STATEMENT OF WORK (SOW)
FOR
Breakroom Building 110
AT
Watervliet Arsenal Building #, Rm #
Project #: 7PW123 Installation Management, Public Works Division Facilities Engineering (TAWV-IMP-F)
PART 1 GENERAL
SUMMARY OF WORK
1.1.1 Project Description
a. This Statement of Work (SOW) defines the effort required for renovation and construction of a break room within building 110, room to be shown on the drawing set. This project will include but not limited to, demolition of walls, new flooring, paint, plumbing, electrical, painting, ductless split A/C and data cables.
1.1.2 Location of Work
a. The work is located at Watervliet Arsenal, Watervliet, New York, Building 110, approximately as indicated on the drawing set. The exact location will be shown by the Contracting Officer.
1.1.3 Period of Performance
a. The contractor shall be required to (i) commence work under this contract within 7 calendar days after the date the contractor receives notice to proceed, (ii) prosecute the work diligently, and (iii) complete the entire work ready for use not later than [60] calendar days after notice to proceed was given. The time stated for completion shall include final cleanup of the premises.
1.1.4 Hours of Operation
a. Normal duty hours are 7:00 AM through 4:00 PM, Monday through Thursday, and 7:00 AM through 3:00 PM on select Fridays (the arsenal is closed every-other Friday). Federal holidays are non-work days. If a federal holiday falls on a Saturday or Sunday, the preceding Friday or following Monday is a work holiday.
SUBMITTALS
1.1.5 General Submittal Requirements
a. The contractor shall provide submittal documents via e-mail to the Contracting Officer for review and approval. Provide all physical (non-document) submittals (including but not limited to material swatches, samples, etc.) directly to the Contracting Officer for review and approval. All submittals require government approval unless otherwise explicitly stated.
b. The contractor shall reference APPENDIX A “Consolidated List of Submittals” for the complete list of required submittals.
1.1.6 Contractor Safety Plan
a. The contractor shall submit a safety plan which is specific to the work activity being performed under the scope of this project for Government review and approval.
1.1.7 Lockout / Tagout (LOTO) Acknowledgement
a. The Contractor shall sign and submit a “Lockout/Tagout (LOTO) Notification Acknowledgement Record” prior to working on or de-energizing any electrical equipment.
1.1.8 Asbestos Notification Acknowledgement Record
a. The contractor shall sign the “Asbestos Notification Acknowledgement Record” and submit this form to the Project COR prior to starting project work.
1.1.9 Project Schedule
a. The contractor shall create a project schedule and submit for Government review and approval.
b. The project schedule shall detail the phases of work and the overall timeline for the execution of the work. The contractor shall include delivery dates, appropriate milestones, and shall clearly detail any requirements for work performed outside of regular hours of operation.
GENERAL REQUIREMENTS
1.1.10 Watervliet Arsenal Information for Contractors (WAIC)
a. The contractor shall perform all work in strict conformance with Watervliet Arsenal Instructions for Contractors (WAIC). The contractor shall read and understand this document prior to submitting a proposal. The WAIC is publically available at:

1.1.11 Utility Outage Requests and Utility Connection Requests
a. Schedule work to minimize outages. For utility outages and connections required during the execution of work that affect existing systems, coordinate schedule with COR. Schedule utility outages and connections to minimize disruptions to the Government. No additional payment will be provided for utility outages and connections required to be performed outside the regular work hours.
b. The contractor shall notify the Contracting Specialist a minimum of two (2) business days before the contractor starts work. The contractor shall notify the COR a minimum of seven (7) business days before the shut-down of a utility or building service.
1.1.12 Safety Requirements
a. The contractor is required to provide the proper control of hazardous energy (lockout/tagout) per OSHA 29 CFR 1910.147.
b. The contractor shall inspect the project work site prior to starting work, and shall request a copy of the asbestos building inspection records to verify the location and description of known asbestos-containing materials.
ON-SITE PERMITS
1.1.13 Excavation (Dig Permit)
a. The clearance of buried utilities will be performed by the Government in advance of the Contractor performing subsurface work, unless otherwise explicitly stated.
b. The Contractor shall obtain a copy of the completed Utility Clearance Permit (WVA Form 420-2-E) from the COR prior to performing subsurface work. Verify existing utility locations indicated on contract drawings, within area of work.
1.1.14 Hot Work and Confined Space
a. Obtain permit for welding, burning, use of flame, or entry into confined space from WVA Fire Department by calling (518)266-5990 each day the permit is to be used.

