The RFP specifies firm, fixed pricing on Exhibit C, Pricing Pages, which vendors must provide at proposal submission. The contractor must meet minimum experience requirements including three years of supportive services to older adults, at least two years working with Alzheimer's disease and related dementias, five years of grant and contract administration experience, and two years of care coordination experience. Pricing for Track 2 (Caregiver Relief) should be capped at $2,500 per year per person, with exceptions to $5,000 available at contractor discretion; Track 3 (Assistive Technology) pricing should be capped at $1,500 per year, with exceptions to $2,000 available at contractor discretion. Missouri Department of Health and Senior Services will be the payor of last resort, requiring documentation that all other payment sources, including insurance and government assistance programs, have been exhausted. The RFP references a current contract (RFPS30034902302654; contract numbers CS232654001) that may be reviewed through the Division of Purchasing's Awarded Bid & Contract Document Search System. Federal funds may be utilized for this contract, requiring compliance with applicable Federal Funds Requirements outlined in Attachment 2. INTRODUCTION AND BACKGROUND INFORMATION SECTION
Introduction:
Purpose: This document constitutes a request for competitive, sealed proposals for the provision of Customized In-Home Caregiver Training and Caregiver Relief to families caring for a person with Alzheimer’s disease or related dementias as set forth herein for the Department of Health and Senior Services (hereinafter referred to as “state agency”).
The program provides customized caregiver training for caregivers and reimburses caregivers for respite-related services. These services are intended to postpone the institutionalization of individuals with Alzheimer’s disease or related dementias.
Minimum Experience Qualifications: There are minimum experience qualifications in the RFP (See Section 2.2, Minimum Experience Requirements in Section 2: Scope of Work).
Titles: Titles of paragraphs used herein are for the purpose of facilitating reference only and shall not be construed to infer a contractual construction of language.
Background and Historical Usage Information:
Alzheimer’s disease is the most common form of dementia, accounting for sixty percent (60%) to eight percent (80%) percent of dementia cases. Alzheimer’s disease is not a normal part of aging and worsens over time. The greatest known risk factor is increasing age, and the majority of people with Alzheimer’s disease are 65 and older. Alzheimer’s disease may begin with mild memory loss in the early stages, but will eventually lead to individuals losing the ability to carry on a conversation and respond to their environment in the late stages. According to the CDC, Alzheimer’s disease is the sixth (6th) leading cause of death in the United States. There's no cure for Alzheimer's, but there are treatments that may change disease progression, and drug and non-drug options that may help treat symptoms.
It is estimated that there are 122,000+ citizens with Alzheimer’s disease within the State of Missouri, which equates to 11.2% of the over-65 population. The number of Missourians with the disease is expected to increase as the state’s population ages.
The impact of Alzheimer’s disease reaches beyond the person with the disease to include spouses, parents, children, grandchildren, and other caregivers who are responsible financially, physically, and emotionally for providing or arranging care. In Missouri in 2024, the direct costs of caring for those with Alzheimer’s disease were estimated to total $6.4 billion. There are an estimated 223,000 family caregivers for persons with Alzheimer’s disease or related dementias in Missouri. Alzheimer’s disease takes a devastating toll on caregivers. Twice as many caregivers of those with dementia indicate substantial emotional, financial, and physical difficulties when compared with caregivers of people without dementia.
The Department of Health and Senior Services’ Alzheimer’s Service Program was initiated in 1987 with funding appropriated by the State of Missouri to assist individuals with Alzheimer’s disease or related dementias and their families by providing respite and other related services to caregivers, aiming to postpone institutionalization. The Customized In-Home Caregiver Training Program was initiated in 2007 to provide personalized training that included care consultations in the caregiver's home, aiming to reduce caregiver stress, improve communication, and make the home safer for individuals diagnosed with Alzheimer’s disease or related dementias.
In 2020, these two (2) funding opportunities were combined to better serve Missourians. This new opportunity, Customized Caregiver Training and Relief Program, was divided into two (2) tracks. Track 1 focused on caregiver training while Track 2 focused on caregiver relief. In 2024, the program was expanded further to include a third track. Track 3 focuses on assistive technology utilized to keep the person living with dementia safe and comfortable in their home.

