The general description of the work for Paint Creek is to fabricate and install protective guards that will cover the rotating machinery equipment associated with the line shafts. All locations that have pillow block bearings and couplings shall have their respective guards hinged for fast and easy access to the equipment. (See attached photographs.)

These projects will be constructed in accordance with the plans and specifications provided. All Drawings and Specifications are for Official Use Only.

1.0 GENERAL

1.1 Alum Creek Lake

1.1.1 The scope of work covers the fabrication and installation of machinery guards for the

Alum Creek Dam in Lewis Center, Ohio. Contractor shall provide/install all fasteners

(painted) and appurtenances as needed for a complete installation. The guards shall be installed in each of the four (4) control houses that are used to operate the Tainter gates.

The guards will provide protection to project personnel to prevent injury from the rotating machinery. The fabrication of the guards will take place off site and the guards will then be shipped to the project site where they will be installed by the contractor.

Painting shall be completed at the time of fabrication and touch up painting will occur after installation. In situations where two or more specifications apply to this work, the most stringent requirements govern.

1.2 Deer Creek Lake

1.2.1 These specifications cover the fabrication and installation of the line shaft machinery guards. Contractor shall provide/install all fasteners (painted) and appurtenances as needed for a complete installation. The guards shall be installed on each of the abutments and on both of the interior piers. The fabrication of the guards shall take place off site and be delivered to the project. Painting shall take place at the time of fabrication and any damaged paint will be touched up after installation. The installation of the guards shall be completed by the contractor. In situations where two or more specifications apply to this work, the most stringent will govern.

1.3 Paint Creek Lake

1.3.1 These specifications cover the fabrication and installation of the line shaft machinery guards at Paint Creek Lake. Contractor shall provide/install all fasteners (painted) and appurtenances as needed for a complete installation. Guards shall be installed over each of the rotating lineshafts that operate the Tainter gates. The fabrication of the guards shall take place off-site and the guards will then be shipped to the project site where they will be installed by the contractor. Painting shall take place at the time of fabrication and paint damaged during installation will be repaired after installation. In situations where two or more specifications apply to this work, the most stringent requirements govern.

The Contractor shall fabricate the covers in accordance with sheets S-003 through S-008.

2.0 Materials

2.1 Structural Steel

2.1.1 All W-shape steel that may be used shall conform to ASTM A992 steel

2.1.2 Angles shall conform to ASTM A36 Steel.

2.1.3 All other structural steel shall conform to ASTM A36 Steel.

2.1.4 Dimensional tolerances shall be equal to +/- 1/16”.

2.2 Steel Hardware

2.2.1 Bolts for connections shall conform to ASTM A307 grade A.

2.2.2 Nuts shall conform to ASTM A563

2.2.3 Washers shall conform to ASTM F436

2.3 Aluminum

2.3.1 Aluminum structural components shall conform to requirements of 6061-T6. With

Aluminum type 5052 being an acceptable alternative.

2.3.2 Aluminum Weld filler shall be 5356.

2.3.3 Aluminum welding shall be done in accordance with AWS D1.2 Structural Welding

Code-Aluminum.

2.3.4 A fabricated member shall not vary from straight or from its intended curvature by more than its length divided by 960.

2.3.5 Dimensional length tolerances shall be +/- 1/16”

2.4 Expanded Metal

2.4.1 Expanded metal shall be Aluminum 6061-T6

2.4.2 Expanded metal shall be flattened.

2.4.3 Expanded metal shall be ½# 0.51 or equivalent

2.4.4 Tolerances

2.4.4.1 SWD: -0 + ¼” per foot of width

2.4.4.2 LWD: -0 + ¼” per foot of length

3.0 Fabrication

Material before being laid off or worked must be straight. If straightening is necessary, it shall be done by methods that will not injure the metal. Sharp kinks or bends shall be cause for rejections of the material.

Material with welds will not be accepted, except where otherwise approved. Where heating is required, precautions shall be taken to avoid overheating the metal and it shall be allowed to cool in such a manner as not to destroy the original properties of the metal. Finished members shall be free from twists, bends and open joints. All bolt holes shall be 9/16” Dia. For a ½” Bolt unless otherwise noted.

