Section C - Descriptions and Specifications

PERFORMANCE WORK STATEMENT

PERFORMANCE WORK STATEMENT (PWS)

Elevator Service

Part 1

General Information Part C - 1: General

C - 1.01 PURPOSE

GENERAL: The purpose of this request is for the contractor to provide all labor, material, and equipment to provide full maintenance of ten elevators in Building 4500 and 1 elevator in building 4555, for Moncrief Army Community Hospital (MACH), Fort Jackson, South Carolina.

C-1.02 NOT USED

C-1.03 SCOPE

The elevator maintenance Statement of Work (SOW) establishes the minimum requirements for a full maintenance program which shall be contracted by government with an approved contractor. The full maintenance contract shall cover the elevator as listed in Part 5 ofthis SOW. Contractor shall be required to provide full maintenance during the hours of 7:30 AM to 4:00 PM Monday through Friday. In addition, the contractor shall provide afterhour's emergency services Monday through Sunday.

C-1.04 CONTRACTOR COMPETENCE AND QUALIFICATIONS:

The contractor shall be competent and experienced in the field of elevator service with a minimum of five (5) years prior experience on comparable or more complex elevator equipment and currently have service contracts on similar equipment. The contractor shall be on-site at the hospital with all tools necessary for repairs to answer a service call within one (1) hour notification by online services calls or telephonic service calls.

C-1.05 NOT USED

C-1.06 INSPECTION OF EQUIPMENT AND CONDITIONS AT JOB SITE:

Prior to submitting a proposal, it shall be the responsibility of the offerer to visit the job site and inspect each elevator to establish to their satisfaction the condition of the equipment to be maintained and any other conditions affecting the work to be performed.

This equipment is offered in an "as is" condition. No claims shall be allowed for correction of deficiencies claimed to exist prior to award of contract.

C-1.07 WIRING DIAGRAMS:

The contractor shall provide to MACH a set of reproducible wiring diagrams covering all changes, modifications, etc., which take place during the contract term. The diagrams are to be furnished to MACH immediately following modifications at the sole expense of the contractor.

C-1.01 PERFORMANCE CONFERENCES AND REPORTS:

At the end of each ninety (90) day period, the contractor shall meet with the Contracting Officers Representative (COR) to review performance and schedule major repairs contemplated during the next ninety (90) day period. The contractor shall submit to the COR written performance reports at the end of each six (6) month period if requested.

The contractor shall submit, when applicable, any written recommendations related to safety issues, improvements, modernization, etc.

The contractor's supervisory personnel shall be available at all times to report and confer with the COR. The contractor shall submit to the COR such records and reports as may be identified in the SOW or such other records and reports as MACH may request. The COR shall specify the method of report and record delivery such as hard copy or e-mail format.

On all elevator shutdowns involving passenger entrapments, a comprehensive report shall be prepared and submitted to the COR within 24 hours of the entrapment. The report shall include building location, elevator identification, date and time of entrapment, time entrapped passenger (s) were released, cause of entrapment and corrective action taken by the contractor.

C-1.09 CHECKING IN AND OUT

The contractor's personnel shall check in with the COR or MACH's Representative and sign in the log book located in Room 5-8C, 5th Floor of Building 4500, prior to commencing work and sign out after completing work. This requirement applies to regular maintenance, repair and callbacks. At time of check-in, at the COR or MACH's representative shall provide the contractor's personnel with a list of any reported problems requiring the contractor's attention.

C-1.10 TIME SHEETS:

A time sheet or ticket shall be submitted to MACH or approval within twenty-four (24) hours of servicing or repairing an elevator unit if requested. The time sheet or ticket shall include the date the work was performed, the time of day the work started and finished and the building name and elevator number.

C-1.11 CONTINUITY OF SERVICE:

The contractor will correct any failure which removes any equipment from service within three (3) regular workdays from the day of the failure. If the equipment is not back in service by such time, the Government shall receive a credit of $130.00 per day per elevator to the monthly maintenance payment for the equipment, out of service beyond such time.

