http://www.sam.gov/ mailto:lornia.j.villalobos.civ@mail.mil mailto:randall.p.cummins.civ@mail.mil.
mailto:lornia.j.villalobos.civ@mail.mil mailto:%20randall.p.cummins.civ@mail.mil.
Unacceptable Proposal does not meet the minimum requirements in accordance with the solicitation.
*An unacceptable rating will be assessed if offeror fails to submit verifiable proof of RMF/ATO accreditation and certification in accordance with Defense Health Agency guidelines.
NOTE: The combined technical response for Volume II shall not exceed 30 pages.
Volume III. Price
Factor 2: Price. Failure to provide a price for any test, unless it is specifically identified as "not separately priced" within the proposal, will be construed to mean the offeror cannot provide the test. All proposals will initially be reviewed to determine whether each proposal contains the required pricing for each test. Any proposal failing to price all required testing, unless the test is identified as “not separately priced”, will not be evaluated further and will be ineligible for award.
The Government will evaluate price proposals on the basis of fair and reasonable pricing once the proposal has received at least an acceptable overall technical rating. An offeror's pricing must be determined to be fair and reasonable in order to be considered for contract award.
b. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
c. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

52.212-2 ADDENDUM
Award will be made using the lowest price technically acceptable (LPTA). The Government will award a contract resulting from this solicitation to the responsible offeror whose quote meets all technical evaluation factors and whose price has been determined fair and reasonable. Award will be made to the responsible offeror on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors.
Award may be made without discussions with offerors (except communications conducted for the purpose of minor clarification). Therefore, each initial offer should contain the offeror's best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if it is later determined by the contracting officer to be necessary.
Award will be made on an all or none basis.
The criteria for evaluation of proposals is provided below.
Factor 1: Technical Capability. The Government will evaluate each subfactor independently. The offerors detailed response to each paragraph and subparagraph of the Procurement Description will be evaluated to ensure that the offeror’s response meets or exceeds the requirement. Offeror must meet or exceed each requirement listed in the
Purchase Description in order to be deemed acceptable. Failure to provide a response in sufficient detail which meets the minimum requirement for any paragraph or subparagraph under the Technical Subfactors below shall result in an offeror receiving a rating of unacceptable. Each factor and sub-factor will be evaluated using the criteria stated in Table 1 below.
Subfactor 1. Proof of RMF/ATO accreditation and certification certified in accordance with Defense Health
Agency guidelines.
Subfactor 2. Instruments/Equipment and Reagents/Consumables- Statement of Work paragraph 3.2.
Subfactor 3. Maintenance/Service/Telephone Support- Statement of Work paragraph 5.6.
Subfactor 4. Training-Purchase Description paragraph 3.
Subfactor 5. Delivery/Installation/Method Validation-Purchase Description paragraph 5.9.1.
Note: A rating of acceptable must be achieved on all technical subfactors in order to receive an acceptable rating for the overall Technical Capability Factor 1.