RFP Questions:
Buyer is Single Point of Contact for Solicitation: Vendors and their agents (including subcontractors, employees, consultants, or anyone else acting on their behalf) must direct all of their questions or comments regarding the RFP, the evaluation, etc., to the buyer indicated on the first page of this RFP. It is preferred that questions be emailed to the buyer.
Except as noted herein, vendors and their agents are instructed not to contact any other state employee regarding any of these matters during the solicitation and evaluation process. Inappropriate contacts are grounds for suspension and/or exclusion from specific procurements. Vendors can be sanctioned for unauthorized contact with any evaluator under 1 Code of State Regulation (CSR) 40-1.060(8)(G) and (H) available at http://www.sos.mo.gov/adrules/csr/csr.asp.
Minority Business Enterprise/Women Business Enterprise (MBE/WBE) and Service Disabled Veteran Enterprise certification or subcontract questions should be directed to the Office of Equal Opportunity (OEO) at http://oeo.mo.gov.
Vendor is Responsible for Asking Questions About the RFP: It is the vendor’s responsibility to ask questions, request changes or clarifications, or otherwise advise the Division of Purchasing if the vendor believes that any language, specifications, or requirements are: (1) ambiguous, (2) contradictory or arbitrary, (3) violate any state or federal law or regulation, (4) restrict or limit the requirements to a single source, or (5) restrict or limit the vendor’s ability to submit a proposal.
Vendor Question Deadline: Every attempt shall be made to ensure that the vendor receives an adequate and prompt response. However, in order to maintain a fair and equitable procurement process, all vendors will be advised, via the issuance of an amendment to the RFP, of any relevant or pertinent information related to the procurement. All questions and issues should be submitted no later than ten (10) calendar days prior to the proposal closing date and time of the proposals. If not received prior to ten (10) calendar days before the proposal closing date and time, the Division of Purchasing may not be able to fully research and consider the respective questions or issues.
State’s Response to Vendor Questions: Upon the Division of Purchasing’s consideration of questions and issues, if the Division of Purchasing determines that changes are necessary, the resulting changes will be included in a subsequently issued RFP amendment(s); absence of such response indicates that the questions and issues were considered but deemed unnecessary for an RFP amendment. All vendors will be advised of any change to the RFP’s language, specifications, or requirements by a formal amendment to the RFP. There will be no posted written records of the questions/communications (i.e., formal question/answer document).
RFP is State’s Only Official Position: The only official position of the State of Missouri shall be that which is contained in the RFP and any amendments thereto.
Amendments:
If the Division of Purchasing determines that changes to the RFP are necessary, the resulting changes will be included in a subsequently issued RFP amendment(s) prior to the proposal closing date and time.
Glossary of Terms and Acronyms:
Whenever the following terms and acronyms appear in the RFP document or any amendment thereto, the definitions or meanings described below shall apply.
RFP specific Glossary, Acronyms, and Abbreviations:
Alzheimer’s Disease – A degenerative brain disease of unknown cause that is the most common form of dementia, that usually starts in late middle age or in old age, that results in progressive memory loss, impaired thinking, disorientation, and changes in personality and mood, and that is marked histologically by the degeneration of brain neurons especially in the cerebral cortex and by the presence of neurofibrillary tangles and plaques containing beta-amyloid.
Assistive Technology – Items, software, or equipment that helps people with disabilities and their caregivers increase, maintain, or improve their functional capabilities, enabling greater independence and safety in daily living, learning and working. Can range from simple, low-tech tools to more complex, high-tech solutions.
Caregiver – A person who resides in the home of and provides direct care to a person with Alzheimer’s disease or related dementias (residing in the home can be waived for Track 1 but is required for Track 2).
Dementia – A progressive condition (such as Alzheimer’s disease) marked by the development of multiple cognitive deficits such as memory impairment, aphasia, and the inability to plan and initiate complex behavior.
