PERFORMANCE WORK STATEMENT (PWS)

REPLACE NEW ELEVATOR CPU

US ARMY CORPS OF ENGINEERS

FORT PECK PROJECT

PART 1

GENERAL INFORMATION

1. General: This is a services contract to provide, install, test a new elevator CPU compatible to the existing elevator system for the US Army Corps of Engineer (USACE) at the Fort Peck Project.

1.1 Description of Services/Introduction: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform elevator CPU, door operator, replacement and inspection for USACE as defined in this Performance Work Statement except for those items specified as government furnished property and services. The Contractor shall perform to the standards in this contract.

1.2 Background: The existing elevator system was installed and manufactured by Otis Elevator Company in 1947. The elevator controls were upgraded in 1999 by KONE elevator services. The elevator has a 2500 pound capacity. The existing specifications are listed below:

1.3 Objectives: Contractor shall provide for the safe, careful and efficient operations of all equipment and protection of Government funds and property under the direction of the COR. The Contractor shall maintain a clean and orderly work area.

1.4 Scope: The Contractor shall provide, install and test a new nonproprietary elevator CPU on the PP1 Passenger Elevator. In addition, the Contractor shall perform annual inspection and full load test (FLT) on the elevator. Safety upgrades shall meet the current s t a t e a n d federal requirements for a passenger elevator such as A17.1/CSA B44 - 2013 Safety Code for Elevators and Escalators.

1.5 Period of Performance: The period of performance shall be for one (1) year from date of award.

1.6 Quality Control: The Contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract.

1.7. Quality Assurance: The government shall evaluate the Contractor’s performance under this contract, IAW Part 7 of this Performance Work Statement. This plan is primarily focused on what the Government must do to ensure that the Contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

1.8 Recognized Holidays: Contractors will not be permitted to work on the Federal Holidays specified in the instructions and notices section.

1.9 Hours of Operation: The Contractor shall be required to complete all inspections between 7:00 a.m. & 5:00 p.m., Monday through Thursday.

1.10 Place of Performance: The work to be performed under this contract shall be performed at the Fort Peck Power Plant. The power plant is located on Yellowstone Road, 2.5 miles East of Fort Peck, Montana. All work will be performed in:

1.11 Type of Contract: The government will award a Firm Fixed Price Contract.

1.12 Security Requirements: All Contractor personnel shall notify the Government upon entering and leaving the facility perimeter. All Contractor personnel shall sign the visitor registry and log entry and leaving times. The Government will issue all Contractor personnel a visitor badge which shall be worn at all times. A Government representative will be provided to escort Contractor personnel during the performance of work.

1.13 Physical Security: The Contractor shall be responsible for safeguarding all government equipment, information and property provided for Contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.

1.14 Lock Combinations: N/A

1.15 Special Qualifications: All employees shall possess required licenses/certificates/ insurance as required by federal, state, and local authorities. Elevator Contractors, Mechanics, and Inspectors must be qualified to inspect and test elevators as specified in the QEI-1 Standard for the Qualification of Elevator Inspectors. Inspectors shall use the Guide for Inspection of Elevators, Escalators, and Moving Walkways, ASME A17.2-2013, as a reference for the inspection of all elevators.

1.16 Post Award Conference/Periodic Progress Meetings: The Contractor shall meet with USACE personal on-site prior to beginning work to review contract requirements.

1.17 Government POC: The POC monitors all technical aspects of the contract and assists in contract administration. The POC is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance;

maintain written and oral communications with the Contractor concerning technical aspects of the contract; issue written interpretations of technical requirements, including Government drawings, designs, specifications; monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. The POC is not authorized to change any of the terms and conditions of the resulting order. The POC for this contract shall be: Mr.

1.18 Key Personnel: The following personnel are considered key personnel by the government: Contractor shall at a minimum furnish one (1) adult (18 years or older) to provide the elevator repair, inspection and testing services. The individual selected to provide the elevator inspection and testing services shall not be substituted for, unless prior approval by the POC is obtained.

1.19 Identification of Contractor Employees: All contract personnel attending meetings, answering Government telephones, and working in other situations where their Contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by the Contractor are suitably marked as Contractor products or that Contractor participation is appropriately disclosed. All Contractor personnel shall be required to wear identification badges which will include name of the person and the company who he/she is employed with.

