PROJECT FORMS

PART 1 - GENERAL

1.1 SCOPE

A. This Section lists specific, KC VAMC prepared or approved forms that are required to be used by the Contractor during construction of the Project.

B. Before starting work on-site, obtain the most recent version of the following forms from the

Government Project Manager (GPM).

C. Copy and fill-in all required values indicated as applicable to the Work of the Project, and submit to the GPM.

1.2 KC VAMC ADMINISTRATIVE FORMS:

A. SCHEDULE OF VALUES – PROGRESS PAYMENT: For the Contractors submittal of material and associated labor quantities and costs for project management and Progress Payment justification and accounting.

B. PROJECT MASTER SCHEDULE: For the Contractors submittal of project timeline with milestones and phasing to manage project execution over time.

C. CONTRACTOR DAILY LOG: For the Contractors submittal daily of weather conditions, work underway, trades and staff on site, and project status.

D. AUTOMATIC PAYMENT FORM: For the Contractor to enroll in the Automatic Clearing House

(ACH) system for the purpose of submitting Progress Payment Requests, Invoices, in order to receive payment of same.

E. CERTIFIED PAYROLL FORM: For Contractor to submit Certified Payroll information to the GPM as required by law, for all Contractors employees, Sub-contractors employees, and all others working under this contract not employed by the Government.

F. CONTRACTOR COST PROPOSAL: For Contractor to submit cost proposals in response to

Government Request for Price (RFP).

G. RELEASE of CLAIM: For Contractor to submit with final payment request.

H. STORED MATERIAL TRACKING FORM: For Contractor to submit and track material stored on and off the job site.

I. BACKGROUND DATA COLLECTION FORM: For the Contractor’s submittal of proposed Workers intended to be working on KC VAMC property.

J. HOT WORK PERMIT: For the Contractor’s submittal of proposed dates and time for any Work involving use of heat or a flame (including brazing of copper pipes), to include proposed means and methods to be used to mitigate risks and to eliminate fire-system alarms caused by that

Work.

K. INTERIM LIFE SAFETY MEASURES (ILSM) FORM: A document for initial submittal, that includes an assessment of risks and description of deficiencies that will be caused or affected by construction activities.

L. INFECTION CONTROL RISK ASSESSMENT (ICRA) FORM: A document for initial submittal, describing risks and mitigation measures to be used during construction in regards to infection control.

M. ILSM & ICRA DAILY MONITORING FORM: For daily submittal of life-safety and infectious disease control measures being maintained by the Contractor during construction activities.

N. NEW EQUIPMENT FORM: A concise form to list the description, type, model number, serial number, utility/service connections, operational loads, and other salient characteristics of all new “equipment” provided by the Contractor as a part of the Work of the Project. Equipment includes, but is not necessarily limited to operational items with a service life, such as door or gate operators, lifts, elevators, fire-suppression systems and devices, HVAC equipment (not ductwork), plumbing and electrical equipment (not piping or conduit), and light fixtures.

O. OUTAGE ACTION PLAN: A document for submittal by the Contractor for each existing utility service disconnection or outage, describing measure planned to be undertaken to minimize risks and disturbance of existing utility service conditions.

P. PAST PERFORMANCE QUESTIONNAIRE: The form for reporting five (5) past projects of the same class (scope & complexity) as this project.

Q. PLANNED UTILITY SYSTEM SHUTDOWN REQUEST FORM: The “request” form to be submitted by the Contractor for each existing utility service proposed for disconnection or outage, to include the “Outage Action Plan” above, and to include specific dates / times proposed for the affected Work.

1.01 GENERAL INTENTION:

Prime Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for Project 589- 17-106, Renovate Elevators Building 26, for the Kansas City, MO VAMC, as required by drawings and specifications.

The Contracting Officer (CO) for the convenience of all Bidders will schedule a single Pre-bid Site Visit conducted by the Government Project Manager (GPM) for this project.

The design for this project is In-House.

Prior to commencing work, Prime Contractor shall provide proof that an OSHA designated “competent person” (CP) (29 CFR 1926.20(b) (2) will maintain a presence at the work site whenever the general or subcontractors are present.

