SUMMARY OF CHANGES

SECTION A - SOLICITATION/CONTRACT FORM

The required response date/time has changed from 29-Sep-2017 11:00 AM to 16-Oct-2017 11:00 AM.

SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS

The following have been modified:

ADDENDUM TO SOLICITATION

INSTRUCTIONS TO OFFERORS

L.1.0 INTRODUCTION

L.1.1 The pending Virtual Patient Simulator Systems (VPSS) Whole Body Patient

Simulator (WBPS) Multiple Award Contract (MAC) will be comprised of two distinct commercial Lots both with the NAICS 333318:

Lot I – High Fidelity Patient Simulators (HFPS)

Lot II – Veterinary Simulators (VS)

L.1.1.1 This solicitation contains proposal instructions required for potential Original Equipment

Manufacturer (OEM) Offerors, as well as specific instructions for each Lot and its respective proposal volumes. The Government will ONLY consider offers from OEMs that have a current capability to produce commercial WBPSs in accordance with the SOW. See FAR Part 2.101 for the definition of “Commercial Item”.

All LOTs will be Full and Open Competition. The MAC will consist of a five-year ordering period. An Offeror may propose for one or more Lots and one or more commercial products within each Lot. The Government may consider set-asides for small businesses for future competed Delivery Orders.

L.1.1.2 Each Lot may result in one or more base contract awards. The Government intends to award approximately 10 contracts per Lot from this solicitation. The ID/IQ base contracts shall not be funded. All VPSS WBPS requirements/funded contract actions shall be issued on subsequent orders to the VPSS WBPS ID/IQ base contract. Each VPSS WBPS Lot awardee shall be provided a fair opportunity to compete for future VPSS WBPS requirements. The

Government intends to advertise these opportunities on the ACC-Orlando Business

Opportunities Board. Specifics of the first order to be issued to each VPSS WBPS awardee, meeting the contract minimum of $2,500, is outlined within section H.1 of this solicitation.

The Government will compete future ID/IQ orders using procedures of FAR Part 16.505.

L.1.1.3 All Offerors shall submit a single proposal in response to this solicitation. An Offeror may submit responses for all VPSS WBPS Lots. An Offeror shall only submit one VPSS WBPS commercial item per Lot proposed.

Offerors proposing on multiple Lots are instructed to submit one proposal with separate sections, as required. If multiple Lots are proposed, Volume I will require separate sections. The Offeror shall submit one section for Volumes II, III and IV, even if proposing multiple Lots. Volumes are identified, below, in section L.3.0.

Each VPSS WBPS Lot is represented by a separate CLIN on a single contract. Not proposing or qualifying for contract award in one Lot, does not preclude the Offeror from being awarded a contract for another proposed Lot.

VPSS WBPS contract awardees shall only be eligible for future VPSS WBPS requirements that correspond with the Lot(s) for which the base contract is awarded.

L.1.1.4 The Government anticipates an award date of 1st Quarter Fiscal Year 18 for this requirement. This information is provided for use as a basis for applicable cost and schedule calculations.

L.1.1.5 Submission Process. Proposal Volumes I-IV are due by 11:00 A.M. Eastern Time on 16

October 2017. The proposal shall be valid for 120 calendar days from the proposal due date.

L.1.2 All proposal submissions, regardless of submission date, shall be addressed to the solicitation Points of Contact (POCs). All questions and comments regarding this solicitation shall be submitted by electronic mail to the POCs with the solicitation number in the subject line utilizing the Questions and Comments Form, listed as Attachment 02 to the RFP. Phone inquiries regarding this solicitation are prohibited and will not be answered or acknowledged.

The POCs for this solicitation are Mr. Bobby Verica and Ms. Patricia Cole Soucy:

L.1.3 RFP Question and Answer Periods

For all VPSS WBPS Lots, potential Offerors may submit RFP questions and comments.

Deadline is 1:00 P.M. Eastern Time on 25 September 2017.

L.1.3.1 The Government will post its response(s) to appropriate questions and comments, without Offeror identification, to the FedBizOpps website. If the Government determines that a question or concern warrants a change to the solicitation, the change will be published through the issuance of a solicitation amendment prior to the deadline for proposal submissions. In the event that an amendment to the solicitation is issued, the Government may elect to extend the submission deadline date. Questions received after the referenced deadline in Section L.1.3 may not be answered.

L.1.3 Proposal Compliance. All proposals submitted must be in compliance with Section

L of this solicitation and must demonstrate a thorough understanding of the associated requirements stated in the evaluation criteria, the requirements identified in the VPSS WBPS base Statement of Work (SOW).

