HANDICAPPED RAMP WAREHOUSE 15

A BAY

STATEMENT OF WORK

I. INTRODUCTION

1 SCOPE

The contractor shall install a fully functional, complete, and usable handicapped ramp and stairs in accordance with the American with Disabilities Act (ADA) and with the Architectural Barriers Act (ABA) as well as any other pertinent codes or references. Included is a large canopy to cover the entrance door. A photo of what the finished product shall look like is at the end of this SOW.

2 BACKGROUND

Currently, there is no handicapped access ramp to allow personnel to enter Warehouse 15 A bay South end.

A handicapped ramp and stairs are needed to ensure all personnel can access warehouse 15 A bay.

3 OBJECTIVES

The contractor shall provide all tools, materials, equipment and labor to install a handicapped ramp in accordance with the American with Disabilities act and with the Architectural Barriers Act as well as any other pertinent codes or references for warehouse 15 A bay South end. Included is a large canopy to cover the entrance doors.

4.1 DOCUMENTATION AND RECORDS

All documentation records, and schedules, as described in this SOW, which are the responsibility of the Contractor and the property of the Government, shall remain so upon termination or completion of the contract. The Contractor shall keep these items current. Documentation, record, and schedules shall be turned over to the Government upon termination or completion of the contract. The Contractor shall provide submittals for all material, equipment, designs and plans to the government prior to the start of any work.

Submittals are required for all materials and shall be sent via email. Two hard copies of submittals shall also be provided to the COR/PM.

5 REQUIREMENTS

5.1 SPECIFIC REQUIREMENTS

The contractor shall provide all tools, materials, equipment and qualified labor to install an ADA/ABA accessible concrete handicapped ramp and stairs to enter warehouse 15A bay. This handicapped ramp/stairs shall match the other ramp systems around DSCR (bldg. 33 South side). Except that stairs are only required on the left side of the entrance facing North. The contractor shall install anti slip tread caps on all stair treads, full width.

The contractor shall replace the existing door and frame with new aluminum anodized door, frame threshold and all seals etc. The contractor shall provide and install a knob outside capable of accepting a best core. The contractor shall buy and install the core pre pinned. The government will provide the pinning requirements. The contractor shall provide and install panic hardware on the inside per code.

The door shall open out to the right when facing the door with lite to the left. The contractor shall include a handicapped opener and an operator button on each side of the door. Relocate the card reader and phone/communication box to the opposite side.

The contractor shall remove and dispose of the existing stairs and dispose of. The contractor shall demolish the existing sidewalk to the extent necessary for this ramp and stairs. Replace any as needed.

All concrete to be 3000psi.

An awning of the same type/style etc. that is used on DSCR shall be provided and installed. The government representative will approve the awning submittal.

A photovoltaic globe type light fixture with LED bulb is required under the canopy, same dimensions as existing located at building 33. A 120v 15A duplex electrical outlet shall be located under the soffit on the canopy (conduit concealed). Relocate the photovoltaic wall mounted light pack currently over the existing door to the wall above the landing for illumination. Provide all electrical components for all electrical work. Patch all holes to match.

Provide powder coated steel handrails to match existing handrails used at building 33. Color to be Sherwin Williams DTM Manor Brown.

The contractor shall install the same brick type, pattern etc. as building 33.

The contractor shall provide and install aluminum letters/numbers the same size as existing at building 33.

The contractor shall provide a park bench similar to the one depicted in the photo.

The contractor shall account for any drainage needs and provide as needed. No standing water anywhere on the concrete will be acceptable.

The contractor shall ensure a smooth transition from the concrete sidewalk to the ramp start point and through the doorway.

Any concrete washout shall be properly contained (i.e. kiddie pool, plastic etc.) and allowed to dry prior to disposal. No concrete washout will be permitted directly on the ground anywhere on DSCR.

The contractor shall submit an engineered drawing(s) approved by a licensed engineer showing all details, dimensions, elevations etc. for this project for approval.

Relocate the picnic table in the vicinity per government representative.

The contractor shall provide a portable bathroom for this project.

The contractor has 180 calendar days to complete this project from the notice to proceed.

Work hours will be from 7:00am – 5:00pm Monday-Friday.

5.2 PRE-CONSTRUCTION MEETING AND OTHER MEETINGS

After award and prior to the initiation of work, a meeting will be held with the Contractor, Contracting Officer, Contract Specialist/Contract Administrator, and any other individuals designated by the Government.