In-home Services – Services provided at the residence of the person living with dementia, including personal care, household help, companionship, and minor medical support.
Track 1/Caregiver Training – Training provided to the caregiver/caregivers of a person affected by Alzheimer’s disease or related dementias. The training should reduce caregiver stress, improve communication with the person suffering from memory loss, and make the home safer for individuals with Alzheimer’s disease or related dementias.
Track 2/Caregiver Relief – Reimbursement provided to a caregiver of someone with Alzheimer’s disease or related dementias to cover respite and related services. Respite can be provided in the home or at a facility (short term stay). Related services could include: assessment and care coordination, adult day care, safety and supportive programs, education programs, counseling services or personal care items to enable the caregiver to better care for the individual with Alzheimer’s disease or a related dementia.
Track 3/Caregiver Assistive Technology – Monitoring, safety, and other assistive devices supplied to caregivers to provide families with flexibility in supporting loved ones with dementia in their homes. Supports could include cameras, door alarms, smart home components, medication management tools, or communication devices, as well as installation and training in how to utilize the technology efficiently.
General Glossary, Acronyms, and Abbreviations:
Agency and/or State Agency means the statutory unit of state government in the State of Missouri for which the equipment, supplies, and/or services are being purchased by the Division of Purchasing (Purchasing). The agency is also responsible for payment, unless otherwise specified herein.
Amendment means a written, official modification to a solicitation or contract.
Attachment applies to all documents which are included with an RFP to incorporate any informational data or requirements related to the performance requirements and/or specifications.
Buyer means the procurement staff member of Purchasing.
Code of State Regulation (CSR) contains the current administrative rules of executive agencies of Missouri government. The regulations are arranged by agency rather than by subject.
Contract means a legal and binding agreement between two or more competent parties, for a consideration for the procurement of equipment, supplies, and/or services.
Contractor means a supplier, offeror, person, or organization who is a successful vendor as a result of an RFP and who enters into a contract.
Exhibit applies to forms which are included with an RFP for the vendor to complete and submit with their proposal prior to the specified closing date and time.
May means that a certain feature, component, or action is permissible, but not required.
Must means that a certain feature, component, or action is a mandatory condition.
Party refers to either the State of Missouri or the contractor as an entity that may enter into a contract pursuant to the terms herein.
Pricing Page(s) applies to the form(s) on which the vendor must state the price(s) applicable for the equipment, supplies, and/or services required in the RFP. The pricing pages must be completed and submitted by the vendor with the proposal prior to the specified proposal closing date and time.
Proposal Closing Date and Time and similar expressions mean the exact deadline required by the RFP for the receipt of sealed proposals.
Purchase Order means the authorized document issued by the state agency to the contractor indicating descriptions, quantities, and agreed prices for products and/or services.
Reasonable, Necessary or Proper as used herein shall be interpreted solely by the State of Missouri.
Request for Proposal (RFP) means the solicitation document issued by Purchasing to potential vendors for the purchase of equipment, supplies, and/or services as described in the document. The definition includes the following sections: Introduction and Background Information; Scope of Work; Terms and Conditions (“terms and conditions” and “Terms and Conditions” are used interchangeably throughout the RFP); General Contractual Requirements; and Vendor Submission, Evaluation, and Award Information; and the RFP Vendor Response Exhibits, Attachments, and Amendments of the RFP.
RSMo (Revised Statutes of Missouri) refers to the body of laws enacted by the Legislature which govern the operations of all agencies of the State of Missouri. Chapter 34 of the statutes is the primary chapter governing the operations of Purchasing.
Shall has the same meaning as the word must.
Should means that a certain feature, component and/or action is desirable but not mandatory.
State collectively referring to the state government and/or the agencies thereof.
Supplier has the same meaning as the word, vendor.
Vendor means the supplier, offeror, person, or organization that responds to an RFP by submitting a proposal with prices to provide the equipment, supplies, and/or services as required in the RFP document.
