FORMER SECTION “L” NOTICE TO OFFERORS

Attachment 1 – Technical Proposal Evaluation Factors

Proposals will be evaluated for all non-cost/price evaluation factors/technical proposal and cost or price reasonableness following the evaluation procedures set forth in Section M. Additional Source Selection Evaluation Procedures in this solicitation. Attachment 1 has been formatted to assist you in preparing a complete technical proposal. In order to provide full consideration of your qualification for contract award, you are encouraged to ensure that the information furnished in support of your technical proposal is factual, accurate and complete.

1. The DOD is committed to applying “Best Value” contracting as a means to rely on industry for timely delivery of quality products, while reducing the Government’s administrative costs associated with contractor oversight. To this end, we are evaluating factors in addition to price under this solicitation.

Since effective production and quality control practices translate into better quality products, fewer delinquencies, reduced administrative costs and lower overall cost to the Government, we will be using your Technical Proposal to assess the efficiency of your production methods and the effectiveness of your quality control procedures. Moreover, those offerors who consistently demonstrate an ability to deliver on time while consistently improving the quality of the products they produce will receive more favorable consideration than those who do not.

2. You may provide additional technical information you believe will enhance your proposal; however, overly elaborate presentations are not desired.

3. Failure to provide the information requested may render the offeror’s proposal technically unacceptable.

4. The Government reserves the right to verify the information provided for evaluation purposes.

Offerors are required to submit a technical proposal consisting of: 1) Product Demonstration Model(s) (PDM(s)) and 2) Past Performance – Performance Confidence Assessment as part of their offer, and proposals must be received before the time set forth for solicitation closing. The technical proposal will be evaluated to determine compliance with all characteristics listed for evaluation in Section M. Additional Source Selection Evaluation Procedures. Failure to furnish this information by the time specified in the solicitation may be cause for rejection of offers if not otherwise acceptable under the provisions for considering late offers.

5. Cost and Pricing Data: A full cost and pricing proposal is not required; however, the offeror may be required to submit information other than cost and pricing data during discussions. Therefore, offerors are cautioned to retain cost proposal worksheets, vendor quotations and related support information.

[X]52.215-9020 Instructions for Submitting Product Demonstration Models (PDM) for Supply Chains at DLA Troop Support.

INSTRUCTIONS FOR SUBMITING PRODUCT DEMONSTRATION MODELS (PDM) FOR

SUPPLY CHAINS AT DLA TROOP SUPPORT (NOV 2011)

(a) The offeror shall submit product demonstration models (PDM), as specified below, as part of its proposal. Models will be submitted at no expense to the Government and must be received prior to the time and date set forth for closing of offers. PDMs will be returned at the offeror’s request and expense so long as the PDM(s) is(are) not destroyed by testing, retained by the Government as a manufacturing standard, and/or unless otherwise specified by the solicitation. PDMs will become Government property unless return is requested within 30 days after date of award.

NSN: 8405-01-268-2698 Item Number ___White Size 7_ Number of Units Required __1__

(b) Models will be tested or evaluated to determine compliance with all characteristics specified for such test or evaluation as referenced in Federal Acquisition Regulation (FAR) Subpart 15.3. Failure of models to conform to all such characteristics may result in the rejection of the entire offer. Failure to furnish models by the time and date specified in the solicitation may be cause for rejection of the entire offer if not otherwise acceptable under the provisions for considering late offers.

The following applies only when checked as paragraph (c):

[ ] If the PDM will be used as a manufacturing standard, for the successful offeror(s), the Government will return one approved product demonstration model (PDM) which will serve as a manufacturing standard. Products delivered under any resulting contract shall conform to the approved model as to the characteristics listed for test or evaluation and shall conform to the specification/commercial product description characteristics.