Training:

1. All employees of Prime Contractor and subcontractors shall have successfully completed the 10-hour or 30-hour OSHA Construction Safety course and other relevant competency training, as determined by Government Project Manager and/or the Construction Safety Officer (CSO) with input from the facility Construction Safety Committee.

2. The Prime Contractor will submit Safety Training records of all employees (to include Sub-contractors, vendors, Consultants, etc. performing work on government property) for approval before the start of work.

VHA Directive 2011-36, Safety and Health during Construction, dated 9/22/2011 in its entirety is a part of this section (see attachment A).

The lists of Construction Specifications Institute (CSI) Divisions represent the major categories labor and materials are to be broken into for bidding purposes. Some or all of these Divisions maybe required for bidding this project. For further breakdown of work within each division, refer to the Specifications and the Specification Index. Bidders are notified that all work will be categorized in the 50 CSI Divisions, the detail provided by the Specifications and the Specification Index may or may not include all work required by this project and only provides the second tier breakdown and not the final details required for biding.

1.02 STATEMENT OF BID ITEM(S):

BID ITEM 1: Base Bid, All work including general construction, alterations, concrete work, light-gauge framing, finishes, elevators, electrical, necessary demolition, and certain other items as required by the Specifications, Drawings, and other documents of Project 589-17-106, Renovate Elevators Building 26 and as expressed in attachment B, Statement of Work. (420 calendar days).

BID ITEM 2: Deductive Alternate 1: All work in BID ITEM 1 excluding the scope to replace the elevator hoist-ropes. (399 calendar days)

BID ITEM 3: Deductive Alternate 2: All work in BID ITEM 2 excluding the scope to replace the counter weight roller guides. (396 calendar days)

BID ITEM 4: Deductive Alternate 3: All work in BID ITEM 3 excluding the scope to replace the car roller guides. (393 calendar days)

1.03 SPECIFICATIONS AND DRAWINGS FOR PRIME CONTRACTOR:

After contract award, the Government will provide electronic drawing files in PDF format to the successful biding Contractor.

Prime Contractor may print, at own expense additional hardcopy drawing sets from electronic media provided by the Government.

Return all copies, in all mediums to the Government Project Manager prior to final payment.

1.04 CONSTRUCTION SECURITY REQUIREMENTS:

Security Plan:

1. The security plan defines both physical and administrative security procedures that will remain effective for the entire duration of the project.

2. The Prime Contractor is responsible for assuring that all sub-contractors working on the project and their employees also comply with these regulations.

Personnel Background Check and Badging:

1. All personnel hired to perform work on project 589-17-106, Renovate Elevators Building 26 will be scheduled by the Prime Contractor with the GPM in groups no larger than 15, for Background Data Collection form completion, Government orientation, and Safety Training.

2. Submit schedules 3 weeks in advance of need for their performance of work at the VA Medical Center. Lead-time is necessary for an adequate interval for background check, Safety Training, and Badging.

Contract personnel shall not enter the project site or perform work at the medical center without the appropriate badge issued by the VA.

The VA security management program requires compliance with VA security policies, by all employees, consultants, or other such persons associated with the Prime Contractor and/or its Subcontractors having business in connection with this project. These persons will wear picture ID badges prominently displayed at all times with name, project name and number, and employer.

Contract personnel may be subject to inspection of their personal effects when entering or leaving the project site.

For working outside the “regular hours” as defined in the contract, The Prime Contractor shall give 21-day notice to the CO and The GPM, to make security arrangements for the employees.

This notice is separate from any notices required for utility shutdown described later in this section.

Photography of VA premises is NOT allowed without written permission of the CO.

VA reserves the right to close down or shut down the project site and order Prime Contractor’s employees off the premises in the event of a national emergency. The Prime Contractor may return to the site only with the written approval of the CO.

Key Control:

1. The Prime Contractor shall provide duplicate keys and lock combinations for all locked spaces including toolboxes and storage containers, to the CO and GPM for the purpose of security inspections and emergency action.

2. The Prime Contractor shall turn over all temporary construction lock cylinders to the VA locksmith for pin configuration and installation. Keys will be issued to the Prime Contractor for distribution as needed. The Prime Contractor is responsible for security of the construction site and key control.