L.1.3.1 All proposals must clearly and convincingly demonstrate that the Offeror has a thorough understanding of the requirements and associated risks, and currently has commercially available products that meet the requirements. Mere reiteration of the requirement or standard reference material is inadequate. It is the Offeror’s responsibility to present sufficient information to be meaningfully evaluated without discussions. The Offeror is advised that quality of information is more important than quantity. The Offeror’s proposal shall be clear, concise, and include sufficient detail for effective evaluation and for substantiating the validity of stated claims. Clarity and readability are very important. The Government will not infer meanings from vague or ambiguous language (e.g., “The Offeror understands/can comply/will comply with the SOW;” “Standard procedures will be employed;” “Well known techniques will be used.”).

L.1.3.2 Any data previously submitted in response to any other solicitation should be assumed to be unavailable during this proposal evaluation and source selection process.

Proposal data shall not be incorporated into the proposal by referring to another proposal or source.

L.1.3.3 If a discrepancy exists between the original paper copy of the proposal and the

CD/DVD copy, the electronic version will take precedence.

L.1.3.4 Proposals shall not contain classified information.

mailto:patricia.c.soucy.civ@mail.mil

L.1.3.5 The Offeror shall not make any changes to the solicitation. Any objections to the terms and conditions of the solicitation shall be addressed to the Procuring Contracting Officer

(PCO) IAW L.8.2 (f).

L.1.3.6 Failure to submit all the required documentation in exactly the manner stated with regards to timeliness and format specified in the solicitation will constitute a basis for the

Government to reject a proposal as non-compliant and, therefore, unacceptable at the sole discretion of the PCO.

Furthermore, the Government may determine, at the discretion of the PCO, that the Offeror’s proposal is non-compliant and, therefore, ineligible for award if an Offeror indicates in its proposal that:

Offeror cannot or will not meet one or more contract requirements;

Offeror includes information that prompts the Government to question the Offeror’s compliance with one or more contract requirements.

L.1.4 The Government intends to evaluate proposals and award a contract without conducting discussions with Offerors. However, in the event discussions are in the best interest of the Government, the PCO will establish a competitive range prior to entering into discussions.

Therefore, the Offeror’s initial proposal should contain the Offeror’s best terms from a price and non-price standpoint.

L.1.5 If discussions are conducted, the PCO will schedule the discussion sessions, and each

Offeror will be notified of the time and place at least three (3) business days prior to their discussion session. The PCO will provide additional instructions with the notification. If discussions are deemed necessary, they will take place at a facility of the Government’s choosing in the Orlando, Florida area.

L.2.0 PROPOSAL DELIVERY/FORMAT/CONTENT REQUIREMENTS

L.2.1 The Offeror shall mark the outside shipping container with the solicitation number, W900KK-17-R-0030. The Offeror shall mail or hand-carry proposals to the POCs identified in

Section L.1.2.

L.2.2 Hand-carrying proposals or final proposal revisions (as applicable), is authorized. If hand-carrying the proposal, the Offeror bears the responsibility to contact both the Primary and

Alternate POC (as identified in Section L.1.2) via e-mail to coordinate the Government receiving agent, date, time, and place of delivery. The Offeror must e-mail the Government not less than

48 hours in advance to schedule delivery. This e-mail shall include:

1) The name of the Offeror;

2) The name of the individual delivering the proposal;

3) The individual’s office and cellular phone numbers.

L.2.3 Upon the delivery of any hand-carried proposal, the Offeror will obtain a Proposal

Receipt Form from a Government receiving agent designated by name by a POC identified in

Section L.1.2. The receiving agent will:

1) Annotate the date and time of proposal receipt;

2) Identify the number of boxes/packages received;

3) Sign in the “Signature of Receiving Agent” portion of the form; and,

4) Provide a copy of the Proposal Receipt Form to the Offeror.

The Government is not responsible for the proposal content or condition, and their signature only denotes the receipt of the proposal. Surrender of the proposal to any party not named as a

Solicitation POC will be considered non-delivery (i.e. gate guard, individual in office).

L.2.4 The hard-copy volumes shall be submitted in standard 8-1/2 x 11-inch three-ring binders with 4-1/4-inch hole spacing between rings. Each binder shall be clearly labeled with its

Volume number.

The binder shall be able to accommodate the insertion of change pages and additional information provided as a result of discussions and subsequent proposal revisions. Any pages that are changed as a result of discussions should be of a different color and have changed information clearly marked by a vertical line in the right margin of the page. The revised pages shall be dated.

L.2.5 The proposal shall use no smaller than Microsoft (MS) Word 12-point Times New

Roman font for all proposal text on 8-1/2 x 11 inch paper. If both sides of sheets of paper are used, each sheet will be counted as two pages. Tables or figures shall not exceed 11 x 17 inches.

