STATEMENT OF WORK (SOW)

FOR

Install Chain Curtains in Storm Drain System

AT

Watervliet Arsenal

POST

Project #: 6PW228

Installation Management, Public Works Division Facilities Engineering (TAWV-IMP-F)

PART 1 GENERAL

1.01 Overview.

A. This Statement of Work (SOW) defines the effort required for the fabrication and installation of seven

(7) chain curtains at four (4) locations in the storm drain system to mediate odor from discharge of the Industrial Waste Treatment Plant (IWTP).

This project will also include repair/replacement of three (3) deteriorated catch basins. Locations are indicated on the Malcolm Pirnie General Sanitary and Storm Sewer System Map, attached.

1.02 Hours of Operation.

A. Normal duty hours are 7:00 AM through 4:00 PM, Monday through Thursday, and 7:00 AM through

3:00 PM on select Fridays (the arsenal is closed every-other Friday). Federal holidays are non-work days. If a federal holiday falls on a Saturday or Sunday, the preceding Friday or following Monday is a work holiday.

1.03 General Requirements.

A. The contractor shall perform all work in strict conformance with Watervliet Arsenal Instructions for Contractors (WAIC). The contractor shall read and understand this document prior to submitting a proposal. The WAIC is publically available at:

B. The contractor shall supply all tools, materials, supplies, equipment, labor, temporary utilities and supervision necessary for the complete execution of the requirements within the contract documents and requirements of this SOW.

C. The contractor shall, prior to submitting a proposal, review (i) all contract documents, and (ii) the project work site, and shall provide written notice to Contracting Specialist detailing any discrepancies that are found between the contract documents and the project work site.

D. The contractor shall notify the Contracting Specialist a minimum of two (2) business days before the contractor starts work. The contractor shall notify the COR a minimum of seven (7) business days before the shut-down of a utility or building service.

E. The contactor shall clean all work areas, at the end of each work day, to remove all tools, equipment, debris, materials, and all waste from the work site.

1.04 Safety Requirements.

A. General.

1. The contractor shall be responsible for complying with all DOD, Army, Federal, and State health and safety requirements which are applicable to this project, and shall use all means and precautions available to ensure the safety of the occupants and protection to the existing buildings and structures.

B. Existing Conditions.

1. The contractor shall inspect the project work site prior to starting work, and shall request a copy of the asbestos building inspection records to verify the location and description of known asbestos-containing materials. The contractor shall sign the “Asbestos Notification Acknowledgement Record” and submit this form to the Project COR prior to starting project work.

A. Contractor Safety Plan.

1. The contractor shall submit a safety plan which is specific to the work activity being performed under the scope of this project for Government review and approval.

1.02 Period of Performance.

A. The contractor shall be required to (i) commence work under this contract within 7 calendar days after the date the contractor receives notice to proceed, (ii) prosecute the work diligently, and (iii) complete the entire work ready for use not later than [30] calendar days after notice to proceed was given. The time stated for completion shall include final cleanup of the premises.

1.03 Project Schedule.

A. The contractor shall create a project schedule and submit for Government review and approval. The project schedule shall detail the phases of work and the overall timeline for the execution of the work.

The contractor shall include delivery dates, appropriate milestones, and shall clearly detail any requirements for work performed outside of regular hours of operation.

1.04 Permits.

A. The Contractor shall obtain the following WVA Permits from the Contracting Officer’s

Representative (COR), IAW the Watervliet Arsenal Information for Contractors (WAIC). The Project COR will provide the Permit forms and the direction necessary to complete each Permit.

1. Hot Work (Flame) Permit

2. Confined Space Entry Permit

3. Utility Marking Permit

1.05 Equipment Identification.

A. The contractor shall create and affix a label to all large equipment, dumpsters, and other mobilized property, which is weather-resistant (laminated, or within a see-thru label hanger), securely affixed to the equipment or visible through a window, and which clearly displays the General Contractor’s:

1. Company name

2. POC name (first, last)

3. POC phone number

4. Project Number (see title page of SOW)

1.06 Project Drawings.

A. The contractor shall complete the Items of Work in accordance with the following associated Project Drawing(s), attached to this SOW.