3.1 Steel

3.1.1 Cutting

3.1.1.1 Structural steel shall be thermally cut provided an accurate profile with a smooth and regular surface, free from cracks and notches is obtained. All thermal cut surfaces shall be produced using a mechanically guided torch unless otherwise approved by the engineer. Thermal cut surfaces produced by a manually guided torch, when allowed, shall be smoothed by a machining or grinding. Surfaces and edges shall be prepared in accordance with AWS D1.1M / D1.1.

3.1.2 Welding

3.1.2.1 Welding of structural steel shall be by an electric arc welding process using a method which excludes the atmosphere from the molten metal and shall conform to the requirements specified in the provisions of AWS D1.1M / D1.1

3.2 Aluminum

3.2.1 Cutting

3.2.1.1 Cutting shall be by shearing, sawing, nibbling, routing, arc cutting, laser, or abrasive water jet. Edges which have been arc or laser cut shall be planned to remove edge cracks. Oxygen cutting is prohibited.

3.2.1.2 Cut edges shall be true, smooth, and free from excessive burrs or ragged breaks.

3.2.2 Welding

3.2.2.1 Welding of aluminum shall be done conforming to AWS D1.2.

3.2.2.2 The flat bars on the frame shall all be flush with each other and the expanded metal welded onto the frame.

3.3 Weld testing

3.3.1.1 All welds shall be visually inspected in accordance with AWS and performed by

AWS certified welding inspector.

4.0 Painting

1. Prior to painting, the steel shall be prepared using a SSPC-SP5 sandblasting.

2. Structural angles and plate work shall be painted with a ppg paint mixture to match the existing grey color of the current machinery in the control housing.

3. The selected primer and paint shall be applied according to manufacturer’s specifications.

4. The primer shall be a MIL-DTL-24441 / 19B or Equivalent

a. The thickness of the primer shall be a minimum of 2.5 mils or no greater than 6 mils.

5. The primary coat shall be 40-AH-51 (Grey) or equivalent.

a. The thickness of the paint be a minimum of 12 mils for the completed system.

b. There shall be a minimum of two coats.

6. There shall be no paint applied to the aluminum material.

5.0 Workmanship

1. General: Workmanship shall be of the highest grade in accordance with the best modern practices to conform to the specifications and drawings for the items being furnished.

2. Quality Control: The Contractor shall establish and maintain a quality control system to assure compliance with the contract requirement. The Contractor’s Quality Control Staff will ensure that workmanship is of the highest quality and provide documentation that supports this quality for all work performed by the Contractor, and Sub-Contractor, Fabricator of Supplier.

6.3.1 Marking. Paint Creek Lake

7.0 Submittals

1. General

a. Contractor submittals shall be furnished in accordance with the specifications below. All listed items shall be submitted for Government approval by the Contracting Officer.

Associated work items shall not be performed until receiving approval from the

Contracting Officer. Contracting Officer shall provide a response to the shop drawings within one week of the receipt.

2. Shop Drawings

a. Use of the attached drawings for shop drawings shall be at the Contractor’s option and risk. If the Contractor’s shop practice requires details in the additions to, or different from, those shown on the contract drawings, prepare shop drawings and furnish three sets for approval. If approved by the Contracting Officer, each copy of the drawings will be identified as having received such approval by beings so stamped and dated, and one set will be returned to the Contractor. The Contractor shall make any corrections required by the Contract Officer. The approval of the drawings by the Contracting Officer shall not be construed as a complete check, but will indicate only that the general method of construction and detailing is satisfactory. Approval of shop drawings by the Contracting

Officer shall not relieve the Contractor of the responsibility for complete conformity with the specifications.

3. List of Materials

a. The contractor shall submit evidence, satisfactory to the Contracting Officer, stating that all materials conform to the requirements of these specifications. The Contractor shall also furnish a shipping bill or memorandum of each shipment of finished pieces or members, giving the designation mark and weight of each piece, the number of pieces and the total weight.