Accuracy of Background Information:
Although an attempt has been made to provide accurate and up-to-date information, the State of Missouri does not warrant or represent that the background information provided herein reflects all relationships or existing conditions related to this RFP.

SCOPE OF WORK SECTION
General Requirements:
The contractor shall provide Customized Caregiver Training and Relief Program Services (hereinafter referred to as the program) for the Department of Health and Senior Services (hereinafter referred to as the state agency) in accordance with the provisions and requirements stated herein and to the sole satisfaction of the state agency. The services requested are intended to postpone the institutionalization of persons with Alzheimer’s disease or related dementias
The contractor shall provide the three (3) different tracks, Caregiver Training (Track 1), Caregiver Relief (Track 2), and Assistive Technology Services to Relieve Caregivers (Track 3) in accordance with the requirements stated herein for all counties and St. Louis City in the State of Missouri, which is broken into two (2) different regions (A & B) as indicated on Attachment 1.
The contractor’s customized caregiver training and relief program shall include services that reduce the caregivers’ burden and stress, as well as improve communication, enhance quality of life, and make the home safer for individuals diagnosed with Alzheimer’s disease or related dementias. The contractor’s customized caregiver training and relief shall strive to meet the following goals:
Increase awareness and participation in Alzheimer’s disease services available through the program;
Increase understanding of Alzheimer’s disease and options available for care;
Increase caregivers’ ability to cope with persons with Alzheimer’s disease or related dementias;
Improve the safety and well-being of persons with Alzheimer’s disease or related dementias;
Improve the well-being and reduce the stress of family caregivers;
Increase awareness and access to assistive services in the community; and
Increase the proportion of long-term care provided in homes for persons with Alzheimer’s disease while reducing the number of persons with Alzheimer’s disease who are prematurely or unnecessarily institutionalized.
The contractor shall survey program participants annually to determine their satisfaction with the program and to gauge their opinions on the goals outlined above (2.1.3 a through g). The survey results shall be reported to the state agency at the close of the program year to be utilized in the budget book metrics.
The contractor shall report to the Adult Abuse and Neglect Hotline (1-800-392-0210) or enter the information on the on-line portal (https://moapss.health.mo.gov/) any instances of abuse, neglect, bullying or exploitation of a vulnerable adult between the ages of eighteen (18) through fifty-nine (59) or an individual sixty (60) years or older pursuant to the laws of the state of Missouri. The contractor shall report to the Child Abuse/Neglect Hotline (1-800-392-3738) any instances of child abuse or neglect pursuant to the laws of the state of Missouri.
The contractor shall make the program available to qualified persons in all counties of the State of Missouri. The estimated Alzheimer’s disease population per county and region map are shown in Attachment 1. A participant may be allowed to be in multiple tracks (see section 2.4) at the same time.
The contractor’s customized caregiver training and relief program must be fully operational within sixty (60) days after the contractor receives the of the notice to proceed with services from the state agency.
Contractor’s Obligation: Unless otherwise specified herein, the contractor shall furnish all material, labor, facilities, equipment, and supplies necessary to perform the services required herein.
Minimum Experience Requirements:
The contractor must meet or exceed the following minimum experience requirements at the time of proposal submission and for the duration of the contract:
The contractor shall have three (3) years of experience in provision of supportive services to older adults, people with disabilities and/or caregivers.
The contractor must have at least two (2) years’ experience working with individuals with Alzheimer’s and related dementias and their caregivers. The experience must include working with the Alzheimer’s Association, Area Agencies on Aging, and other local service providers for individuals with Alzheimer’s or related dementias.
The contractor shall have at least five (5) years of experience in grant and contract administration.
The contractor must have at least two (2) years’ experience in care coordination for individuals with Alzheimer’s or related dementias and their caregivers.
Team/Personnel Qualifications:
The contractor shall include the following personnel to administer and perform the contract requirements:
Working Team: The working team personnel shall have three (3) years of experience in Alzheimer’s disease and related dementias, caregiver training, care coordination, and respite fund administration.
