ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT
YOUR EVALUATIONOF THE CONTRACTOR’S PERFORMANCE
RATING DEFINITION NOTE
(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.
(VG) Very Good Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.
The contractor’s proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
(N) Not Applicable
No information or did not apply to your contract

In general, herbicide services shall be applied at various locations throughout the government property boundaries surrounding the locations identified below (including, but not limited to flood control prime facilities/dam embankments and recreation/park facilities). The Contractor shall possess a valid State of Texas Commercial Applicator’s License, and insurance, and shall comply with the conditions specified in the sub-paragraph 3-3e and 3-4 in ER 1130-2-540. Inspections will be made by the Lake Manager, COR, as applicable, or QA Inspector after each application is completed. No payment will be made until the areas treated have been inspected and accepted by the inspector. Inspections may take up to two weeks after the completion of any service, so results can be seen. Any work that is rejected as unsatisfactory by the inspector (less than 90% kill) will be redone by the Contractor at the Contractor’s expense.
1.2.2 Application Treatment Specifications: Applications will be done at a time when, according to the label, satisfactory results can be expected. The type of herbicide will be determined by the Contractor; colored dye will be added to the herbicide to show coverage of application. The herbicide applications will be for herbaceous (annual and perennial grasses, broadleaf weeds), and woody vegetation. Woody vegetation may be girdled to achieve a successful kill. Two applications will be performed annually: spring and fall applications (dependent on lake elevations and park closures). Fall application must be completed before the contract term expires. Contract will begin with the spring application. The exact date of when an application will be performed will be determined by weather conditions. When conditions such as excessive moisture, potential rainfall, wind speed, or other factors prevail to such an extent that satisfactory results cannot be obtained, no applications should be made. Work should resume only when desired results can be obtained. If work cannot be commenced or completed by the agreed upon dates, the Contractor shall provide written notification to the Lake Manager, or COR, as applicable. Delays in service requiring an extension to the completion date due to inclement weather or conditions beyond the Contractor’s control may only be allowed with the written authorization of the Lake Manager, or COR, as applicable. The specifications are split into 2 major categories and relate directly to the legend on the maps.
1.2.2.1 Herbicide Park Roads and Road Shoulders – Benbrook only. This application treatment will be for an area a minimum of three-foot (3’) wide along each edge of the roadway, boat ramp, parking lot, pullout, or pull-through. Any vegetation that is growing up in between the edges shall also be treated. Maps are included to show these areas in further detail.
Treatable surface is approximately 27 miles total within 7 recreation areas.
1.2.2.2 Herbicide Prime Facilities (Benbrook and Joe Pool):
1.2.2.2.1 Stilling Basin Steps and Walkways - This application shall cover a minimum of three-foot (3’) wide strip along the steps and concrete walkways, and around guardrails. Treatable surface area is approximately 650 feet at Benbrook, and 1100 feet at Joe Pool.
1.2.2.2.2 Dam - This application treatment of herbicide will occur on all rip-rap associated with the dam (entire face upstream of both dams, and Joe Pool’s spillway and stilling basin); shoulders of the paved and gravel portions of the crest service road; and on the uncontrolled spillway to include u-channels, weep-holes, and at other areas related to dam operation. Maps are included to show these areas in further detail. This would include both broad application and spot treatment. Treatable surface is approximately 31 acres at Benbrook, and 65 acres at Joe Pool.
1.2.2.2.3 Project Office - This application treatment will be for an area a minimum of three-foot (3’) wide along the perimeter of the pavement around the main office parking lots and compound, and spot treatments for any vegetation growing through cracks in the pavement.
Treatable surface is approximately 2 acres at Benbrook and 0.5 acres at Joe Pool.

1.2.3.1 Safety Meetings - The contractor shall hold a safety meeting with all his employees prior to each application, and will notify the Lake Manager or COR as applicable, of location and time of each meeting.
When a new employee is hired, the contractor will thoroughly brief that person in all safety procedures and requirements. Any contractor employee who persists in unsafe performance will be removed from the job at the request of the Lake Manager, COR as applicable, or designated representative. A copy of the safety meeting report shall be submitted to the Lake Manager, or COR as applicable, with the weekly QC report.
1.2.3.2 Accidents – The contractor shall immediately notify the Lake Manager, or COR as applicable, of any accident occurring during the operation of the contract on government property.
1.3 Period of Performance: This contract is set up with a base year and two option years.
Base Year (01 August 2017 – 31 July 2018) or award date Option Year 1 (01 August 2018 – July 31 2019) Option Year 2 (01 August 2019 – July 31 2020)
Two applications will be performed per year in the late Spring and early Fall, generally in April and September. Applications will be done at a time when according to the label, satisfactory results can be expected. The exact date of when an application will be performed will be determined by weather conditions. When conditions such as excessive moisture, potential rainfall, wind speed, or other factors prevail to such an extent that satisfactory results cannot be obtained, no applications should be made. Work should resume only when desired results can be obtained.
