About this file
This is a Vendor Affidavit document issued by Hillsborough County Public Schools (HCPS) in Florida regarding compliance with anti-human trafficking provisions. The affidavit is required for all nongovernmental entities executing, renewing, or extending contracts with HCPS and must be signed by an authorized officer or representative of the contractor under penalty of perjury. The affidavit certifies that the contractor does not engage in coercive labor practices, including use or threat of physical force, unlawful restraint or confinement, exploitative debt arrangements, confiscation of identification documents, threats of financial harm, fraudulent inducement, or provision of controlled substances for exploitation. This certification is mandated by Section 787.06(13) of the Florida Statutes and applies to the Crowd Based Innovation Services contract opportunity issued by HCPS on January 13, 2025, with quotes due by January 27, 2025.
The affidavit requires vendors to declare under penalties of perjury that they comply with all specified anti-coercion and anti-human trafficking requirements. The document serves as a compliance mechanism for HCPS to ensure that all contractors meet Florida's statutory obligations regarding human trafficking prevention. Authorized representatives must provide their signature, printed name, title, and date on the affidavit form. This certification is a prerequisite for contract award and must be submitted as part of the vendor's proposal documentation through VendorLink.
