury or illness such as an over-pressurization injury and/or decompression illness, divers shall be re-examined by a hyperbaric physician and be re-certified as “Fit to Dive”.

SD-11 Closeout Submittals

1. Closeout Report; Report submitted within 30 days of project completion overall description of completed project and work accomplished including copies of the diver’s logs. Photos detailing any problems found shall also be included. GA – PO

1.8 SPECIAL SAFETY REQUIREMENTS

In addition to the Safety and Health Requirements Manual, EM 385-1-1, the Contractor shall comply with the requirements listed below.

a. An oxygen resuscitation system capable of delivering 100-percent oxygen for a minimum of 30 minutes shall be at the dive location.

b. For each dive where decompression sickness is suspected, has occurred, or the symptoms are evident, the following additional information shall be recorded and attached to the dive plan to become part of the official contract file.

1. Description of decompression symptoms (including depth and time of onset).

2. Description and any results of treatment given.

c. The Contractor's diving supervisor shall be on-site at all times while diving is underway. He/she shall have the authority to make decisions for the Contractor during diving operations, and shall have responsibility for the diving operation and the dive crew members.

d. In addition to the pre-dive meeting above, just prior to commencement of the daily dive operations the Contractor shall conduct a safety meeting at the site. All of the Contractors employees involved in the dive operation shall attend that meeting without exception. At the meeting the Contractor's Potential Hazard Analysis will be reviewed and discussed by all personnel that will be involved with the dive operation. Additional input shall be encouraged from all of those present.

1.9 CLOSEOUT REPORT