3. The Prime Contractor shall turn over all permanent lock cylinders to the VA locksmith for permanent installation. See Section 08 71 00, DOOR HARDWARE and coordinate.

Document Control:

1. The General Prime Contractor is responsible for safekeeping of all drawings, project manual, and other project information. Share this information only with those with a specific need to accomplish the project.

2. Before starting any work, the Prime Contractor/Sub Contractors shall submit an electronic security memorandum describing the approach to following goals and maintaining confidentiality of “sensitive information”.

3. Certain documents, sketches, videos or photographs and drawings may be marked “Law Enforcement Sensitive” or “Sensitive Unclassified”. Secure such information in separate

01 00 00, General Conditions Page 5 of 61 containers and limit the access to only those who will need it for the project. Return the information to the CO or GPM upon request.

4. Do not remove or transmit these documents from the project site without the written approval of CO.

5. All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the VA.

6. Notify GPM and CO immediately when there is a loss or compromise of “sensitive information”.

7. All electronic information shall be stored in specified location following VA standards and procedures using an Engineering Document Management Software (EDMS).

a. Security, access, and maintenance of all project drawings, both hardcopy and electronic shall be performed and tracked through the EDMS system.

b. “Sensitive information” including drawings and other documents may be attached to e-mail provided all VA encryption procedures are followed.

Motor Vehicle Restrictions:

1. Vehicle authorization request are required for any vehicle entering the site. Submit such request 24 hours before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies.

2. The Prime Contractor will arrange off sight parking for all workers associated with this project.

3. The Medical Center at its discretion may issue Separate parking permits for the Prime Contractor and its employees to parking in designated areas only.

Fire Safety Plan: Establish and maintain a fire protection program in accordance with 29 CFR 1926. Prior to Notice to Proceed, Prime Contractor will prepare a plan detailing project-specific fire safety measures, including periodic status reports, and submit to the GPM and CO for review for compliance with VHA Directive 2005-007, NFPA 101 and NFPA 241.

Prior to Notice to Proceed, all employees of the Prime Contractor and/or any Subcontractors shall undergo a safety briefing provided by the Prime Contractor’s Competent Person per OSHA requirements. This briefing shall include information on the construction limits, VAMC safety guidelines, means of egress, break areas, work hours, locations of restrooms, use of VAMC equipment, etc. Provide documentation to the GPM and CO that all construction workers have undergone Prime Contractor’s safety briefing.

Site and Building Access: Maintain free and unobstructed access to facility emergency services and for fire, police, and other emergency response forces in accordance with NFPA 241.

Fire Extinguishers: Provide and maintain extinguishers in construction areas and temporary storage areas in accordance with 29 CFR 1926, NFPA 241, and NFPA 10.

Store, dispense, and use flammable and combustible liquids in accordance with 29 CFR 1926, NFPA 241, and NFPA 30.

Smoke Detectors: Prevent accidental operation. Schedule outages 7 days in advance when activation is emanate. Coordinate with GPM and CO.

Hot Work: Perform and safeguard hot work operations in accordance with NFPA 241 and NFPA 51B. Coordinate with the GPM. Obtain permits from GPM at least 1 week in advance.

Fire Hazard Prevention and Safety Inspections: Inspect entire construction areas daily. Coordinate with, and report findings and corrective actions daily to the GPM.

Smoking: Smoking is prohibited in and adjacent to construction areas inside existing buildings and additions under construction, in separate and detached buildings under construction. Smoking is prohibited except in designated smoking rest areas.

Dispose of waste and debris in accordance with NFPA 241. Remove from buildings daily.

PERFORM OTHER CONSTRUCTION, ALTERATION, AND DEMOLITION OPERATIONS IN

ACCORDANCE WITH 29 CFR 1926.

1.06 OPERATIONS AND STORAGE AREAS:

The Prime Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the GPM and CO.

The Prime Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Prime Contractor their employees, Subcontractors, Vendors, and Consultants performance.

Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the Prime Contractor only with the approval of the GPM and the CO and shall be built with labor and materials furnished by the Prime Contractor without expense to the Government. The temporary buildings and utilities shall remain the property of the Prime Contractor and the Prime Contractor shall remove at its expense upon completion of the work. The buildings and utilities may be abandoned in place, with the written consent of the GPM and the CO.

