Statement of Work
DEMO TRAILERS ON OKLAHOMA AVE AND MINNESOTA AVE
FORT LEONARD WOOD, MO
(WO#LEO1ENG055078J)
29 May 2019
I. GENERAL PROVISIONS
1. GENERAL. This project consists of providing all labor, equipment, and materials to demolish the Re-locatable buildings located on Oklahoma Ave and Minnesota Ave as identified below in Section II. Work shall also include, but is not necessarily limited to removing the demolished buildings and contents, as required. All work specified herein shall be installed in accordance with (IAW) this Scope of Work, the project drawings (as applicable), and other specifications or applications referenced or otherwise made a part of this Contract.
2. LOCATION. The site of the proposed work: Oklahoma Ave (2 locations, 33 trailers) and Minnesota Ave (43 trailers) to include trailer Bldg 708A located on the corner of Minnesota Ave and Alabama Ave on Fort Leonard Wood, MO Army installation.
3. VERIFICATION OF CONDITIONS AND DIMENSIONS. It is incumbent upon the contractor to review plans and specifications, field verify applicable dimensions before starting work, and to notify the Government of any perceived discrepancies between the plans/specifications and actual conditions immediately upon discovery, and before work affected by the discrepancy is performed.
4. COORDINATION. Two (2) weeks prior to planned work start up for initial notification, Contractor shall contact the Government Project Manager:
- Government Project Manager (PM)/Quality Assurance Representative (QAR)
The Contractor shall coordinate daily access to the facility with the facility POC, or his designated alternate. Failure to coordinate daily access with the Facility POC resulting in a Contractor’s inability to access work areas shall not constitute grounds for modifications to this Contract. Any work deemed necessary by the Contractor to be performed on Saturdays, Sundays, or legal holidays to maintain or correct Contractor schedules for the purpose of meeting the negotiated or Government-specified completion period for this project, shall be performed without additional cost to the Government. The Contractor shall notify the Contracting Officer or his authorized representative of work planned or scheduled for other than normal working hours. This notification shall be submitted in writing a minimum of 72 hours in advance of the proposed work. Approval for work during other than normal duty hours (other than that specified by the Government) will be contingent upon availability of Government (facility) personnel to open the affected facilities for access, and the Contractor shall reimburse the Government for any additional expense thus incurred by the Government, such as, but not limited to, overtime pay for Government employees (inspectors, facility representatives, etc.), utilities service, etc. Failure to provide coordination IAW this paragraph resulting in the Contractor’s inability to gain access to the job site at his convenience shall not constitute grounds for change orders for either time extensions or additional consideration due to lost time.
5. SECURITY REQUIREMENTS. Prior to physically starting work on this project, the Contractor shall furnish the Government PM/QAR, with a list of all Contractor / Sub-Contractor personnel who will require access to the facility during the course of the subject project. This list shall be provided a minimum of 5 working days before access to the facility is required. This list shall be signed by an authorized representative of the Contractor, and shall contain the following information: full name of employee(s), date of birth, identification number correlating to the employee's photo identification (e.g., driver's license number, employee number, etc.), and Corporate points of contact who shall be further responsible for identifying employees, subcontractors, vendors and delivery personnel. This list shall be updated and resubmitted as often as necessary for it to be current. Depending on current events, personnel shall be required to show photo identification (cross checked against the required roster) prior to gaining access to a facility. Failure to provide this information IAW this paragraph resulting in the Contractor's inability to gain access to the job site shall not constitute grounds for change orders for either time extension or additional consideration due to lost time
5.1 OPSEC
The OPSEC Plan and User Agreement will be available for Contractors to review at the Contracting Office located in Building 11400, Fort Leonard Wood, MO, from the hours of 0730 to 1600. The COR will ensure the Contractor is aware of the Installation's OPSEC Plan and is in compliance with the Installation's OPSEC Plan during contract performance.
Pursuant to AR 530-1, all Contractor personnel must complete initial Level I OPSEC training within 30 calendar days beginning performance on this contract. For contracts that cross fiscal years, or for contracts with option years, all contractor personnel shall complete annual OPSEC awareness training. The MSCoE OPSEC Officer will provide training. Contact information for this Government employee will be provided at the kick-off meeting.
The MSCoE OPSEC Officer will provide a one hour training session conducted on FLW for Contractor personnel working on this installation. The COR will coordinate this training at the time of award.
Contractor personnel working at installations other than FLW shall coordinate with the COR and local OPSEC Offices to obtain training applicable to that installation.
