Statement of Work, 12 month Lab Equipment Service Agreement

Introduction The Tropical Agriculture Research Station (TARS) in Mayaguez, Puerto Rico, as part of the USDA’s Agricultural Research Service is seeking a one-year service contract to provide preventive maintenance service and immediate repair service to a variety of lab equipment listed below. The service must be carried out by trained service engineers that are familiar with Perkin Elmer lab equipment. This service must include labor, necessary and instrument-compliant replacement parts, 1-2 hr travel time to the location, emergency breakdown visits, 10% - 20% discount on training and one preventive maintenance visit.

1. Key Assumptions

The parties agree that the schedule, level of effort, and outcomes listed in this SOW are based on the following assumptions:

Seller has the necessary trained personnel, personal protection equipment, means of transportation, packing containers and skills to perform the services.

1. The Contractor must provide all the services described in the introduction of the SOW. All services will be provided on a timely manner and emergency repairs will have be conducted within 24 hours once is informed.

1. All personnel must sign in at the Main Building upon arrival where they will be directed to the location chemistry lab where all the equipment is used.

3. Buyer Responsibilities 0.

Buyer will provide designated contacts: Elkin Vargas, Acting Administrative Officer; phone: 787-831-3435 Ext. 230 OR 787-464-6999.

Buyer contacts will review all Seller invoices. Contract will be in effect once awarded and contractor is notified.

4. Schedule

The Location of services will be at USDA, ARS, TARS 2200 Pedro Albizu Campos Avenue, Mayaguez, Puerto Rico 00680.

The Contractor must coordinate in advance the day and time with designated contact Elkin Vargas (787-464-6999) to do the job.