[ X] It is preferred that the materials used to manufacture the PDM(s) be in accordance with any specifications defined in Section C of this solicitation. However, if the offeror uses alternate materials to manufacture the PDM(s) a letter must be supplied with them stating (i) which materials depart from the end item specification and (ii) that, although the materials used in producing the PDM(s) differ from those specified, the materials required by the specification will be utilized under any resultant contract.

Note: The use of alternate materials will not affect the rating of the PDM, either favorably or negatively when the use of the alternate materials is identified in accordance with the requirements of this section.

However, the Government reserves the right to give a negative rating to (a) PDM(s) that is(are) made with materials that depart from the end item specification and the use of those alternate materials was not identified in accordance with the requirements of this section. Accordingly, to preclude a negative assessment of a PDM that is comprised of materials that are not in accordance with the specification(s), it is in the offeror's best interest to identify any and all alternate materials that have been used.

Although the offeror is permitted to use alternate materials to construct the PDM(s), the offeror is not permitted to utilize alternate manufacturing operations or change the construction/design of the PDM. The PDM must be manufactured (i) at the same facility as the production quantity and (ii) must be in accordance with applicable specifications, including the placement of the labels, unless otherwise identified. However, labels need not be printed for the purpose of the PDM but there must be an indication as to the size of the PDM, when applicable."

(FORMER SECTION “M”)

Attachment 1 – Technical Proposal Evaluation factors

EVALUATION FACTORS FOR AWARD

(a) The Government will make award to the responsible offeror whose offer conforms with the solicitation and is most advantageous to the Government, cost or price, and other evaluation factors considered. The Government will award a contract only to the concern whose technical proposal establishes that it can meet the requirements of the Government. For this solicitation, all evaluation factors other than cost or price, when combined, are –

[ ] significantly less important than cost or price. As the evaluated cost/price becomes more equal, relative importance of all other evaluation factors becomes more significant.

[ ] approximately equal to cost or price.

[X ] significantly more important than cost or price. As other evaluation factors become more equal, the evaluated cost or price becomes more important.

(b) Non-Cost/Price Evaluation Factors (listed in descending order of importance):

1. Technical Factor - Product Demonstration Model(s) (PDM(S)) – and Technical Risk Sub-factors:

a. Manufacturing Operations

b. Visual Requirements

c. Dimensional Requirements

NOTE: The PDM(s) sub-factors above are of equal importance; however, the overall PDM(s) rating will be no higher than the lowest sub-factor rating.

2. Past Performance - Performance Confidence Assessment

Sub-factors:

a. Past Performance Relevancy

b. Quality and Delivery Performance

- Quality of Items

- Delivery Performance

NOTE: The Past Performance sub-factors above are of equal importance. Under the second subfactor, “Quality and Delivery Performance sub-factor,” the first and second elements of Quality and Delivery Performance are of equal importance.

The first non-cost or non-price evaluation factor of PDM(S) is the most important evaluation factor as it clearly shows an offeror’s capability and understanding of the specification. The second non-cost or non-price evaluation factor is Past Performance - Performance Confidence Assessment, as this factor evaluates the responsibility and reliability of an offeror based on their prior performance under commercial and Government contracts.

SPM1C1-11-R-0137 Page 59 of 61

COMBINED TECHNICAL RISK RATINGS

Technical Factor - Product Demonstration Model(s) (PDM(S))

The Government will utilize the following rating scale to evaluate Product Demonstration Model(s):

Outstanding Proposal meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses. Risk of unsuccessful performance is very low.

The PDM meets all stated requirements of the acquisition documentation (i.e., specification; commercial item description; purchase description, etc.) and has no weaknesses.

Good Proposal meets requirements and indicates a thorough approach and understanding of the requirements.

Proposal contains strengths which outweigh any weaknesses. Risk of unsuccessful performance is low.

The proposal’s strengths outweigh any weaknesses when the PDM substantially meets the stated requirements of the acquisition documentation (i.e., specification; commercial item description; purchase description, etc.), but the PDM contains a weakness or weaknesses that are not significant and are very easily correctable during production.