C-1.12 ANALYSIS AND ADJUSTMENTS:

On each group of elevators consisting of three (3) or more cars a computerized traffic analysis shall be performed as soon as possible after commencement of contract.

Readjustment of the group elevator systems shall be performed within six (6) months of commencement of the contract. After readjustments have been completed, computerized traffic analysis shall be performed for comparison of before and after conditions.

Computerized traffic analysis shall be performed annually to ensure proper operation.

All computerized traffic analysis shall be for a consecutive five (5) day (Monday through Friday) 24-hour period.

C-1.13 SAFETY DATA SHEETS:

The contractor shall furnish to MACH an approved Safety Data Sheet for all cleaning solvents, lubricants, oils, greases, paints, etc., used during their performance of the elevator preventive maintenance. A copy of the applicable Safety Data Sheets shall be visibility mounted in each elevator equipment room that contains these items in a protective cover.

C-1.14 EMPLOYEE SAFETY:

The contractor shall maintain employee safety as one of its important concerns. The contractor shall provide employees with information and supplies necessary to comply with MACH Safety Standards, ASHMS, and Occupational Safety and Health Act (OSHA) and procedures stated in the Elevator Industry Field Employee's Safety Handbook. The contractor shall conduct regular and systematic safety meetings with employees to discuss new procedures and improWhave on safety awareness.

Part C-2: Scope of Elevator Maintenance C-2.01 This SOW provides for complete maintenance coverage including examinations, cleaning, painting, lubrication, adjusting, parts replacements, repairs and testing on all parts of the elevator equipment including, but not limited to, machines, worms, gears, thrust bearings, drive sheaves, sheave bearings, brake pulleys, brakes, brake coils, linings, rope grippers, motors, SCR, VVVFD, hydraulic power units, hydraulic pumps and valves, silencers, mufflers, selectors, relays, contactors, solid state devices, transformers, resistors and all related control equipment.

It also includes safety governors, governor sheaves, car safeties, hydraulic plunger, bolster plate, jack packing, deflector and secondary sheaves, bearings, car and counterweight buffers, car and counterweight guide rails, limit switches, guide shoes (slide and roller), door operators, car and hatch door hangers, contacts, interlocks, auxiliary door closing devices, safety edges, photo eyes, emergency light systems including batteries, car fans, car lighting fluorescent lubes, light diffusers, lay-in ceiling panels, handrails, car frames, platforms and all other elevator related devices.

This SOW does not include car enclosures (including removal panels, door panels, car gates, plenum chambers, mirrors, bulletin boards, carpets and tile flooring), hoist-way enclosures, doors, frames, and sills, underground hydraulic jack cylinders including underground piping and fittings, muzak, two-way communications equipment, fluorescent ballasts, and smoke sensors. These items are the responsibility of the Government.

C-2.02 SCHEDULED MAINTENANCE:

All preventive maintenance performed by the contractor shall be scheduled elevator by elevator prior to commencement of the contract and subject to final approval of the COR.

Minimum preventive maintenance frequency visits shall be weekly for gearless equipment, semi-monthly for geared equipment and monthly for hydraulic equipment.

The contractor shall spend a minimum of 4 hours a week doing on-site preventative maintenance.

The preventive maintenance schedule, as prepared by the contractor, shall show building name, elevator serial numbers, examination frequency, examination hours and be keyed to a preventive maintenance schedule prepared for the specific equipment covered by this sow.

Maintenance schedules shall be permanently located in the equipment rooms for each elevator. The schedules shall be accessible for MACH's monitoring. Schedules shall be maintained by indicating the work performed, signature of the mechanic performing the work was performed.

