X X X X The above referenced solicitation identified in block 9a for the construction of the parking garage at the Providence VA Medical Center is hereby amended as follows:
1.) See the attached responses to the RFIs which are hereby incorporated into the solicit ation and resulting contract.
2.) See the attached sign-in sheet from the site visit.
3.) All other terms and conditions remain unchanged.

Karla Rotondo Contracting Officer Q1: An onsite storage area for staging of 20-25 loaded trailers of precast will be required. Is there a location on the VA campus contractor can utilize for this staging area? The project site requirements are too restricted to allow ample staging area.
Per Specification 01 35 13 – Special Project Procedures, Part 1.4; there are no exclusive areas within the campus that can be given to the contractor for their storage needs.
The Contractor will be responsible for coordinating the staging, storage, and delivery of precast units, materials, and equipment in a manner that minimizes impacts to the operation of the Hospital, patients, staff, public and motor vehicle traffic. The costs associated with this coordination, including offsite storage, vehicle escorts, police details, and other costs associated with storage, staging and delivery of materials, equipment, etc. shall be included in the Contractors bid price.
Q2: Reference Drawing C-101, General Note 7. - Please confirm contractor is required to pay required private utility fees.
General Note #7 on C-101 referrers to the fees associated with National Grid providing a new electrical service originating from the utility pole along Chalkstone Ave. as shown on the plans. The estimate from National Grid to complete the work is $26,000 and shall be included in the base bid for the project.
Q3: Are there any utilities located below the sidewalk along Chalkstone Ave. that will be encountered during installation of the underground electric conduit.
Yes. Fiber optic cables are known to exist below the sidewalk along Chalkstone Ave., between the utility pole and the VA property.
Q4: If the elevator manufacturer is unable to fit the selected car in the elevator shaft shown, can the shaft be expanded to support the rough opening requirements? Otherwise, is a 350FPM elevator acceptable?
The elevator shaft will be modified to accept specified elevator design requirements. This is an item that must be coordinated by the contractor. Adjustment of the shaft dimensions to endure the approved elevator is accommodated shall be at no cost to the government.
Q5: Can the Site Superintendent share other duties, such as the Site Safety & Health Officer (SSHO)?
In general the Site Superintendent may share other duties in accordance with the FAR / VARR. Please note that the Specifications (Section 01 35 26 – Safety Requirements, Part 1.4-B.4.g.3) the site superintendent can share other duties such as Site Safety and Health Officer/SSHO provided the site supervisor duties are not neglected. Please note that the SSHO must meet the requirements of all applicable OSHA standards. Please review specification section 01 35 26 – Safety Requirements for SSHO requirements.