Acceptable Proposal meets requirements and indicates an adequate approach and understanding of the requirements.

Strengths and weaknesses are offsetting or will have little or no impact on contract performance. Risk of unsuccessful performance is no worse than moderate. The proposal’s strengths and weaknesses are offsetting or will have little or no impact on contract performance when the PDM substantially meets the stated requirements of the acquisition documentation (i.e., specification; commercial item description;

purchase description, etc.), but the PDM contains a weakness or weaknesses that require preventative corrective action in production.

Marginal Proposal does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The proposal has one or more weaknesses which are not offset by strengths. Risk of unsuccessful performance is high. The PDM does not meet all stated requirements of the acquisition documentation (i.e., specification; commercial item description; purchase description, etc.), but has potential to become acceptable through significant corrective action. The PDM exhibits a significant weakness or weaknesses that indicate that the offeror did not completely understand the technical requirements and exhibits a low probability to produce and acceptable item.

Unacceptable Proposal does not meet requirements and contains one or more deficiencies. Proposal is unawardable.

The PDM substantially fails to meet many significant designs related to and/or other requirements of the acquisition documentation (i.e., specification; commercial item description; purchase description, etc.), and is indicative of a lack of understanding the technical requirements. The PDM exhibits a very significant weakness or weaknesses determined by the evaluation team to render the PDM completely unacceptable for its intended function. The PDM cannot become acceptable without extensive corrective action or remedy that would be tantamount to a new PDM.

NOTE: Please note that the term “weakness” is synonymous with the term “defect” as contained in the specification.

SPM1C1-11-R-0137 Page 60 of 61

Technical Risk Ratings- Past Performance – Performance Confidence Assessment

Since the anticipated offerors have varying degrees of relevant past performance as it relates to these items, the Government will utilize the following rating scale to evaluate Performance Relevancy:

Past Performance Relevancy:

Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.

Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.

Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.

Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.

Neutral rating The offeror has no record of past performance history

The Government will utilize the following rating scale to evaluate Quality and Delivery Performance:

Quality and Delivery

Outstanding An overall Outstanding rating indicates the offeror’s past performance record consistently meets or exceeds contract requirements. Past contractual performance has been accomplished with few minor problems for which corrective actions taken by the offeror have been highly effective.

Good An overall Good rating indicates the offeror’s past performance record consistently meets or exceeds contract requirements. Past contractual performance has been accomplished with some minor problems for which corrective actions taken by the offeror have been effective.

Acceptable An overall Acceptable rating indicates the offeror’s past performance record met contract requirements.

Offeror’s record of past performance contains reports of some minor problems for which corrective actions taken by the offeror appear to have been or were acceptable.

Marginal An overall Marginal rating indicates the offeror’s past performance record has not met some contract requirements. Offeror’s record of past performance contains reports of serious problems for which the offeror did not identify corrective actions or proposed corrective actions that were only marginally effective or were not fully implemented.

SPM1C1-11-R-0137 Page 61 of 61

Unacceptable An overall Unacceptable rating indicates the offeror’s past performance record has not met most contractual requirements, recovery was not accomplished in a timely manner and proposed corrective actions appear or were ineffective.

Neutral rating The offeror has no record of past performance history

The following rating symbology will be utilized to arrive at the overall rating for Past Performance. The rating is arrived at by combining and applying equal weight to the Past Performance Relevancy Sub- Factor and Quality and Delivery Performance Sub-Factor. “Quality and Delivery Performance sub-factor,” the first and second elements of Quality and Delivery Performance are of equal importance. This overall rating is known as the Performance Confidence Assessment:

Past Performance - Performance Confidence Assessment

Substantial Confidence Based on the offeror’s recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort.

Satisfactory Confidence Based on the offeror’s recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.

Limited Confidence

Based on the offeror’s recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort.

No Confidence

Based on the offeror’s recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort.

Unknown Confidence (Neutral)

No recent/relevant performance record is available or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned.