1. Chain Curtain Diagram.

B. The Project Drawing(s) do not indicate each component, device, fastener, fixture, or similar incidental item necessary for the performance of the work. The contractor shall provide, furnish, and install all necessary and appropriate components, devices, fasteners, fixtures, or similar incidental items as necessary and required to provide complete, operable, and functional system to suit the intended purpose.

C. Project Drawings are illustrative in nature. The contractor is required to review the project site, and obtain and/or verify all measurements and project data/information required to perform this project work. In addition, the contractor is required to review all contract documents, and provide written notice to Contracting Officer of any discrepancies that are found

1.08 Submittals.

A. The contractor shall e-mail all submittals to the Contracting Specialist for review and approval. All submittals require government approval unless otherwise explicitly stated.

B. The contractor shall reference APPENDIX A “Consolidated List of Submittals” for complete list of required submittals.

PART 2 PRODUCTS

2.01 General.

A. Unless otherwise stated, all materials for this project shall be new. All products shall not contain any asbestos, mercury, chromium/lead paint, Class I Freon, and Polychlorinated Biphenyls (PCB), and shall state such on the manufacturer’s original packaging.

B. All work shall be in strict accordance with product manufacturer’s instructions and specifications, including but not limited to manufacturer guidance for storage, staging, application, installation, and disposal of the product.

C. All construction materials shall meet the applicable requirements of the DoD Green Procurement Program, specifically recycled content products, environmentally preferable products and services, biobased products, energy-and water-efficient products, alternate fuel vehicles and alternative fuels, products using renewable energy, and alternatives to hazardous or toxic chemicals as found at <http://www.acq.osd.mil/dpap/cpic/cp/docs/USA001967-08-DPAP.pdf>.

2.02 Products and Materials.

A. Chain:

1. Stainless Steel Twist Link Chain as manufactured by Suncor Stainless or Equivalent.

a. Grade 304 Stainless Steel

B. Hardware:

1. Chain Swivel Snaps as manufactured by Suncor Stainless or Equivalent.

a. Grade 316 Stainless Steel

2. Welded Eye Bolt with Nut and Washers

a. Standard UNC Thread

b. Grade 304 Stainless Steel

3. Fasteners

a. ¼” X 2-1/4” Tapcon Screws

i. Slotted Hex Washer Head

ii. Grade 410 Stainless Steel

C. Angle Steel:

1. 6061-T6 Aluminum.

D. Concrete:

1. Compressive Strength of 5,000 psi after 28 days.

PART 3 ITEMS OF WORK

3.01 Storm Sewer Manhole #6.

A. The contractor shall refer to the Malcolm Pirnie General Sanitary and Storm Sewer System Map, attached, for exact location.

B. The contractor shall maintain the free flow of vehicle and pedestrian traffic during project work through the use of traffic control devices, including flagmen, traffic cones, detour signs, signal devices, and flashers.

1. Contractor personnel acting as road guides shall be equipped with reflective vests and comply with all applicable safety regulations.

C. The contractor shall fabricate and install two (2) chain curtains, one at each 36” invert, to prevent the backdraft of unpleasant odors from the discharge of the IWTP.

D. The curtains shall be constructed to cover the invert opening, but NOT restrict the flow of storm water through the Reinforced Concrete Pipes (RCP’s). Curtains shall consist of the following components for corrosion resistance and durability:

1. 1/8” Stainless Steel Twist Link Chain

2. 1” x 1” x ¼” Aluminum Angle

3. 3/16” Stainless Steel Chain Swivel Snaps

4. 3/16” Stainless Steel Eye Bolts with Lock Nuts and Washers

E. The contractor shall refer to the “Chain Curtain” diagram, attached, for assembly of the aforementioned items.

F. The contractor shall affix the aluminum angle to the manhole structure using stainless steel Tapcon screws suitable for the application. Contractor shall follow all manufactures recommendations during installation.