CONTRACTOR EQUIPMENT
1.1.18 Equipment Identification
a. The contractor shall create and affix a label to all large equipment, dumpsters, and other mobilized property, which is weather-resistant (laminated, or within a see-thru label hanger), securely affixed to the equipment or visible through a window, and which clearly displays the General Contractor’s:
i. Company Name, POC Name (first, last), POC phone number, Project Number.
1.1.19 Waste Dumpster
a. The contractor shall place any waste dumpsters in a manner that does not interfere with thru-traffic.
b. Place waste dumpsters with a minimum of 33’ clearance from any building. The final location shall be coordinated with, and approved by the COR before any dumpsters are mobilized.
c. Cover all waste dumpsters when not in use, and before the end of each work shift. The cover shall be weather-tight.
d. Note: The Contractor shall be required to retain and submit copies of all Bills of Lading and Weight Slips associated with the disposal of construction and demolition C&D) debris.
PART 2 PRODUCTS
GENERAL
PART 3 All work shall be in strict accordance with product manufacturer’s instructions and specifications, including but not limited to manufacturer guidance for storage, staging, application, installation, and disposal of the product. Where a specific material is noted, the contractor shall assume –or equivalent- as acceptable for submittals.
3.1.1 Hazardous Materials
a. The contractor shall submit Safety Data Sheets (SDS) for all hazardous materials to be used.
3.1.2 Asbestos, Mercury, Lead, PCB
a. Unless otherwise stated, all materials for this project shall be new. All products shall not contain any asbestos, mercury, chromium/lead paint, Class I Freon, and Polychlorinated Biphenyls (PCB), and shall state such on the manufacturer’s original packaging.
3.1.3 DoD Green Procurement Program
a. All construction materials shall meet the applicable requirements of the DoD Green Procurement Program, specifically recycled content products, environmentally preferable products and services, biobased products, energy-and water-efficient products, alternate fuel vehicles and alternative fuels, products using renewable energy, and alternatives to hazardous or toxic chemicals as found at <http://www.acq.osd.mil/dpap/cpic/cp/docs/USA001967-08-DPAP.pdf>.
3.1.4 Paint
a. To meet the Affirmative Procurement Program (Federal Government’s Buy Recycled Requirements) all paint used in the project shall adhere to the below listed requirements of the Comprehensive Procurement Guideline for recovered materials. This applies only to interior latex paint, only if available at the time of the project.
i. Reprocessed Latex Paint (White, Off-White, Pastel Colors) shall contain 20% Post consumer content.
ii. Reprocessed Latex Paint (Grey, Brown Earth tones, and Other Dark Colors) shall contain 50-99% Post consumer content.
iii. Consolidated Latex Paint shall contain 100% Post consumer content.

· Plywood carcass construction, 1/2” thickness minimum, finished exposed exterior sides, finished natural interior sides.
· Solid wood face-frame, 3/4” thickness, maple.
· Solid wood doors and drawer-fronts, 3/4” thickness, maple.
· Plywood shelves, finished, adjustable, 3/4” thickness.

3.1.11 Drawer and Door Hardware

3.1.12 Fasteners

3.1.13 Plumbing

1. Brass ball valves. Valves shall NOT be manufacturer in China. COR will verify at every jobsite upon final walkthrough. Note: Mueller Industries manufactures acceptable valves.
· Faucet shall be ADA compliant, industrial grade, color nickel with 0.5gpm low flow aerator installed.
· Commercial Grade grinder pump: Shall be 0.33hp and 88GPM minimum with a vertical lift of 15’ minimum at 20GPM.

3.1.14 Finish Materials
· Toe Kicks:
1. 4” Vinyl Cove molded toe-kick, construction adhesive to base cabinet provided toe-kick. Note: If cabinet manufacturer has provided finish wood toe-kick, vinyl shall not be used.

