1. Defense Logistics Agency (DLA) Troop Support, Medical Supply Chain, is the contracting activity.
2. This justification is for the purchase of AddedVue Packs and PRK Treatment Cards for use with the Star S4 IR Laser system currently installed at the Womack Army Medical Center, Fort Bragg, NC.
3. The required equipment, p/n 0040-7000 - AddedVue Pack and p/n 0040-1010-11 – PRK Treatment Card, are available from the manufacturer Abbot Medical Optics (AMO) Sales and Services, Inc. 4. The authority under the Multiple-Award Schedule Program waiving the competition requirements is FAR 8.405-6(a)(1)(i)(B), only one source is capable of providing the supplies required at the level of quality required because the supplies are unique or highly specialized.
The AMO Star S4 IR laser system is an excimer laser which allows surgeons to deliver guided laser vision correction treatments to patients. The AddedVue packs and PRK Treatment Cards to be purchased are needed to ensure that the laser precisely reshapes the curvature of the soldier’s cornea by removing microscopic amounts of tissue following the pattern on the treatment card and packs which are unique to each solider.
The brand name AMO AddedVue packs and PRK Treatment Cards are the only ones compatible with the AMO Star S4 IR laser currently installed. This equipment is essential for the continuation of the Warfighter Refractive Eye Surgery Program (WRESP), which enhances the readiness of our deploying Soldiers. No other vendor can provide the items requested, as AMO has Sole proprietorship of Star S4 IR Laser system.
5. DLA Troop Support has determined the price of the AddedVue Packs and PRK Treatment Cards to be fair and reasonable based upon pricing evaluated at the time of award of AMO’s ECAT contract.
6. Market research in the ECAT system found no additional sources for the AddedVue Packs and PRK Treatment Cards. In addition to the Services, DLA Troop Support will attempt to locate other acceptable sources inside and outside the ECAT program for future requirements as they arise.
7. There are no other facts supporting the justification.
Limited Source Justification
8. ECAT vendors have the opportunity to add products that fall within the scope of the ECAT contract at any time; however, currently there are no other known ECAT vendors that would be able to offer the AMO AddedVue Packs and PRK Treatment Cards.
