STATEMENT OF WORK

PROPANE SERVICES

SEBA DALKAI BOARDING SCHOOL

I. Background: Arizona Navajo South Agency, Bureau of Indian Education (BIE), Facility Management must have uninterruptible supply of LP Gas to Seba Dalkai Boarding School.

The project is located on the Navajo Indian Reservation at the BIE, Arizona Navajo South Agency, Seba Dalkai Boarding School (SDBS), Winslow, Navajo County, Arizona.

II. Tasks:

a. Provide uninterruptible supply of LP gas to Seba Dalkai Boarding School.

b. SDBS has two (2) 1000 gallons tanks located Northeast of main school building 1407

c. Two 1000 gallons located north of building 1363.

Total of four (4) tanks.

Spillage: Accidental release or spillage during delivery of the product by the supplier shall result in the supplier being held responsible for all costs associated with the cleanup and disposal of all contaminated soil.

When a spill occurs, the supplier shall immediately notify the Department of Environmental Protection (DEP). Cleanup and disposal must be conducted in accordance with state and federal regulation guidelines.

III. Responsibilities:

The Contractor shall furnish and pay for all supervision, labor, equipment tools, and materials, in performing all operations and construction/installation of new parts, pipe and related fittings, shutoff valves, transportation, insurance, Arizona Gross Receipt Taxes, Navajo Nation Business Activity taxes at 6%, and other necessary incidentals for improvement and fixture installation requirements.

IV. Repair Service:

a. . Repair service shall be defined as a service call to correct any problem with propane tanks or condition of the propane tanks that requires the contractor to provide repair prior to the next scheduled service.

b. Incidental charges associated with this repair, including but not limited to, fees, charges, tickets, mileage, etc. shall be incurred by the Contractor at no additional charge to the Government.