9.0 Corps of Engineers Inspection

a. Corps of Engineers personnel shall be allowed to inspect the progress of any portion of this project at the contractor’s worksite. All parts or assemblies shall not be shipped until the units have been inspected and approved by the Contractor Quality Control Representative (CQCR). The CQCR shall provide reports to the Contracting Representative prior to the shipment of any part of assembly certifying that the part of assembly meets or exceeds the contract requirements.

10.0 Delivery

a. Delivery shall be coordinated with each projects respective point of contact (POC) prior to shipping.

Monday through Friday, Federal holidays excluded.

b. A minimum of seven (7) days’ notice shall be given prior to shipping as determined by the individual projects’ POC at each project location. The Contractor shall confirm that intended method of shipment is satisfactory for unloading by the Government.

c. The items shall be delivered via overland means.

d. Delivery shall be made to the following physical addresses for each respective project:

11.0 Installation

Installation shall proceed within seven days after delivery of Machine Guards. Contractor must follow the drawings and the EM385-1-1 for all safety matters.

Restoration requirements:

The Contractor shall clean all debris and waste resulting from construction operations and restore all surfaces, structures, ditches, and property to its original condition to the satisfaction of the on-site Government representative.

Any areas disturbed by the installation of the waterline shall be restored to their undisturbed (pre-project) condition before the end of the contract as shown in the attached plan drawings. Note: The tap and valve for the restroom is under the concrete sidewalk, and there may be other areas of concrete/asphalt disturbed during installation of the waterline. Complete restoration shall consist of proper trench refill and surface treatment. Proper trench refill shall include adequate compaction. In forested and lawn areas, mound soil approximately two-inches higher than the surrounding grade to provide for possible settlement UNLESS this mound will interfere with surface drainage, then omit any mounding and grade to drain. Seed mixture shall contain at least 40% Creeping Red fescue, 40% Perennial

Rye grass, and 20% Kentucky bluegrass. Seed and mulch application rates shall be per the manufacturer’s instructions. All seeded areas shall be thoroughly mulched (straw) and watered. Paved areas, including gravel, asphalt, or concrete surfaces shall be restored in accordance with the details provided in the plan drawings. All work, staging, and stockpile areas shall be thoroughly cleaned before the Contractor completes work at the site.

The contractor shall clean the work area at the end of each workday and shall maintain a clear and safe means of ingress and egress from the work area at all times. The use of the site for construction staging shall be coordinated with the on-site Government representative. Upon completion of construction, the Contractor shall clean all debris and waste resulting from construction operations and restore all surfaces, structures, ditches, and property to its original condition to the satisfaction of the Resource Manager. All waste materials shall be disposed at an approved landfill facility. After placed into operation, the water shall be tested for bacteria in accordance with AWWA

Standard C651. The contractor shall provide the laboratory results to the Government.

Environmental Protection – General. For the purpose of this specification, environmental protection is defined as the retention of the environment in its natural state to the greatest possible extent during project construction and to enhance the natural appearance in its final condition.

Environmental protection requires consideration of air, water, and land and involves noise and solid waste management, as well as other pollutants. In order to prevent and to provide for abatement and control of, any environmental pollution arising from the construction activities in the performance of this contract, the Contractor and its subcontractors shall comply with all applicable Federal, State, and local laws and regulations concerning environmental pollution control and abatement. The Contractor's attention is invited to Drawing Numbers GI004 and GI005 concerning environmental protection measures. The Contractor shall comply with all applicable local, state and federal environmental laws and regulations.

Permits. Obtaining and complying with all environmental permits and commitments required by Federal, State, Regional, and local environmental laws and regulations is the Contractor's responsibility. The Contractor is responsible to obtain any other permits needed for the project and for the payment of all fees associated with said permits.

Hours of Operation

Hours of work shall be 0730-1600 hours Monday through Friday. Work may continue outside these times with permission from the on site government representative. All work shall be performed during daylight hours.

Delay of work may occur during excessive rainfall or wet periods due to access to the site.