The Prime Contractor shall, under regulations prescribed by the CO, use only established roadways, or use temporary roadways constructed by the Prime Contractor when and as authorized by the GPM. When materials are transported in prosecuting the work, vehicles shall

01 00 00, General Conditions Page 7 of 61 not be loaded beyond the loading capacity recommended by the manufacturer of the vehicle or prescribed by any Federal, State, or local law or regulation. When it is necessary to cross curbs or sidewalks, the Prime Contractor shall protect them from damage. The Prime Contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads. (FAR 52.236-10)

Working space and space available for storing materials shall be as shown on the drawings.

Workers are subject to rules of the Medical Center applicable to their conduct.

Execute work to interfere as little as possible with normal functioning of the Medical Center as a whole, including operations of utility services, fire protection systems, and any existing equipment, and with work being done by others. Use of equipment and tools with the potential to transmit vibrations and/or noises through the building structure, are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Prime Contractor's personnel, except as permitted by the GPM where required by limited working space.

1. Do not store materials and equipment in other than assigned areas.

2. Schedule delivery of Materials and equipment to the immediate construction working areas, quantities shall not exceed a two-workday supply.

3. The Government will not receive material or equipment for the Prime Contractor.

4. Provide unobstructed access to Medical Center areas required to remain in operation.

5. Where access by Medical Center personnel to vacated portions of buildings is not required, the Prime Contractor's may store materials and equipment, subject to fire and safety requirements.

Utilities Services: Where necessary to cut existing pipes, electrical wires, conduits, cables, etc., of utility services, or of fire protection systems or communications systems, they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by the GPM or Utility Company involved:

1. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the construction project, for such items as water, sewer, electricity, gas or steam, payment of such fee shall be the responsibility of the Government and not the Prime Contractor.

Phasing: The Medical Center must maintain its operation 24 hours a day 7 days a week.

Therefore, schedule and coordinate any interruption in service with the GPM to ensure that no lapses in operation occur. It is the PRIME CONTRACTOR'S responsibility to develop a work plan and Project Schedule detailing, at a minimum, the procedures, the equipment, and materials, the interim life safety measure, and a timeline defining the duration of the work with phasing, milestones, and subtasks. The Project Schedule shall include such detail as is required by Specification Section 01 32 16.15, PROJECT SCHEDULES.

1. To insure such executions, the Prime Contractor shall furnish the GPM with a schedule of approximate phasing dates on which the Prime Contractor intends to accomplish work in each specific area of the site, building(s), or portion(s) thereof. In addition, the Prime Contractor shall notify the GPM three (3) weeks in advance of the proposed date of starting work in each specific area of the site, building(s), or portion(s) thereof. The Prime Contractor shall arrange such dates to insure accomplishment of the work in successive phases is mutually agreeable with the Medical Center Director, GPM, CO, and Prime Contractor.

All Medical Center buildings will be occupied during performance of work.

1. The Prime Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and Medical Centers operations in these areas is not hindered.

2. The Prime Contractor is to pay particular attention to and mitigate the infiltration of smoke or fumes generated by construction work into the Medical Centers various air intakes.

3. The Prime Contractor shall permit access to Government employees, patients, and visitors through construction areas, which serve as routes of access to such affected areas and equipment. These routes whether access or egress shall be isolated from the construction area by temporary partitions and have walking surfaces, lighting etc. to facilitate patient and staff access.

4. The Prime Contractor shall coordinate alteration work in occupied areas so that Medical Center operations will continue during the construction period.

When a building and/or construction site is turned over to the Prime Contractor, The Prime Contractor shall accept entire responsibility including upkeep and maintenance therefore:

Utilities Systems: Building services and Utility systems include domestic water, treated or filtered water systems, natural gas, steam, medical gas systems, sanitary sewers, storm sewer, electricity, fire protection, detection, and notification systems, vertical transportation systems, and communications systems, including voice, data, public address, Wi-Fi networks, patient monitoring systems, building management systems, and audio/video broadcast networks.

Interruption of any of these systems is prohibited without prior written approval of the GPM.