Removal of elevators from service shall be coordinated with and approved by MACH. To the extent possible all preventive maintenance which requires removal of elevator from service shall be scheduled during off peak hours of building operation. No elevator shall be taken out of service during normal business day without notification to MACH except under emergency conditions. The contractor shall not remove from service more than one elevator at a time in any bank of elevators (except in emergencies). Emergency circumstances are understood to be those that pose imminent possibility of equipment damage or passenger injury as judged by the contractor's employees. Elevators taken out of service shall be identified by placing signage at every floor-indicating elevator is out of service. Signs shall be removed after service is restored.

C-2.03 EXAMINE:

The contractor shall examine the equipment at regular intervals sufficient to preserve life of the equipment.

When, as a result of examination, corrective action is determined to be the responsibility of the contractor, the contractor shall proceed immediately to make (or cause to be made) replacements, repairs and corrections.

When such work is determined not to be the contractor's responsibility, a written report signed by the contractor, shall be delivered to the COR. If the COR disagrees with the contractor's determination, the COR and the contractor shall use their best efforts to resolve the disagreement in a manner mutually agreeable to the parties. If an agreement is not reached, the COR and the contractor shall meet with the Contracting Officer (KO) to resolve the issue.

Items of an emergency nature shall be communicated by the contractor to the COR immediately and followed up in written form if requested.

Examinations of the equipment shall follow the basic procedures recognized by the elevator service industry. (Paragraph C-2.02 states minimum visits)

C-2.04 CLEAN:

The contractor shall clean all of the elevator equipment as well as the elevator equipment room, hoist-ways and pits.

Cleaning of the equipment shall occur at regular intervals sufficient to maintain a professional appearance and preserve the life of the equipment. Minimum cleaning intervals shall be as set forth in Part C-6 of this SOW.

The contractor shall report to the COR the need for cleaning and/or janitorial services for any items not covered by this contract or which are otherwise not the responsibility of the contractor.

C-2.05 PAINT:

The contractor shall paint the elevator equipment at intervals to maintain a professional appearance, preventrusting and preserve the equipment. All paint shall be suitable for the purpose intended and be of a highquality. Paint shall not emit hazardous or offensive odors. Application of the paint shall, in all circumstances, comply with applicable local codes and/or current ANSI Elevator and Escalator Codes.

C-2.06 LUBRICATE:

The contractor shall lubricate all moving part of the equipment. Lubricants shall be applied at intervals recommended by the equipment manufacturer or as dictated through use of the equipment. All lubricants shall be suitable for the purpose intended and shall meet or exceed the minimum requirements specified by the manufacturer of the equipment to which the lubricant is applied. All lubricants shall be stored in approved containers and properly labeled.

C-2.07 ADJUST:

The contractor shall make the necessary equipment adjustments when the operation of the equipment varies from its normal designed performance standards. See Part C-7 for performance standards.

The contractor's employees shall make adjustments with appropriate tools and instruments. Adjustments shall be made at regular intervals to maintain the elevator in optimum operating condition.

Parts or assemblies which are worn or otherwise deteriorated beyond normal adjustment limits shall be replaced.

C-2.08 REPLACE:

The contractor shall replace all items covered under this contract during the course of scheduled preventive maintenance. In the opinion of the contractor and/or COR, replacement shall be made to prevent an unscheduled elevator shutdown and to ensure the continued normal operations of the elevator.

Replacements shall be made to extend the useful life of the elevator.

C-2.09 REPLACEMENT PARTS:

The contractor shall be required to furnish metal cabinets with a supply of spare parts sufficient for normal maintenance and repair of the elevators. The quantity of the replacement parts maintained on the job shall be adequate to perform an effective preventive maintenance program.

Motors, armatures, field coils, and any other major compartment shall be delivered to the job site within two (2) days of known requirement.

Any used parts or parts that are not equal to or better than genuine manufacturer's parts are not acceptable and shall not be used by the contractor on MACH's elevator equipment.