1. Shall be color matched to the selected kitchen materials.
3.1.15 Resilient Plank Flooring

3.1.16 Split Unit A/C
· Daikin FTK24NMVJU single zone ductless split AC 12,000 BTU. Refrigerant type R410A. Minimum of 18 SEER. Or equivalent
· Daikin 12,000 BTU wall mounted indoor unit. Or equivalent
· Daikin Handheld Wireless Remote Controllers
· Pre-insulated line set.
· Rectorseal WBB300 outdoor condenser wall bracket. Minimum support rating of 300 Lbs. Or equivalent
· Daikin Condensate pump(s) Model SI30-230. Or equivalent.
· Fortress enclosure (Brown) or equivalent
PART 4 EXECUTION
ITEMS OF WORK
0. Drawing Package:
1. Contractor shall keep on the work site a copy of the drawings and specifications and shall at all times give the COR access thereto. Anything mentioned in the specifications and not shown on the drawings, or shown on the drawings and not mentioned in the specifications, shall be of like effect as if shown or mentioned in both. In case of discrepancy in the figures, in the drawings or in the specifications, the matter shall be promptly submitted to the COR and a determination shall be made. Any adjustments by the Contractor without such a determination shall be at the risk and expense of the Contractor.
0. Furniture Removal, demolition
a. Existing office furniture, computer equipment, etc. will be removed by government personnel. Any remaining items shall be removed and relocated by the Contractor. Any damage incurred to the remaining items during the construction period shall be covered in full by the contracting element.
b. Contractor shall conform to D1, Demo Plan for all demolition.
0. Lead Safe Paint Scraping ** (Only where paint was deemed lead containing)
3. Contractor shall scrape all loose paint as wall prep for new paint to be applied.
3. Contractor shall be lead certified and conform to all NYS regulations and 5PW070, “Lead Based Paint Abatement Project Requirements for Non-Housing” document provided by Watervliet Arsenal.
3. Prior to encapsulation, the space shall be visually inspected by Government officials.
3. Contractor shall encapsulate the remaining lead using industrial Lead Encapsulating Paint.
0. General Demolition.
4. The contractor shall carefully remove the following items for re-installation:
1. Fire alarm enunciator strobe, any all fire detection devices.
1. Baseboard heat register covers **Contractor shall label for location.
1. Lighting fixtures to be reused
4. The contractor shall exercise caution while working around sprinkler head and fire suppression equipment.
4. The contractor shall remove all interior finishes from Building 110 to include, but not limited to:
3. Ceiling tile
3. Ceiling Grid
3. Baseboard heat register (brackets and covers) **To be labeled and replaced
3. Light fixtures (shall be saved for reuse).
3. Window Coverings, to include privacy coatings.
3. Baseboard Trim
3. Outlets, Switches and Outlet and Switch cover plates.
3. Wall paper/paneling if any
3. Floor and wall cabinets, where applicable
3. Ceiling fan /if any **Talk to government official about possible reuse.
3. Anytime electrical wiring is removed, it shall be brought back to the panel.
3. Per the drawing set, there shall be several walls removed. As stated above, all electric that is involved with the removal of said walls shall be removed and brought back to the panel, labeling where necessary.
0. Kitchenette:
5. The Contractor shall adhere to the drawings for countertop, cabinet and sink locations.
5. The sink height, accessibility and faucet shall be ADA compliant.
5. All cabinets shall be constructed of solid wood and have soft close features on all openings.
5. The countertops shall all be solid surface, built to spec.
5. The Contractor is responsible for all measurements prior to ordering materials.
5. The Contractor shall install 18” x 30” basin in the 36” cabinet adjacent to the sink, a 0.33hp grinder pump shall then be installed and plumbed in accordance with the drawings to the drain southeast of room 139 also shown on drawing. The plumbing for the grinder pump shall be in-wall and above the drop ceiling where applicable within room 139. Where the plumbing shall drop into the bathroom space and attach to the drain the plumbing shall be allowed to be fastened to the wall in a neat and orderly fashion. The connection shall be made under one of the bathroom sinks of which shall be discussed with the government prior to install.
0. Ceiling Grid and Tiles:
6. The Contractor shall provide and install new 2 ft. x 2 ft. ceiling grid system in accordance with the selected product for all spaces noted on the drawing set.
6. The Contractor shall provide and install all new 2 ft. x 2 ft. acoustical ceiling tiles.
6. The Contractor shall install the ceiling at the highest possible height taking into account the HVAC, structure and other possible obstructions. When installed, there shall be enough room such that removal of all ceiling tile is still possible for access.
6. Upon completion, the new drop ceiling system shall meet the edges of the room, be level throughout and appear uniform and complete.
0. Paint.
7. The Contractor shall prepare and paint all, existing doors, radiator covers, window framing and trim surfaces within building 110 Room 139, to include and all surrounding walls.
7. All paint work shall be performed in accordance with WVA Specification 09 90 00 - INTERIOR PAINTS AND COATINGS.
7. Colors and sheen are to be chosen by Contracting Officer Representative after a color wheel or samples are submitted for review and approval.
7. At completion of painting phase, walls and ceiling shall be smooth and even coated in appearance with no bleed through. Painting of walls and ceiling shall have a professional neat appearance. Doors and windows shall operate without interference. Any over-brushed paint shall be removed and the underlying surface repaired. Visible differences in paint sheen and or texture shall not occur when completed.
7. At completion of the painting phase, finished areas shall be smooth and evenly coated in appearance. Painting of walls, trim, doors and windows shall appear neat and professional.
0. Baseboard Trim:
8. The Contractor shall provide and install new 6 inch wood baseboard trim as described throughout the perimeter of all renovated spaces in building 110, Room 139.
8. The Contractor shall terminate at all openings and use matching caulk to complete as necessary.
0. Electrical Outlets/ Switches and plates:
9. The Contractor shall adhere to the drawing set and install electrical boxes above the drop ceiling in various locations for power and data poles to be installed not in contract.
9. The contractor shall replace all wall outlets and light switches with new receptacles and plates. Color shall be white.
9. The contractor is responsible for counting the number of switches and outlets and confirming with government official.
9. The contractor shall identify and label all new circuits/used.
9. The contractor shall adhere to NFPA 70 – National Electric Code (NEC).
9. Where outlets is within 6 feet of a water source the outlet shall be GFI.
9. Where electrical has been removed, this shall be taken all the way back to the panel and labeled accordingly.
4.0.10 Resilient Plank Flooring
a. The contractor shall install luxury resilient plank flooring in the specified areas of building 110, room 139 per the drawing and flooring schedule.
b. The contractor is responsible for all measurements in regard to quantity required for install.
c. The direction of the flooring shall be installed as described in the drawing set. Where the drawing was incapable of showing direction or pattern, the contractor shall confirm with the Government prior to laying floor.
d. The contractor shall adhere to the installation instructions provided by the manufacturer.
e. When complete, the flooring shall have no visible gaps, and the joints shall be no closer than 4” to the next nearest joint.
f. The contractor shall install matching thresholds to the doorway, except in the cases where the material require no transition.
g. When complete, the height differential between the hall and room shall be no more than 1/4”.
4.0.11 Thermostats:
a. The contractor shall move the thermostat to the south side of the entry door.
b. The wires shall be run into the wall cavity for a neat and clean appearance.
c. When complete the thermostat shall be fully functional.
4.0.12 Ductless Split Unit A/C:
a. Installation of new single zone ~ 2 ton ductless split AC. One indoor unit will be ~24,000 BTU wall mounted
b. The contractor shall provide new pre-insulated line set running from the indoor unit to the condensing unit; this line set shall run outside the building extending to the condensing unit, the condensing unit shall be installed on a wall mounted bracket on the North side of the building providing a minimum of 36” clearance to the front and sides of the new condenser, this unit shall be mounted approximately 4’ from ground level. Condenser wall bracket paint shall match finish of condensing unit.
c. Penetrations through exterior wall shall be core drilled, sleeved, properly sealed and caulked. Contractor to ensure adequate clearance on all sides of lines inside sleeve.
d. Contractor shall provide and install a line set enclosure, brown in color, to encase the line set on the exterior of the building, copper line set shall be installed to look aesthetically pleasing. Contractor shall properly fasten this enclosure in a neat and orderly manner.
e. Contractor shall install line set, electrical and condensate lines for indoor units above drop ceiling and inside wall so it is not visible.
f. Indoor units shall be mounted on interior walls at a height of ~ 8 ft. location to be shown on site visit
g. Contractor shall supply pumps, or equivalent for condensate removal from each indoor unit. Condensate lines will be run outside through enclosure and extend past the bottom of the enclosure. Condensate line shall be rigid copper pipe leaving the bottom of the enclosure. Pipe must be securely fastened to wall and discharge to a height of no more than 18” above ground level
h. Low voltage wiring and line voltage wiring shall be connected to new units using liquid tight flexible conduit from disconnect box into the condensing unit.
i. Contractor shall insure the new air conditioning unit is installed properly according to manufacturer recommendations and properly charged, started and tested.
j. All refrigerant fittings shall connect with factory supplied flare fittings where applicable, any other connections shall be brazed connections, using brazing rod containing no less than 15% Silver.
CLOSEOUT REQUIREMENTS
4.0.13 Construction and Demolition Debris
a. The Contractor shall submit copies of all Bills of Lading and Weight Slips associated with the disposal of construction and demolition (C&D) debris and/or recycled materials, under this scope of work. Submit documentation per Appendix A, “Consolidated List of Submittals”.
4.0.14 Cleanup of Work Site
a. The contactor shall clean all work areas and remove all tools, equipment, debris, materials, and all waste from the premises.
b. The contractor shall remove all debris from any remote holding locations, to include any excavation and concrete equipment chute cleanout debris.
c. The contractor shall remove their equipment from Watervliet Arsenal premises within five (5) working days after completion of project work.
4.0.15 Restoration of Work Site
a. The contractor shall repair any items, building surfaces, equipment or miscellaneous items which were damaged during the execution of this SOW. Items that cannot be repaired to restore original function or aesthetics shall be replaced in kind by the Contractor at no additional charge to the Government.
b. Grass, trees, and other vegetation affected by project work shall be restored to same or better condition as existed prior to the start of project work. The Contractor shall prepare the affected areas to eliminate uneven areas and low spots. Remove foreign materials, weeds, and damaged soil. The Contractor shall scratch the area to a depth of three (3) inches where topsoil is to be placed. Spread topsoil to a uniform, compacted depth of less than five (5) inches over areas to be reseeded. Rake until smooth. The Contractor shall seed, fertilize, and water the area to establish grass.