Pre-Construction Meeting purpose is to discuss matters of mutual interest concerning resulting order. General conditions, work schedule and coordination, security, safety, permits, and other matters pertinent to work accomplishments shall be discussed in this meeting.

The contractor is required to follow the U.S Army Corps of Engineers three phase inspection process. If the contractor is not familiar with this process it can be found on the internet.

Preparatory phase inspection – The contractor shall give the government 3 business days’ notice prior to calling for a preparatory meeting for each Definable Feature of Work (DFOW). No work can start on a DFOW without a preparatory meeting. The supervisor leading that DFOW MUST attend the preparatory meeting AND stay on the work site acting as a supervisor until that DFOW has been completed.

Initial phase inspection – The contractor shall give the government two business days’ notice prior to calling for an initial inspection.

Follow up inspections shall be conducted daily by the on-site supervisor to ensure all contract requirements are being met. The contractor shall schedule a pre final inspection after they confirm all the work conforms to the contract for either that DFOW and/or when all work is completed. The contractor shall give the government 3 business days’ notice of when they would like to hold any pre final inspection(s).

Final inspection – The contractor shall give the government 5 business days’ notice of when they would like to hold a final inspection/acceptance inspection. All punch list items previously identified must be completed before a final acceptance inspection will be scheduled.

The contractor shall not hold any required training the contract may require on the same day any startups of equipment are also required. They must be held on different days. The contractor shall provide manufacturer’s training on all equipment/systems installed. The contractor shall give the government 5 business days’ notice for any training and 5 business days’ notice for any startups of equipment so the proper government maintenance personnel can schedule to be present.

6 QUALITY CONTROL PLAN (QCP) AND SURVEILLANCE SCHEDULE

The Contractor shall submit a Quality Control Plan within 10 business days of contract award. This plan shall describe the Contractor’s methodology for compliance with the Deliverables and Performance Requirements Summary above. The CO, or his/her authorized representative will notify the Contractor of concurrence or required modifications to the QCP within 10 business days of receipt. The Contractor shall make appropriate modifications within 5 business days of the CO, or his/her authorized representative’s notification and provide a revised QCP within 5 business days.

The contractor shall perform daily inspections of this project conducted by a qualified employee. The contractor shall provide the COR/PM with daily inspection reports detailing the work accomplished that day, weather conditions, safety related topics etc. The daily report is due the next business day by 8:00am.

The contractor shall, within two weeks after receipt of the notice to proceed, provide the COR/PM with a construction schedule. This schedule shall be updated and provided weekly (on Monday morning by 8:00am or first business day of the week) to the COR/PM until the final inspection is completed.

7 SECURITY REQUIREMENTS

7.1 INSTALLATION SECURITY

CONTRACTOR ACCESS AND USE OF PREMISES

1. Security Requirements Defense Supply Center Richmond (DSCR):

a. No employee or representative of the Contractor will be admitted to the work site unless he/she furnishes satisfactory proof that he is a citizen of the United States or an alien who has been lawfully admitted for permanent residence, or who presents evidence from the Immigration and Naturalization Service that employment will not affect his immigration status.

b. Personnel Information: All authorized non-military personnel utilizing the base are required to have in their possession at all times an installation visitor pass or access control badge. This pass is issued by the DSCR Welcome Center, located at Building 210. The Welcome Center is open Monday through Friday except federal holidays from 6:30 a.m. to 4:30 p.m. In order to obtain pass/badge, the Contracting Officer or designee will request personal identifiable information (PII) from each contractor and submit it to Security a minimum of 7 working days prior to start of work.

c. Official Capacity: The Contractor shall comply with visitor pass requirements for the performance of work at DSCR.

d. The DSCR Security Department will run a criminal history check on Contractor employees as a condition of employment. Official proof of identity or naturalization papers may be required, which will be retuned once entered into the security computer system. Based on the results of the criminal history check, the Vetting Official (VO) will or will not grant entry to the installation based on guidance set forth by the Installation Commander. If entry is denied, the Contractor will be immediately notified. DSCR Security may require updating or rechecking criminal history based on the duration of the contract. NOTE:

The VO will deny access to any person who the Installation commander has deemed unfit for unaccompanied access.