1. The Prime Contractor will maintain existing building services and utility systems for the Medical Center at all times and provide temporary facilities, labor, materials, equipment, connections, and utilities services as required by activities of this project to assure uninterrupted service.

2. The Prime Contractor shall submit a request to interrupt any building services or utility systems to the GPM, in writing, four (4) weeks in advance of proposed interruption. Request shall state reason, date, exact time of, and approximate duration of requested interruption and include a detailed outline in 15-minute increments, of the work to be accomplished during the interruption. The request will include a detailed plan for maintaining the building service or utility being interrupted with redundancies and an emergency reconnection/failure plan.

3. Prime Contractor will receive (in writing) an approval of request, or of which other date and/or time, such interruption will cause least inconvenience to operations of Medical Center. Interruption time approved by Medical Center may occur at other than Prime Contractor's normal working hours.

4. Additional advance notice and planning will be required for major interruptions of any system.

5. Electrical work shall be accomplished with all affected circuits or equipment de-energized.

When an electrical outage cannot be accomplished, work on any energized circuits or equipment shall not commence without a detailed work plan, the Medical Center Director’s prior knowledge, and written approval. Refer to specification Sections 26 05 11, REQUIREMENTS FOR ELECTRICAL INSTALLATIONS; 27 05 11 REQUIREMENTS FOR

COMMUNICATIONS INSTALLATIONS; and 28 05 00, COMMON WORK RESULTS FOR ELECTRONIC SAFETY AND SECURITY for additional requirements.

6. In case of a construction emergency, service will be interrupted on approval of GPM. Such approval will be confirmed in writing as soon as practical.

7. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the construction project, such fee shall be the responsibility of the Government and not the Prime Contractor.

Abandoned Lines: All service lines such as wires, cables, conduits, ducts, pipes and the like, and their hangers or supports, which are to be abandoned are required to be entirely removed, sealed, capped or plugged at the main, branch or panel or closest point practicable to their origin.

The lines shall not be capped in finished areas, but shall be removed and sealed, capped or plugged in ceilings, within furred spaces, in unfinished areas, or within walls or partitions so that they are completely behind the finished surfaces, in a manner compliant to applicable codes and standards.

1.07 INFECTION PREVENTION MEASURES:

Implement the requirements of VAMC’s Infection Control Risk Assessment (ICRA) team. ICRA Group may monitor dust in the vicinity of the construction work and require the Prime Contractor to take corrective action immediately if the safe levels are exceeded.

Establish and maintain a dust control program as part of the Prime Contractor’s infection preventive measures in accordance with the guidelines provided by ICRA Group. Prior to start of work, prepare a plan detailing project-specific dust protection measures, including periodic status reports, and submit to the Facility ICRA team through the GPM for review for compliance with contract requirements in accordance with Section 01 33 23, SHOP DRAWINGS, PRODUCT DATA AND SAMPLES. The Government provided IRCA and ILSM forms will be completed and submitted with the required plan.

1. All personnel involved in the construction or renovation activity shall be educated and trained in infection prevention measures established by the medical center prior to performing work.

Medical center Infection Control personnel shall monitor for airborne disease (e.g. aspergillosis) as appropriate during construction. A baseline of conditions may be established by the medical center prior to the start of work and periodically during the construction stage to determine impact of construction activities on indoor air quality. In addition:

1. The GPM and VAMC Infection Control personnel shall review pressure differential monitoring documentation to verify that pressure differentials in the construction zone and in the patient-care rooms are appropriate for their settings. The requirement for negative air pressure in the construction zone shall depend on the location and type of activity. Upon notification, the Prime Contractor shall implement corrective measures to restore proper pressure differentials as needed.

2. In case of any problem, the medical center, along with assistance from the Prime Contractor, shall conduct an environmental assessment to find and eliminate the source.

In general, the following preventive measures shall be adopted during construction to keep down dust and prevent mold.

1. Dampen debris to keep down dust and provide temporary construction partitions in existing structures where directed by GPM, blank off ducts and diffusers to prevent circulation of dust into occupied areas during construction.

