About this file

This Statement of Work (SOW) details a sewer service line installation project in Henryetta, Oklahoma, for the Indian Health Service. The project involves furnishing and installing a 4-inch PVC gravity sewer service line from a residence at 201 West Cummings Street to the community sewage system, utilizing both open cut and pipe bursting (directional drilling) methods. Key technical specifications include using SCH40 PVC piping, maintaining specific horizontal and vertical separations between water and sewer services, and installing two-way cleanouts.

The contractor is required to perform work Monday through Friday between 8:00 a.m. and 5:00 p.m., obtain necessary permits from the City of Henryetta, conduct video inspection with CCTV equipment, and provide a minimum one-year warranty on materials and labor. The existing sewer service line is clay tile with a collapse underneath a neighboring property, necessitating the pipe bursting technique. The project involves careful trench preparation, backfilling, and restoration of the site to its original condition, with specific requirements for pipe depth, compaction, and settlement correction.