C-2.IO REPAIRS:

Repairs which are the responsibility of the contractor:

The contractor shall make (or cause to be made) all repairs which become necessary due to normal wear and use of the elevator system. The contractor shall be solely responsible for all cost for labor, materials, expenses and supplies which occur as a result of the stated repair.

Repairs which are the responsibility of the Government:

Prior to any repairs being made by the contractor, the contractor shall submit a written proposal to the KO to obtain formal approval to proceed. The Government reserves the right to obtain comparison proposals from other contractors.

Repairs shall be made by the contractor, when authorized by the KO, where such repairs are not included in the contract.

The contract shall supply all labor, materials and supplies as outlined in the proposal. On completion of all repair work, the contractor shall submit to the Government for payment an invoice detailing the nature of the work performed and related charges.

C-2.11 PERFORMANCE OF PERDIODIC TEST (S):

The contractor shall perform periodic Safety Test(s) of the elevator compartments. The periodic test(s) shall be conducted in accordance with standards and procedures stated in ANSI A 17. Elevator and Escalator Code. Test results shall be recorded on forms supplied by the contractor and acceptable to the COR. Upon completion of test(s), copies shall be submitted to the COR signed by the elevator mechanic who performed the test(s) and authorized representative of the contractor. Inspections performed by others are not included in this SOW.

C-2.12 CALLBACK SERVICE:

For the purpose of this SOW, a callback is a request from MACH to the contractor requesting the contractor to go to a specific elevator to correct any elevator problem and/or condition which needs attention before the contractor's next scheduled preventive maintenance visit. Additionally, a callback is work that is performed by one person working alone, at the job site, for a period of less than two (2) hours.

Work in excess of two (2) hours or requirement of a second person shall be considered repair work and shall be governed by Part C-2, Paragraph C-2.10 of this SOW.

The contractor shall provide 24-hour callback service to MACH at no additional cost. The contractor shall respond to any callback, i.e., shall be on-site within a minimum of one ( 1) hour from the time in which the callback was reported to the contractor.

Part C-3: Contract Terms

C-3.01 FAILURE TO PERFORM:

The contractor shall fully guarantee all work performed during the term of the Contract and for a period of ninety

(90) days after the termination date. Should the Government determine during the Term or within thirty (30) days after termination that any required work has not been fully performed, has been performed improperly or not performed at all, the contractor shall, after written notification by the Government, correct said deficiency within ten (I 0) days. Failure to correct will be construed as a default under the contract and the Government has the right to secure others to perform the services and deduct the costs of these services from the contractual amount due to the contractor.

The Government reserves the right to engage in independent party to perform in inspection to determine responsibility pursuant to this Paragraph C-3 .01 .

C-3.02 EMPLOYEES OF CONTRACTOR TO BE SATISFACTORY:

The contractor agrees that all work shall be performed by and under the supervision of a sufficient number of skilled, experienced elevator service mechanics and repairmen directly employed by contractor, who are permanently assigned to support the execution of this contract for maintenance and repair work and who shall be qualified to keep the elevators properly adjusted and/or repaired (with the exception of helpers who need not be so qualified). All personnel shall be employees of the contractor and shall not be subcontractors, franchises, contract labor, etc. the contractor and its employees shall use all reasonable care to maintain the elevator equipment in proper and safe operating condition. All employees performing work under this contract shall be satisfactory dressed in clean uniforms (identifying them as elevator service personnel) with acceptable demeanor and possessing full technical qualifications in the Government.

Contractor employees will be denied access to the property if determined to be under the influence of a controlled substance and/or alcohol. Employees shall not have in their possession firearms, explosives, controlled substances or other potentially harmful, dangerous or illegal items on the Government's properties. Any employees found to be unacceptable to the Government shall be replaced after being given reasonable notice to the contractor. All employees are subject to security screening upon entering and/or exiting the Government's property. MACH is a smoke campus. Smoking is not allowed on this campus at all.