2. Do not perform dust-producing tasks within occupied areas without the approval of the GPM.

For construction in any areas that will remain jointly occupied by the medical Center and Prime Contractor’s workers, the Prime Contractor shall:

a. Provide dust proof temporary drywall construction barriers to completely separate construction from the operational areas of the hospital in order to contain dirt debris and dust. Barriers shall be sealed and made presentable on hospital occupied side. Install a self- closing rated door in a metal frame, commensurate with the partition, to allow worker access. Maintain negative air at all times. A fire retardant polystyrene, 6-mil thick or greater plastic barrier meeting local fire codes may be used where dust control is the only hazard, and an agreement is reached with the GPM and Medical Center.

b. HEPA filtration is required for negative air exhaust. The Prime Contractor shall verify that construction exhaust to exterior is not reintroduced to the medical center through intake vents, or building openings. Install HEPA (High Efficiency Particulate Accumulator) filter vacuum system rated at 95% capture of 0.3 microns including pollen, mold spores, and dust particles. Insure continuous negative air pressures occurring within the work area.

HEPA filters should have ASHRAE 85 or other pre-filter to extend the useful life of the HEPA. Provide both primary and secondary filtrations units. Exhaust hoses shall be heavy duty, flexible steel reinforced and exhausted so that dust is not reintroduced to the medical center.

c. Adhesive Walk-off/Carpet Walk-off Mats, minimum 600mm x 900mm (24” x 36”), shall be used at all interior transitions from the construction area to occupied medical center area. These mats shall be changed as often as required to maintain clean work areas directly outside construction area at all times.

d. Vacuum and wet mop all transition areas from construction to the occupied medical center as often as is necessary to prevent tracking of dust and construction debris to the occupied area and at the end of each workday. Vacuum shall utilize HEPA filtration.

Maintain surrounding area frequently. Remove debris as they are created. Transport these outside the construction area in containers with tightly fitting lids.

e. The Prime Contractor shall not haul debris through patient-care areas without prior written approval of the GPM and the Medical Center. When, approved, debris shall be hauled in enclosed dust proof containers or wrapped in plastic and sealed with duct tape.

No sharp objects should be allowed to cut through the plastic. Wipe down the exterior of the containers with a damp rag to remove dust. All equipment, tools, material, etc.

transported through occupied areas shall be made free from dust and moisture by vacuuming and wipe down.

f. Use of a HEPA vacuum, to clean inside the barrier and vacuum ceiling tile prior to replacement is required. Any ceiling access panels opened for investigation beyond sealed areas shall be sealed immediately when unattended.

g. There shall be no standing water during construction. This includes water in equipment drip pans and open containers within the construction areas. All accidental spills must be cleaned up and dried within 12 hours. Remove and dispose of porous materials that remain damp for more than 72 hours.

h. At completion, remove construction barriers and ceiling protection carefully, outside of normal work hours. Vacuum and clean all surfaces free of dust after the removal.

1.08 BRAND NAME OR EQUAL:

VAAR 852.211-73 Brand name or equal

(Note: As used in this clause, the term “brand name” includes identification of products by make and model.)

A. If items called for by this invitation for bids have been identified in the schedule by a “brand name or equal” description, such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of products that will be satisfactory. Bids offering “equal” products (including products of the brand name manufacturer other than the one described by brand name) will be considered for award if such products are clearly identified in the bids and are determined by the Government to meet fully the salient characteristics requirements listed in the invitation.

B. Unless the bidder clearly indicates in the bid that the bidder is offering an “equal” product, the bid shall be considered as offering a brand name product referenced in the invitation for bids.

1. If the bidder proposes to furnish an “equal” product, the brand name, if any, of the product to be furnished shall be inserted in the space provided in the invitation for bids, or such product shall be otherwise clearly identified in the bid. The evaluation of bids and the determination as to equality of the product offered shall be the responsibility of the Government and will be based on information furnished by the bidder or identified in his/her bid as well as other information reasonably available to the purchasing activity. CAUTION TO BIDDERS. The purchasing activity is not responsible for locating or securing any information that is not identified in the bid and reasonably available to the purchasing activity. Accordingly, to insure that sufficient information is available, the bidder must furnish as a part of his/her bid all descriptive material (such as cuts, illustrations, drawings or other information) necessary for the purchasing activity to:

a. Determine whether the product offered meets the salient characteristics requirement of the Invitation for Bids, and

b. Establish exactly what the bidder proposes to furnish and what the Government will furnish binding itself to purchase by making an award. The information furnished may include specific references to information previously furnished or to information otherwise available to the purchasing activity.