Contact Person: The contractor shall designate a contact person who shall serve as the contractor’s contact and shall be the liaison between the contractor and the state agency. By no later than five (5) state business days after authorization by the state agency to proceed with services, the contractor shall provide the state agency with the name, address, email address, and telephone number of the contractor’s contact person. The contractor’s contact person shall (1) oversee all services being provided, (2) assume responsibility and liability for services performed per the contract, and (3) serve as the primary point of contact with the state agency.
The contractor shall understand and agree that the state agency reserves the right to request any of the contractor’s personnel assigned to provide the services be removed without just cause. The contractor shall understand and agree that the state agency will allow the contractor a reasonable time to find a replacement. The state agency shall approve all replacement personnel. The state agency reserves the right to require background checks and non-disclosure agreements (NDA) of selected contractor personnel, at the contractor’s expense.
Caregiver Training (Track 1) Requirements:
The contractor shall provide Caregiver Training (Track 1), which shall include the following services:
An in-home assessment of the person with Alzheimer’s disease or related dementia and his/her unmet needs that will be utilized to develop customized training for the caregiver to help meet the identified needs of the individual with Alzheimer’s disease or related dementia and reduce stress on the caregiver.
Care plan development to help meet the unmet needs of the individual with Alzheimer’s disease or related dementias and reduce stress on the caregiver.
Training materials and resources for caregivers to help them modify the daily activities of the individual diagnosed with Alzheimer’s disease or related dementias to promote independence.
Follow-up calls and/or visits to ensure the care plan is addressing the needs of the individual with Alzheimer’s disease or related dementias and their caregiver and to provide additional resources as necessary.
Where necessary, assistive safety devices to help with activities of daily living and/or increase the safety and well-being of a person with Alzheimer’s disease or related dementias while remaining in their home.
Referrals to other community supports and resources as necessary to help meet the needs of the individual with Alzheimer’s disease or related dementias.
REVISED PER AMENDMENT 01
The contractor shall provide a minimum of three hundred fifty (350) Caregiver Trainings (Track 1) distributed in an equitable manner throughout the state.
The contractor’s trainings shall be provided in those counties determined by the contractor to have the greatest need for the services. The trainings shall not be concentrated in large metropolitan areas (urbanized area of 50,000 or more inhabitants).
The contactor shall understand and agree that participation shall be limited to one training per person/family. Exceptions may be made on a case-by-case basis with approval from the state agency, with priority given to new caregiver enrollees.
Caregiver Relief (Track 2) Requirements:
The contractor shall provide Caregiver Relief (Track 2), which shall include the following services:
Assessment and Care Coordination – This shall include an initial assessment and reassessment(s) of the individual with Alzheimer’s disease or related dementias and needs of the family. Additionally, the contractor shall provide care options, counseling to aid in planning the future care of their loved ones, assistance in developing an individualized plan to meet the needs of the family, and information and referrals for family support services offered by the contractor’s program and through community organizations.
Along with assessment and care coordination required in Track 2, the contractor shall provide reimbursement for one (1) or more of the following services to persons with Alzheimer’s disease or related dementias and their families.
Respite Assistance – In-home or facility-based care to provide caregivers a temporary rest from caregiving, while the person with Alzheimer’s disease or related dementias continues to receive care in a safe environment. Care can be provided professionally or by another family member/family friend.
Adult Day Care – This shall include supervision, meaningful social activities, nutrition, and other care to persons with Alzheimer’s disease or related dementias in a group setting outside of the home for periods of two (2) to twenty-four (24) hours per day.
Safety and Supportive Programs – This shall include relief to family caregivers through assistive services (e.g. legal assistance, transportation, and therapeutic activities) and products (e.g. mobility devices, medication related to the Alzheimer’s disease or related dementias, nutritional shakes and incontinence supplies).
Education Programs – This shall include opportunities to persons with Alzheimer’s disease or related dementias, the caregiver, and family members to participate in educational classes and/or furnish information on the nature of the disease and its progression, disease management, treatments and medications, falls prevention and safety considerations, nutrition and health promotion, caregiver guidance and supports, emerging research advances, and the array of community-based services.