PART 2

DEFINITIONS & ACRONYMS

2. DEFINITIONS AND ACRONYMS:

2.1. DEFINITIONS:

2.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

2.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and to make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

2.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the Contracting Officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.4. DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.

2.1.5. DELIVERABLE. Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.

2.1.6. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

2.1.7. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

2.1.8. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

2.1.9. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of Contractor performance.

2.1.10. QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.

2.1.11. SUBCONTRACTOR. One that enters into a contract with a prime Contractor. The Government does not have privity of contract with the subcontractor.

2.1.12 WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.

2.1.13 WORK WEEK. Monday through Friday, unless specified otherwise.

GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES

3. GOVERNMENT FURNISHED ITEMS AND SERVICES:

3.4 Equipment: N/A

3.5 Materials: The Government shall supply required test weights for performance of Category 5 full load tests.

PART 4

CONTRACTOR FURNISHED ITEMS AND SERVICES

4. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES:

4.1 General: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract that are not listed under Section 3 of this PWS.

4.2 Secret Facility Clearance: N/A

4.3 Materials: N/A

4.4 Equipment: The Contractor shall furnish all equipment. Equipment used by the Contractor shall be in compliance with OSHA, EPA and other DOD Directives.

4.5 Liability on Tests: The contractor shall not be liable for damage to Government equipment being tested when the damage results directly from and as a result of the tests specified in this specification and not due to neglect by the Contractor, his agents or employees. The Government will assume liability for the cost of labor and replacement parts and may choose to negotiate the cost of repair work with the Contractor.

PART 5

SPECIFIC TASKS

5.1 BASIC SERVICES: The Contractor shall provide, install and test a new nonproprietary elevator CPU on the PP1 Passenger Elevator. In addition, the Contractor shall perform annual inspection and full load test (FLT) on the elevator. Safety upgrades shall meet the current s t a t e a n d federal requirements for a passenger elevator such as A17.1/CSA B44 - 2013 Safety Code for Elevators and Escalators. All work must be completed in accordance with statement of work and attached work schedules. Storage or work areas will be assigned to the Contractor by the POC at the site. The Contractor shall notify the Contracting Officer two (2) weeks prior to commencing work. The Contractor shall guarantee that all requirements for the inspections and tests are completed and documented.

5.2 NEW EQUIPMENT: The new elevator CPU system shall support the existing 2,500 pound capacity.

The door operator shall be compatible with the existing elevator system. All equipment and work shall conform to all applicable codes and standards for all aspects of the work including but not limited to:

• American Society For Testing And Materials (ASTM)

• American Society of Mechanical Engineers (ASME) A17.1, A17.2.1, QEI-1

• American National Standards Institute (ANSI) Architectural Barriers Act (ABA)

• Code of Federal Regulations (CFR) America with Disabilities Act (ADA) Accessibility Guidelines for

Buildings and Facilities.

• Institute of Electrical & Electronics Engineers (IEEE)

• International Building Code (IBC)

• National Electrical Manufacturers Association (NEMA) National Fire Protection Association (NFPA)

101, 70, and 72

• Unified Facilities Criteria (UFC) 3-600-01

• Underwriters Laboratories (UL)

5.3 MANUALS: Submit three (3) copies of operation manual outlining the step by step procedures for system startup, operation and shutdown. Manuals shall include manufacturer's name, model number, service manual parts list and brief description of all equipment, including basic operating features. Submit three copies of maintenance manual listing routine maintenance procedures, possible breakdowns and repairs, and troubleshooting guides. Manuals shall include equipment layout and complete wiring and control diagrams of the system as installed. Operation and maintenance manuals shall be provided prior to training course.

5.4 OPERATOR TRAINING: The contractor shall conduct a formal training course for operating Government personnel which shall include care, lubrication, adjustment and maintenance of the elevator equipment. Training period shall consist of a total of eight (8) hours of normal working time and shall start after the system is functionally completed but prior to final acceptance tests. Field instructions shall cover all of the items contained in the operating and maintenance instructions, includ ing demonstrations of routine maintenance operations.

5.5 WORKMANSHIP: Only personnel who are competent, qualified, and have demonstrated experience in using accepted trade practices of the type of work for which they are to be employed on this Contract shall be used. All fieldwork shall be done to journeyman standards. The Government reserves the right to require written verification of personnel qualifications from the Contractor or his sub- contractors.