G. The contractor shall obtain a Confined Space Entry Permit from the Project COR IAW the Watervliet Arsenal Information for Contractors (WAIC) prior to commencement of any work within the manhole.

H. The contractor shall exercise caution so as to avoid damage to the manhole structure and masonry.

Contractor shall be responsible to repair any such damage at no additional cost to the Government.

I. The contractor shall periodically check all components during installation to ensure they are in their proper position, free from damage, and that all connections are tight.

3.02 Storm Sewer Manhole #3C.

A. The contractor shall refer to the Malcolm Pirnie General Sanitary and Storm Sewer System Map, attached, for exact location.

B. The contractor shall maintain the free flow of vehicle and pedestrian traffic during project work through the use of traffic control devices, including flagmen, traffic cones, detour signs, signal devices, and flashers.

1. Contractor personnel acting as road guides shall be equipped with reflective vests and comply with all applicable safety regulations.

C. The contractor shall fabricate and install three (3) chain curtains, one at each 36” invert and one at the

16” invert, to prevent the backdraft of unpleasant odors from the discharge of the IWTP.

D. The curtains shall be constructed to cover the invert opening, but NOT restrict the flow of storm water through the RCP’s or the Vitrified Clay Pipe (VCP). Curtains shall consist of the following components for corrosion resistance and durability:

1. 1/8” Stainless Steel Twist Link Chain

2. 1” x 1” x ¼” Aluminum Angle

3. 3/16” Stainless Steel Chain Swivel Snaps

4. 3/16” Stainless Steel Eye Bolts with Lock Nuts and Washers

E. The contractor shall refer to the “Chain Curtain” diagram, attached, for assembly of the aforementioned items.

F. The contractor shall affix the aluminum angle to the manhole structure using stainless steel Tapcon screws suitable for the application. Contractor shall follow all manufactures recommendations during installation.

G. The contractor shall obtain a Confined Space Entry Permit from the Project COR IAW the Watervliet Arsenal Information for Contractors (WAIC) prior to commencement of any work within the manhole.

H. The contractor shall exercise caution so as to avoid damage to the manhole structure and masonry.

C. The curtains shall be constructed to cover the invert opening, but NOT restrict the flow of storm water through the tunnel. Curtains shall consist of the following components for corrosion resistance and durability:

1. 1/8” Stainless Steel Twist Link Chain

2. 1” x 1” x ¼” Aluminum Angle

3. 3/16” Stainless Steel Chain Swivel Snaps

4. 3/16” Stainless Steel Eye Bolts with Lock Nuts and Washers

D. The contractor shall refer to the “Chain Curtain” diagram, attached, for assembly of the aforementioned items.

E. The contractor shall affix the aluminum angle to the manhole structure using stainless steel Tapcon screws suitable for the application. Contractor shall follow all manufactures recommendations during installation.

B. The contractor shall jack hammer existing concrete apron and excavate as needed to remove the existing catch basin.

C. The contractor shall follow all dig safe procedures prior to excavating.

D. The contractor shall dispose of all debris generated during the course of this project. The contractor shall haul pavement and soil to Carver Sand and Gravel or Mead Enterprise for recycling. A bill of lading shall be provided to the Project COR at the end of each work day to verify where the pavement and soil was disposed.

E. The contractor shall install a new precast catch basin in kind to existing.

F. Prior to ordering precast portion of new catch basin structure, the Contractor shall verify proposed invert elevation, and size and direction of all lateral and underdrain pipes.

G. Where existing lateral pipe is to be reconnected to new catch basin, existing lateral pipe is to be thoroughly cleaned of all extraneous material before making connection and grouting in place.

H. The structure is not to be backfilled until all mortar has completely set.

I. Material for leveling and backfilling under and around catch basin, is to be select granular fill in accordance with NYSDOT Section 203-2.06.