Counseling Services – This shall include individual and/or group therapeutic counseling for persons with Alzheimer’s disease or related dementias and/or caregiver/family members.
Other – If the contractor provides other services than those listed above the contractor shall obtain state agency approval prior to performing other services.
The contractor may allot a portion of the award from Track 2 to outreach. The contractor’s outreach shall promote the availability of services to the public with targeted efforts to reach and increase service to low-income, minority and rural populations.

The contractor shall provide a minimum of three hundred (300) Caregiver Relief (Track 2) reimbursements across the state.
The contractor shall provide Caregiver Relief in those counties determined by the contractor to have the greatest need for the services. The contractor’s Caregiver Relief should not be concentrated in large metropolitan areas (urbanized area of 50,000 or more inhabitants).
To ensure a maximum number of caregivers have the opportunity to participate, the contractor should cap reimbursements for Track 2 at $2500 per year per person with Alzheimer’s disease or related dementias. However, exceptions may be made to allow up to $5000 per year per person at the discretion of the contractor.
The contractor may allow for larger one-time reimbursements upon the written approval of a waiver from the state agency. Participation in caregiver relief shall be limited to two years, but exceptions may be made on a case-by-case basis with written approval from the state agency.
Assistive Technology Services to Relieve Caregivers (Track 3) Requirements:
The contractor shall provide Assistive Technology Services to Relieve Caregivers (Track 3), which shall include the following:
Assessment and Coordination – Assessment and Coordination shall include an assessment of the individual with dementia to ensure they are appropriate to be home alone with assistive technology and the caregiver’s comfort level with using technology to monitor the individual when they are home alone.
The contractor shall maintain an approved list or database of possible devices or services that may be selected to meet the caregiver’s needs. The state agency will approve these lists before they are presented to families or caregivers.
Once a determination by the contractor is made that the individual is safe to be home alone and the caregiver is comfortable with using assistive technology to monitor them, the contractor shall discuss potential devices and services available. The contractor shall order the device/service and shall ensure that all installation and training are scheduled at a time convenient for the caregiver and family.
The contractor shall ensure the family has access to broadband, if needed for the device.
The contractor shall set up monthly payments, if needed, to keep the services going for the caregiver.
The contractor may retrieve devices given out once the caregiver no longer needs them for the person assessed in the following situations:
If the person being cared for is no longer able to stay home with just assistive technology;
If the person being cared for enters a facility such as a nursing home, or;
If the person being cared for passes away.
The contractor may establish a lending library to loan out devices retrieved from clients to provide to other families as needed and as devices are available to loan out.
REVISED PER AMENDMENT 01
The contractor shall provide a minimum of five hundred (500) individual Assistive Technology devices/units of service across the state which will be utilized in the home of the individual with Alzheimer’s or related dementias to help monitor their activities when they are home alone. These services shall be provided to a minimum of one hundred twenty-five (125) participants.
The contractor should only consider Assistive Technology for those caregivers providing services to individuals in the early stages of Alzheimer’s and Dementia who can safely be left home alone with the assistive technology services in place.
To ensure a maximum number of caregivers have the opportunity to participate, the contractor should cap reimbursements for Track 3 at $1,500 per year per person with Alzheimer’s disease or related dementias. However, exceptions may be made to allow up to $2000 per year per person at the discretion of the contractor, and as long as the budget will allow such an increase.
The contractor shall give priority to those caregivers with a lack of other resources in their area such as adult day cares, nursing facilities providing respite services, in-home providers who can provide respite services due to staff shortages or other issues
Reporting Requirements:
Publicity: Any publicity release mentioning contract activities shall reference the contract number and the state agency. Any publications, including audiovisual items produced with contract funds, shall give credit to the contract and the state agency. The contractor shall obtain approval from the state agency prior to the release of such publicity or publications.
On a monthly basis by no later than ten (10) calendar days after the end of the reporting period, the contractor shall submit a monthly data report along with the monthly invoice.
