9. Prices shown herein are “net” ceiling prices-not to exceed rates. The net ceiling rates include malpractice insurance for non-personal services task-orders and exclude travel and lodging. Personal Services task-orders will also be accepted. See page 3 of this price list.

11. Medical Liability Insurance Limits: In the amount of $5 million per occurrence/$5 million aggregate.

Shift Differentials: No shift differential
Work Week: Sunday – Saturday
Overtime Rate(s): : 1.3x base rate for hours worked in excess of 40 hours in accordance with your federal, state and local labor laws. 1.6x base rate for hours worked in excess of 12 hours for California.
Holiday Rate(s): 1.3x the normal hourly rate, such rate applies to recognized federal holiday in which the placement actually works.
On-Call Rate(s): On-Call offered at $2.00 per hour. Provider must carry a cell phone or beeper and provide phone consultation if needed. The individual must remain within 30 minutes from facility or within a 30 mile radius from the facility.
Call Back Rate(s): Regular rates apply for Call-Back with 1.3x base rates after 40 hours. Call- Back applies when physically on site.
Minimum Qualifications of Professionals:

Registered Nurse - General
Graduation from an accredited School of Nursing, Current R.N. license in state of assignment.
Minimum 1 Year experience within preceding 3 years.
Valid RN License in state of assignment.

Registered Nurse
– Specialty
Graduation from an accredited School of Nursing, Current R.N. license in state of assignment.
Minimum one (1) year recent clinical experience in specialty within the past three (3) years.
Valid RN license in state of assignment.

Pharmacist
Bachelors Degree in Pharmacy from an accredited university or PharmD.
One year of applicable experience in the past 3 years.
Valid RPH,license in state of assignment.
Physician Assistant
Graduation from a physician assistant educational program, accredited by the Accreditation Review Committee on Education for the Physician Assistant (ARC- PA) or its predecessors, at a college, university, or educational institution that is accredited by an accrediting body or organization recognized by the Secretary, U.S.
Department of Education.
One year of applicable experience in the past 3 years.
Valid Bachelor’s degree and Accredited PA program required.
Certification from NCCPA.