e. Proof of Identity: Official identification consists of any current Government issued picture identification. Examples- of official identification are valid state issued driver's license, military ID card, military dependent ID card, green card, etc. Expired identification is not valid.

f. Pass or Badge Management: Expired, invalid, inaccurate, inoperative or terminated Pass or Access Control Badge shall be returned to the Contracting Officer or his designee for disposition. These cards are the property of the U.S. Government and shall not be retained by the cardholder upon expiration, replacement, or when the DoD affiliation of the employee has been terminated. If the Pass or Access Control Badge is lost or stolen a police report will be completed and a new request will be submitted by the Contracting Officer or designee. Compliance with this requirement is mandatory and certification thereof to the Contracting Officer is required prior to submitting final invoices. Failure to return badges will hold up Contractor's final payment.

g. Contractor Security Responsibilities: The contractor is responsible for employees and subcontractors under their employment. Contractors will ensure that employees are familiar with and obey installation traffic, safety, and security regulations. Contractor employees, including subcontractors, are required to wear the ID card conspicuously on their outer clothing and above the waist at all times while working on this Center. Personnel are subject to challenge and removal from the work area if the ID card is not being worn. It is the COR/designee’s responsibility to enforce this requirement. Failure to do so is just cause for ordering that work on a contract be stopped.

h. Unofficial photography, cameras, or photographs are prohibited on DSCR property. When official photographs are required, submit a written approval request containing specific justification and details to the Contracting Officer 5 days prior.

i. Firearms, Weapons, Alcoholic Beverages, Illegal Drugs, and Contraband are prohibited on DSCR property. Violators may be prosecuted. State issued/authorized concealed weapons permits are not valid on Federal (DSCR) property.

j. Trusted Traveler Program (TTP): Contractors are not authorized to use the TTP to escort individuals on to the installation.

k. DSCR is not responsible for job site security. Removal of material from job-site and Installation are subject to security checks. The contractor is to notify DSCR security of any special measures (watchmen, alarm, etc.), security concerns, security problems, or unusual activities within the job site.

2. TRAFFIC LAWS AND ENFORCEMENT:

a. Motor Vehicle Operation: Ingress and egress of personnel will be subject to the DSCR security regulations. All personnel must be made aware of the base speed limits which is 25 mph (unless otherwise posted). Speed limits are enforced and violators will be ticketed. DSCR traffic violations are processed through the federal court system.

b. Seatbelts are MANDATORY.

c. Use of cellphones while driving is prohibited.

d. Parking is in designated areas only, between two white lines. No parking is permitted in fire lanes, on seeded areas, in reserved parking slots, or assigned handicapped parking. Any equipment or vehicle that does not fit within a standard parking space must contact the Contracting Officer to determine a designated area for parking such equipment or vehicle.

e. Traffic accidents should be reported immediately to the DSCR Emergency Dispatch by calling 911 or (804) 279-4888 for non-emergencies.

f. All personnel entering the installation are subject to random vehicle inspections/searches at any time while on the Installation. The purpose of these inspections is to detect the theft of Government and private property, firearms, weapons, alcoholic beverages, illegal drugs, or contraband.

3. TRUCK DELIVERIES

a. It is the contractors’ responsibility to notify the Contracting Officer of truck deliveries and escort trucks from the truck gate to its destination. This will not be done by DLA Police.

b. All commercial vehicles will enter the installation through the Commercial gate adjacent to the North Gate access control point (ACP). All commercial vehicles will also depart the installation via the North Gate. Commercial vehicles include, but are not limited to, semi-trucks, box trucks, passenger busses, cargo vans, dump trucks and walk-in vans. All such vehicles entering the gate will be processed through an entrance security inspection point before continuing onto the Installation.

c. Vehicle Inspection: Commercial/large vehicle inspections will be conducted on all vehicles by DLA police per the most current Police Standard Operating Procedure. The inspection may be upgraded based upon Force Protection Conditions (FPCON).

d. Shipment Validation: All deliveries to DSCR will have some form of identifying documentation. For example, a bill of lading must identify: 1) a DSCR POC and 2) a delivery location. Validation will consist of reviewing the delivery documentation by DLA police officers as provided by the delivery truck driver.