Track 1: The monthly data report shall include, at minimum, the following:
The counties in which the services were provided;
The number of assessments completed;
The number of care plans developed;
The number, type and cost of safety products provided; and
The number of trainings completed and information regarding follow-up visits.
Track 2: The monthly data report shall include, at minimum, the following:
The counties in which the services were provided;
The number of unduplicated persons that received services; and
The number of units of service delivered in each allowable category.
Track 3: The monthly data report shall include, at minimum, the following:
The counties in which the assistive technology devices/services were provided;
The number of assistive technology assessments completed; and
The number, type, and cost of assistive devices/services provided. (Cost should include total annual cost including installation, training, broadband access, monthly service fees, device purchase costs, etc.)
The contractor shall submit an annual report no later than thirty (30) days after the conclusion of the original contract period and after any subsequent contract renewal periods. The contractor shall provide an annual comprehensive programmatic evaluation of the services provided during the period. At a minimum, the contractor shall provide the following information within the programmatic evaluation:
A brief description of accomplishments, collaborations, outcomes, and barriers;
Quantifiable outcomes that demonstrate the impact of services provided by the program;
Increase awareness and participation in Alzheimer’s disease services available through the program;
Increase understanding of Alzheimer’s disease and options available for care;
Increase caregivers' ability to assist their care recipient living with Alzheimer’s disease or related dementias;
Improve the safety and well-being of people living with Alzheimer’s disease or related dementias;
Improve well-being and reduce the stress of family caregivers;
Increase awareness and access to assistive services in the community; and
Increase the proportion of long-term care provided in homes and communities for persons with Alzheimer’s disease or related dementias and reduce the number of persons with Alzheimer’s disease or related dementias who are prematurely or unnecessarily institutionalized.
If utilizing an online training system, the total number of participants that accessed the training and the number of participants that completed the training;
The total number of unduplicated persons that received the services through the contract reported by county and zip code;
The total number of units of service delivered by category;
The total number of persons that received services in each allowable category of service;
The number of family caregivers and persons with Alzheimer’s disease or related dementias in Missouri who requested services but were not served or served through alternate program funding due to lack of resources from this program; and
The results from the annual participant survey.
Contract Monitoring: The state agency reserves the right to monitor the contract throughout the effective period of the contract to ensure financial and contractual compliance. If the state agency determines the contractor to be at high-risk for non-compliance, the state agency shall have the right to impose special conditions or restrictions. Written notification will be provided to the contractor of the determination of high-risk and of any special conditions or restrictions to be imposed. The special conditions or restrictions may include, but not limited to, those conditions specified below:
Requiring additional, more detailed financial reports or other documentation;
Additional contract monitoring;
Requiring the contractor to obtain technical or management assistance; and/or
Establishing additional prior approvals from the state agency.
Budget/Price Analysis: Within ten (10) calendar days of notice to proceed by the state agency, the contractor must provide a budget or price analysis for each of the firm, fixed prices identified on Exhibit C, Pricing Page. The budget/price analysis must reflect the contractor’s breakdown of the quoted price and should be shown in sufficient detail to demonstrate those factors affecting the price such as staffing patterns and proposed hours, etc. The budget or price analysis must be in a format mutually agreed to by the contractor and state agency.
Financial Records and Document Retention:
The contractor must maintain financial and accounting records and evidence pertaining to the contract in accordance with generally accepted accounting principles.
The contractor shall maintain all financial records, supporting documentation, and all other records pertinent to the contract for a period of five (5) years from the date of the submission of final report to the state agency or the completion of an audit, whichever is later, or as otherwise stated in the contract.
If any litigation, claim, negotiation, audit, investigation, or other action involving the records has been started before the expiration of the five (5) year period, the contractor shall retain the records until completion of such action and resolutions of all issues that arise from it or until the end of the regular five (5) year period, whichever is later.
If the state agency is subject to any litigation, claim, negotiation, audit, or other action involving the records, the state agency will notify the contractor in writing to extend the contractor’s retention period.