J. The contractor shall reinstall the existing grating and drain collar. Frame and cover are to be placed true to line and grade.

K. The contractor shall install a concrete apron around the new catch basin per Specification 32 WVA

100.

3.06 Replace Catch Basin, South of Building 115.

B. The contractor shall saw cut the pavement a minimum of thirty-two (32) inches from the catch basin frame and excavate as needed to remove the existing catch basin.

C. The contractor shall follow all dig safe procedures prior to excavating.

D. The contractor shall dispose of all debris generated during the course of this project. The Contractor shall haul pavement and soil to Carver Sand and Gravel or Mead Enterprise for recycling. A bill of lading shall be provided to the Project COR at the end of each work day to verify where the pavement and soil was disposed.

E. The contractor shall install a new precast catch basin in kind to existing.

F. Prior to ordering precast portion of new catch basin structure, the Contractor shall verify proposed invert elevation, and size and direction of all lateral and underdrain pipes.

G. Where existing lateral pipe is to be reconnected to new catch basin, existing lateral pipe is to be thoroughly cleaned of all extraneous material before making connection and grouting in place.

3.07 Repair Catch Basin, Northwest Corner of “Body Forge”, Building 40-6.

attached, for exact location.

B. The contractor shall complete all work in accordance with Specification 32 WVA 100 – Resetting

Manholes, attached.

C. The contractor shall saw cut the pavement a minimum of thirty-two (32) inches from the catch basin frame and excavate as needed to expose the underground structure. Removal of the adjacent curb and sidewalk is not required. Any damage to the sidewalk or curb as a result of excavation work shall be the responsibility of the contractor.

D. The contractor shall follow all dig safe procedures prior to excavating.

E. The contractor shall remove all deteriorated blocks, bricks, and concrete down to sound structural material.

F. The contractor shall dispose of all debris generated during the course of this project. The contractor shall haul pavement and soil to Carver Sand and Gravel or Mead Enterprise for recycling. A bill of lading shall be provided to the Project COR at the end of each work day to verify where the pavement and soil was disposed.

G. Upon project completion, the contractor shall clean out catch basin and dispose of all contents.

PART 4 CLOSEOUT REQUIREMENTS

4.01 Cleanup of Work Site.

A. The contactor shall clean all work areas and remove all tools, equipment, debris, materials, and all waste from the premises.

B. The contractor shall remove all debris from any remote holding locations, to include any excavation and concrete equipment chute cleanout debris.

C. The contractor shall remove their equipment from Watervliet Arsenal premises within five (5) working days after completion of project work.

APPENDIX B: Quality Assurance Surveillance Plan (QASP)

PURPOSE

This Quality Assurance Surveillance Plan (QASP) will be used as a guide to Government Quality Assurance Personnel (QAP). It provides a systematic method to evaluate the services the contractor is required to furnish.

SURVEILLANCE PROCEDURES

Timeliness: Progress is maintained to assure compliance with completion time.

Quality: Conformance with statement of work and industry standards.

Tasks: Performed according to statement of work requirements.

Problem Resolution: Contractor submits deliverables in accordance with the contract and resolves any deficiencies identified by the government in the final submission. Following final submission, contractor resolves any additional deficiencies identified by the government and resubmits and or corrects within one week.

Delivery: Contractor submits each deliverable in accordance with the statement of work and terms of the contract.

PERFORMANCE AREAS

Management Responsiveness: Evaluate the adequacy of the contractor’s responsiveness and their ability to keep the government informed of work progress and provides for early problem identification and effective corrective action plans.

Schedule Control: Evaluate the contractor’s adherence to completion schedule.

Performance: The contractor will be evaluated as to the workmanship and adherence to the statement of work.

EVALUATION CRITERIA

(Outstanding). Indicates performance clearly exceeds contractual requirements. The area of evaluation contains few minor problems for which corrective actions appear highly effective.