Failure to do so may result in delivery delay or denial of the delivery. The source of delivery will be identified and verified by DLA police officers.

e. Driver Vetting: All delivery/commercial truck drivers will be vetted by DLA police officers prior to allowing entrance onto DSCR. Vetting will consist of conducting a background check of the driver and all passengers using Virginia Criminal Information Network (VCIN). Any driver or passenger having an outstanding warrant will be handled in accordance with DLA/DSCR procedures. If the driver and/or passengers have personal weapons (firearms) or dangerous weapons, they will not be authorized entrance to the facility. If procedural requirements have not been met, the driver will not gain access.

f. Increased FPCON requires increased screening requirements. FPCON Charlie and Delta require the opening and visual inspection of each package.

8 SAFETY AND HEALTH REQUIREMENTS

The contractor shall comply with all applicable federal and state safety rules and regulations pertaining to this SOW in Attachment I. The contractor shall conduct daily “tool box safety briefs” every day they are conducting active work on this project. Document on the daily reports that these safety briefs were held with a description of the topic discussed.

9 ENVIRONMENTAL REQUIREMENTS

The contractor shall comply with all applicable federal and state environmental laws and regulations pertaining to this SOW in Attachment II.

10 DISCLOSURE OF INFORMATION

Information made available to the Contractor by the Government for the performance or administration of this effort shall be used only for those purposes and shall not be used in any other way without the written agreement of the Contracting Officer. The Contractor agrees to assume responsibility for protecting the confidentiality of Government records, which are not public information. Each Contractor or employee of the Contractor to whom information may be made available or disclosed shall be notified in writing by the Contractor that such information may be disclosed only for a purpose and to the extent authorized herein.

10.1 PERMITS

The Contractor shall, without additional expense to the Government, obtain all appointments, licenses, and permits required to perform under this contract. Evidence of such permits and licenses shall be provided to the Contracting Officer and/or his designated representative before work commences and at other times as requested by the Contracting Officer and/or his designated representative. The contractor shall obtain a hot work permit anytime hot work will be accomplished. Grinding, cutting, welding etc. are considered hot work. The contractor shall call the fire department a minimum of 24 hours in advance to obtain a hot work permit. The contractor shall provide a minimum 10# ABC fire extinguisher which shall be properly tested and ready for use anytime hot work will be completed.

11 PAYMENTS FOR UNAUTHORIZED WORK

No payments will be made for any unauthorized supplies/services or for any unauthorized changes to the work specified in this SOW; this includes any services performed by the Contractor of their own volition or at the request of an individual other than a duly appointed Contracting Officer. Any additional work to be performed beyond the terms of this SOW must be approved by a Contracting Officer in writing prior to the commencement of such work; thus only a duly appointed Contracting Officer is authorized to change the specifications, terms, and conditions under this effort.

11.1 INVOICING AND PAYMENT SCHEDULE

Invoices and payments will be processed electronically via the iRAPT System (formerly WAWF).

12 PERSONNEL AND RELATED REQUIREMENTS

12.1 CONTRACTOR EMPLOYEES

The prime contractor shall always have at least one government approved supervisory representative on the installation whenever the prime contractor or sub-contractor is working. This supervisory person shall have complete authority to direct all prime and/or sub-contractors in all areas including quality control and safety and have the authority to stop work due to quality or safety related issues.

Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees by wearing a Security Identification Badge at all times above the waist on an outer garment while on Government premises. Tank top type shirts are not authorized for wear on DSCR Richmond at any time. Shorts are not permitted. Shirts must have sleeves a minimum of 4” down the arm as measured from the top of the shoulder.

No music of any kind is permitted to be played while working on DSCR Richmond. This includes radios, MP3, CD players, headphones, ear buds etc. A single ear piece device used solely for answering phone calls may be permitted at the discretion of the COR/PM. When Contractor personnel attend meetings, answer phones, and work in other situations where their status is not obvious to third parties they must identify themselves as such to avoid creating the impression that they are Government employees.

12.2 CONTRACT OVERSIGHT POSITION

The contractor’s proposal shall include the name and contact information of the individual responsible for the following functions:

1) Authority to act as the principle point of contact with the Contracting Officer.

2) Authority to submit invoices and act on the vendor’s behalf in contract performance matters.

13 PERIOD OF PERFORMANCE

The contractor shall complete this project within 180 days after the NTP is issued.

13.1 HOURS OF OPERATION

The Contractor shall provide personnel to diligently pursue completing this project as soon as possible after the NTP is issued. The contractor’s work hours for this project shall be 7:00 a.m. through 5:00 p.m. Monday through Friday except on days when the installation is closed. If the contractor would like to work outside these hours they must request, in writing to the CO, 7 business days in advance.