The contractor shall maintain a financial management methodology that, at a minimum, records expenditures in a manner that readily identifies the expenditure as an allowable activity and allows required federal financial reports to be easily prepared.
The contractor shall have written policies and procedures to comply with the Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards (2 CFR 200) and shall make its policies and procedures available to the state agency, upon request.
Electronic Funds Transfer, Invoicing, and Payment Requirements:
Electronic Funds Transfer (EFT): The State of Missouri will submit contract payments to the contractor at the remittance address listed in the contractor’s MissouriBUYS (WebProcure/Proactis) vendor registration. However, the contractor understands and agrees the state reserves the right to make contract payments to the contractor through EFT. Therefore, prior to any payments becoming due under the contract, the contractor must verify and update, if applicable, their vendor registration with their current remittance address and ACH-EFT payment information at https://MissouriBUYS.mo.gov.
Budget – The state agency reserves the right to reallocate or reduce contract funds at any time during the contract period due to underutilization of contract funds or changes in the availability of program funds. The state agency will provide the contractor with thirty (30) days prior written notification of any reallocation
If the contractor identifies specific needs within the Scope of Work, the contractor may rebudget up to 10% of the total budget between object class categories of the budget without obtaining prior written approval of the state agency. The contractor must understand and agree to a written contract amendment for any rebudgeting 10.1% and over.
Invoicing: The contractor shall submit invoices monthly. Invoices shall be due by the last day of the month following the month in which the contractor provided services under the contract. The contractor shall perform the services prior to invoicing the state agency.
The contractor shall invoice the state agency on the contractor’s original descriptive business invoice form and submit the invoice the following address/email address:

The contractor shall use uniquely identifiable invoice numbers to distinguish an invoice from a previously submitted invoice and shall include on the invoice the remittance address listed in the contractor’s MissouriBUYS (WebProcure/Proactis) vendor registration.
Each contractor invoice must be on the contractor’s original descriptive business invoice form and must contain a unique invoice number and the remittance address included in the contractor’s MissouriBUYS (WebProcure/Proactis) vendor registration. The invoice number will be listed on the state’s EFT addendum record to enable the contractor to properly apply state payments to invoices. The contractor must comply with all other invoicing requirements stated in the RFP.
The statewide financial management system has been designed to capture certain receipt and payment information. For each purchase order received, an invoice must be submitted that references the purchase order number and should be itemized in accordance with items listed on the purchase order. Failure to comply with this requirement may delay processing of invoices for payment.
The contractor shall not invoice federal or state taxes unless otherwise required under law or regulation.
The contractor shall include the following certification statement on any invoice submitted to the department:
“I certify to the best of my knowledge and belief that the information provided herein is true, complete, and accurate. I am aware that the provision of false, fictitious, or fraudulent information, or the omission of any material fact, may subject me to criminal, civil, or administrative consequences including, but not limited to violations of U.S. Code Title 18, Sections 2, 1001, 1343 and Title 31, Sections 3729-3730 and 3801-3812.”
Payment:
Payments are due upon receipt of a valid invoice, payable in 30 calendar days. All invoices for equipment, supplies, and/or services purchased by the State of Missouri shall be subject to late payment charges as provided in section 34.055, RSMo.
The State of Missouri does not pay state or federal taxes unless otherwise required under law or regulation.
The State of Missouri assumes no obligation for equipment, supplies, and/or services shipped or provided in excess of the quantity ordered. Any unauthorized quantity is subject to the state's rejection and shall be returned at the contractor's expense.
The contractor may obtain detailed information for payments issued for the past 24 months from the State of Missouri’s central accounting system (SAM II) on the Vendor Services Portal at https://www.vendorservices.mo.gov/vendorservices/Portal/Default.aspx.
Inspection and Acceptance Specifications: For purposes of acceptance, no equipment, supplies, and/or services received by the state pursuant to a contract shall be deemed accepted until the state has had reasonable opportunity to inspect said equipment, supplies, and/or services.
All equipment, supplies, and/or services which do not comply with the specifications and/or requirements or which are otherwise unacceptable or defective may be rejected by the state.