11 x 17 pages are only acceptable for tables/graphic representations. Font size for tables and figures shall be no smaller than 8-point font and shall only include text that is applicable to the table or figure. If proposal volumes/sections exceed the limitations set forth herein, excess text will not be evaluated. All pages of each volume shall be appropriately numbered (X of Y) and shall contain the Volume number, company name, date, and solicitation number in the header and/or footer.

Use at least 1-inch margins on the top, bottom, left, and right side margins. Pages shall be numbered sequentially by volume and Lot and marked with the Offeror’s name and any applicable data use restrictions. The CD/DVD copies of the proposal shall be MS Office 2013 compatible. MS Word Files shall use the following page setup parameters:

Margins – Top, Bottom, Left, Right - 1”

Gutter – 0”

From Edge – Header, Footer - 0.5”

Page Size, Width – 8.5”

Page Size, Height – 11”

L.2.6 Electronic File Packaging. The Offeror shall submit each proposal volume on a separate Compact Disc (CD) or Digital Video Disc (DVD). Each disc shall contain all proposed

Lots corresponding to the respective volume. CDs/DVDs shall be appropriately marked with the

Offeror’s name and address, volume identification, and distribution restrictions. These markings shall be identified on every CD/DVD volume copy along with “Copy X of Y”. All files on each

CD/DVD shall be directly supported by MS Office 2013 and free of any virus or malware and can be opened and read by the Government. All files shall be clearly identified with a filename appropriate to the content. An Adobe Acrobat Portable Document Format (PDF) version of all electronic documents shall also be included as a control copy. If the hard-copy proposal differs from the electronic version, the electronic version will take precedence.

L.2.7 All information shall be confined to the appropriate volume. The referencing of information in one Volume to any other Volume is explicitly prohibited; information from any other volume will not be considered. Pricing information shall not be included in any volume other than Volume III, Cost/Price. The Offeror shall confine submissions to essential matters, sufficient to define the proposal and provide adequate basis for evaluation. The Offeror is responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each proposal. Proprietary information shall be clearly marked.

L.3.0 PROPOSAL VOLUMES

The proposal must consist of Volumes I through IV, where each Volume must comply with the guidance set forth in the table below:

L.3.1 Page Count Evaluation and Exclusions. Those pages that exceed the page counts in

Section L.3.0 for each Volume will not be evaluated. Page counts do not include any applicable

Cover Sheet, Table of Contents, Executive Summary, List of Figures and Tables, Lists of

Acronyms (if used), section separators, and any blank pages marked “Not used.”

L.4.3 For the above L.4.1 – L.4.2 requirements, reference SOW PEO-STRI-16-W073.

L.4.4 The proposal should not simply rephrase or restate the Government's requirements but rather shall provide convincing rationale addressing how the Offeror’s proposed commercial item meets the VPSS WBPS HFPS and/or VS requirements. Paraphrasing the Statement of Work or parts thereof is discouraged and may be considered inadequate and non-compliant. It is the Offeror’s responsibility to present enough information to be meaningfully evaluated without discussions.

L.5.0 FACTOR II, VOLUME II - SMALL BUSINESS PARTICIPATION

The Small Business Participation Plan shall stand on its own merit without reference to another source, volume, or proposal. If proposing multiple WBPS Lots, only one plan is required with separate sections for each Lot.

L.5.1. All Offerors, both small and large businesses, are required to submit a Small

Business Participation Plan that clearly demonstrates a plan for meeting your proposed Small

Business Participation Requirement. A positive participation is required from no less than one of the following categories: small business (SB), small disadvantaged business (SDB), service disabled veteran owned small business (SDVOSB), woman owned small business

(WOSB) and Historically Underutilized Business Zone small business (HUBZones).

The Small Business Participation Requirement is a percentage that is applied towards the total funded value over the assessed annual period. For example, if the total amount of DOs awarded within an annual period have a funded value of $1,000,000, the contractor should satisfy the Small Business Participation Requirement by taking the funded value and multiplying the positive percentage to identify that amount that should be subcontracted to small businesses. Small business offerors can meet the small business participation requirement through performance as a prime small business, or a combination of performance and small business subcontracting.

Offerors are cautioned that failure to meet the proposed Small Business Participation

Requirement for each reported period will affect the Performance Risk Assessments and may be reflected in their CPARS.

L.5.2. Offerors shall submit Small Business Participation Plan information in accordance with DFARS 215.304. The Plan shall include the following:

a. Type of Business of prime contractor (Check all applicable boxes):

{ } Large

{ } Small (also check type of Small Business below)

{ } Small Disadvantaged Business

{ } Woman-Owned Small Business

{ } Historically Underutilized Business Zone Small Business

{ } Veteran Owned Small Business

{ } Service Disabled Veteran Owned Small Business

b. The Offeror shall propose a positive Small Business Participation Requirement from no less than one of the following categories: small business (SB), small disadvantaged business (SDB), service disabled veteran owned small business (SDVOSB), woman owned small business (WOSB) and Historically Underutilized Business Zone small business (HUBZones).

c. The Offeror shall populate the following table to identify all anticipated Prime and subcontracted supplies/services.

