Section SF 30 - BLOCK 14 CONTINUATION PAGE

BASIS FOR AWARD

In accordance with FAR 52.212-2, the Government intends to make an award to the Contractor who submits the lowest price, technically acceptable quote, and is deemed responsible by the Contracting Officer. The anticipated award will be a Firm Fixed-Price contract. Offerors must submit quotes on the full quantities identified. Failure to submit all information requested in the instruction to offerors could be cause for rejection of the entire quote. The Contracting Officer reserves the right to make no award under this procedure.

The evaluation of quotes will be based off of the following factors:

a. Technical: Facility Assessment consisting of cleanliness, security, amenities and in conformance with the terms in the PWS.

b. Price: Pricing for each Line Item. Pricing must be submitted in the proper format. The proper format consists of QUANTITY * UNIT AMOUNT = NET AMT. In the event there is a difference between a unit price and the extended total, the unit price will be held to the intended price. If the offeror shows only the total price but fails to enter a unit price, the total divided by the estimated quantity will be held to be the intended price. The total evaluated price shall include the base and all option years; partial quotes will not be accepted. Offerors shall provide enough detail that their quote can be properly evaluated.

INSTRUCTIONS TO OFFERORS

1.. QUESTIONS: All questions shall be in writing. Questions may be emailed to Valerie Norris at valerie.l.norris2.civ@mail.mil.

2. DUE DATE: All quotes are due no later than the time & date stated in block #8 of the solicitation. Quotes shall be emailed to valerie.l.norris2.civ@mail.mil.

3. LOCATION: The hotel must be located within 15 miles of 1236 Gate 3 Road, Anniston, AL 36206. The distance will be measured by utilizing Google Maps (https://www.google.com/maps).

4. QUOTES MUST INCLUDE THE FOLLOWING REQUIRED INFORMATION:

1. Name & Address of proposed facility:

2. A list of facility amenities.

3. Federal Emergency Management Agency (FEMA) Certification of the proposed facility.

4. The contractor must provide Point of Contact (POC) who will be available from commencement to conclusion of lodging.

5. PRICING: Please note that the Government is not authorized to pay prices higher than the allowable per diem rate of $91.00 for the required location.

Contractors shall quote on all CLINS. The proper format consists of QUANTITY * UNIT AMOUNT = NET AMT. CLIN pricing should be all inclusive all applicable taxes, fees, gratuities, commissions and service charges), G&A and profit.

THE FEDERAL GOVERNMENT IS TAX EXEMPT.

In the event there is a difference between a unit price and the extended total, the unit price will be construed as the intended price. If the offeror shows only the total price but fails to enter a unit price, the total divided by the estimated quantity will be construed as the intended price.

6. The U.S. Government will only pay for the actual number of rooms used, up to the maximum quantity identified in this contract. When possible, the Government will provide 24 hour advance notice to the contractor of any “No Show’s” . The Government will be responsible for payment only of the first night after notice was provided. Then, on the morning following the check in date, any rooms not filled will be released for sale to the general public and will not be chargeable as “No Shows” under the contract. The Contractor will notify the Contracting Officer of any unused rooms that are being released. If additional rooms or nights are needed in excess of the quantity reserved under this contract, contact the Contracting Officer to notify of the situation. Contract changes are authorized only by the Contracting Officer. Inform any personnel that all changes are to be in accordance with this paragraph. The

7. Complete blocks 12, 17 and 30 of the SF1449 as well as any amendments. In doing so, the offeror accedes to the contract terms and conditions as written in the Request for Proposal. Failure to provide this information with your quote may deem your quote non-responsive.

8. For quotes submitted by Third Party Vendors, the following must also be submitted with the quote. Failure to provide this information with your quote may deem your quote non-responsive/unacceptable.

(A). The Third Party Vendor must provide a Point of Contact (POC) at the facility from commencement to conclusion of the event. The POC listed below (who will be at each event) cannot be changed without prior written approval from the Contracting Office at least 3 business days in advance.

(B). The Third Party Vendor must submit all subcontracting agreements to reflect an agreement or contract between the prime hotel and the third party vendor. For any quote submitted by an offeror that is not a hotel, the offeror must describe in its quote the technical approach for the offeror's services as a prime contractor and forward a copy of a signed, written agreement between the offeror and a representative who has binding authority of stated hotel facility. This agreement must state that it covers the period of performance requested in the solicitation. In addition, a copy of all insurances(s) (standard business, liability, extraordinary insurances) must be submitted with the offeror’s technical proposal for evaluation purposes.

(C). For third party vendors, your quote shall include the additional information requested in paragraph 8a-8b above. Failure to provide this information with your quote may deem your quote to be technically unacceptable and/or non-responsive.

9. The Hotel(s) presented to the Government must be Federal Emergency Management Agency (FEMA) approved.

10. No smoking rooms required. Contractor is put on notice that the Government will not be held liable for any requirement that is to be paid for by participants or any amounts associated with those items, including any guaranteed minimum purchases that the contractor may require.

Price does not cover nor include personal expense items, and, or services in guest rooms such as long distance phone charges, internet, movie rentals, laundry, room services, etc., will be paid directly by the guest, and shall not be billed under the resultant purchase order.

