PERFORMANCE WORK STATEMENT

STATEMENT OF WORK

11 April 2022

1. SCOPE: This Statement of Work (SOW) specifies the requirements of DEVCOM-SC-TSD-STTC EEO Reasonable Accommodations Personal Assistant Services for DEVCOM-SC-TSD-STTC employee.

2. APPLICABLE DOCUMENTS: The applicable top-level documents are contained in Contract Document Requirements List (CDRL), by number, title, date, contract reference, and category. The document versions specified on the DSL take precedence over the generic references (those without the revision letters) cited in the SOW. Unless otherwise indicated, copies of specifications, standards, and handbooks listed are available from the Document Automation and Production Service, at the Acquisition Streamlining and Standardization Information System (ASSIST) website: https://assist.daps.dla.mil/quicksearch/.

3. REQUIREMENTS: The contractor, as an independent contractor and not an agent of the U.S. Government, shall provide services in support of DEVCOM-SC-TSD-STTC for the following:

3.1 Primary Place of Performance: The United States Military Academy, 606 Thayer RD, West Point, NY 10996.

3.2 Period of Performance: No more than 48 months to include a base period of twelve (12) months and three (3) option periods of twelve (12) months each.

3.3 The contractor shall provide one full time equivalent, 40 hours a week, personal assistance services (PAS) for a government employee with disabilities. There should be arrangements for back-up personal assistant if the one assigned cannot provide the agreed-upon services for any reasons.

i The contractor will not provide PAS on federal holidays or when the employee is on leave (sick, annual, etc.). For purposes of estimation, there are 2,080 total work hours in a year. This employee should not take less than 325 hours of total leave including federal holidays and other leave for a balance of 1755 work hours per year.

ii The contractor will have to provide up to 14 hours of PAS per day when the employee is traveling for work. For purposes of estimation, assume there will be up to 20 days of PAS needed while the employee is in a travel status. This means that in addition to the 1755 work hours per year, PAS will be needed for no more than an additional 120 overtime hours (6 hours x 20 days).

3.4 The contractor shall assist the employee with opening doorways and accessing the elevators. The contractor will meet the employee at his vehicle in the parking lot next to his place of work at West Point Military Academy each morning to insure the employee can safely exit his vehicle. The contractor will escort the employee to all work locations throughout the day, as required. At the conclusion of the day, the contractor will escort the employee to the parking lot/awaiting transportation and carrying the employees bags to insure he is safely in his vehicle.

3.5 The contractor shall assist the employee with trips to the restroom to include ensuring there are no other occupants in the restroom and toileting assistance will be needed. This can include maneuvering, orienting, reaching and grabbing items such as paper towels.

3.6 The contractor shall take notes during presentations and meetings for the employee acting as a scribe. Also assistance will be required for some fine motor skills, use of any buttons, snaps, CAC card inserted in the elevator, some computer assistance, opening food packages, carrying of bags/laptops. The contractor shall assist in the organizing and maintenance of files and manuals, photocopying, and help with other duties requiring physical demands according to the limitations of the employee.

3.7 The contractor shall assist at lunch with carrying of lunch trays, opening food packages or drinks, and any logistical needs involved with food and/or lunch.

3.8 Contractor personnel providing PAS shall be eligible to receive and maintain a common access card or CAC. To obtain a CAC the recipient will have two background checks: 1) a Federal Bureau of Investigation (FBI) fingerprint check, and 2) A National Agency Check with Written Inquiries (NACI) check. Generally, the background check is to insure the individual is reliable, trustworthy, and of good moral character. A history of substance abuse, criminal behavior, or association with terrorist organizations will almost always disqualify someone from passing a background check and obtaining a CAC. The government will assume the financial cost of the background check needed to obtain the CAC. If the vendor cannot adequately provide an individual(s) who can pass a background check to receive a CAC, the contract may be terminated by the government.

3.9 The contractor should be physically able to lift a 130-pound person in the case of a fall and able to assist in an emergency or accident type situation (illness, fire drills, building evacuation, etc.)

3.10 The contractor shall travel to work in various US Army locations in conjunction with the employee’s needs and meetings. The contractor shall be available for the employee’s various work locations to include telework.

3.11 The contractor shall attend official travel with the employee as needed. Travel-related duties may include as described above, transferring to/from all modes of transportation including aircraft, busses, taxis, rental vehicles, etc. Contractor shall assist in dressing and grooming, preparing meals, transferring to and from bed, and facilitating the logistical needs of the employee. Job-related duties/support shall also be required during official travel such as reading and note-taking.

i The Government will provide payment for all travel related expenses of contractor including parking at terminal, airfare, ground transportation, rental vehicle, hotel, and standard government meals and incidental expenses allowance in accordance with the Joint Travel Regulations (JTR).

ii The Government will make all travel arrangements for contractor and employee through the Defense Travel System to insure they are on the same flights/schedule and are staying at the same hotel. Whenever possible, contractor shall have an adjacent or nearby hotel room to employee to be available in the event of a critical need (e.g., fire alarm evacuation, toileting in the middle of the night).

4. MANAGEMENT:

4.1 The contractor shall plan, schedule, budget and control all resources allocated to meet the requirements of this contract.

4.2 The contractor shall prepare a monthly Performance and Cost Report IAW CDRL and Other Direct Cost.

5. SECURITY:

5.1 The Government will provide access to all training content/systems required for all training requirements.

i AT Level I Training. This provision/contract text is for contractor employees with an area of performance within an Army controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to government installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the Government Technical Representative or to the contracting officer, if a Government Technical Representative is not assigned, within 30 calendar days after completion of training by all employees and subcontractor personnel. ii iWATCH Training. This standard language is for contractor employees with an area of performance within an Army controlled installation, facility or area. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity ATO). This local developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the Government Technical Representative. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the Government Technical Representative NLT 30 calendar days after contract award.

iii Access and General Protection/Security Policy and Procedures. This standard language text is for contractor employees with an area of performance within an Army controlled installation, facility or area. Contractor and all associated sub-contractor’s employees shall comply with applicable Installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

iv For Contracts that require OPSEC Training. Per AR 530-1, Operations Security, new contractor employees must complete Level I OPSEC training within 30 calendar days of their reporting for duty. All contractor employees must complete annual OPSEC awareness training.

v Contractor Employees Who Require Access to Government Information Systems. All contractor employees with access to a government info system must be registered in the ATCTS (Army Training Certification Tracking System) at commencement of services, and must successfully complete the DOD Information Assurance Awareness prior to access to the IS and then annually thereafter.

vi For Information assurance (IA)/information technology (IT) training. All contractor employees and associated sub-contractor employees must complete the DoD IA awareness training before issuance of network access and annually thereafter. All contractor employees working IA/IT functions must comply with DoD and Army training requirements in DoD 8570.01, DoD 8570.01-M and AR 25-2 within six months of employment.

vii For information assurance (IA)/information technology (IT) certification. Per DoD 8570.01-M, DFARS 252.239.7001 and AR 25-2, the contractor employees supporting IA/IT functions shall be appropriately certified upon contract award. The baseline certification as stipulated in DoD 8570.01- M must be completed upon contract award.
