W911YU-17-T-1003 ATTACHMENT 1
REQUIREMENTS SUMMARY
Scope. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to provide/supply bulk vaccinations as detailed below in support of the Guam National Guard’s annual Periodic Health Assessment. The contractor shall perform to the standards in the contract as well as comply with all Federal, State, and local, laws and applicable regulations.

DELIVERY SCHEDULE AND ADDRESS
A minimum of 80% of the required items must be delivered no later than 1 August 2017 to the following address:
Guam National Guard ATTN: SFC Aldrin Borja 430 Army Drive, Bldg 100 Barrigada, GU 96913-4421
The remaining 20% of the shipment must be delivered no later than 15 August 2017 to the same address. Offerors may ship the entire requirement as a whole or bulk as long as the entire shipment is received no later than the earlier of the two dates.

ADDITIONAL TERMS AND CONDITIONS
1. Item Condition: New items ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty.
2. Shipping: Shipping must be free on board (FOB) destination OCONUS (OUTSIDE
Continental U.S.), which means that the offeror must deliver the goods on its conveyance to the destination specified and the offeror is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified destination.
3. SAM Requirement: This solicitation requires registration with the System for Award
Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov.
Registration must be "ACTIVE" at the time of award.
4. SPECIAL HANDLING OF VACCINATIONS/IMMUNIZATIONS: Offerors must include with their offer/quote, a detailed description of the entire shipping process— quality control plan. The quality control plan should include how the vaccinations will be packaged, shipped, and the method used to control and monitor the temperature of the shipment throughout the process. Failure to include quality control information may result in an offer/quote rating of “not technically acceptable.”