11 . All representations and certifications to include provision of Dun & Bradstreet number are to be returned with the offer. Dun and Bradstreet information can be found by calling (800)-333-0505. Registration in the System for Award Management (SAM) database is not required. However, the successful contractor shall be registered in the SAM database prior to submitting an invoice and through final payment of any contract resulting from this solicitation. 3th BDE (CM), 102TH DIV (MS)

Lodging accommodations in support of the USAR Lodging In Kind Program for the Det 2, 4/108th BN at Anniston, AL 36206

1.0 INTRODUCTION

The 3rd Brigade (CM), 102th Division (MS) and subordinate Battalions conduct monthly unit assemblies at multiple geographically dispersed locations with the continental USA. The 3rd Brigade and subordinate Battalions provides MOS-T, Transition and NCOES training for the United State Army AC, RC and NG CBRN community.

2.0 BACKGROUND

The 3rd BDE (CM), 102nd DIV (MS) was created to enhance the US Army capabilities to train Active and Reserve CBRN soldier in support of the War effort and other contingency operations.

3.0 OBJECTIVES

Lodging is required to support Soldiers assigned to the Det 2, 4-108th CM Battalion who reside 50 miles or more from the unit address. The Det 2, 4-108th CM Battalion is located at 1236 Gate 3 Road, Anniston, AL 36206. The contracted hotel is required to provide all rooms, equipment, personnel, tools, materials, supervision, amenities, and other items necessary to perform the requested task/service as described in this Performance Work Statement (PWS). The Army Standard for lodging in Kind is two (2) Soldiers per room. However, selected Soldiers may be authorized single rooms with approval from the serviced organization/Unit as long as it does not exceed the quantity or total cost of rooms contracted during the service period.

The hotel shall only be paid for the number of rooms used at the contracted cost. The contracted cost shall not exceed the local authorized Per Diem rate. The selected hotel shall be able to support the unit with a combination of up to 8 double rooms per battle assembly NTE 2 nights on a monthly basis with the exception of October 2017 and March 2018. The requirement for October 2017 and March 2018 will be for up to 8 rooms for 3 nights.

The selected hotel will be located within fifteen (15) miles of the unit address. The location will be easily accessible to major roads and highways, restaurants and coffee shops.

Double type room shall have 2 standalone beds (a pullout sofa bed does not constitute as a bed).

Rooms shall be constructed and finished to provide good light and sound attenuation.

Sufficient electrical outlets shall be available and in good working condition.

All entrance doors to rooms shall have interior security locks, either deadbolt or double locking doors. Each room occupant shall be provided with a room key. Access to sleeping rooms shall be via interior corridors.

Bathrooms must be in a sanitary condition. Bathrooms must contain a shower and/or tub combination with a wash basin, a properly functioning toilet, and a mirror. Additionally, bathrooms should have adequate lighting and grounded electrical outlets for use of electrical razors, hair dryers etc. The shower or shower/tub combination shall have shower doors or curtains. Preferred sink and shower area separated by door.

Rooms shall have adequate heating and cooling capabilities.

Each room must contain at a minimum; a television with cable access, internet access, iron and ironing board, alarm clock, a refrigerator, a microwave, and a safe.

The approved facility must have an accessible parking area and be located in a low crime area, free of drugs and prostitution with security cameras or security guard on property for after hour protection.

All rooms will be non-smoking, and free of bed bugs, lice, rodents or anything that will make the living conditions unsafe and/or unhealthy.

A Health and Fitness Center and Business Services Center available at no cost to the soldiers. Recreational facilities (ie. Pool) is preferred but not required.

Daily custodial services shall include, but is not limited to, room cleaning, bed making, linen change (as required), adequate quantities of soap, bath towels, hand towels, sanitized drinking glasses, facial tissues, and a cloth bath mat.

All items specified in the SOW must be present for the Facility Assessment of the Hotel and are subject to verification at time of quote submission.

A complete room list will be provided to the Lodging Facility at least 72 hours prior to the required start date.

Period of Performance: The Period of Performance is from 01 May 2017 to 30 April 2018.

4.0 DEFINITIONS

4.1.CONTRACTOR: The term as used in this requirement refers to the primary (prime) contractor or vendor.
4.2.CONTRACTING OFFICER (KO): The government official who has authority to enter into contract, administer or terminate contracts. He/she can make related determinations and findings on behalf of the government. The Contracting Officer is the ONLY individual who can legally bind the government.
4.3.CONTRACTING OFFICER’S REPRESENTATIVE (COR): An employee of the U.S. Government appointed by the Contracting Officer to monitor contractor’s performance. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

1. Technical: Facility Assessment consisting of cleanliness, security, amenities and in conformance with the terms in the PWS.

2. Price: Pricing for each Line Item. Pricing must be submitted in the proper format. The proper format consists of QUANTITY x UNIT PRICE = NET AMT. In the event there is a difference between a unit price and the extended total, the unit price will be held to the intended price. If the offeror shows only the total price but fails to enter a unit price, the total divided by the estimated quantity will be held to be the intended price.

Technical and Price, when combined, are equal in importance.

(b) RESERVED.

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(a) Definitions. As used in this clause--

Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.

Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.

(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall--

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF.
