Overtime Rates. Will be charged at a Rate of 1.3x the Regular Hourly Rate as quoted in this Price List.

(Not including H&W and all Insurances and Non-Allowable Costs.)

Shift Differential Charges. Shirt Differential Charges are applicable only to weekend hours (defined as

7:00p.m. to Monday Morning 7:00a.m. (Local Time). The rates charged during those hours shall incur a 5% increase above the normal hourly rates as charged in our normal “net” ceiling prices above.

On-Call. On-Call nurses will carry a beeper and remain within a 30-minute radius of the facility.

Telephone Consultation is included. $5.00 per hour will be paid while the nurse is on On-Call Status.

Call-Back. Call-Back is when a nurse is called back to work and out of on-call status. The nurse will be paid the regular hourly rate in this Price List (with a minimum of one hour, regardless if less time is worked), unless overtime is being incurred (over 40 hours worked) and does not include on-call time.

Holidays. Will be charges at a Rate of 1.3x the Regular Hourly Rate as quoted in this Price List. (Not including H&W and all Insurances and Non-Allowable Costs.)

Workweek. The Workweek as defined in this Schedule Contract runs from 12:00am Sunday through

11:59pm Saturday.

MINIMUM QUALIFICATIONS OF PROFESSIONALS

POSITION: REGISTERED NURSE – GENERAL

Experience: Minimum: Twelve (12) Months Experience in the Past Twenty-Four (24) Months Preferred.

Education: (Minimum): RN – AD or Diploma, BSN Preferred.

Certifications: ACLS (Not Required).

Description: The Physical Therapist evaluates the extent of the patient’s disability in such areas as neuromuscular, musculoskeletal, sensorimeter, cardiovascular and respiratory functions. The Physical Therapist plans, implements and evaluates an appropriate treatment program under the direction of a physician.

POSITION: LABORATORY TECHNICIAN I

Description: The Laboratory Technician I typically performs tests and analysis in either a clinical or research laboratory. Task and Responsibilities include the processing of specimens, preparation of reagents, performance of testing procedures, reporting of laboratory results, quality control testing, instrument maintenance and troubleshooting. Other areas of work include verification of analytic accuracy, precision, sensitivity and reference ranges for test methods. Performs laboratory procedures; records observations, measurements and reports results. Is responsible for maintenance of cleans rooms and instruments and assists with the receipt and inspection of raw materials, their ordering and stocking. The Laboratory Technician I is also familiar with standard concepts, practices and procedures within a particular field.

Experience in Specialty Area: (Minimum): One or more years of full-time laboratory experience with excellent references.

Education: (Minimum): Associates Degree; successful completion of a Medical Technology Program that is accredited by the Committee on Allied Health Education and Accreditation (CAHEA), or equivalent training, education and experience. ASCP certification or equivalent nationally recognized medical technology certification or categorical certification.

Certifications: As Required, or ASCP certification or equivalent nationally recognized medical technology certification or categorical certification.

Description: The Laboratory Technician II typically performs a variety of defined research lab tests/experiments under general supervision. Minimum of a Bachelor’s Degree with post-graduate level training in biological, behavioral, social science or public health research environments: hands-on experience is a pre-requisite for this position. The Laboratory Technician II is also able to lead or assist in the planning, execution, budgeting and reporting of testing and research on time and in compliance with processes and regulatory requirements. Performs complex tasks, utilizing standard research protocols in clinical; health sciences laboratory and/or field settings. Additional duties include conducting experiments, collection, analysis and interpretation of results. Has a knowledge base of practices, procedures and concepts within a particular area of specialization.

Experience in Specialty Area: (Minimum): Two or more years of full-time laboratory experience with excellent references.

Education: (Minimum): Bachelor’s Degree and successful completion of a Medical Technology Program that is accredited by the Committee on Allied Health Education and Accreditation (CAHEA), or equivalent training, education and experience.

This individual is a strong team player with excellent judgement, critical thinking skills and strong collaboration abilities. Tasks and responsibilities include performing complex tasks that involve standard research protocols in clinical, health sciences laboratory and/or field settings. Executes complex biological, hematological, chemical, immunologic, microscopic and bacteriological testing. Performs laboratory procedures, records observations, measurements and test results. Individuals in this position may specialize in categories such as Immunology, Immunohematology (Blood Bank), Clinical Chemistry, Microbiology, Cytotechnology and Molecular Biology.

Experience in Specialty Area: (Minimum): Three or more years of full-time laboratory experience with excellent references.

Education: (Minimum): Bachelor’s Degree with a focus on one or more of the following disciplines: Life Sciences, Medical Courses, Microbiology, Chemistry, Statistics or other specialized courses relating to laboratory and medical technologies.