Pursuant to Section 8(d) of the Small Business Act, a concern is small for subcontracts that relate to Government procurements if it does not exceed the size standard for the North American

Industry Classification System (NAICS) code that the prime contractor believes best describes the product or services being acquired by the subcontract. In other words, the size of the Offeror’s subcontractors and suppliers is determined by the NAICS code of the subcontract, which may or may not be the same NAICS code as the one for Offeror’s contract with the Government.

L.6.0 FACTOR III, VOLUME III – Cost/Price

L.6.1 All assumptions pertaining to price shall be included in the Cost/Price Volume only.

Additionally, the Price Volume shall include the requirements set forth in this section, L.6.0. All price information shall be limited to this Volume.

Mortar location, etc.). The offeror shall provide completed invoices or sales receipts, if available, to Government or non-Government customers that show, at a minimum, the product name, product number and price.

L.6.2.1 For catalog items, the offer shall include:

A copy of or identification of the catalog and its date, or the appropriate pages for the offered items, or a statement that the catalog is on file in the buying office to which the proposal is being submitted.

A copy or description of current discount policies and price lists (published or unpublished) (e.g., wholesale, original equipment manufacturer, or reseller).

An explanation of the basis of each offered price and its relationship to the established catalog price, including how the proposed price relates to the price of recent sales in quantities similar to the proposed quantities.

L.6.2.2 For market-priced items, the offeror shall include:

The source and date or period of the market quotation or other basis for market price, the base amount, and applicable discounts.

A description of the nature of the market.

L.6.3 Section B of the RFP for the ID/IQ establishes the contract celling-price for the five year base period. Offerors shall not submit any pricing within Section B of the RFP. Contract actions will be funded at the ID/IQ order level.

L.6.4 Certified cost and pricing data is not currently required.

L.7.0 VOLUME IV- ADMINISTRATIVE

Volume IV is required to be eligible for award but will not be evaluated. Offerors shall prepare and submit an Administrative Volume (Solicitation, Offer, and Award Documents and

Certifications/Representations) that contains the following information, by section:

L.7.1 Certifications/Representations. Each offeror shall complete (fill-in and signatures) the solicitation sections indicated below using the file (without modification to the file) provided with the solicitation. Section A – Standard Form 30 (signed), Amendment(s) to Solicitation (if applicable)

Section K – Representations, Certifications, and Other Statements of Offerors

Section K – 52.219-1, Small Business Program Representation

Appendix A – Small Business Subcontracting Plan (Required from Large Businesses only)

L.7.2 Offerors shall submit the following:

a) The Offeror’s DUNS, Tax Identification Number (TIN) & CAGE code

b) Identification of the Offeror’s cognizant DCMA and DCAA offices, to include Point of

Contact information

c) A statement as to which LOT(s) the proposal is to be considered for award

d) A statement of acceptance of all RFP terms and conditions. No alternate proposals will be accepted. The Government cautions Offerors that in order for its proposal to be eligible for award, the proposal shall be in compliance with the terms and conditions set forth in the RFP.

e) Names, titles, telephone numbers, and email addresses of persons authorized to negotiate on the Offeror's behalf with the Government in connection with this solicitation in the event that the Government deems discussions to be necessary.

f) A statement of acceptance of all RFP terms and conditions. The Offeror shall not make any changes to the solicitation. Any objections to the terms and conditions of the solicitation should be addressed to the PCO. If the proposal takes exception to the terms and conditions of the solicitation, all exceptions shall be denoted in a cover letter to the proposal; however, proposed exceptions taken to the solicitation may result in the offer being non-compliant. Assumptions to the solicitation submitted within an offeror’s proposal must be supported with data necessary to illustrate the adequacy of the assumption.

L.7.3 Subcontracting Plan. Large Business Offerors Only shall provide a Small Business

Subcontracting Plan meeting the requirements of FAR 52.219-9 and DFARS 252.219-7003 (or

DFARS 252.219-7004 if the Offeror has a Comprehensive Small Business Subcontracting Plan).

The Small Business Subcontracting Plan is a requirement for award to a Large Business and will be incorporated into any resultant contract. The Small Business Subcontracting Plan shall reflect and be consistent with the commitments stipulated in the Offeror’s proposed Small Business

Participation Plan.

Small Business Subcontracting Plan is a different/separate requirement than the above outlined Small Business Participation Plan. This plan shall be submitted separately from the

Small Business Participation Plan information required above, which applies to both large and small businesses. The Small Business Subcontracting Plan is not a requirement for evaluation in source selection but rather a requirement for award to large business and will be incorporated into any resultant contract.