2. If the bidder proposes to modify a product so as to make it conform to the requirements of the Invitation for Bids, he/she shall:

a. Include in his/her bid a clear description of such proposed modifications, and

b. Clearly mark any descriptive material to show the proposed modifications.

3. Modifications proposed after bid opening to make a product conform to a brand name product referenced in the Invitation for Bids will not be considered.

1.09 ALTERATIONS:

Survey: Before any work is started, the Prime Contractor shall make a thorough survey with the GPM, areas of buildings in which alterations occur and areas which are anticipated routes of access, and furnish a report, signed by both, to the CO. This report shall list by rooms and spaces:

1. Existing condition and types of resilient flooring, doors, windows, walls, and other surfaces not required to be altered throughout affected areas of buildings.

2. Existence and conditions of items such as memorials, plaques, and accessories, etc., required by drawings to be either reused, relocated, or both.

3. Shall note any discrepancies between drawings and existing conditions at site.

4. Shall designate areas for working space, materials storage, and routes of access to areas within buildings where alterations occur and upon which the Prime Contractor and the GPM have agreed.

Any items required by drawings to be either reused or relocated or both, found during this survey to be nonexistent, or in opinion of the GPM, to be in such condition that their use is impossible or impractical, shall be furnished and/or replaced by the Prime Contractor with new items in accordance with specifications furnished by the Government. Provided the contract work is changed by reason of this subparagraph B, the contract will be modified accordingly, under provisions of clause entitled "DIFFERING SITE CONDITIONS" (FAR 52.236-2) and "CHANGES" (FAR 52.243-4 and VAAR 852.236-88) and the CO issues a written contract modification.

Re-Survey: Thirty days before expected partial or final inspection date, the Prime Contractor and the GPM together shall make a thorough re-survey of the areas of buildings involved. They shall furnish a report on conditions then existing, of resilient flooring, doors, windows, walls, and other surfaces as compared with conditions of same as noted in first condition survey report:

1. Re-survey report shall also list any damage caused by Prime Contractor to such flooring and other surfaces, despite protection measures; and, will form basis for determining extent of repair work required of Prime Contractor to restore damage caused by Prime Contractor's workmen in executing work of this contract.

Protection: Provide the following protective measures:

1. Protection of interior of existing structures at all times, from damage, dust, and weather inclemency. Wherever work is performed, floor surfaces that are to remain in place shall be adequately protected prior to starting work, and this protection shall be maintained intact until all work in the area is completed.

1.10 DISPOSAL AND RETENTION:

Materials and equipment accruing from work removed and from demolition of buildings or structures, or parts thereof, shall be disposed of as follows:

1. Reserved items, which are to remain property of the Government, are noted on drawings as items to be stored. Items that remain property of the Government shall be removed or dislodged from present locations in such a manner as to prevent damage, which would be detrimental to re-installation and reuse. Store such items where directed by the GPM.

2. Items not reserved shall become property of the Prime Contractor and be removed by Prime Contractor from Medical Center.

3. Items of portable equipment and furnishings located in rooms and spaces in which work is to be done under this contract shall remain the property of the Government. When rooms and spaces are vacated by the Department of Veterans Affairs during the alteration period, such items, which are NOT required by drawings and specifications to be either relocated, or reused, will be removed by the Government in advance of work to avoid interfering with Prime Contractor's operation.

1.11 RESTORATION:

Remove, cut, alter, replace, patch, and repair existing work as necessary to install new work.

Except as otherwise shown or specified, do not cut, alter, or remove any structural work, and do not disturb any ducts, plumbing, steam, gas, or electric work without approval of the GPM.

Existing work to be altered or extended and that is found to be defective in any way, shall be reported to the GPM before it is disturbed. Materials and workmanship used in restoring work

01 00 00, General Conditions Page 13 of 61 shall conform in type and quality to that of original existing construction, except as otherwise shown or specified.

