STATEMENT OF WORK

Jemez Day School

Pest Control

1. SCOPE OF WORK:

The contractor shall provide pest control to the school located at Jemez Pueblo, quarterly.

Service will include classrooms, administration offices, hallways, IT rooms, cafeteria, janitor closets, crawl space, modular buildings, restrooms, locker rooms, gym, maintenance shop, loading dock, and door entry ways. Pesticides used will be EPA approved, approved for healthcare/low odor.

2. PROJECT LOCATION:

3. PURPOSE:

The purpose of the project is to provide a comfortable learning and working environment for students and staff.

4. SPECIFIC REQUIREMENTS:

A. SCHOOL SITE VISIT:

Not required.

B. SERVICES:

The contractor shall perform the following:

• Will follow all label safety measures on pesticides.

• Will follow safety policy regarding PPE.

• Spray with EPA approved pesticides, indoor use.

• Spray with EPA approved pesticides, outdoor use.

• Placement of mouse traps and sticky pads.

• All bait is enclosed in child proof bait stations; will be placed where only targeted pests will find them.

• Sweep spider webs.

C. GENERAL REQUIREMENTS:

a. QUOTE/PROPOSAL:

Quotes/Proposal shall be itemized by material, supplies, equipment, labor costs, and NM State Taxes.

b. WORK HOURS:

Work hours for this project shall be schedule with school maintenance worker. With safety in mind, contractor prefer to spray before or after school with minimal staff in the building.

c. SAFETY:

The contractor shall adhere to all BIA/BIE adopted codes and the contractor shall abide by all OSHA regulations while on school grounds, and shall review all applicable safety regulations before beginning the project. The contractor shall provide signs, barriers, and or caution tape as a safety precaution around the work site. The contractor shall have a fall protection plan if workers are to be performing duties above six (6) feet from the ground or finished floor. The contractor shall bear sole responsibility of taking all appropriate actions necessary to prevent accidents, injuries to individuals and damage to property in and around the work area.

d. BACKGROUND INVESTIGATION:

The awarded contractor shall provide a designated individual(s) from their company who will be subject to a BIA Security Background Investigations in accordance with Section 231 of the Crime Control Act of 1990, Public Law 101-647. Background Investigation forms will be provided at the pre-construction meeting.

e. ACCESS:

BIE Facility Management personnel shall provide access to Jemez Day School at all times. The contractor shall minimize disruption of classroom instructions and normal school activities expected in such areas.

f. DAMAGES:

Where existing surfaces are damaged the contractor shall repair and finish with matching existing structure. The contractor shall be responsible for replacement of any items (interior or exterior) damaged during the performance of this project.

g. CLEANING:

The Contractor shall keep the work area reasonably clean at all times. Contractor shall accomplish a complete cleanup at the end of the project. The Contractor shall properly dispose of all debris at an approved dumping site.

h. PERFORMANCE:

Performance time for this project is 30 Calendar Days from Notice to Proceed.

(Exception: Work days maybe interrupted with closures of the Pueblo due to Tribal Traditional Activities). All work shall be completed in a professional manner and without delay once the on-site work is started.

i. SALVAGE:

All salvageable materials and equipment shall be the property of the contractor unless otherwise noted in the Statement of Work and/or drawings. The Contractor is responsible for the removal of all salvageable materials and equipment from the installation.

5. PRODUCTS:

A. WARRANTY:

The Contractor shall provide a written warranty on the material and workmanship from date of acceptance by the Government.

B. MATERIALS:

The Contractor shall provide a written statement noting all material being utilized are asbestos free.
