PERFORMANCE WORK STATEMENT (PWS)

DEPLOYMENT PHYSICIAN SERVICES

SCOPE OF WORK

General: These specifications will provide professional medical services of a licensed physician with specialty in Civilian Contingency Deployment physical examination criteria as indicated by US ARMY Central Command.

The Contractor shall be licensed as a Board Certified practicing medical doctor in Occupational & Environmental Medicine (MD or DO) in State of Michigan.

The Contractor shall have experience in the specialty in South West Asia (SWA) Contingency Deployment Physical Examinations.

The Contractor shall not be required to relinquish any private practice except to the extent required to perform this contract.

The main function of the contractor is to provide Medical Direction related to the Occupational Health Clinic and Civilian Expeditionary Workforce (CEW) medical program oversight.

The Contractor in the performance of his/her duties shall not assume the role of a private physician.

Administrative duties and necessary preparation required to perform these duties are included in the services as long as they are performed at the HDI Federal Center.

The selected Physician will work a minimum of 4 hours a week and a maximum 16 hours a week. If circumstances prevent the adherence to this time frame, prior approval must be obtained from the Contracting Officer’s Representative (COR) to deviate.

The contract award from this solicitation will be a fixed hourly rate labor hour contract.

Services are identified under duties Duties section of this contract and are performed at the Federal Center.

TERM OF CONTRACT

This contract will be awarded with a twelve-month base period and TWO (2) twelve-month option periods

CONTRACT TYPE

The contract award from this solicitation will be a labor hour contract with a fixed labor rate.

DUTIES

Provide Occupational Health (OH) Exams. Occupational Health Care Provider must offer occupational health exams when medically determined as appropriate and necessary.

OH provider may offer baseline occupational medical examinations to provide baseline measurements for comparison to periodic measurements in the detection of early or subclinical health effects. Based on guidance of DoDI 6055.5-M, Occupational Medical Examinations and Surveillance Manual May 2, 2007.

OH health provider providers must offer termination of exposure examinations when the employee’s exposure to a specific hazard has ceased due to employee reassignment, changes in existing work processes, or termination of employment where required by OSHA standards (e.g. 29 CFR 1910.1001).

OH health provider will act as overseeing of the hearing conservation audiograms for employees at risk of noise-induced hearing loss using the procedures of the DoDI 6055.12 DoD Hearing Conservation Program.

Provide pre-placement periodic job related and fitness for duty physical examinations as required.

Provide Courtesy Employment physical examinations as required by DHRC.

Comply with the following manual: DoD-D 1404.10, DoD I 6490.03, and CENTCOM PPG Mod 13 Contractor will be provided with copies if requested.

Authorize and/or prescribe only those drugs or supplies which are essential to maintain the health and welfare of deploying personnel.

When physician is in the building will provide emergency response oversight, otherwise is to direct care given by professional nurses.

Provides guidance and oversight in the development of Travel Medicine Program to include review and appropriate protective immunization guidance for employees planning work related foreign travel.

Participate in maintaining records for each individual employee in accordance with paragraph 1.4 D - Health Records. Records shall indicate symptoms, diagnosis, treatment plan and all prescribed substances or over the counters to be dispensed with the instructions to be given to the patient.

Provide advice and oversight of the Medical Surveillance program. Conduct medical surveillance examinations for asbestos, chemical, radiation and other exposure agents. Consultation services for air quality, asbestos, noise exposure, chemical sensitivities, heat/cold stress, muscle-skeletal injuries, and ergonomics.

Provide Emergency Essential (EE) or Civilian Expeditionary Workforce (CEW) and Deployment physical examinations as required.

Serve as Medical Review Officer for Pre-Employment of Civilian Expeditionary Workforce (CEW) Program as well as those with deployment limiting medical conditions.

Comply with the following manual: DoDI 6490 and DoDI 6490.07;DoD-D1404.10 and AR 40-501, as well as OPM 5 CFR 339; Medical Qualification Determinations. Contractor will be provided copies of these documents if requested.

Act as liaison with other services physicians to improve care and facilitate smooth deployment and EE/CEW employment.

Participate in maintaining records for each individual employee in accordance with 5 Code of Federal Regulations (CFR) 293 Sub part E.3.5 health records. Records shall indicate any substance prescribed or dispensed and treatment or instruction given to deployer.

Along with Occupational Health Nurses, maintain and support medical Emergency Essential and Deployment policies and procedures required for the functioning of the Emergency Essential Physicals or Deployment Processing.

Conduct post deployment physicals on those employees returning from deployment. Referrals to outsources as directed in DoDI 6490.03.

CONTRACT REQUIREMENTS

MEDICAL LICENSE

The Contractor shall be licensed as a practicing medical doctor with a specialty in Army South West Asia (SWA) Contingency Deployment Physical examination criteria and Board Certification in Occupational & Environmental Medicine (M.D. or D.O.) by the State of Michigan with occupational health experience. The contractor shall provide proof that he is a licensed medical doctor in the State of Michigan (see evaluation criteria for the contract solicitation phase).

PRIVATE PRACTICE

The Contractor shall not be required to relinquish any private practice except to the extent required to perform this contract. The main function of the Contractor is to provide emergency and occupational health treatment as it relates to deployment and day to day activities of Occupational Health Clinic operations. The Contractor shall refer HDIFC employees to a private physician for follow up treatment.

