9. Prices shown herein are “net” ceiling prices-not to exceed rates. The net ceiling rates include malpractice insurance for non-personal services task-orders and exclude travel and lodging.

Work Week: Sunday – Saturday

Overtime Rate(s): 1.4 times hourly rate for all hours worked in excess of 40 hours per week per state and local laws, and 1.8 for hours worked in excess of 12 hours per day for double time which applies only to allied healthcare payments.

Holiday Rate(s): 1.5 times hourly rate for all hours worked in excess of 40 hours per week per state and local laws for Federal holiday’s placement actually works.

On-Call Rate(s): $7.00 per hour. Vendor includes telephone consultations. When on call, the individual is to carry a beeper or cell phone and answer calls. The individual must be within 30 minutes from facility or a 30 mile radius from facility or an approved radius as determined by the task ordering facility.

Call Back Rate(s): Regular hourly rate applies minimal 2 hours,vendor must be physically on site to receive call back rate
