STATEMENT OF WORK

ALERT FACILITY & DORMITORIES

INTERNET SERVICES

OFFUTT AFB, NE

1. DESCRIPTION OF SERVICES

The Contractor shall provide commercial internet service to selected areas at Offutt AFB, NE. All work shall be performed in accordance with all applicable federal, state, and local laws, regulations, codes, including any supplements or revisions, industry standards, and this Statement of Work (SOW).

2. SCOPE OF WORK

The contractor shall provide internet services at the required speeds to the designated locations, which include the Alert Facility (Building 524) and common areas located in select Dormitories (Buildings 364, 365, 367, and 369).

2.1 The contractor shall provide all labor, tools, materials, and services required to install and provide high-speed internet services to the designated buildings. Services shall include any equipment, wiring, or infrastructure to ensure the service provider’s service solution is compatible with the government’s current infrastructure without additional expense to the government. Internet speeds required:

a. Alert Facility (Building 524)- Speeds shall not be lower than 1 Gbps download and 35

Mbps upload.

b. Dormitories (Buildings 364, 365, 367, and 369)- Speeds shall not be lower than 8

Mbps download and 2 Mbps upload.

2.2 The contractor shall install hand holes and ducting as necessary in order to install the necessary cabling to support commercial internet services within the designated buildings.

The government does not authorize aerial cable installations. The service provider must exercise extreme caution to avoid any structural or property damage. If any damages occur during installation, said damages shall be reported to the government’s point of contact (POC), the contractor will be held responsible for any costs associated with the repairs.

The service provider is responsible for the removal of all debris directly resulting from this project.

2.3 The Contractor shall connect to government-provided power connection points and termination frames. Contractor shall install all hardware and services shall be operational within 45 days of award.

2.4 The Contractor shall coordinate with the government POC prior to any installation. Upon contract award, a listing containing the primary and alternate POCs, email addressed, and telephone numbers will be provided.

2.5 The Contractor shall provide and install transmission equipment and cables for local internet access and service circuits to the buildings.

2.6 If receivers are wireless, coordination must be made with the 55 CS/SCXT (POC for Offutt AFB is Installation Spectrum Manager Mr. David Clingerman) in regards to allowable frequencies and permissions for those frequencies. Contractor shall provide proof of or lack thereof in regards to Part 15 certification as listed in Title 47 CFR 15 (http://www.access.gpo.gov/nara/cfr/waisidx_04/47cfr15_04.html).

2.7 For any transmitting device to be installed on Offutt AFB the contractor shall provide Mr.

David Clingerman (and submit a copy to the contracting officer) the following information:

• Power output

• Modulation type

• Exact Frequency

• Equipment Manufacturer http://www.access.gpo.gov/nara/cfr/waisidx_04/47cfr15_04.html)

• Model Number

• Location of equipment physical address/description of location

2.8 Contractor Equipment Listing. Upon request, the contractor shall provide the government with maps, plans, or equivalent documents describing the location of all equipment, facilities, and material on the base.

2.9 Service Support and Repair Services. Selected service provider must guarantee that the contracted service will be available 24 hours per day, 7 days per week (24/7). Selected service provider shall identify a point of contact and customer service phone number, whose responsibility will be to provide assistance with any internet related issues. The identified personnel must provide a solution within 24 hours of a reported malfunction or outage. If a solution cannot be identified within 24 hours, the POC must advise government personnel on the estimated timeframe for repairs or solutions. If the problem cannot be resolved within the arranged timeframe, the government should receive credit for the loss of service.

2.10 Alert Facility (Building 524) specific requirements:

2.10.1 The contractor shall install the hardware required to establish connection within the parameters of the common areas, ensuring that no internet is available beyond those zones.

2.10.2 Entire second floor of Building 524 (17,269 sq ft) requires coverage (stairwells not included) while first floor requires only partial coverage (~15,000 sq ft) to include rooms 107, 114, and 117, 133, and 144 (Bldg. 524 schematic available upon request and after signature of non-disclosure).

2.10.3 Provide one (1) 50 Mbps Cable Modem for room 134 to utilize for Electronic Flight Bag (EFB) program utilization and download requirements.

2.10.4 The 595 C2G may require a transfer services to another bldg/location (based on runway closure/construction at Offutt AFB). In the event a relocation of services is required, a request for quote may be solicited from the contractor for the transfer of the equipment or the purchase of new equipment, whichever is most advantageous to the government. Following any relocation, Bldg 524 tenants may choose to continue WiFi services.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES

3.1 The government will furnish site access, restroom facilities, building electrical power, security police, and fire protection. In the event of an emergency, dial 911. Emergency services are at the contractor’s expense.

3.2 The government will provide any required permits that may be necessary for work to be performed.

3.3 The government will arrange access into all applicable facilities in conformance with the installation schedule to include providing on base transportation where commercial vehicle access is restricted.

4. GENERAL INFROMATION

4.1 Normal hours of operation are Monday through Friday 0730 to 1630 excluding federal holidays. The following federal holidays are observed: New Year's Day, Martin Luther King, Jr Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day.

4.2 Contractor will require access to a Federal Installation. For access to Offutt AFB and facilities the contractor shall coordinate not less than 24 hours in advance with the Primary POCs provided in a separate letter. In the event that access is required immediately following a weekend or holiday, the contractor shall coordinate during normal hours of operation with the appointed POC the business day prior to that weekend or holiday. If deemed necessary, pass applications will be provided by the government. Access to certain areas of the base can be limited to certain days and times.

4.3 The contractor shall not attempt coordination with personnel not identified as a Contracting Officer Representative (COR) or designated government POC unless instructed to do so by the Contracting Officer or Contract Administrator. The contractor shall accomplish all communication in writing and include, the Contract Administrator, COR or designated government POC, and the Contracting Officer.

5. CONTINUATION OF MISSION ESSENTIAL FUNCTIONS

In accordance with Department of Defense Instruction (DoDI) 1100.22, Guidance for Determining Workforce Mix, and any applicable Air Force implementation, unless otherwise directed by an authorized Government representative, it is determined that services provided under this contract are not mission essential for performance during a crisis.
