9. Prices shown herein are “net” ceiling prices-not to exceed rates. The net ceiling rates include malpractice insurance for non-personal services task orders and exclude travel and lodging. Personal Services task orders will also be accepted. See pages 3 through 5 of this price list.

11. Medical Liability Insurance Limits: $1 Million per occurrence; $3 Million aggregate.

For Physicians:
No overtime - physician receives the normal hourly rate.
For Allied Healthcare Providers (SINs 621-030 (Physician Assistants), 621-053 (CRNA), 621-054 (Nurse Practitioner), 621-050 (Rad Tech, CT, Ultrasound) and 621-058 (Cert.Nurse Midwife):
1.4x the normal hourly rate for any hours worked in excess of 40 hours; in accordance with state and local labor laws.
Holiday Rate(s) (for all Federal Government Holidays):
For Physicians:
No overtime–the normal hourly rate applies.
For CRNAs and CNMs:
1.4x the normal hourly rates for any hours worked in excess of 40 hrs; in accordance with state and local labor laws.
For Physicians (excludes 621-002 Emergency Medicine):
Weeknight On-Call Rate: Weeknight is a one -time flat charge for a 15.5 hour period when a physician is required to be available by phone or pager and be able to provide phone consultation. The weeknight period is generally 4:30 pm- 8:00 am. Physician must be within a 30 mile or a 30 minute radius from the facility.
Weekend On-Call Rate: Weekend on call is for a 24 hour period on Saturday and Sunday when a physician is required to be available by phone or pager and be able to provide phone consultation. Physician must be within a 30 mile or a 30 minute radius from the facility.
Call Back Rate: If the physician must report to the facility and see patients, the normal hourly rate applies when physically onsite for all hours worked.
For Allied Healthcare Providers (SINs 621-030 (Physician Assistants), 621-053 (CRNA), 621-054 (Nurse Practitioner), 621-050 (Rad Tech, CT, Ultrasound) and 621- 058 (Cert. Nurse Midwife):
Weeknight/Weekend On Call/Call Back (excludes 621-050 RTT, CT, Ultrasound Techs): $10.00 per hour for the time the provider is on call. On call provider is to carry a phone or pager and be able to provide phone consultations. The provider must be within a 30 minute or a 30 mile radius from the facility.
Call Back: The normal hourly rate applies if less than 40 hours worked. In excess of 40 hours, overtime of 1.4x the normal hourly rate applies for each hour worked when called back and physically onsite at the facility. No minimum billable hours.
Minimum Qualifications of Professionals:
621-001 through 022 Physician Qualifications
Each physician will possess: A Medical (MD) or Osteopathic (DO) degree or Certification by The Educational Commission for Foreign Medical Graduates (ECFMG) if appropriate. Completion of a Residency Training Program in their specialty approved by the Accreditation Council of Graduate Medical Education (ACGME). Completion of a Fellowship Training Program in their subspecialty approved by the Accreditation Council of Graduate Medical Education (ACGME), if required for certification. Board Certification or Eligibility by their specialty American or Osteopathic Board. A current, valid medical license in one of the U. S. states, territories or commonwealths. Applicable certifications according to requirements for specialties (i.e. BLS,ACLS, PALS, NALS, etc…).
Minimum one year experience in their respective specialty.
621-002 Emergency Medicine Trauma I
In addition to the Physician qualifications listed above each physician shall be a Doctor of Medicine or Osteopathy who has successfully completed an approved Emergency Medicine Residency and is Emergency Medicine Board Certified.
621-030 Physician Assistants
A Bachelor of Science Degree and completion of an accredited Physician Assistant Program.
Certification by the National Commission on Certification of Physician Assistants. Subspecialty training as appropriate. A current, valid license in one of the U. S. states, territories or commonwealths.
Must have a minimum of one year experience.
621-050 Radiation Therapist/Radiological Therapeutic Technologist (RTT)
Must have completed an accredited educational program in Radiation Therapy. Must have certification by the American Registry of Radiologic Technicians (ARRT). A current valid license in one of the states, territories or commonwealths. Must have a minimum of one year experience.
621-050 Radiography, Computer Tomography (CT)
Must have completed an accredited educational program in Computed Tomography. Must have current valid registration/certification in one of the U.S. states, territories or commonwealths. Must have a minimum of one year experience.
621-050 Radiography, Ultrasound
Must have completed an accredited educational program in Ultrasound Technology. Must have current valid registration/certification in one of the U.S. states, territories or commonwealths. Must have a minimum of one year experience.
621-053 Registered Nurse (Anesthetist)
A Bachelor of Science in Nursing degree and a Masters degree in their specialty areas, such as Family Practitioner, Anesthetist or Midwifery. Certification in their specialty by the appropriate certifying organization, i.e. American Academy of Nurse Practitioners, American Association of Nurse Anesthetists or the American College of Nurse Midwifes Certification Council. Subspecialty training is appropriate. A current, valid license in one of the U. S. states, territories or commonwealths. Must have a minimum of one year experience.