C-3.03 OWNER'S RIGHT TO INSPECT AND REQUIRE WORK:

The Government reserves the right to make such inspections and tests as are necessary to ascertain that the requirements of this contract are being fulfilled. The Government's right to make inspections or tests may be exercised by its elevator consultant, as it may designate, who will, if the COR so advises, have the same authority to inspect and test as the Government, as provided here-in. The contractor agrees to furnish personnel and tools necessary to conduct such tests. Deficiencies reported shall be promptly corrected at the Contractor's expense. If the contractor fails to perform the work required by the terms of the contract in a diligent and satisfactory manner, the Government may, in addition to any other remedies the Government may have, after ten (I 0) days written notice to the Contractor, perform or cause to be performed all or any part of the work required here-in. Contractor agrees that it shall reimburse the KO for any expenses incurred by the Government in exercising its right under this paragraph

3.03 and the Contractor agrees that the Government, in the Government's sole and absolute discretion, may deduct the amount of such expenses from any sum owing the Contractor.

C-3.04 CONTRACTOR TO COMPLY WITH CODES:

All repair, parts replacement or adjustments called for here-in shall be performed in full compliance with ANSI A 17. I safety Code for Elevators and Escalators, latest edition, and ANSI Al 7.2 Inspections Manual, latest edition, including amendments thereto.

C-3.05 OWNERSHIP OF EQUIPMENT:

The Contractor shall not at any time assume possession or control of any part of the equipment, but such shall remain the Government's exclusively.

C-3.06 SINGULAR/PLURAL:

Whenever required by the context, the use of singular form shall be deemed to include the plural, and the use of the plural form shall be deemed to include the singular.

Part C-4: Contract Exclusions

C-4.01 THE CONTRACTOR SHALL NOT BE RESPONSIBLE FOR THE FOLL WING:

A. Repairs because of negligence, accident or misuse of the equipment by anyone other than the contractor, his employees, subcontractors, servants, or agents, or other causes beyond the contractor's control except ordinary wear and tear.

B. Making other safety test or installing new attachments on the elevators when recommend by insurance companies or governmental authorities.

C. Making any alterations to the elevator equipment, including control circuits, without prior written approval from the Government.

D. For maintaining the mail line power feeders and associated dis-connect switches and breakers.

E. For maintaining elevator components specifically excluded under Part C-2, paragraph C-2.01.

Part C-6: Minimum Preventive Maintenance Frequency and Task

The following preventive maintenance frequency and task are not inclusive of all required maintenance points for the variety of equipment available throughout the elevator industry. These maintenance frequency and task lists are designed to be examples of the preventive maintenance quality level required by the SOW. The Contractor is required to submit specific preventive maintenance frequency and task lists for each type of equipment prior to commencement of contract for Government approval in paragraph C-

2.02 - Scheduled Maintenance.

C-6.01 GEARED PASSENGER AND SERVICE-VARIABLE VOLTAGE CONTROL:

Each Visit:

l. Ride each elevator and observe performance, leveling, floor stops, door opening and closing operation and noise.

2. Test safety edges, photo eyes, detectors, door open buttons, and alarm be ll.

3. Check for proper car and hall button operation and all indicator illuminations and lantern operations.

Monthly:

4. Clean and inspect machine, controller, sector, motor, VVVFD/SCR and governor.

5. Clean and inspect car top, operating switches, door operator and controls, car door hangers, gibs, photo eyes and safety edge. Lubricate and adjust door operator and door accessory equipment.

6. Clean and inspect hoist-way door hangers, interlocks, linkage, pickup assembly, door gibs, nonvision wing and hoist-way switches.

7. Clean and inspect governor tension sheave, car and counterweight buffers, compensating sheave assembly.

8. Check hoist motor and motor generator commutators and brushes. Replace or reseat as required.

9. Check for leaks and oil level in machine.

Quarterly:

l 0. Inspect motor coupling, end thrust, and backlash, motor generator bearings, vacuum and clean motor.

