VA ALTOONA MEDICAL CENTER

CABLE TELEVISION

STATEMENT OF WORK

The Altoona VA Medical Center (VAMC) is seeking a five-year contract for analog Cable television at these locations.

Service Requirements:

1) State College Community Based Outpatient Clinic (CBOC) 2581 Clyde Ave. State College, PA 16801

a. Basic/Standard analog Cable TV service - Total number of buildings is 1.

b. Total number of cable drops is 1.

c. The contractor shall use already existing outlets for installation/connection.

2) Johnstown Community Based Outpatient Clinic 598 Galleria Drive Johnstown, PA 15904

a. Basic/Standard analog Cable TV service- Total number of buildings is 1.

b. Total number of cable drops is 1.

c. The contractor shall use already existing outlets for installation/connection.

3) James E VAN Zandt VA Medical Center (VAMC) 2907 Pleasant Valley Blvd. Altoona, PA 16602

a. Basic/Standard analog Cable TV service - Total number of buildings is 2.

b. Total number of cable drops is (see d. below)

c. The contractor shall use already existing outlets for installation/connection.

d. Includes a 500 line of cable feeding 2 VECIMA units in 198 IT closet across from cashier’s cage supplying 69 Analog Cable channels.

4) Dubois Community Based Outpatient Clinic 5690 Shaffer Road Dubois, PA 15801

a. Basic/Standard analog Cable TV service- Total number of buildings is 1.

b. Total number of cable drops is 1.

c. The contractor shall use already existing outlets for installation/connection.

5) At the Altoona VA Medical Center and community-based outpatient clinics, the Contractor shall maintain all head end equipment. Equipment shall include all ancillary equipment necessary to supply a strong signal to all cable TV outlets.

6) Any required installment work shall be performed in accordance with Medical Center Requirements.

7) The number and types of television channels will be comparable to what’s commonly available in the local areas based on a basic + standard (expanded) level of service.

8) Cable Boxes & remote-control devices: At the Altoona VAMC and CBOC(s) the Contractor must minimize the use of set top boxes and remotes to the maximum extent possible. The use of existing TV remotes is preferable and a cable box per outlet is not desirable.

9) On-site response time: Contractor shall provide an onsite response time capability of four (4) hours to respond to cable/internet problems or outages. Immediate telephone response capability is also required for reporting problems or outages.

Period of Performance: One year base period, with four one-year options.

Security/Privacy: Contractor shall be required to comply with VAPHS physical security guidelines by either checking in with the VA Police each time they come on-site to perform contracted services or by obtaining a VA Contractor ID badge from the VA Police.

Contractor shall not have access to any VA-sensitive information under this contract.

Contractor shall be escorted by appropriate VA personnel at all times while performing work in sensitive areas.

Program/Technical POC: The POC for this contract is Stacy Trusik, AUS/COR, at 814-943-8164 ext. 16279.

“VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors”:

1. Contractor employees who work in or travel to VHA locations must comply with the following:

a. Documentation requirements:

1) If fully vaccinated, shall show proof of vaccination.

NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination

2) If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test.

3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.

4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the following website: COVID-19 Screening Tool. Regularly check the website for updates.

a. Contractor employees who work away from VA locations, but who will have direct patient contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