13.2 FEDERAL HOLIDAYS

The Contractor and all sub-contractors shall not work on the ten Federal holidays observed at DLA unless approved by the CO. A written request to the contractor officer is required a minimum of 7 business days in advance of a holiday. The Contractor shall make sure no sub-contractors work when DLA is closed. The following holidays are observed:

• New Year's Day (January 1)

• Martin Luther King's Birthday (Third Monday in January)

• Presidents’ Day (Third Monday in February)

• Memorial Day (last Monday in May)

• Independence Day (July 4)

• Labor Day (first Monday in September)

• Columbus Day (Second Monday in October)

• Veteran's Day (November 11)

• Thanksgiving Day (Fourth Thursday in November)

• Christmas Day (December 25th)

When one of the above holidays falls on Sunday, the following Monday will be observed as a legal holiday.

When the federal holiday falls on a Saturday, the preceding Friday is observed as the holiday.

13.3 OTHER DAYS OF INSTALLATION CLOSURE

When the Installation Commander closes the installation for any reason, including but not limited to days of inclement weather, the Contractor and all sub-contractors shall not work. The contractor may call the installation weather hotline at 1-888-325-6733 to determine if the base will be closed or have a delayed opening. If the base has a delayed opening the contractor may enter the base at whatever time is listed to continue their work effort. The Contractor shall make sure no sub-contractors work when DLA is closed.

14 PLACE OF PERFORMANCE

Place of performance will be the Defense Supply Center Richmond (DSCR) located at 8000 Jefferson Davis Hwy, Richmond, VA 23297 Richmond, Virginia.

15 ADMINISTRATIVE CONSIDERATIONS

To promote timely and effective administration, correspondence will be subject to the following procedures:

• Technical correspondence (where technical issues relate to compliance with the requirements herein) will be addressed to the CO with an information copy to the COR/PM.

• All other correspondence, including invoices, (that which proposes or otherwise involves waivers, deviations or modifications to the requirements, terms or conditions of this SOW) will be addressed to the COR with an information copy to the Contracting Officer.

• Whenever corresponding to the COR/PM via email the subject line needs to start with the title of the contract.

17 GENERAL

17.1 OTHER CONSIDERATIONS

The Contractor should conduct a site visit to each location to verify all measurements, sizes, interferences, quantity of material etc. for this project. The contractor shall consider ALL electrical, mechanical, plumbing, security or any other interferences that may impede their work to be part of their work under this SOW. The contractor shall remove, disable, relocate etc. all interferences as needed ensuring all items function as they were intended to at the conclusion of the contract. All items that need to be terminated for any reason are the responsibility of the contractor.

The contractor is required to request that all utilities be marked/located prior to any digging, earthwork etc.

to prevent injury or damage to personnel or utilities. The contractor shall request these marking through the COR/PM a minimum or 7 business days’ in advance and shall keep the marking updated throughout the life of the contract.

Anytime any utilities (water, heat, AC, gas etc.) will need to be shut down for any period of time during the life of the contract the contractor shall give the COR/PM 10 days’ notice.

The contractor is required to provide a back flow preventer for use on the hydrant. The contractor shall provide documentation that the back flow preventer has been inspected within the previous six months prior to using the hydrant. No hydrant shall be used without the proper documentation. The contractor shall give the government COR/PM three business days’ notice if they will need to access a hydrant. The contractor shall fill out and submit to the COR/PM the hydrant application 3 business days in advance. The application must be approved by the fire department and a copy provided to the contractor prior to hydrant use. At no time will the contractor use the hydrant without an approved hydrant application. The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish cleaning vehicles and equipment etc.

The Contractor shall diligently prosecute their work and must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this SOW when the Government facility is not closed for any of the above reasons.

Weather - The contractor shall always keep weather conditions in mind throughout the life of this contract.

Whenever weather conditions indicate strong winds, tornados, hurricanes etc. the contractor shall take all necessary action to prevent all materials, debris etc. from becoming missile hazards. Any damage or harm caused by contractor material, debris etc. will be the sole responsibility of the contractor.