Contractor shall provide a list of personnel trained to the COR within five (5) business days of completion of training.
5.2 Antiterrorism:
ACCESS AND GENERAL PROTECTION/SECURITY POLICY AND PROCEDURES:
All Contractor and sub-contractor personnel performing tasks on this SOW shall comply with all applicable installation, facility and area commander installation/facility access and local security policies and procedures which will be provided to the Contractor by a Government representative upon written request to the Contracting Officer. Contractor shall provide all information required for background checks to meet installation access requirements as performed by the installation Provost Marshal Office, Director of Emergency Services or Security Office. Requesting Contractor shall submit request for installation pass to the COR at https://vcc1.icmvcc.com/im3500/. There is no charge to the Contractor for this pass. Contractor personnel shall comply with all personal identity verification requirements as directed by DoD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of the base contract, should the Force Protection Condition (FPCON) at any facility or installation change, the Government may require changes in Contractor security matters or processes.
In the event that the automated system at https://jkodirect.jten.mil is not available (e.g., server problems), Level I AT Awareness Training can be provided by a qualified instructor. However, if the training is not completed online, the Level I AT Awareness Instructor qualification must be coordinated with the Installation Antiterrorism Officer (or Installation Security equivalent) and the resultant name(s) of approved instructors shall be provided to the KO.
Antiterrorism (AT) Level I Training: All contractor and sub-contractor employees requiring access to Army installations, facilities and controlled access areas shall complete AT Level I awareness training available at https://jkodirect.jten.mil.Users need to enroll in "Course #JS-US007-14" to receive credit for the training within 10 calendar days of contract award; or for new employees within 10 working days of beginning performance on this contract. Contractor shall submit certificates of completion for all personnel to the KO within five calendar days of completion of the training. There is no charge to the Contractor for this training. The burden to complete this training is approximately 1-hour per employee.
iWatch Training: Within 10 calendar days of contract award and within 10 calendar days of new Contractor employees commencing work on this contract, all Contractor and sub-contractor employees shall review the FLW Directorate of Emergency Services (DES) web link for iWatch training located at http://www.wood.army.mil/LEC/iWatch/iWatch.htm. This locally developed training will inform employees of the types of behavior to watch for and how to report suspicious activity. Suspicious activity shall be reported to the KO. There is no charge to the Contractor for this review. The burden to complete this training is approximately 20-minutes per employee.
6. ENVIRONMENTAL. a. Waste / Debris Disposal. The Contractor shall conform to all applicable contract clauses, Federal, State, and Local laws & regulations in regard to the removal, handling, and proper disposal of demolition debris, and the Contractor shall lawfully dispose of all demolition waste and debris off of the government property, at landfill(s) specifically licensed to accept such wastes, unless previously identified and negotiated for salvage or recycling. Burning of wastes, debris, etc. is not authorized.
(1) For wastes not identified for salvage, the contractor shall provide to the Government PM/QAR within 2 weeks of the wastes having been hauled, receipts from the receiving activity for those wastes. The receipts shall include as a minimum the name & address of the receiving activity, the transporting/delivering activity, and the volume &/or weight and type of the wastes received.
(2) Should the Contractor make use of a dumpster for normal debris disposal, the responsibility for proper final disposition shifts to the applicable waste management company.
However, in order to comply with the provisions of paragraphs 6.a. and c. of these General Provisions, the contractor shall maintain a record of all debris waste streams.
(3) Demolition Waste Reporting – The Contractor shall make reasonable efforts to recycle demolition waste to the fullest extent possible. The Contractor shall complete the current Fort Leonard Wood Demolition Waste Debris Reports (attached) in accordance with the applicable guidance. The completed form shall be submitted through the DPW Project Management Branch to the responsible Environmental activity no later than at the Pre-Final Inspection. No Payments beyond 95% progress shall be made prior to receipt of this documentation.
b. Erosion Control. Erosion control measures are required on all projects which disturb exterior sites to prevent soil from eroding outside the project boundaries. The contractor shall develop a separate storm water pollution prevention plan (SWPPP) for approval by the government prior to commencing work. The subsequent job-site protective measures from the approved plan shall be installed before any land disturbance operations commence, and maintained throughout the duration of the project, and until permanent surface stabilization is achieved. As soon as technically feasible, permanent turf or other approved surface stabilization is required on the disturbed areas.