(Satisfactory). Indicates performance clearly meets contractual requirements. The area of evaluation contains some minor problems for which the corrective actions appear satisfactory.

(Unsatisfactory). Indicates the contractor failed to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains serious problems for which the corrective actions were ineffective.

DOCUMENTATION

Documentation used to perform surveillance will consist of random government inspections and inspection records, as well as the evaluation of final work product delivery by the project COR/COTR.

RATING

Contractor will be given an overall rating of Outstanding, Satisfactory, or Unsatisfactory by the Government Technical point of contact or Contracting Specialist Representative, if appointed, 3 using the above evaluation criteria.

Resetting Manholes and Catch Basins

1.01 REFERENCES

PART 3 EXECUTION

3.01 Cutting and Excavation

A. Contractor shall saw cut the concrete/ pavement a minimum of eighteen (18) inches from the manhole/ catch basin frame.

1. Contractor shall remove all asphalt, concrete and the manhole/catch basin frame itself.

2. Contractor shall be sure to not allow any material to accumulate at the bottom of the catch basin.

B. Contractor shall remove all debris and excavate around the manhole to expose the underground structure.

1. All deteriorated blocks, bricks, and concrete shall be removed down to sound structural material.

2. Contractor shall compact all exposed soil and remove undesirable backfill material.

3. Contractor shall clean out the catch basin and dispose of all contents.

3.02 Rebuilding and Setting

A. Contractor shall repair the structure to reinstall the frame.

1. Install new cement bricks, patch concrete and grout to reset the existing frame onto the manhole structure. Ensure proper final elevation of top of frame by properly sizing brick and grout placement. All storm sewer frames shall be placed one (1) inch below the existing perimeter surface elevation. All other frames shall slightly depress (1/2”) from the existing perimeter surface elevation.

2. Contractor shall install a new frame and manhole cover/ catch basin on new foundation.

Frame shall be flat and level. Frame shall be of same dimensions as existing frame and conform to all ASTM and DOT standards.

B. Concrete Apron

1. Contractor shall backfill around the new manhole/ catch basin. The backfill shall be compacted in six inch lifts and meet all DOT standards for roadways. The top six (6) inches shall consist of compacted crushed stone.

2. Using 5000 psi concrete, fill the perimeter area around frame, forming an apron. The apron’s surface shall be finished smooth with square edges. At storm sewers, an evenly sloped smooth sloped surface shall be provided, which facilitates drainage in the structure. Perimeter edge shall be troweled.

3. The minimum thickness of the apron shall be at least six (6) inches thick.

4. Roadway barricades shall be maintained during curing period. 6 mil minimum thickness, plastic sheeting shall cover concrete apron during the seven (7) day curing period.

5. After a minimum of seven (7) days the perimeter of the apron shall be sealed with a hot asphalt emulsion. The joint shall be cleaned of all dirt and debris prior to application. The joint sealant shall be applied according to all manufacturers’ instructions.

1.01 Scope.

A. This Statement of Work (SOW) defines the effort required for the repair and repointing of the brick wall adjacent to the Motor Pool, Building 15. This SOW includes the associated project management, code and regulation compliance, and quality assurance requirements.

1.02 Existing Conditions.

A. Due to water penetration and freeze/thaw cycles, the mortar has loosened and the wall has begun to separate at the joints. To keep the wall stable and prevent any safety concerns, the wall has been shored until a permanent repair is made.

1.03 Submittals.

A. The contractor shall e-mail all submittals to the Contracting Specialist for review and approval. All submittals require government approval unless otherwise explicitly stated.

B. The contractor shall reference APPENDIX A “Consolidated List of Submittals” for complete list of required submittals.

1.04 Hours of Operation.

A. Normal duty hours are 7:00 AM through 4:00 PM, Monday through Thursday, and 7:00 AM through 3:00 PM on select Fridays (the arsenal is closed every-other Friday). Federal holidays are non-work days. If a federal holiday falls on a Saturday or Sunday, the preceding Friday or following Monday is a work holiday.

