Shiprock IHS Service Unit
Northern Navajo Medical Center Shiprock NM, 87420
STATEMENT OF WORK
2021 Diesel Fuel for Generator and Boiler's
1.0 Introduction
The Shiprock Service Unit requires having an outside vendor to refill the diesel fuel tanks for each of the emergency generators and also to measure the tanks when completed with the filling of the tanks. The diesel fuel shall be a #2 Dyed for use wi th other than highway vehicles. A total of tank capacity is 10000 gallons for the north generator and 8000 gallons for the south generator. The requirement for the reserve capacity of each tank is approximately 3500 gallons, which is a 4 day supply to operate the emergency generators. Additionally, there will be one 100 gallon tank with a 2" opening which will be nearby the reserve tanks and shall also be fueled at the same time as the reserve tanks with the same fuel. An es t imated total of 22,200 gallons m a y be required to be distributed through the year.
2.0 Staffing roles and Government Responsibilities
• Start of Project shall begin from Date of award through one year.
• NNMC Contracting Officer has decision making authority, including approval of changes, reports, documentation and deliverables. The Shiprock Service Unit will provide no items. The Contracting Officers Representative (COR), will represent IHS and will monitor the contract, work performance and approve invoices for payment. Contractor shall report to the COR when contacted for delivery of fuel.
COR shall request delivery of fuel via email on an As Needed Basis,
2.1 Contractor Responsibilities
The Contractor shall provide phone number and email contact information for 2 contract personnel who shall be available Monday - Friday 0800 to 1630 and respond to requests within 4 hours of contact by the Government personnel.
• The Contractor shall schedule the delivery within 3 business days of the request from the Government.
• The Contractor shall provide labor, tools, materials and transportation to refill the above ground tanks, and the 100 gallon tank located near each generator.
• The refilling of the tanks shall be during the working hours of 0800 to 1630, Monday through Friday, except Federal Holidays.
• Contractor shall provide identification tags or personal picture identification to each employee, indicating they are employees of the Contractor.
• Contractor shall schedule work, as not to interrupt the daily operations of the
Shiprock Service Unit.
• Contractor shall be responsible to collect trash and diesel fuel spill generated and dispose of daily, at the Contractors Facility.
• Parking of vehicles or equipment shall be designated by the Security
Department, Shiprock Service Unit.
3.0 Assumptions
• The generators shall not be affected by the filling of the above tanks.
• A spill kit shall be onboard the transport, and the operator shall have working knowledge of the usage of the spill kit
4.0 Risks
• The contractor shall identify any risk, he/she is aware of and how he/she plans to mitigate these risks.
5.0 Scope of Work
• Contractor shall post signs reflecting the work is in progress.
• Contractor shall wear proper attire PPE when working on the NNMC facility.
• Grounding of transport shall be required before dispensing fuel.
• Fuel shall be dispensed into the above tank for the North generator and the
South Generator.
• Fuel levels shall be verified with tank 'stick' or sounding tape and recorded.
• Area shall be cleaned upon completion of project.
5.1 Inclusion
• All task identified in 5.0 (Scope of Work).
5.2 Deliverables
• Delivery tally sheet shall be delivered to the NNMC personnel upon delivery of fuel. Invoice will be mailed to Finance department and a copy can be forward to Maintenance Facility to complete a receiving report for payment.
6.0 Work Approach
• All work shall be done in a workmanlike manner.
7.0 Completion Criteria and Final Acceptance Criteria
• Contractor and Project Officer shall make final inspection of delivered fuel.
8.0 Final Acceptance
• Contractor shall be provided a completed delivery tally sheet and invoice.
9.0 Project Management
• N/A
10.0 Timeline and Period of Performance
• Start of project shall start within five (5) working days after Purchase Order is received and the completion shall be the final delivery of diesel fuel.