Federal Holidays: When one of the designated holidays falls on a Sunday the following Monday is observed as a legal holiday. When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday. Work that interferes with other ongoing functions at the project shall be scheduled around normal working hours to be approved by Project Manager. Work shall be scheduled around the above listed Federal holidays unless approved in advance.

Contractor and Government Equipment and Supplies

The contractor shall furnish all personnel, equipment, transportation, disposal, supplies and materials to perform the work. No government equipment or supplies shall be used for this project.

Contractor Accident Prevention Plan (APP).

A. General. Contractor shall comply with all Federal, State and Local safety requirements including those specified by the Occupational Safety and Health Act (OSHA) in 29 CRF 1910 General Industry Standard, 1926

Construction Standard and the U.S. Army Corps of Engineers Safety & Health Requirements Manual EM 385-1-1 as currently revised. Where a conflict exists between safety standards/requirements the contractor shall comply with the more stringent requirement. If the Contractor fails or refuses to promptly comply with safety and security requirements as specified herein, any project employee may stop all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such work stoppage shall be made subject to any claims for extension of time or for excess costs or damages sustained by the Contractor. Also, the Contractor will not be paid for work not performed as a result of the stop order.

B. Contractor Accident Prevention Plan (APP). The Contractor shall prepare a site-specific Accident

Prevention Plan following the outline provided in EM 385-1-1, Appendix A as a minimum. The contractor shall address each element and sub-element in the outline in the order they appear in Appendix A. Any item from the outline that does not apply must be listed, in its assigned place and a brief explanation of the reason it is not required/provided. The APP shall address any unusual or unique aspects of the project or activity. This plan must be accepted by the Contracting Officer prior to commencement of any work. Contractors are encouraged to contact the USACE Safety & Occupational Health Office for questions or information regarding the development of contract Accident Prevention Plans. (1) The contractor shall have on site an employee designated as a Site Safety and Health Officer (SSHO) who has completed the OSHA 30-hour Construction Safety training or its equivalent as specified in EM 385-1-1, 01.A.17 and has at least five years experience relative to the work to be performed. No work will be permitted unless the SSHO is present at the work site.

(2) Work crews of two or more must have two people holding current certification in First Aid and CPR from the American Red Cross, American Heart Association or a licensed physician. Training must include a hands-on component that cannot be taken online and the contractor must maintain appropriate First Aid kit on site. Single individuals working alone must have current certification in First Aid as a minimum.

(3) All work that potentially exposes the worker to falls of six feet or more will require the contractor to submit a Site Specific Fall Protection & Prevention Plan in accordance with EM 385-1-1, Section 21.D.

(4) Cranes and hoisting equipment shall be operated only by qualified personnel (operators licensing, physical and drug testing in accordance with EM 385-1-1 Section 16.B). Note: CDL Medical Clearance is not acceptable. Please see attached medical clearance requirements.

(5) Effective November 8, 2010 all “Signalpersons” and “Riggers” shall be qualified. This is a mandatory requirement in accordance with 29 CFR 1926.1400 Regulations. Riggers must be a qualified person for the performance of specified hoisting activities such as during assembly/disassembly, in accordance with OSHA

1926.1404(r)(1) and 1926.1425(c)(3).

(6) Work orders involving potential exposure to Hazardous, Toxic or Radiological Waste shall require development of Site Specific Safety and Health Plan (SSHP) by qualified personnel as specified in EM 385-1-1

Sections 6 and 33.

C. Risk Management Process. The Contractor shall utilize risk management tools such as Activity

Hazard Analysis (AHA), Job Hazard Analyses (JHAs), Job Safety Analyses (JSAs) or similar tools to identify, assess, prioritize risks and develop controls to minimize, monitor and control the probability and/or impact of unfortunate events to an acceptable level. Within each major phase, all activities, tasks or Definable Features of

Work (DFOWs) shall be identified that will require an AHA/JSA/JHA. For each work activity, task or DFOW contractor shall prepare the initial AHA/JSA/JHA. A Risk Assessment Code (RAC) is assigned to each step, to the http://www.publications.usace.army.mil/Portals/76/Publications/EngineerManuals/EM_385-1-1.pdf risk that remains after controls have been applied (residual risk) a final RAC (cannot be lower than the highest step

RAC) shall be assigned. See EM 385-1-1 Section 01.A.14.b. for further guidance or contact the SOHO for assistance. The AHA/JSA/JHA shall be reviewed and accepted by the proper authority in order to proceed with the activity. All on-site employees shall review the accepted AHA/JSA/JHA prior to performing work and during weekly and “Tool Box” safety meetings as appropriate.

