STATEMENT OF WORK
FOR
Booster in Military Medicine
LUKE AFB CLINIC, AZ 85309
A. BACKGROUND: During the 2019 Medical Group squadron re-organization, the Flight Medicine Clinic became an element of the Military Medicine Flight. The new re-organization caused the Flight Medicine Clinic to move the unit location to another section within the military treatment facility.
The location of the military medicine flight has too many Land Mobile Radio (LMR) blinds spots and it limits the Field Response Team/Flight Line Response team the ability to monitor the LMRs during an emergency and maintain radio contact with the incident commander and the Luke AFB Alarm Room. It is our understanding that the reason for the blind spots is due to our close physical connection with the radiology lead walls. Base communication advised that a booster will fix the problem and we would no longer have any radio reception issues.
The Luke AFB Military Medicine Flight is requesting a booster to be installed in the duty section in order to amplify the Land Mobile Radio (LMR) signal for Emergency Flight Line and Filed Response Team requirements. The LMRs need to be monitored at all times and personnel assigned for Flight Line response and Filed Team Response continuously run into the problem that in certain locations within the duty section the radios completely lose their signal. The inability for personnel to monitor radios at all times has resulted in delaying care for a pilot during an in-flight emergency and the lack of communication between the fire chief/Luke Alarm Room and the Field Response Team during an active shooter incident. The Flight Line and Field Treatment Response personnel need to be in direct communication with the Incident Commander. We are in need of a system that will ensure the LMRs maintain good reception throughout the Military Medicine Flight at all times. The booster will need to be connected to the current Luke AFB Emergency Communication System. This desired booster will be a new addition. A complete walk-through of the project will be required to ensure that the new booster resulted in fixing our Land Mobile Reception Problem.
B. REQUIREMENTS:
1. Contractor will provide all personnel, all tools, installation, materials, test equipment and technical requirements, as listed in detail below, necessary to perform this service.
2. Contractor will provide a complete walkthrough of the project to ensure successful system function.
3. Contractor will NOT deviate from the listed equipment and options listed in Pt I Materials required and Pt II Materials Required. Any deviation from and/or failure to provide materials as listed will result in the purchase of equipment that will not meet organization needs (**CAN BE BRAND NAME OR EQUAL AS LONG AS IT MEETS THE REQUIRED
SPECIFICATIONS**)
C.TECHNICAL REQUIREMENTS:
This is a comprehensive, two-part, Bi-Directional Amplifier (BDA) and Distributed Antenna System (DAS) solution to address 1) the UHF (LMR) coverage issues and 2) the cellular coverage issues within Building 1130. The two systems should be independent of one another so that if one goes down, the other remains operational.

To determine optimal antenna placement for improved UHF (380 – 400 MHz) LMR coverage, current coverage for BLDG 1130 was assessed on 20191011.
For improved UHF (380 – 400 MHz) LMR coverage within BLDG 1130, purchase BDA and DAS, w/ installation services as defined in PT I Materials Required.
NOTE: The LMR BDA & DAS will be custom built and we must ensure this portion of this requirement is limited to 380-400 MHz operation for key equipment. Failure to ensure this key criteria is met will render installed system unusable.
Authority to use this system/subsystems is granted on a secondary, non-interference basis. If this system, or any subsystem, is found to cause interference, it must be removed from service (SHUT DOWN) until the cause of such interference is resolved.

(PART II) Cellular BDA & DAS, w/ Installation Services:
To determine optimal antenna placement for improved cellular coverage, current cellular coverage for BLDG 1130 was assessed on 20200106.
For improved cellular coverage within BLDG 1130, purchase cellular BDA and DAS, w/ installation services as defined in PT II Materials Required below.
NOTE: For PART II purposes, the term ‘Carrier’ and ‘Cellular Service Provider’ are synonymous.
This system is a Federal Communications Commission (FCC), PART 20, and Consumer Signal Booster and must be professionally installed.
Carrier consent is required to operate this system. Approval to utilize this device is implied, given the label, reading as presented in A1, CONSUMER BOOSTER LABEL 20200309, is affixed by the equipment manufacturer. Per FCC NOTE: Verizon Wireless, AT&T, Sprint, and T–Mobile have “approved” all boosters having this label. NOTE: The warning label may look different, but it must contain the same information. (See Attachment)
This system must be registered with the using organization’s carrier BEFORE USE.
REGARDLESS of carrier consent or system registration, authority to use these systems/subsystems is granted on a secondary, non-interference basis. If this system, or any subsystem, is found to cause interference by the FCC, the organization’s carrier, any other carrier, or the Installation Spectrum Manager, then the system/subsystem must be removed from service (SHUT DOWN) until the cause of such interference is resolved.
This system is comprised of FCC certified subsystems. System/subsystem FCC ID(s) is/are validated.
With validated FCC IDs, FCC defined Network Protection Standards are inherent to this/these system/subsystem(s) for virtue of the aforementioned label.
The using organization may not modify this system, or any subsystem, as built, in any way.
Work required by other work centers/agencies: the requestor must provide shipping address to vendor. The requestor will resolve any equipment installation issues that may emerge as a result of this purchase.