1.05 General Requirements.

A. The contractor shall perform all work in strict conformance with Watervliet Arsenal Instructions for Contractors (WAIC). The contractor shall read and understand this document prior to submitting a proposal. The WAIC is publically available at:

B. The contractor shall supply all tools, materials, supplies, equipment, labor, temporary utilities and supervision necessary for the complete execution of the requirements within the contract documents and requirements of this SOW.

C. The contractor shall, prior to submitting a proposal, review (i) all contract documents, and (ii) the project work site, and shall provide written notice to Contracting Specialist detailing any discrepancies that are found between the contract documents and the project work site.

D. The contractor shall notify the Contracting Specialist a minimum of two (2) business days before the contractor starts work. The contractor shall notify the COR a minimum of seven (7) business days before the shut-down of a utility or building service.

E. The contactor shall clean all work areas, at the end of each work day, to remove all tools, equipment, debris, materials, and all waste from the work site.

1.06 Safety Requirements.

A. The contractor shall be responsible for complying with all DOD, Army, Federal, and State health and safety requirements which are applicable to this project, and shall use all means and precautions available to ensure the safety of the occupants and protection to the existing buildings and structures.

B. The contractor shall inspect the project work site prior to starting work, and shall request a copy of the asbestos building inspection records to verify the location and description of known asbestos-containing materials. The contractor shall sign the “Asbestos Notification Acknowledgement Record” and submit this form to the Contracting Specialist.

1.07 Period of Performance.

A. The contractor shall be required to (i) commence work under this contract within 7 calendar days after the date the contractor receives notice to proceed, (ii) prosecute the work diligently, and (iii) complete the entire work ready for use not later than [21] calendar days after notice to proceed was given. The time stated for completion shall include final cleanup of the premises.

1.08 Project Schedule.

A. The contractor shall create a project schedule and submit for Government review and approval. The project schedule shall detail the phases of work and the overall timeline for the execution of the work.

The contractor shall include delivery dates, appropriate milestones, and shall clearly detail any requirements for work performed outside of regular hours of operation.

1.09 Equipment Identification.

A. The contractor shall create and affix a label to all large equipment, dumpsters, and other mobilized property, which is weather-resistant (laminated, or within a see-thru label hanger), securely affixed to the equipment or visible through a window, and which clearly displays the General Contractor’s:

2. POC name (first, last)

3. POC phone number

4. Project Number: “5PW029”

1.10 Applicable Documents.

A. Watervliet Arsenal Instructions for Contractors (WAIC).

2.01 General.

A. Unless otherwise stated, all materials for this project shall be new. All products shall not contain any asbestos, mercury, chromium/lead paint, Class I Freon, and Polychlorinated Biphenyls (PCB), and shall state such on the manufacturer’s original packaging.

B. All work shall be in strict accordance with product manufacturer’s instructions and specifications, including but not limited to manufacturer guidance for storage, staging, application, installation, and disposal of the product.

C. All construction materials shall meet the applicable requirements of the DoD Green Procurement Program, specifically recycled content products, environmentally preferable products and services, biobased products, energy-and water-efficient products, alternate fuel vehicles and alternative fuels, products using renewable energy, and alternatives to hazardous or toxic chemicals as found at <http://www.acq.osd.mil/dpap/cpic/cp/docs/USA001967-08-DPAP.pdf>.

2.02 Materials.

A. Mortar

1. Mortar shall be of similar characteristics of the existing mortar.

2. This includes strength, (Typically Type “k” for the historic masonry at the Arsenal), texture, hardness, and color.

B. Replacement Bricks

1. Replacement bricks shall be of similar vintage as the surrounding bricks in size, color, texture, material strength etc.