III ATTACHMENTS

1 ATTACHMENT I: SAFETY AND HEALTH REQUIREMENTS

Contractors shall comply with all federal, state, and local occupational safety and health regulations, such as but not limited to US Department of Labor and Industry Occupational Safety and Health Administration (OSHA) , 29 CFR 1910,1926,1960 or whichever is applicable; in addition to safety requirements of US Army Corps of Engineers EM385-1-1 (latest edition), National Electric Codes (NEC), National Fire Codes (NFC), Defense Logistics Agency (DLA) safety regulations, or other safety requirements as established for the life of the contract. The safety provisions listed above are only general in nature and not intended to be all inclusive.

1.1 VOLUNTARY PROTECTION PROGRAM (VPP)

The Defense Supply Center, Richmond is committed to becoming an OSHA Voluntary Protection Program (VPP) site. Contractors shall comply with all OSHA rules and regulations as applicable in addition to all other Local, State and Federal Occupational Safety and Health Regulations. The objective is for all contractors to be familiar with VPP and abide by the Defense Supply Center Richmond Safety Requirements.

DELIVERABLE

Contractors and subcontractors shall adhere to all safety and health regulations and be aware of The Defense Supply Center OSHA VPP initiative. Contractors and subcontractors shall read the Visitor & Contractor Safety at Defense Supply Center Richmond Brochure prior to performance of any on-site work. This is a one-time process but should be completed for all new contractors.

Contractors shall acknowledge and document that all Contractor and subcontractor employees have read the Visitor & Contractor Safety at Defense Supply Center Richmond Brochure Contractors and subcontractors shall document receipt of a physical copy of the Visitor & Contractor Safety at Defense Supply Center Richmond Brochure by detaching the tear-off and leaving it with the COR, COTR or CO who, in return, will forward it to the Occupational Safety and Health Office (DS-FRES, building 80, 2nd Floor) before coming on site.

1.2 SAFETY PLAN

DELIVERABLE

The Contractor shall submit a site specific Occupational Safety and Health Plan (OSHP) for approval prior to the commencement of any on-site work for this SOW. Failure to submit an OSHP will delay contracted activities. All site specific OSHP shall be developed to comply with all federal, state, and local occupational safety and health regulations, consensus standards, and any trade specific practices which will include but not limited to the following; Occupational Safety and Health Administration (OSHA)29 CFR General Industry - 1910, Construction Standards – 1926, Federal Employee – 1960; Army Corps of Engineers EM385-1-1 manual;

National Electric Codes (NEC); National Fire Protection Association (NFPA) Codes; American Conference of Industrial Hygienist (ACGIH); and must at a minimum, be strictly adhered to by all contracted personnel. The following are required and must be submitted to the safety office via the PM;

Contractor safety plan, site specified activity hazard analysis, OSHA 300A, Safety Data Sheets and Contractor safety tool box meetings.

An OSHP will be required for all contracted services involving the construction, alteration, and/or repair, including painting or decorating of government buildings, public works, and equipment in addition to all activities using/applying products requiring safety data sheets (formerly material safety data sheets (MSDS)). OSHPs will also be required for all activities were there are potential health hazards which could be a result of the contracted services (ie. hazardous material surveys and clean-up, confined space entry, etc.). All contractors who are subjected to the submittal of OSHP shall also submit their most recent OSHA 300 log in accordance with 29 CFR 1904.

An OSHP will not be required for nonpersonnal services contracts including, but not limited to, those which are administrative only, deliveries/pickups, transportation or for nondestructive surveys. However, if contracted services specify a DSCR provided workspace the contractor should submit an OSHP or provide their COR and/or COTR with a letter stating that they will adopt and operate under DSCR's written occupational safety and health control programs. For all nonpersonnal service contracts not requiring an OSHP, please adhere to the Visitor and Contractor Safety Brochure which can be obtained from the COR and/or COTR. Please contact the Occupational Safety and Health office at 804-279-4830 if you need assistance in determining if your contracted services apply to an OSHP submittal.

All Contracted services requiring an OSHP where operations involving demarcating work areas with protective coverings, shields, protective barriers, barricades, warning signs and the like, will be furnished and installed by the Contractor where a hazard or potential hazard exists. Notices shall be provided twenty-four (24) hours prior to the planned installation of protective barriers and barricades, the Contractor will coordinate with the COR and/or COTR for concurrence.

Barricades and barriers will be removed by the Contractor at the completion of the work in the area.