11. Check and adjust brake and rope gripper. Inspect and lubricate pivot pins.

12. Clean and adjust controller and sector components including contracts, relays and timers. Check transformers and rectifiers. Vacuum and selector parts.

13. Check out complete safety circuit.

14. Check selector cables and/or tapes. Lubricate selector drive worm. Inspect selector drive.

15. Clean, inspect and lubricate governor linkage.

16. Inspect, rotate and equalize hoist cables. Inspect cable shackles and fastenings.

17. Check adjustments of roller/slide car and counterweight guides. Check bearings/liners and fastening.

18. Inspect TM, slowdown, leveling and/or limit switches.

19. Clean and inspect all car and hoist-way door contacts and interlocks.

20. Check door closing force. Check car and hoist-way hanger rollers and adjust up thrust.

21. Inspect door operator bearings and cams.

22. Clean and inspect governor tail sheave, compensating sheaves, compensation ropes and hitches and/or compensating chains, guides and hitches.

23. Clean and inspect car and counterweight buffers. Check buffer oil level and operation.

Semi-Annual:

24. Check control and main line fuses, voltage readings, motor and VVVFD/SCR wire connections, overloads, armature clearance and brake cores.

25. Check motor overload devices, resistor and resistance connections.

26. Check car safety mechanism and governor rope hitch.

Annual:

27. Drop brake shoes, clean, lubricate and adjust. Flush and replace worm gear oil.

28. Check all controller and selector terminals. Check and clean all fuse holders.

29. Check car frame, overhead, car and counterweight sheaves, sills and pit.

30. Annual lubrication of motor, motor generator and machine bearings, deflector, compound and compensating sheaves and governor tension sheave bearings. Check all fastenings.

31. Annual car safety test. Clean, inspect and lubricate governor and safety mechanisms. Check buffer and rope gripper oil level.

32. Adjust motor control and perform logic systems operation check.

33. Clean hoist-way equipment including guide rails, counterweights, hoist-way door hangers, interlocks, closers, headers and related devices. Check all fastenings on guide rails, brackets and entrances. Check traveling cables.

Five Year:

34. Perform full load governor, safety and buffer test.

C-6.02 HYDRAULIC PASSENGER AND SERVICE:

Each Visit:

1. Ride each elevator and observe performance, leveling, floor stops, door opening and closing operation and noise.

2. Test safety edges, photo eyes, detectors, door open buttons, and alarm bell.

3. Check for proper car and hall button operation and all indicator illumination and lantern operations.

4. Check for leaks in power unit, hydraulic control valve, silencer, sound couplings and oil line.

Monthly:

5. Clean and inspect power unit, controller and motor. Check drive belt tension and condition.

6. Check for leaks and oil level in power unit. Clean power unit oil drip pan.

7. Clean and inspect car top, operating switches, door operator and controls, car door hangers. gibs, photo eye and safety edge. Lubricate and adjust door operator and door accessory equipment.

8. Clean and inspect hoist-way door hangers, interlocks, linkage, pick up assembly, door gibs, nonvision wing and hoist-way switches.

9. Clean and inspect hydraulic plunger, seals and packing. Check jack oil drip container.

I 0. Check guide rail lubrication drip containers in pit.

Quarterly:

11. Check motor and pump sheave alignment. Check motor and pump mounting fastenings.

12. Adjust hydraulic control valve. Clean hydraulic control valve strainers. Check hydraulic control valve locking nuts, adjustment screws and mounting fastenings.

13. Clean and adjust controller components including contacts, relays and timers. Check transformers and rectifiers. Vacuum or brush all controller parts.

