STATEMENT OF WORK (SOW)

THE PURCHASE AND INSTALLATION OF 1ea CORRUGATED BOX MAKER at DLA DISTRIBUTION Warner Robins, GA

1.0 Background:

A. The Defense Logistics Agency (DLA) is a United States (U.S.) Department of Defense (DOD) agency that provides worldwide logistics support for the missions of the Military Departments and the Unified Combatant Commands under conditions of peace and war.

B. DLA Distribution Warner Robins (DDWG) has a requirement for a Contractor to purchase, install, provide all management, supervision, labor, equipment, materials, tools, and supplies to perform this contract.

2.0 Scope of Work to be performed:

A. Contractor shall provide materials, supervision, tools, installation, labor, training, and materials to install and make operational the corrugated box makers. Contractor shall provide all transportation and rigging services to off-load, move, position, install one corrugated box maker within a designated location on DDWG.

B. At the time of contract award, the Contracting Officer (KO) will identify the on-site Government point of contact (POC) who will monitor performance to ensure services are received and carried out in accordance with the requirements in this SOW. All references in this SOW to “Contractor personnel” include both Contractor employees and subcontractors.

C. Contractor personnel shall be escorted by Government personnel at all times when accessing these secured buildings. Installation date shall be coordinated with the POC.

2.1 The following are the requirements for one corrugated box maker for the purchase and installation:

A. Box maker shall be able to cut, slot, crease, and process single wall and double wall corrugated fiberboard with a capacity bursting (Mullen) test up to 450#.

B. Digital entry and display for quick entry and recall of box data without having to reset. Programmable sequence control system to allow data entry time. (For box styles and length, width and height) Set up for the machine should be between 30 seconds to 2 minutes for each new box.

C. The box maker shall accept a width of 86”

D. The machine should have the ability to make boxes of these types:

FTC- full telescope carton RSC- regular slotted cartons OSC- overlap slotted carton FOL- full overlap slotted carton HSCC-half slotted carton DBLCC- double cover carton FTHS-full telescope half slotted (2pc) OPF- one-piece folder FPF- five-panel folder TS- triple slide carton PADS and LINERS

E. Variable crease adjustments

F. Slot depth capacity of 19”, with a 38” width capacity

G. Include 5 cut heads & 4 score heads

H. Machine main voltage to be 208 AC volts 3 phase

I. Automatic air blower to clean out cut-off section

J. In-feed & out-feed table

K. Safety interlocks at access doors to prevent accidental injury

2.2 Training

Upon completion of installation and testing the contractor shall provide the following documentation and training.

A. Maintenance manuals for corrective and preventative maintenance (3 copies) and list of spare parts.

B. Operator manuals (3 copies) Provides details for operational procedures for equipment and safety procedures for equipment operation.

C. Operator training: The contractor shall provide minimum 8 hours training for 4 DDWG warehouse employees for the equipment as follows:

a. Procedure for operation of the equipment

b. Provide training of programming sequence control for data entry.

c. Safety training

d. Troubleshooting procedure for jam clearing and set-up

2.5 Period of Performance

A. The Contractor shall complete all equipment installation and training within 24 weeks after the date of contract award.

3.0 Responsibilities of the Government

A. The Government shall provide floor space not less than 96” X 189” that is sufficient to withstand 10,000 lbs.

B. The Government will provide an electrical power for the box machine.

(3 phase/30 amp/208 volts)

C. The Government will make available air pressure line (90 psi) for the box machine.

D. The Government will provide sufficient corrugated board stock to run machine during operating training.

E. The Government will make available at no cost to the Contractor electricity for Contractor connection at Government provided work areas.