1. AHA/JSA/JHA shall define the activities being performed and identify the work sequences, the specific anticipated hazards, site conditions, equipment, materials, and the control measures to be implemented to eliminate or reduce each hazard to an acceptable level of risk. AHA/JSA/JHA should indicate compliance with EM 385-1-1 personal protective equipment requirements (safety-toed work boots, safety glasses, gloves, hard hats, etc.), work crews of two or more must have two people currently qualified in first aid and CPR and the appropriate first aid kit on site. Single individuals working alone must have current certification in First Aid only.

2. The names of the Competent Person(s) and Qualified Person(s) required for a particular activity (e.g., excavation, scaffolding, fall protection, or other activities as specified by OSHA and this manual) shall be identified and included in the AHA/JSA/JHA, as well as proof of their competency/qualification.

D. Accident/Mishap Reporting: The Contractor shall report all recordable mishaps to the GDA within

24-hours in accordance with EM 385-1-1 Section 01.D. Accidents shall be investigated to determine causal factors and corrective actions shall be implemented. Accidents resulting in: days away injuries, days away illnesses, restricted/transfer injuries, property damage of $5,000.00 or more shall be investigated and reported to the GDA using the USACE Accident Investigation Report Form ENG 3394 within 24 hours of the occurrence.

(1) Immediate notification to the GDA is required for all serious accidents:

a. Fatal Injury/illness;

b. Permanent totally disabling injury/illness;

c. Permanent partial disabling injury/illness;

d. One (1) or more persons hospitalized as inpatients as a result of a single occurrence;

e. $500,000 or greater accidental property damage;

f. Three (3) or more individuals become ill or have a medical condition suspected to be related to a site condition, or a hazardous or toxic agent on the site;

g. USACE aircraft destroyed or missing;

The Contractor shall maintain an OSHA Log of Work Related Injuries and Illnesses, OSHA Form 300 and comply with all accident reporting and recordkeeping requirements specified in 29 CFR 1904.39.

(2) In addition to the above, any mishap occurring in any of the following high hazard areas shall be reported immediately to the GDA, investigated and corrective actions implemented.

a. Electrical (includes Arc Flash, electrical shock, etc.);

b. Uncontrolled Release of Hazardous Energy (includes electrical and non-electrical);

c. Load Handling Equipment (LHE) or Rigging;

d. Fall-from-Height (any level or other than same surface);

e. Underwater Diving

SECURITY REQUIREMENTS

All Contractors and subcontractor employee(s) working on this contract who require access in and/or around any

Huntington District facilities shall receive authorization to be on site at these facilities based upon a favorable background investigation in accordance with “Homeland Security Presidential Directive 12: Policy for a Common

Identification Standard for Federal Employees and Contractors” (HSPD 12). The Huntington District Security

Management Office shall submit contractor(s) personal information to complete the basic background investigation in order to make a recommendation to the Contracting Office of an employee’s suitability for work on Government property. The Procuring Contracting Officer (PCO) has the right to deny any contractor or subcontractor employee from working on the contract based upon the results of the background investigation provided by the Huntington

District Security Management Office. The Government, Contractor, and subcontractors shall not permit any employee(s) access to drawings, blueprints, records, photos, sites, or projects until the Huntington District Security

Management Office has completed an Operations Security (OPSEC) review of such items and determined they can be released.

FOR ALL CONTRACTS 1-YEAR AND SUBSEQUENT OPTION YEARS:

Contractors shall submit a complete investigation packet for each contractor and subcontractor employee to the

Huntington District Security Management office within 10 working days after award of any contract, or prior to the individual being permitted unescorted access in or on District facilities.

