ATTACHMENT 1

Statement of Work (SOW) Equipment Impact System

Background:

a. Requiring Organization DLA Distribution Warner Robins Georgia

b. History DDWG has severe problems with forklift impact to storage racks, elevator doors, building columns, overhead doors, fire doors and conveyor systems. DDWG needs a system that will identify the operator when a vehicle impacts the items above, shuts down use of the vehicles until it is reset by the maintenance department or warehouse supervisor, maintains a log of the offenders with the time, date and location of the accident and the duration of time the vehicle is locked out until the vehicle is reset.

c. Purpose of the Statement of Work (SOW):

Supply and install vehicle impact manager ID system (Shock Watch ID 1300-PR-T or equivalent) on 109 DDWG forklifts/stock selectors/turret trucks and pivot mast vehicles. The system shall provide ID access control and impact and utilization monitoring devices for the material handling equipment. This system shall be integrated with the forklifts and the Impact Manager ID that shall detect impacts and record the event details, such as the driver ID, date, time, and location of the incident. The data shall then be downloaded through user interface options for the ID such as a data ranger data collection device that can download

1. Scope of Work to be Performed by Contractor:

The contractor shall provide all necessary engineering, manufacturing and labor to provide and deliver 109 vehicle impact manager ID systems and accessories needed on the DDWG fleet of stock selectors, turret trucks, swing mast, pivot mast and forklift vehicles. The initialization of the vehicle shall be started by the employee doing a swipe of operator I buttons that the contractor provided. The contractor shall oversee the installation of the Impact manager ID w/proxy reader and accessories to be located on the vehicle and which shall allow the vehicle to be turned on. The operator information shall be recorded in memory such as the time, date, operator name, time logged on, time logged of, record of any impact, location of the impact, time and date the vehicle was reset, the person who reset it, vehicle utilization and automatic logoff time due to vehicle inactivity. The contractor designed, supplied and government installed vehicle impact sensor system shall record this information and shall be downloaded manually using the data ranger to the contractor supplied and installed vehicle manager software system installed on the Government stand alone computer 12 black I Buttons will be supplied to give the warehouse supervisors and maintenance personnel access to reset the vehicles when a vehicle accident occurs.

The contractor shall supply On-Site implementation/over site of the installation and training of the systems on the lifts for 3 days. The contractor shall do a follow up on-site visit 2 weeks after the initial first visit for to 2 days for any further training or trouble shooting needed.

a. Any data tracking system cannot be an RF or Web Link based system. No data can be collected by a wireless access point.

The Impact manager system must be equivalent to or greater than the Manufacturer Shock Watch ID1300-PR-T and accessories

109 each Impact manager ID systems with Proxy reader (Reference number ID1300-PR-T) 72 each filter blocks for the electric forklifts (Reference number RF10076) 37 each 12V relays for 12 V LP trucks (Reference number ID10091) 6 each equipment command software (Reference number EQC1400) 6 each Key kits (Reference number EM11046-PR) 12 each yellow data collection keys (Reference number ID11002) 12 each Black reset keys (Reference number ID11018) Red Program Keys (Reference number ID11001) 12 each Blue lock out keys (Reference number ID11013) 12 each Green operator keys (Reference number ID11000)

Warranty Coverage - Warranty coverage shall be manufacturer’s standard warranty.

4. Description of Contractor’s Tasks:

Task 1: Fabricate and Deliver the units specified above or manufacturers equivalent.

Task 2: Provide oversight of installation and training for 3 days Task 3: Follow up with a return visit 2 weeks after the initial visit for 2 days For trouble shooting and any further training.

5. Governments Responsibility Supply labor and tools to install this equipment

Supply stand alone computers (Windows 7 or XP, 2.0 GHz or faster, 2 GB Ram,
400 MB available disc space.)

6. Certification and Acceptance:

Final acceptance of the ID system will be provided by DDWG

7. Performance and/or Delivery Schedule:

The contractor shall coordinate the delivery with POC provided by DDWG.