5.6 INSPECTION: The Contractor shall contact the POC to schedule a pre-final inspection of the project after all work has been completed, and the site has been cleaned. The POC wi l l meet with the Contractor on site to inspect the work. A Request for Final Payment shall not be submitted by the Contractor unti l after this inspection has been made, any punch list items have been corrected, and the project re-inspected.

5.7 ELEVATOR PERIODIC TESTS

5.7.1 Inspection and Testing: The work shall consist of but not be limited to the inspection and/or testing of the following equipment:

• Hoistway and car doors, hangers, guides and operating devices

• Hoisting machines, sheaves and brakes

• Motors, motor generator sets

• Hoisting, governor, compensating and operating cables

• Controllers (including contacts, switches, etc.)

• Signal systems, controls and traveling cables

• Governor, safeties, interlocks and contacts

• Guide rails and oiling devices

• Terminal, slow down and leveling devices

• Elevator car, counterweights and buffers

• Machine room, hoistway and pits

5.7.2 Periodic Inspection and Tests: The Contractor shall conduct Category 5 test requirements in accordance with the ASME A17.1 Elevator Code.

5.7.2.1 Full Load Safety Test (FLT): The FLT shall be conducted in accordance with the ANSI A17.1 Elevator Code, Section 1002. The FLT shall include checking the governor switch and tripping speed settings. After completion of the test and any subsequent repairs that may be required, the governor shall be sealed and metal tagged with the set tripping speeds. The Government will furnish the required load at the site.

5.7.3 Reporting Results of Inspections and Tests: As part of the required work, the inspector shall complete one original and one copy of Elevator Inspection Report, MRO Form 1669, 01 Jun 77, which will be furnished by the Government at the site. A sample copy is included as Attachment 1. Delete requirements pertaining to Item Line No. 9C & 9D on MRO Form 1669. A separate inspection report shall be prepared for each elevator. The report shall include under “Remarks” recommendations for correction of all defects found during the inspection. The recommendation shall show the item line number referred to, a brief description of the defect and the recommended corrective action. An estimated repair cost for each item and a total estimated cost for all repairs listed under “Remarks” shall be included with the original copy of the inspection report. Upon completion of the testing and inspection, the inspector shall give the copy of the inspection report to the Government Representative at the site. In addition, the Contractor shall complete a statement showing that the required inspection and tests were completed and obtain the signature of the Government Representative at the site on that statement. Statement forms will be furnished by the Government at the site. A sample copy is included as Attachment 2. The inspector shall also sign the inspection certificate posted in the car and indicate the date. Sample certificates for each elevator are included in Attachment 3. Within 30 days of completion of the inspection and tests, the Contractor shall forward the original copy of the inspection report and the signed copy of the statement of completion to:

5.8 WARRANTIES: All materials and workmanship shall be warranted by the Contractor for a period of not less than one (1) year from the date of final acceptance by the Government. For products and materials that are normally warranted by the manufacturer or supplier for more than one year, the Contractor shall provide the Government with a written warrant, or certification, indicating the manufacturers or supplier's terms and conditions of the warranty.

5.9. EMERGENCY CONDITIONS: Any condition found affecting the immediate safety of the passengers or elevator equipment shall be immediately reported to the Government Representative at the site. Within 24 hours a written report, clearly listing the defects found, the action recommended and a rough estimate of the corrective action proposed shall be submitted to the Government Representative at the site.

5.10 FINAL PAYMENT: Payment shall be made to the Contractor only after the required tests, inspections and repairs are complete and after the required reports, statements and surveys have been submitted as specified herein.

PART 6

APPLICABLE PUBLICATIONS

6.1 The Contractor must abide by all applicable state and federal regulations.

6.2 The Contractor shall follow all elevator inspection and testing requirements detailed in the Safety Code for Elevators and Escalators per ASME A17.1-2013.

6.3 The Contractor shall utilize the Guide for Inspection of Elevators, Escalators, and Moving Walkways, ASME A17.2-2013, for the recommended inspection and testing procedures required to conform to the Safety Code for Elevators and Escalators.

6.2 The Contractor shall follow all Safety Requirements detailed in the applicable Safety & Health Requirements Manual, EM 385-1-1 date 15 September 2014. The link is:
