1ABCT Forklift Lease PWS

Preparing Organization: 1ABCT, S4
1. BACKGROUND: 1ABCT is required to provide all classes of supply to support battalion maintenance programs. Supplies are required to be redistributed throughout the warehouse requiring forklift support. Current tactical equipment capabilities do not meet the requirements due to width restrictions within the Supply Support Activity
(SSA).
2. SCOPE: This is a non-personnel services contract for leasing of three (3) 5K commercial forklifts. This shall also include the maintenance and repair for leased equipment stored within the 1ABCT footprint; the 1ABCT SSA Warehouse. The contractor shall provide (3) with side shift at the 1ABCT SSA for the following Period of Performance:

3. General Requirements: The contractor shall provide (3) 5K Forklifts that contain all necessary safety equipment and meet the minimum specifications, listed below:

Make/Model: Komatsu Capacity: 5,000 lbs at 24” load center (or greater) Mast: 2 Stage Wide View Mast Maximum Fork Height: Not more than 120” Attachment: Not less than 72” Forks: Side Shift Transmission: Automatic Tires: Pneumatic Drive: 2 wheel Steer: 2 wheel Turning Radius: At least 90 degrees Travel Speed Empty: Not less than 5 mph Forklift Weight Empty: At least 900lbs

4 SPECIFIED TASKS:
4.1 General: Provide preventative maintenance service and emergency response for
(3) 5K Forklifts in the 1ABCT SSA at Fort Stewart, GA. Service is for listed 5K Forklifts.
The contractor shall have a service technician within 1.5 hours drive of FSGA. During the lease period, the contractor shall conduct all maintenance and service requirements in accordance with the original equipment manufacturers (OEMs) guidance and the guidance outlined in paragraphs 4.2-4.6 and the PMCS Checklist. Servicing and maintenance shall take place on-site at the 1ABCT SSA Warehouse located at 1720A McFarland Avenue, Fort Stewart, GA 31314.
4.2 Emergency Service: The Contracting Officer Representative (COR) will contact the contractor in the event of required emergency service. The contractor shall have a technician available and on-site at the 1ABCT SSA within 24 hours of notification.
4.2.1 Upon review, once a thorough investigation has been conducted by the contractor and the COR; if the Government is determined to be responsible for the damages the contractor will contact the Contracting Officer (KO) immediately for the purpose of submitting a formal claim.
4.3 Preventative Checks: The contractor will perform Preventative Maintenance Checks and Services (PMCS) on the forklifts once a month on each forklift after coordination, initiated by the contractor NLT the 10th of each month. This PMCS schedule must be established and validated with the COR. The contractor will utilize the PMCS Checklist outlined in section 4.6. At a minimum, forklifts will be checked on the following areas:
a. Tires- Contractor will ensure tires are operationsal and do not pose a safety hazard
b. Forks- Contractor will ensure the forks and lifting components are operational and without fault
c. Controls- Contractor will ensure levers, actuators, etc. are functioning properly
d. Safety Devices- Contractors will ensure all safety devices/systems are fully operational
e. Steering Components- Contractor will ensure all steering linkage and components are fully operational
f. Braking Components- Contractor will ensure all braking systems are functional
g. Horn Operations- Contractor will ensure all sirens, horns, etc. are operational
h. Engine Operations- Contractor will ensure the engine has no faults and is completely operational
i. Fluid Levels- Contractor will ensure all fluids are filled to capacity
j. Electric Components- Contractor will ensure all system electrical components are fully functional
4.4 Pickup and Delivery: The contractor shall deliver (3) Fully Mission Capable (FMC) forklifts won the first day of the Period of Performance (POP). Upon delivery, the COR will conduct a joint inventory with the contractor in order to determine if the forklifts delivered are the correct quantity and are FMC without damage or faults. If damage, faults, or any other disqualifying deficiencies are found during delivery, the entire forklift delivery will be denied and the contractor will incur all applicable charges for shipment and redelivery of acceptable forklifts. The contractor will have 24 hours to deliver replacement forklifts. The contractor will pick up the forklifts at the end of the lease period from the 1ABCT SSA Warehouse within 24 hours of the lease expiration date;
the COR must be notified in advance.
4.5 Fuel: Seventeen (17) full propane tanks included with this contract will be delivered to the 1ABCT SSA Warehouse during the duration of this contract: time, dates, locations and amount of tanks will be determined and requisitioned by the COR. The forklifts will be delivered with (17) full propane tanks (breakdown: 1 full propane tank installed on each forklift (3) and 14 full propane tanks on hand – for a total of 17 tanks delivered on the first day of the POP. COR will coordinate with the contractor once 7 of the propane tanks are empty in order to replenish the reserve and avoid having no fuel to operate the equipment. Payment for any additional propane tanks will be funded through this contract.
4.6 COR Information: POC for delivery of all equipment is the Contracting Officer Representative; 2LT Jonathan Berry at 484-797-3363 at jonathan.m.berry14.mil@mail.mil or until another Contracting Officer Representative (COR) is appointed in writing. The Vendor will be notified by e-mail who the COR is, contact information, and their responsibilities.
4.7 Preventative Maintenance Service and Checks: The contractor will utilize the checklist below and submit a signed and dated copy to the COR immediately following the service on the agreed date of inspection with the COR. The contractor will explain any and all deficiencies to the COR on the day of the monthly inspection. If any forklifts are found non-mission capable, the contractor will provide an equal replacement within 24 hours to the 1ABCT SSA Warehouse.