C. Sealer

1. SealClear by Sealoflex Waterproofing Systems or equivalent

D. Crack control reinforcing

1. Thor Helical Crack Stitching Bars or equivalent

E. Paint

1. PPG Pitthane 35 Gloss Urethane Enamel or equivalent

PART 3 ITEMS OF WORK

3.01 Repair Brickwork.

A. The Contractor shall protect the historic wall structure to the greatest extent possible. Contractor shall use the gentlest means to perform the work and take the greatest of care to ensure that the historic materials are not damaged in the process of the work.

B. The Contractor shall remove temporary shoring as needed while working around the wall structure.

C. The Contractor shall keep the roadway (Arnold Lane) open to vehicle and pedestrian traffic at all times.

D. The Contractor shall set up a safety perimeter around the work zone and maintain all barricades, signs, etc.

E. The Contractor shall remove all unsound, weak, or damaged masonry and mortar. Mortar shall be removed to a depth of at least one inch or 2 ½ times the width of the mortar joint, whichever is greater, or until sound mortar is reached.

F. Loose particles, laitance, spalling, cracked, or debonded masonry and mortar and foreign materials shall be removed. Contractor shall be cautious with powered equipment so that the existing brick is not damaged.

G. Surfaces prepared for repair shall be cleaned free of dust, dirt, masonry chips, oil or other contaminants, rinsed with water, and dried before repair work is begun.

H. The Contractor shall protect surfaces of the structure, and surfaces adjacent to the work area from damage which may result from removal, cleaning, and repair operations.

I. The Contractor shall stitch all cracks using helical bed joint reinforcement.

1. Reinforcement shall be long enough to extend a minimum of twenty (20) inches beyond either side of the crack.

2. Where a crack is less than twenty (20) inches from the end of a wall or an opening, the reinforcement shall be continued for at least eight (8) inches around the corner and bonded into the adjoining wall.

J. The Contractor shall repoint joints to match the rest of the wall after loose bricks have been reinstalled and adjusted.

K. New mortar shall match existing in color, texture and hardness. Mortar shall be “Type K” or be appropriate for the structure.

L. Once the mortar is hardened to an appropriate degree, the joint shall be tooled to match the surrounding joints.

M. The Contractor shall pre-wet the substrate with water prior to the application of the repair material to prevent the substrate from drawing out the moisture too quickly. Contractor shall re-wet the surface with water again immediately before applying the repair material.

N. The Contractor shall mix and apply the mortar in accordance with manufacturer’s instructions.

O. The Contractor shall brush diagonally across the wall with a wire brush to remove any mortar crumbs after mortar is firm to the touch. Contractor shall sponge any mortar residue from the brick.

P. The Contractor shall place a tarp over the wall for at least three days to protect it from the weather.

The tarp shall only be removed to give the wall a daily misting.

Q. After those three days, the contractor shall apply a water repellant coating to the brick.

1. The coating shall penetrate and not seal, it shall be specified for use in an exterior setting.

2. The product shall be permeable and allow air flow.

3. The coating shall be silicone or siloxane based, with a 15% solids formula.

4. The contractor shall apply coating according to all of the manufacturer’s recommendations.

5. The product shall be similar to SealClear by Sealoflex Waterproofing Systems, or other approved equivalent.

3.02 Paint Metal Coping.

A. The Contractor shall paint approximately 250 lineal feet of coping that rests atop the brick wall.

B. The Contractor shall remove any surface rust or loose mill scale.

C. Rusty metal surfaces shall be cleaned as per SSPC requirements. Solvent, mechanical, or chemical cleaning methods shall be used to provide surfaces suitable for painting.

D. Heavy rusted areas shall be treated with a rust penetrating sealer and followed with a zinc rich primer coat. Rust penetrating sealer shall be compatible with the paint/primer used and shall be applied in accordance with manufacturer’s instructions.

E. All bare steel shall be primed with a zinc rich primer the same day as cleaned.

F. The Contractor shall apply at least two (2) coats of paint to all existing surfaces in accordance with all manufacturer’s recommendations and requirements.