4.0.17 Warranty
a. The contractor shall provide a [1] year warranty on all construction related items.

APPENDIX B: Quality Assurance Surveillance Plan (QASP)
PURPOSE
This Quality Assurance Surveillance Plan (QASP) will be used as a guide to Government Quality Assurance Personnel (QAP). It provides a systematic method to evaluate the services the contractor is required to furnish.
SURVEILLANCE PROCEDURES
Timeliness: Progress is maintained to assure compliance with completion time.
Quality: Conformance with statement of work and industry standards.
Tasks: Performed according to statement of work requirements.
Problem Resolution: Contractor submits deliverables in accordance with the contract and resolves any deficiencies identified by the government in the final submission. Following final submission, contractor resolves any additional deficiencies identified by the government and resubmits and or corrects within one week.
Delivery: Contractor submits each deliverable in accordance with the statement of work and terms of the contract.
PERFORMANCE AREAS
Management Responsiveness: Evaluate the adequacy of the contractor’s responsiveness and their ability to keep the government informed of work progress and provides for early problem identification and effective corrective action plans.
Schedule Control: Evaluate the contractor’s adherence to completion schedule.
Performance: The contractor will be evaluated as to the workmanship and adherence to the statement of work.
EVALUATION CRITERIA
(Outstanding). Indicates performance clearly exceeds contractual requirements. The area of evaluation contains few minor problems for which corrective actions appear highly effective.
(Satisfactory). Indicates performance clearly meets contractual requirements. The area of evaluation contains some minor problems for which the corrective actions appear satisfactory.
(Unsatisfactory). Indicates the contractor failed to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains serious problems for which the corrective actions were ineffective.
DOCUMENTATION
Documentation used to perform surveillance will consist of random government inspections and inspection records, as well as the evaluation of final work product delivery by the project COR/COTR.
RATING
Contractor will be given an overall rating of Outstanding, Satisfactory, or Unsatisfactory by the Government Technical point of contact or Contracting Specialist Representative, if appointed, using the above 3 evaluation criteria.
