STATEMENT OF WORK
STERILIER AND CABINET INSTALLATION
1.0 INTRODUCTION
1.1 The Navy Medicine Readiness & Training Command (NMRTC) Pearl Harbor, Dental Directorate at Joint Base Pearl Harbor-Hickam requires deinstallation and removal of existing sterilizer and installation of new sterilizer and cabinet package located at Bldg.
1407, 1253 Makalapa Drive, Joint Base Pearl Harbor Hickam, Honolulu, HI 96860.
2.0 SCOPE
2.1 The contractor shall provide all labor, materials and equipment necessary for the removal and disposal of existing sterilizer currently located in Makalapa Sterile Processing Division (SPD) located at NMRTC – Pearl Harbor, and all labor, materials and parts to install the new sterilizer and cabinetry items listed in Enclosure (1). At the conclusion of the work, the newly installed sterilizer and cabinet must be safe for use and function as intended. Equipment parts that are replaced/repaired under this contract become or remain as appropriate, the property of the Government.
IN NO INSTANCE SHALL THE CONTRACTOR PROCEEED WITH WORK
OUTSIDE THE CURRENT SCOPE OF THIS CONTRACT/INCREASE THE COST
OF THIS CONTRACT WITHOUT GOVERNMENT APPROVAL TO USE OF
EXISTING CONTRACT CLIN FOR MAJOR REPAIRS, OR IN THE ABSENCE OF
SUCH A CONTRACT CLIN, RECEIPT OF BILATERAL SIGNED MODIFICATION
TO THE CONTRACT OR MUTUAL AGREEMENT BY BOTH PARTIES TO AN
ALTERNATIVE CONTRACT/PAYMENT ARRANGEMENT (I.E. SEPARATE
CONTRACT OR PAYMENT BY GOVERNMENT COMMERCIAL PURCHASE
CARD (GCPC)). THIS INCLUDES ANY SURCHARGE, AFTER HOURS
EMERGENCY SERVICE, OR ANY OTHER CHARGES/COST THAT ARE
OUTSIDE THE CURRENT BASIC OR MODIFIED CONTRACT PRICE.
3.0 CONTRACTOR CHECK-IN/CHECK-OUT:
The Contractor, or his representative, is required to check-in and out at Navy Medicine Readiness & Training Command Joint Base Pearl Harbor-Hickam, Makalapa Branch Health Clinic with the General Dentistry Department Head or his/her designee during the hours of 0730 - 1600 hours, Monday through Friday, prior to starting work on the first day of work, and upon completion of all work on the final day. The contractor and General Dentistry Department Head (or his/her designee) will perform a final walkthrough upon completion of all work covered in this statement of work, identifying any incomplete work, or damages related to the deinstallation of existing sterilizer and installation of the new sterilizer and cabinet and correcting any identified deficiencies related to the installation covered under this contract.
4.0 GENERAL INFORMATION :
4.1 Exclusions: The Government will only provide the parts listed in Enclosure (1). Any additional parts required to deinstall existing sterilizer and install new sterilizer and cabinet shall be provided at cost to the contractor.
4.2 Title to Equipment: The contractor shall not assume possession or control of any part of the equipment. The Government retains ownership to title thereof.
4.3 Liability: The contractor shall not be liable for any loss, damage, or delay due to any cause beyond his reasonable control including but not limited to, acts of government, strikes, lockouts, fire, explosion, theft, floods, riot, civil commotion, war, malicious mischief or acts of god.
4.4 Utilities: The contractor may use Government utilities, (e.g. electrica1 power, compressed air, and water) that are available and required for any service performed under this contract. Contractor's electrical equipment must be approved by the building engineer to ensure compatibility with NMRTC JBPH-H electrical wiring and equipment.
4.5 Access to Equipment: The contractor shall be provided reasonable access to all spaces, which are to be serviced, and utility outlets required to complete the service. The contractor shall be free to start and stop all primary equipment incidentals to the operation of the maintained equipment after permission is received from on duty personnel responsible for such equipment.
4.6 Government Personnel: NMRTC JBPH-H employees will not perform installations or attempt repairs to materials while such materials is under the purview of this contract unless agreed to in writing by the contractor.
4.7 Labor: All compensation for labor, parts, and materials are included in the contract price unless otherwise stated (i.e. $ limitation, exclusions).
4.8 Work Hours: All work will be completed during normal working hours Monday through Friday 0730-1600.
