STATEMENT OF WORK
38th Rescue Squadron
Tire Carousel
1 PURPOSE This Statement of Work (SOW) defines the scope of work to accomplish the installation of a Semi-Automatic Tire Carousel.
1.1 DESCRIPTION OF SERVICES
1.2 Vendor shall provide all supervision, labor, administrative support, materials, tools, parts, supplies, equipment, and transportation necessary to effectively and efficiently fulfill all the requirements of this Statement of Work (SOW) to accomplish the services described below at our facility.
1.3 The scope of services includes the following:
1.3.1 Installation of Tire Carousel Storage System.
1.4 WORK SCHEDULE, PLANS, AND WORKING HOURS Workdays for this contract will be standard duty hours: Monday through Friday, 9:00 am to 4:30 pm, unless specified below. Building will have vehicles and other support equipment inside. Clear working space will be provided to the contract team;
however, contractors are not to operate government vehicles or support equipment. Written permission from the designated Government Representative shall be obtained before making any deviation from the schedule.
1.5 AVAILABILITY OF UTILITY SERVICES All reasonable quantities of utilities will be made available to the contractor. Appropriate site conditions including necessary utilities to perform the work, accessible paved roads, appropriate dock availability, regular and uninterrupted access to the installation site during normal project site hours. The Government shall not be held responsible for interruptions of utility services, and will not be liable for contractor delays, damages, or increased costs incurred by any such interruption of service. The contractor shall be responsible for providing all labor, equipment, and materials to make necessary temporary utility connections. The temporary connections shall be coordinated with the Government Representative before being made and shall be removed upon project completion.
1.6 CLEAN-UP All debris and rubbish not specified to become Government property resulting from the installation of new flooring shall be disposed of by the contractor off Government property. The Government will designate a location for a contractor provided dumpster, if necessary. Disposal will follow all applicable Federal, State, and local hauling and disposal regulations. Site clean-up shall occur daily.
1.7 SAFETY REQUREMENTS Safety requirement shall be the applicable sections of EM 385-1-1, Oct 1992 and Occupational Safety and Health Administration (OSHA) regulations. OSHA regulations are available for review at the Base Safety Office, Building 105. EM 385-1-1 is available for reference in the Civil Engineering Office, Building 918. The contractor shall ensure that any lifting devices or ladders meet all applicable safety standards. The contractor shall be required to use those lifting devices that will allow for the existing modular systems furniture to be raised for the installation of the new materials requested. The contractor will not be required to disconnect any electrical system connections to or from the modular systems furniture.
1.8 SITE VISIT A site visit will be offered pursuant to Federal Acquisition Regulation (FAR) Clause 52.237-
1. Regardless of site visit attendance, contractors acknowledge satisfaction as to the nature and location of the work, the general and local conditions, access to job site; disposal, handling and storage of materials; availability of labor, water, electric power or similar physical conditions at MAFB; the character of equipment and facilities needed preliminary to and during the performance of the work;
and all other matters that can in any way affect the work or costs related to this requirement upon submission of offers. Contractors further acknowledge that any damage to the existing structure in and around facilities will be returned to existing condition prior to the completion of subject work at no additional cost to the Government in accordance with FAR Clause 52.237-2. The contractor shall provide the Government with a one-year warranty on workmanship. The contractor shall provide any manufacturer’s warranty to the Government.
2 RESPONSIBILITIES
2.1 Perform a thorough inspection upon completion of installation.
2.2 Level equipment.
2.3 Ensure that the tire carousel system is operating efficiently and safely upon completion of installation.
2.4 Provide a detailed report and demonstration of the equipment installed and function.
2.5 Attend any required safety meetings, not to exceed 1 hour.
2.6 Arrange and complete training session with facility staff on any demonstration, if necessary.
2.7 Arrange customer inspection of the storage system and obtain signed completion acceptance sheet prior to leaving the job site…if necessary.
2.8 Consolidate all debris and dispose of it in compliance with applicable regulations.
3 CUSTOMER RESPONSIBILITIES
3.1 Provide clean, clear access to the site with necessary utilities.
3.2 Ensure that facility staff are available and prepared to attend any required safety meetings and training sessions.
3.3 Remove any obstructions or materials that may interfere with the ability to perform maintenance services.
3.4 Provide material and labor for wiring from building power source.
3.5 Inspect the tire carousel upon completion of installation, sign the install completion/acceptance sheet while the crew is on site…if necessary.
3.6 Building permit fees, plan check fees and fess for professional engineer’s seal on drawings and calculations are not included in the preceding system price.
3.7 Ensure appropriate site conditions, including necessary utilities, accessible paved roads, and regular and uninterrupted access to the storage system during normal project site hours. Area must be vacated of all other trades.