14. Check out complete safety circuit.

15. Check adjustment of roller/slide car guides. Check bearings/liners and fastenings.

16. Inspect TM, slowdown, leveling and/or limit switches.

17. Clean and inspect all car and hoist-way door contacts and interlocks.

18. Check door closing force. Check car and hoist-way hanger rollers and adjust up thrusts.

19. Inspect door operator bearings and cams.

Semi-Annual:

20. Check control and main line fuses, voltage readings and motor wire connections.

21. Check motor overload devices, resistor/resistance connections and starters, contractors and contacts.

Annual:

22. Check all controller terminals. Check and clean all fuse holders.

23. Check car frame, sills, and pit.

24. Annual hydraulic pressure test. Check relief valve seal and tag valve with current test information, date test was performed and performing company name.

25. Perform logic system operation check.

26. Clean hoist-way and hoist-way equipment including guide rails, hoist-way door hangers, interlocks, closers, headers and related devices. Check all fastenings on guide rails, brackets and entrances. Check traveling cables.

Part C-7 Performance Standards

C-7.01 It is the intent of this SOW that the elevator equipment be maintained so as to reserve the operating characteristics in accordance with the original manufacturer's design.

C-7.02 The following are performance levels which is a part of the original design and which shall be maintained at all times.

l. Contract speed of all elevators, dumbwaiters and lifts. Variance from rated speed, regardless of load, shall not exceed 5% of the elevator contract speed.

2. Flight and Cycle performance times.

3. Accuracy of leveling shall be measured under all load conditions. The accuracy of leveling of Yz of an inch shall be used in this contract.

4. Opening and closing times of all hoist-way and car doors within limits of ANSI A 17. l Elevator and Escalator Code, latest edition.

5. Door reversal devices, detectors and/or all passenger sensor devices.

6. Variable car and hall door hold open times.

7. Individual car and group operations and devices.

8. Emergency fire service, emergency power, communications, emergency car light and alarm bell.

C-7.03 The following equipment performance criteria shall apply:

1. Flight Time is the elapsed time in seconds from brake open to brake close on one (I) floor run of approximately 12 'O" and/or elevator starting after the interlock circuit is made and elevator stops at the next landing.

2. Cycle Time is the elapsed time in seconds from car doors starting to close until the doors are three quarters open at the next landing on a one ( 1) floor run of approximately 12'0".

3. Door Open Time is the elapsed time in seconds from the time the car doors start to open after arriving at a landing until they are fully open.

4. Door Close Time is the elapsed time in seconds from the time the car doors starts to close after a call is initiated until they are fully closed with the interlock engaged. This must not exceed requirements as set forth in ANSI A 17. I, Rule 112.3 Elevator and Escalator Code, latest edition.

Closing Time will vary slightly depending upon door opening width and weight of the doors.

5. Door closing force must not exceed the requirement of30 pounds as set forth in ANSI Al 7.1 Elevator and Escalator Code, latest edition. This is by stalling and closing car door in high speed and applying a spring pressure gauge against the leading against the leading edge of the door and reading the amount of pressure the door exerts to continue closing.

C-7.04 As a minimum requirement the performance levels set forth in Paragraph C-7.02 shall be as shown in table C-7.04 - Operation and Maintenance Standards Performance table.

Part C-8: Reports and Inspection

C-8. 1 All aforementioned and other required reports shall be submitted for approval to the COR.

C-8.2 Not Used

C-8.3 The Contractor shall immediately report any hazardous condition discovered, whether within the scope of this project or not.

Part C-9: Site Secur ity and Safety

C-9.1 Site security and safety shall be in accordance with this SOW, the primary contact and/or in accordance with the KO's instruction. The contractor shall be responsible for all activity, security and safety inside the project area(s). Adherence to Hazardous Communications (SDS), Hot Work, Lockout tag-Out, Life Safety, OSHA, ASHMS, NFPA, & Joint Commission protocols is required and shall be enforced.

C-9.2 Hot Work permits are acquired by calling the Fort Jackson Fire Department at extension (803) 7 51-4142 prior to the start of the hot work.