7. SAFETY. The Contractor shall use all appropriate safety equipment and apply sound safety practices as required by OSHA and the State of Missouri. A project-specific Safety plan is required from the contractor as a pre-performance submittal which shall be submitted and must be approved prior to physically starting work.
8. SUBMITTALS: Prior to work starting, the Contractor shall submit their QC plan and Safety plan via e-mail to the Contracting Officer for approval. Issuance of Notice to Proceed (NTP) will be withheld until approval of all specified submittals unless it is determined that the Contractor is not pursuing completion of the submittal process in a timely manner; in that instance the NTP shall be issued at the discretion of the Contracting Officer starting the performance period.

9. QUALITY CONTROL:
9.1. The Contractor is responsible for ensuring the requirements of this project are satisfied IAW the contract specifications and drawings, and all applicable environmental laws and regulations;
the presence or absence of a duly authorized Government Inspector, PM or QAR shall not relieve the Contractor of this responsibility. A project-specific Quality Control Plan is required to be submitted and approved prior to any physical work commencing.
10. PERMITS: The Contractor is responsible for acquisition of all permits & licenses, payment of taxes, payments of fines and penalties for noncompliance, and associated fees required and/or assessed by the authorities having jurisdiction in the work area. Copies of all acquired permits & other approvals from these authorities (as applicable) shall be provided to the Project Manager.
Items covered by this requirement include, but are not limited to: building permits, digging permits, fees associated with hot work permits, State/City requirements for specially licensed or registered contractors to perform work, etc.
11. SITE MAINTENANCE:
11.1. The Contractor shall police up all scrap and debris at the end of each workday, and dispose of it properly off of the installation property. After demo operations are completed for the project, all waste, dirt, and debris shall be removed from the work site and the entire work area cleaned as necessary to match the adjacent area.
11.2. All property, including buildings, site improvements, utilities and vegetation at or adjacent to the work site, and which might be damaged during the course of the required work, shall be adequately protected by the Contractor for the duration of the contract. The Contractor shall not operate heavy construction equipment on drives, sidewalks, or lawns without installing suitable protection on these surfaces. The contractor shall repair all damage determined to be due to his negligence at no additional expense to the Government.
11.3. The Contractor shall take all possible precautions to maintain the work site in a condition that will prevent damage or injury to personnel and real property. The above precautions shall include protection from elements of weather, dust/debris mitigation.
12. PRE-FINAL / FINAL INSPECTION REQUEST NOTIFICATION: The Contractor shall contact the Government PM/QAR to schedule a pre-final / final inspection of the project after all work has been completed, and the site has been cleaned. The Government PM/QAR will meet with the Contractor on site to inspect the work. A Request for Final Payment shall not be submitted by the Contractor until after this inspection has been made, any punch list items have been corrected, and the project re-inspected.
II. PROJECT REQUIREMENTS:
1.0. Contractor shall demo all relocatable trailers located on the east (4 trailers) and west side (29 trailers) of Oklahoma Ave and the 2 locations off of Minnesota Ave (43 trailers) and (1) in the 700 area. Contractor shall make every effort to recycle as much of the demo’d material as possible. Contractor shall provide dumpsters or dump trucks for the building demolition and dispose of all items properly. Contractor shall be required to perform housekeeping duties on a daily basis and maintain a clean, hazard free, safe site at all times. Insulation and other airborne debris that can be easily blown away shall be picked up and placed in the dumpsters immediately. All demo’d material not salvaged by the contractor shall be disposed of by the Contractor, in accordance with Federal, State, and Local laws and regulations.
1.1 During overhead demolition, no personnel outside operator’s protective cages on the equipment shall be allowed within the building perimeter work zone. Overhead work shall be performed in strict accordance with the approved Site Specific Safety Plan.
Note: Contractor shall provide dust embankment / control measures, i.e. wet down or mist the area to include all demo’d material, in order to contain the airborne hazards.
2. Fluorescent Light Bulbs: Prior to building demolition, Contractor shall remove and recycle all fluorescent light bulbs for each building scheduled to be demolished. Contractor shall be required to provide a recycling certificate/receipt from the recycler. Certificate shall include the name and address of the recycler and shall include the number of bulbs recycled.
3. Area Restoration: Upon complete removal of the demolition debris, Contractor shall repair all areas disturbed by the Contractor's demolition operations i.e. tire ruts, divots etc. caused by equipment operation. Contractor shall also ensure all demolition debris is policed up prior to final inspection of each project site.