Upon completion of contract, deliver work complete and undamaged. Existing work (walls, ceilings, partitions, floors, mechanical and electrical work, etc.) disturbed or removed as a result of performing required new work, shall be patched, repaired, reinstalled, or replaced with new work, and refinished and left in as good condition as existed before commencing work.

At Prime Contractor's own expense, Prime Contractor shall immediately restore to service and repair any damage caused by Prime Contractor's Workmen to existing piping and conduits, wires, cables, etc., of utility services or of fire protection systems and communications systems (including telephone) which are not scheduled for discontinuance or abandonment.

Expense of repairs to such utilities and systems not shown on drawings or locations of which are unknown will be covered by adjustment to contract time and price in accordance with clause entitled "CHANGES" (FAR 52.243-4 and VAAR 852.236-88) and "DIFFERING SITE CONDITIONS"

(FAR 52.236-2).

1.12 PHYSICAL DATA:

Data and information furnished or referred to below is for the Prime Contractor's information, the Government shall not be responsible for any interpretation of or conclusion drawn from the data or information by the Prime Contractor. (FAR 52.236-4)

Government does not guarantee that other materials will not be encountered nor that proportions, conditions, or character of several materials will not vary from those indicated by explorations. Bidders are expected to examine site of work; and, after investigation, decide for themselves character of materials and make their bids accordingly.

1.13 AS-BUILT DRAWINGS:

The Prime Contractor shall maintain two full size sets of as-built drawings, which will be kept current during construction of the project, to include all contract changes, modifications, and clarifications.

All variations shall be shown in the same general detail as used in the contract drawings. To insure compliance, as-built drawings shall be made available for the GPM’s review, as often as requested.

Prime Contractor shall deliver two approved completed sets of as-built drawings to the GPM within 15 calendar days after each completed phase and after the acceptance of the project by the GPM and the CO.

Paragraphs A, B, & C shall also apply to all shop drawings.

1.14 USE OF ROADWAYS:

For hauling, use only established public roads and roads on Medical Center property.

1.15 TEMPORARY USE OF EXISTING ELEVATORS:

Prime Contractor may not use existing elevators on the medical center campus accept as directed by the GPM.

1.16 TEMPORARY TOILETS:

Prime Contractor and subcontractors and their employees may use toilets located in room F2-155 for men and F3-152 for Women.

Prime Contractor is responsible for cleaning these restrooms and maintaining the path from the construction area to the restrooms.

1.17 AVAILABILITY AND USE OF UTILITY SERVICES:

The Government shall make all reasonably required amounts of utilities for the completion of at the Kansas City VAMC for this project from existing outlets and supplies, as specified in the contract at no charge to the Prime Contractor. The Prime Contractor shall carefully conserve any utilities furnished without charge.

1.18 NEW TELEPHONE EQUIPMENT:

The Prime Contractor shall coordinate with the GPM and OI&T department the work of installation and connection of telephone equipment. This work shall be completed before the building is turned over to VA.

1.19 TESTS:

As per specification section 23 08 00, Commissioning of HVAC Systems, the Prime Contractor shall provide a written testing and commissioning plan complete with component level, equipment level, sub-system level, and system level breakdowns. The plan will provide a schedule and a written sequence of what will be tested, how and what the expected outcome will be. This document will be submitted to the GPM for approval prior to commencing work. The Prime Contractor shall document the results of the approved plan and submit for approval with the as built documentation.

Pre-test mechanical and electrical equipment and systems and make corrections required for proper operation of such systems before requesting final tests. Final test will not be conducted unless pre-tested.

Conduct final tests required in various Commissioning and other sections of specifications in presence of an authorized representative of the CO. Prime Contractor shall furnish all labor, materials, equipment, instruments, and forms to conduct and record such tests.

Mechanical and electrical systems shall be balanced, controlled, and coordinated. A system is defined as the entire system, which must be coordinated to work together during normal operation to produce results for which the system is designed. For example, air conditioning supply air is only one part of entire system, which provides comfort conditions for a building.

Other related components are return air, exhaust air, steam, chilled water, refrigerant, hot water, controls and electricity, etc. Another example of a system, which involves several components of different disciplines, is a boiler installation.