G. Paint shall be Philly brown in color and approved for use on exterior projects.

H. Each coat shall be applied in a uniform layer, completely covering the preceding coat. The Contractor shall correct any spotting, laps, holidays, brush marks, roller tracking, runs, sags, or other surface imperfections before succeeding coats.

I. All paint coatings selected shall be produced by the same manufacturer and used throughout the entire project.

J. If fresh paint is damaged by the elements, the Contractor shall replace or repair the paint at the discretion of the Project COR at no additional cost to the government.

K. The Contractor shall protect surrounding surfaces against damage during paint application. Contractor shall clean any spattered surfaces. Remove spattered paints by washing, scraping, or other methods.

Do not scratch or damage adjacent finished surfaces.

PART 4 CLOSEOUT REQUIREMENTS

4.01 Cleanup of Work Site.

A. The contactor shall clean all work areas and remove all tools, equipment, debris, materials, and all waste from the premises.

B. The contractor shall remove all debris from any remote holding locations, to include any excavation and concrete equipment chute cleanout debris.

C. The contractor shall remove their equipment from Watervliet Arsenal premises within five (5) working days after completion of project work.

APPENDIX B: Quality Assurance Surveillance Plan (QASP)

PURPOSE

This Quality Assurance Surveillance Plan (QASP) will be used as a guide to Government Quality Assurance Personnel (QAP). It provides a systematic method to evaluate the services the contractor is required to furnish.

SURVEILLANCE PROCEDURES

Timeliness: Progress is maintained to assure compliance with completion time.

Quality: Conformance with statement of work and industry standards.

Tasks: Performed according to statement of work requirements.

Problem Resolution: Contractor submits deliverables in accordance with the contract and resolves any deficiencies identified by the government in the final submission. Following final submission, contractor resolves any additional deficiencies identified by the government and resubmits and or corrects within one week.

Delivery: Contractor submits each deliverable in accordance with the statement of work and terms of the contract.

PERFORMANCE AREAS

Management Responsiveness: Evaluate the adequacy of the contractor’s responsiveness and their ability to keep the government informed of work progress and provides for early problem identification and effective corrective action plans.

Schedule Control: Evaluate the contractor’s adherence to completion schedule.

Performance: The contractor will be evaluated as to the workmanship and adherence to the statement of work.

EVALUATION CRITERIA

(Outstanding). Indicates performance clearly exceeds contractual requirements. The area of evaluation contains few minor problems for which corrective actions appear highly effective.

(Satisfactory). Indicates performance clearly meets contractual requirements. The area of evaluation contains some minor problems for which the corrective actions appear satisfactory.

(Unsatisfactory). Indicates the contractor failed to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains serious problems for which the corrective actions were ineffective.

DOCUMENTATION

Documentation used to perform surveillance will consist of random government inspections and inspection records, as well as the evaluation of final work product delivery by the project COR/COTR.

RATING

Contractor will be given an overall rating of Outstanding, Satisfactory, or Unsatisfactory by the Government Technical point of contact or Contracting Specialist Representative, if appointed, 3 using the above evaluation criteria.

ASTM C881/C881M (2015) Standard Specification for Epoxy-Resin-Base Bonding Systems for Concrete

1.2 DEFINITIONS

1.2.1 Epoxy Resin Binder

A two-component epoxy bonding system in low and medium viscosities used by itself as a primer or for producing epoxy concrete or mortars when mixed with aggregate.

1.2.2 Epoxy Concrete

A combination of epoxy resin binder and fine and coarse aggregate used in the repair of spalling along joints or cracks, small surface spalls or "popouts."

1.2.3 Epoxy Mortar

A combination of epoxy resin binder and fine aggregate used in the surface repair of non-structural cracks and filling of saw kerfs.

1.2.4 Non-Pressure Epoxy Grout

A combination of epoxy resin binder, a mineral filler and a thixotropic agent used in cementing dowels in place and the repair of non-structural cracks.

1.2.5 Pressure Grouting Epoxy

A low viscosity epoxy resin system pumped under pressure into structural cracks in walls or pavements.
