Contract Detail:
1. Maximum order: $1,000,000 per order.
2. Minimum order: Four (4) hours for supplemental/per-diem assignments. Cancellation of these assignments must be made at least two (2) hours prior to the start of a shift or a charge for two (2) hours will be made. However, the Government still reserves their rights for Termination for Convenience. 13 weeks at 36-40 hours per week is the minimum for travel assignments.
3. Geographic coverage: AK, AZ, CA, CO, CT, DE, Washington DC, KS, KY, FL, GA, LA, IA, IL, IN, MA, MD, ME, MI, MN, MO, MT, NH, NJ, NY, NC, ND, NV, OH, OK, OR, PA, RI, SC, TN, TX, VA, WA, WV, and WI.
4. Payment terms: Net 30 days.
5. Government Commercial Credit Card: Accepted up to the micro-purchase threshold.

The net ceiling rates include malpractice insurance for non-personal services task-orders and exclude travel and lodging.
9. Medical Liability Insurance Limits: $1 Million per occurrence; $3 Million aggregate.

Work Week: Sunday – Saturday
Overtime Rate(s): 1.4 times the hourly bill rate for hours worked in excess of 8 per day or 40 per week.
1.8 times the base rate for hours worked in excess of 12 per day when double time is legally required.
Overtime and double time will be applied in accordance with applicable state and local labor laws.
Holiday Rate(s): 1.4 times the hourly bill rate for all Federal Government holidays worked.
Shift Differentials: None.
On-Call Rate(s): $7.50/hr. Includes telephone consultation. Individual must remain within a 30 minute or 30 mile radius of the ordering facility.
Call Back Rate(s): 1.5 times the base rate will apply once the individual is physically on-site. There is a 2 hour minimum call back charge.

Minimum Qualifications of Professionals
Favorite requires proof of at least one year of current experience for each professional in the area of placement or as defined by client contract or law or regulations.

Certification: Follow all requirements for licensure and other associated regulations governing the practice of a physician assistant in the appropriate state(s).
Successfully passed the Physician Assistant National Certifying Examination, administered by the National Commission on Certification of Physician Assistants (NCCPA).
Maintain applicable state registration or certification.
Fulfill physician assistant-specific continuing education requirements.