1.3 ACCIDENT/INCIDENT

The Contractor shall notify the CO or his/her designated representative immediately after the occurrence of any incident resulting in injury, illness, or death in addition to major property damage to a Government facility. The Contractor shall comply with the following Mishap Reporting section for reporting all incidents.

1.4 INJURY TREATMENT

Emergency Medical Services are available for Contractor employees at DSCR. Emergency calls on the DSCR Installation are made by dialing 911. If a private ambulance service is used on DSCR, the Contractor must coordinate with DSCR Installation Emergency Communication Center (ECC) by calling 804-279-4888. The Contractor shall be responsible for all expenses incurred due to use of private ambulance service.

1.5 MISHAP REPORTING

A report on each job connected injury, property damage, or motor vehicle mishap will be written by the Contractor and submitted to the CO or his/her designated representative. The Contractor and CO or his/her designated representative will complete the Mishap Report (DLA form 1591) per DSCR guidelines. The estimated absence in days, if any, by employee due to injury and/or cost or property or vehicle damages shall be included.

1.6 SAFETY AND HEALTH VIOLATIONS

The Army Corps of Engineers Manual EM 385-1-1, Safety and Health Requirements Manual, 29 CFR 1910/1926/1960, General Industry/Construction Standards/Federal Employee (OSHA), National Electric Code (NEC) and National Fire Protection Association (NFPA) Codes, American Conference of Industrial Hygienist (ACGIH) must, at a minimum, be strictly adhered to by all contractors. After consultation with the contracting officer, the Installation’s Safety and Health Manager reserves the right to contact the Virginia Occupational Safety and Health (VOSH) Office for assistance if there are frequent safety and health violations by contractors.

DSCR Safety Specialists or Industrial Hygienists shall perform unannounced inspections of Contractor job sites to ensure compliance with applicable standards. Violations shall be reported to the CO, or his/her authorized representative. The Safety Specialist or Industrial Hygienist reserves the right to stop operations when violations could cause severe injury, death, and/or serious property damage. The Safety Specialist or Industrial Hygienist reserves the right to stop any activity where DSCR employees are impacted. Frequent safety violations shall be reported to the Virginia Occupational Safety and Health (VOSH) Office.

The DSCR Safety and Health Manager or Staff shall be available to provide technical assistance as required and may be contacted by calling (804) 279-4830 from a cell phone or extension 4830 from a Government phone.

2 ATTACHMENT II: ENVIRONMENTAL REQUIREMENTS

2.1 COMPLIANCE WITH ENVIRONMENTAL LAWS AND REGULATIONS

Contractor shall comply with all applicable federal, state, local, Defense Logistics Agency (DLA), and Defense Supply Center Richmond (DSCR) environmental laws, statutes, regulations, executive orders, permits. Contractor shall immediately report any conflicts between applicable federal, state, local, DLA and DSCR, and any specifications within this contract to the Contracting Officer Representative (COR) and the DLA Installation Support at Richmond, Environmental Office (Environmental Office).

2.2 COMPLIANCE WITH GREEN PROCUREMENT REQUIREMENTS

Contractor shall follow Federal EPA Comprehensive Procurement guidelines (www.epa.gov/cpg) for acquisition of building materials and products and select materials that have a long life cycle;

the least toxic materials; recyclable materials; materials that are resource-efficient; materials with the maximum recycled content; materials harvested on a sustained yield basis; and products causing the least pollution during their manufacture, use, and reuse.

2.3 REPORTING NONCOMPLIANCE

Contractor shall immediately report any nonconformance and/or noncompliance with applicable federal, state, local, DLA and installation environmental regulations and laws to the COR and Environmental Office. Contractor shall when given a verbal and/or written notice of environmental noncompliance or nonconformance by the COR, take immediate corrective action.

Failure or refusal to comply promptly may be grounds for the Contracting Officer to invoke the appropriate contractual remedies. This may cause all or part of the work to be stopped immediately until satisfactory corrective action has been taken.

2.4 CONFORMANCE WITH ENVIRONMENTAL MANAGEMENT SYSTEM

Contractor shall take the necessary actions to identify, monitor, and control those contract operations and activities that pose risk of contamination, or can negatively impact the natural and/or human environment.