C-9.3 The Contactor shall provide a site-specific safety plan and signed copy of our pre-contract safety agreement at the pre-contract meeting. A copy of the pre-contract safety agreement is available upon request. The contractor will be required to attend a pre-performance contract meeting and safety training approximately 2 weeks prior to the start of the contract period.

C-9.4 Contractors will be required to wear visitor badges and sign in and out while working in the building.

C-9.5 Any work that will create dust or in any way contaminate the air circulated within the hospital shall be performed in such a way as to comply with all rules and regulations concerning Infection Control.

C-9.6 The Contractor shall acquire a smoke/fire wall permit prior to any work in or through an existing smoke/fire wall in the facility and a ceiling tile permit for any ceiling tile that will be disturbed.

C-9.7 All fire wall and smoke wall penetrations shall be sealed per UL specifications using the Hilti System and provide proof of current training from a Qualified Hilti field representative. This training is available free of charge by the Hilti Co., phone number 1-800-897-8000.

C-9.8 Please be advised that due to present fluctuating security levels, delays beyond our control may be encountered. The Government shall not be billed for cost increases due to raised security levels.

C-9.9 All access and demolished materials become the property of the Contractor and must be removed from the Fort Jackson grounds.

1.5 Period of Performance: Base year plus four one-year option periods.

1.6 General Information

1.6.1 Quality Control: Not applicable.

1.6.2 Quality Assurance: The government shall evaluate the contractor's performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

1.6.3 Recognized Holidays: The Contractor will not be required to perform services on Federal Holidays.

1.6.5 Place of Performance: The work to be performed under this contract will be performed at Moncrief Army Community Hospital, Fort Jackson, SC 29207.

1.6.6 Type of Contract: The Government will award a firm fixed price contract.

1.6.7 Security Requirements: See Section 5.16.2.

1.6.7.2 Key Control: See Section 5.16.6.

1.6.7.3 Lock Combinations: Not Applicable.

1.6.8 Special Qualifications: Not Applicable.

1.6.9 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5.

The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced.

Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.6.10 Contracting Officer Representative (COR): The (COR) will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

1.6.11 Key Personnel: The Contractor shall provide at least two points of contact within the company to interface with the COR on issues concerning generator maintenance service. Names, business address, phone numbers and hours of operation are the minimum information requirement. The Contractor shall also provide an afterhours name and telephone for emergency contact. Contractor shall provide a point of contract for 24 hours a day/7 days a week emergency services.

1.6.12 Identification of Contractor Employees: All contract personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed.

1.6.13 Contractor Travel : Not Applicable.

1.6.14 Other Direct Costs : Not Applicable.

1.6.15 Data : Not Applicable.

1.6.16 Organizational Conflict of Interest: Contractor and subcontractor personnel performing work under this contract may receive, have access to or participate in the development of proprietary or source selection information (e.g., cost or pricing information, budget information or analyses, specifications or work statements, etc.) or perform evaluation services which may create a current or subsequent Organizational Conflict of Interests (OCI) as defined in FAR Subpart 9.5. The Contractor shall notify the Contracting Officer immediately whenever it becomes aware that such access or participation may result in any actual or potential OCI and shall promptly submit a plan to the Contracting Officer to avoid or mitigate any such OCI. The Contractor's mitigation plan will be determined to be acceptable solely at the discretion of the Contracting Officer and in the event the Contracting Officer unilaterally determines that any such OCI cannot be satisfactorily avoided or mitigated, the Contracting Officer may affect other remedies as he or she deems necessary, including prohibiting the Contractor from participation in subsequent contracted requirements which may be affected by the

OCI.

1.6.17 PHASE IN /PHASE OUT PERIOD : Not Applicable.

DEFINITIONS & ACRONYMS

2. DEFINITIONS AND ACRONYMS:

2.1. DEFINITIONS:

2.1.1 . CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

2.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

2.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract.

Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