RESPONSIBILITY OF THE GOVERNMENT

The Government will provide all facilities and supplies necessary for the operation of the HDIFC Occupational Health Clinic. The Government will maintain the time records specified in paragraph 1.4.F Time Records.

HEALTH RECORDS

All official records are the property of the U.S. Government and will be maintained at the HDIFC Occupational Health Clinic in accordance with General Provision, 14, Privacy Act of 1974. When employees participate in voluntary examinations or testing programs conducted by the Contractor, the confidentiality is vital to protect the employee. Test results and medical findings of the Contractor will be made available only to the employee or to his/her designated physician.

ADMINISTRATIVE SUPERVISION

Administrative supervision of the HDIFC Occupational Health Clinic is the HQ DLA Installation Support at Battle Creek Safety and Health Clinic Manager or his designee.

TIME RECORDS

Time records (see attachment 1) of the actual number of hours of service will be maintained by the Government and verified by the contractor. The records are the property of the Government. Such time records will be the basis for determining contractor’s conformance to the time requirement set forth in 1.4.H, Duty Hours and for totaling the number of hours of service for each month. Time records will be physically located in the HDIFC Occupational Health Office. The contractor shall be signed in prior to commencing work and sign out time at the end of duty day on a daily basis, with the sign in/out time being the determiner of actual hours worked (i.e. transit time to/from the duty station is not chargeable).

1.4.G FEDERALLY OBSERVED HOLIDAYS

Federal law (5 U.S.C. 6103) establishes the following public holidays for Federal employees. Please note that most Federal employees work on a Monday through Friday schedule. For these employees, when a holiday falls on a non-workday -- Saturday or Sunday -- the holiday usually is observed on Monday (if the holiday falls on Sunday) or Friday (if the holiday falls on Saturday).

The contractor will not be required to work on the following legal holidays observed by this agency: New Year’s Day, Birthday of Martin Luther King, Jr., Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day.

1.4.H DUTY HOURS

The Contractor shall be on duty at the HDIFC Occupational Health Clinic as needed Monday through Friday, excluding federal holidays or days the HDIFC is closed.

The Contractor is required to perform duties between the hours of 8:00 a.m. and 4:00 pm. each day on duty, with the specific week-to-week start times based upon deployment schedules and therefore as specified by the COR on a weekly basis. By COB on Friday of each week, the COR will provide (via email to the contractor) the following week's schedule including required start times.

The Contractor shall work a to in total not exceed sixteen (16) 16 hours a week.

Changes in the performance period hours of work are based on the patient care needs, but only by mutual agreement between the Contractor and the Government representative (COR or COTR). Performance of service over eight (8) hours/day two (2) days a week must be approved in advance by the HQ DLA Installation Support Battle Creek Safety and Health Manager.

1.5 Acceptable Performance Levels: The contracted Physician shall not be late for the designated start time(s) in a given workweek more than two occasions per month, of a duration of not more than 30 minutes late on each occasion, with the number of occasions and/or duration of tardiness beyond these limits subject to possible monetary consideration per FAR 52.212-4.

The completeness and accuracy of all documentation prepared by the contracted Physician shall be maintained to professional standards and is subject to the COR’s inspection for acceptability as per FAR 52.212-4, with all other required services also subject to this same clause coverage for inspection and acceptance, including possible monetary consideration depending on the extent of any deficiencies.

Contractor Manpower Reporting 1.6 The contractor shall report all contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Defense Logistics Agency via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil. 1.6.1 Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. The contactor may direct questions to the help desk at: http://www.ecmra.mil. Contractor Requirements for Contractor Verification System (CVS). Contractor employees requiring access to the HDIFC, DOD Facilities or DLA Information Systems for at least 24 hours a week for at least 6 months: All individual contractor employees whose work under the contract requires on-site access to a Federal controlled facility or logical access to a DLA information system for at least 24 hours a week for at least 6 months a year, will be required to undergo a background investigation in order to receive access to DLA information system and a Common Access Card (CAC). To begin the CVS process, the contractor should submit to the Contracting Officer Representative (COR) within ten (10) days of contract award or contract modification, the following information First Name, Middle Name, Last Name, SSN, Date of Birth, and email address. Information needs to be sent via a secure means (PKI). The point of contact for training or questions on CVS is the Security and Emergency Services Branch, DLA Installation Support at Battle Creek.

Common Access Card (CAC). The CAC is used as a general identification card as well as for authentication to enable access to DOD computers, networks, and certain DOD facilities. It also serves as an identification card under the Geneva Conventions. The CAC enables encrypting and cryptographically signing email, facilitating the use of PKI authentication tools, and establishes an authoritative process for the use of identity credentials. Employees issued a CAC are responsible for their proper safeguard. Loss of CAC shall be reported to the COR within twenty-four hours following discovery of loss. Termination of employees shall be reported to the COR along with a written statement describing the circumstances surrounding the termination/completion of contract/employment and assignment at the HDIFC. The Plant, Property and Equipment Branch, DLA Installation Support at Battle Creek will keep the written statement on file.