The Contractor must also provide the following: (1) Name of the Contract Specialist with whom he/she is working,

(2) Project Name, (3) Contract Number (4) Task Order Number when applicable, and (5) Security Information sheet on each employee working on site must be submitted directly to the Huntington District Security Office.

The Contractor must provide a full legal name, complete SSN#, Date of Birth, Place of Birth (City and State) and

Driver’s license number if available. It is the Contractor’s responsibility to ensure the above information is accurate and legible, otherwise delays may occur.

Contractors shall have background checks completed for their employees annually in the case of Contracts with option periods. For instance, if the employee was cleared in May of 2018, they shall be cleared again in May of

2019. It is the Contractor’s responsibility to ensure that each employee’s security is up to date. Failure to comply may lead to possible removal from the project/jobsite.

The Contractor must ensure if new employees are brought in to work on an existing Contract with the U.S. Army

Corps of Engineers (USACE), they must process the employee through the Huntington District USACE Security

Management Office for background purposes.

FOR CONTRACTORS REQUIRING DOD COMMON ACCESS CARDS (CAC)

The documents listed below are required:

SF 85, Questionnaire for Non-Sensitive Positions (see http://www.opm.gov/forms/ to print form). FD 258, Contractor Fingerprint Card (one card for digital prints, two for ink prints) (NOTE: a trained/qualified individual must take prints, ORI block on the FD 258 must be USOPM000Z) (Contractor Fingerprint Cards (FD-258) may be obtained by Contracting Division by contacting the Huntington District Security Management Office).

Contractor shall report any changes in personnel, or at any time conditions change at least 5 working days prior to individual(s) need to have access to the site. The Contractor shall provide the same information as listed above for all new employees.

Following submission of paperwork, the Huntington District Security Management Office will conduct an initial criminal check based upon the information provided. If there are no issues, the Huntington District Security

Management Office will make recommendations to the PCO or their Designated Representative. The Huntington

District Security Management Office cannot give approval due to unfavorable report(s). The Huntington District http://www.opm.gov/forms/

Security Management Office shall coordinate with Office of Counsel and will jointly make recommendations to the

PCO and/or the Designated Representative should a background investigation come back with derogatory information. The PCO will make the determination whether to allow the Contractor personnel to work on the site based on Security and Office of Counsel recommendations, and the PCO will advise the Contractor of the decision.

FOREIGN NATIONALS

The Contractor shall provide the package listed above, as well as additional materials noted below for all Foreign

Nationals. The Contractor is to expect additional time in the approval process for all Foreign Nationals due to the level of review and elevation of the review to Headquarters staff to complete. All Foreign Nationals must comply with the following paragraph:

In accordance with Engineering Regulation 380-1-18, “Technology Transfer, Disclosure of Information and

Contacts with Foreign Representatives” dated 1 August 1996, Section 4, all foreign nationals who work on Corps of

Engineers’ contracts shall be approved by the Headquarters US Army Corps of Engineers (HQ USACE) Foreign

Disclosure Officer before beginning work on this contract; this regulation includes subcontractor employees. The

Contractor shall submit to the Security Management Office, the names of all foreign nationals proposed for performance under this contract, along with documentation to verify legal admittance into the United States. Such documentation shall include at least one of the following: a U.S. passport, Certificate of U.S. Citizenship (INS Form

N-560 or N-561), Certificate of Naturalization (INS Form N-550 or N-570), Alien Registration Card with photograph (INS Form I-151 or I-551), Employment Authorization Card (INS Form I-688A), etc.

In order to assure forms are properly completed, and to minimize time delays, the Contractor is encouraged to work closely with the Huntington District Security Management Office. The Government will return for correction incomplete forms, illegible forms, etc. which will delay the contractor / subcontractor from beginning work.

Security requirements are subject to change in accordance with HSPD-12, Office of Personnel Management, regulation or policy changes, etc. The Government shall add any changes to security requirements by modification to the contract.