2.5 PROTECTION OF WORK SITE RESOURCES

Contractor shall confine all activities to areas defined by the drawings and specifications. Prior to the beginning of any work, the Contractor shall identify any land resources to be preserved within the work area. Except in areas indicated on the drawings or specified to be cleared, the Contractor shall not remove, cut, deface, injure, or destroy land resources including trees, shrubs, vines, grasses, topsoil, and landforms. The Contractor shall provide effective protection for land and vegetative resources at all times. Contractor shall notify the Environmental Office if any trees are required to be disposed or removed. The Contractor is not authorized to remove or dispose of any tree greater than 6 inches in diameter unless permission has been granted by the Environmental Office.

2.6 NOTIFICATION OF FEDERAL AND STATE REGULATORS

Contractor shall immediately notify the COR and Environmental Office of the arrival on site of any federal, state, and/or Department of Defense (DoD) environmental regulator or enforcement agent and/or the receipt of any correspondence from a federal, state and local environmental agency.

2.7 GENERATION OF SOLID WASTE

Contractor shall remove from the installation and dispose of all solid waste generated, which cannot be recycled to an approved and permitted off-post disposal facility. Contractor shall make every effort to divert 60 percent of all construction and demolition debris waste and 50 percent of all other solid waste. Prior to removing any waste from DSCR for disposal, the Contractor shall coordinate with the installation Pollution Prevention Manager from the Environmental Office to arrange for recyclable materials to be removed and diverted from the waste stream and provided to the installation to receive credit toward meeting diversion requirements. Submit in writing the quantities of waste removed and recycled to the Environmental Office Pollution Prevention Manager on a monthly basis and at the expiration of the contract. The submittal shall include the date of disposal/recycling, the disposal/recycling facility, the types of material disposed/recycled and the quantities of materials disposed/recycled by weight. The Contractor shall establish a program to promote cost-effective waste reduction in all operations and facilities covered by the contract. This includes collection, separation, and processing products or other materials recovered from solid waste streams. The Contractor shall make maximum effort to reduce and prevent waste and comply with Executive Order 13423.

2.8 PREVENTION OF STORM WATER POLLUTION

The Contractor shall perform, track, participate, implement, and comply with storm water pollution prevention minimum control measures, protocols, and best management practices (BMP) and ensure that water quality standards are not violated in accordance with all regulations and policies as they apply to the Virginia Pollutant Discharge Elimination System general permits. Applicable permits include: 1) The installation’s Small Municipal Separate Storm Sewer System (MS4) Permit and 2) All Construction Activity Storm Water permits.

h. Contractors shall comply with the MSGP permit when the activity is identified as a permitted industrial activity. BMPs include, but are not limited to:

i. Practicing spill prevention and good housekeeping.

j. Installing and managing erosion and sediment control.

k. Meeting the requirements of the MS4 permit.

l. Coordinating with the Environmental Office to schedule inspections and provide corrective actions for noted facility deficiencies.

m. Contractors shall obtain permit coverage for construction activities disturbing over one acre of land (total acreage is cumulative across all portions of the project). BMPs include, but are not limited to:

n. Preparing and implementing a site-specific Storm Water Pollution Prevention Plan

(SWPPP) as outlined in the permit and prior to any soil disturbance.

o. Installing and managing erosion and sediment control.

p. Make available, upon request, permit associated documentation.

q. Practicing spill prevention and good housekeeping.

r. Meeting the requirements of the MS4 permit.

s. Coordinating with the Environmental Office to schedule inspections and provide corrective actions for noted facility deficiencies.

2.9 USE OF ASBESTOS AND LEAD-BASED PAINT

Contractor shall not provide Asbestos-Containing Materials (ACM) or products, or paint with a lead content higher than 0.06 percent by weight, to the installation without written approval of the Environmental Office. Contractors must provide a certificate to the COR at the conclusion of the contract verifying that contract materials and products used are asbestos free. Common ACM include, but are not limited to adhesives, mastics, sheetrock muds, and vinyl and tile flooring.

Contractors installing new thermal system insulation must identify the new insulation with a blue band or cap at the locations where the new insulation begins and ends, and stencil “Non ACM” or “Asbestos Free” on the new insulation.

2.10 CULTURAL RESOURCES SITES

Do not excavate, remove, damage, or otherwise deface any archeological resource located on public lands without consent from the environmental office.

2.11 WETLANDS

Do not excavate or take any action that could fill or damage any wetland unless working under a project specific Corps of Engineers permit. Wetlands include streams, riparian areas, bogs, marshes, and swamps.
