Statement of Work Computer Numeric Controlled (CNC) Abrasive Cutting Machine

26 May 2020

1. DESCRIPTION OF SERVICES:

The contractor shall provide all management, tools, supplies, equipment, and labor necessary to provide a Computer Numeric Controlled (CNC) Abrasive cutting machine (water jet) for Hurlburt Field, FL.

2. BASIC SERVICES:

2.1. The contractor shall provide a OMAX 80X Abrasive Jet – Table 180” x 90” CNC Abrasive Water Jet Machine with EnduroMAX P3060V Pump and the following options: Material Holding Kit, Nozzle Spares Kit, Tilt-A-Jet, Precision Optical Locator, Collison Sensing Terrain Follower, Basic EnduroMAX Pump Spares Kit, and a Remote Key Pad.

2.2. The contractor is required to provide unlimited seats of software and software upgrade for the life of the machine.

2.3. The machine shall be fully assembled at the factory and tested prior to shipping. A factory technician shall arrive onsite to assemble the components of the Water Jet and start the machine to assure its operations prior to Government acceptance.

2.4. After machine start up, the factory technician shall provide 3 days of onsite machine operation and maintenance training.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES:

There will be no Government Furnished Property or Services provided for this effort.

4. HOURS OF OPERATION:

4.1. Installation will be performed in an area that requires escorts. Installation dates and times will need to be coordinated and approved in advance.

4.2. Normal Hours of Operation: The contractor shall perform the services required under this contract during the following hours: Monday - Friday 0700 to 1600, except for federal holidays. The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government.

4.3. Recognized Holidays: The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

5. BASE ACESS:

5.1. Contractor/Employee Base Pass and Identification, Special Clearances, and Vehicle

Passes: The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation. The contractor shall submit the employee listing to the Contracting Officer no less than three

(3) days prior to the performance start date and at least one (1) day prior as changes occur. Employees with active warrants or felonies will not be granted access to the base.

5.2. The Contractor Employee List: This form shall list every employee requiring base access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word “escort” or “E” next to their names. Limit the number of individuals designated as escorts to one or two. For work locations use building numbers or specify as base wide if applicable. Once completed, email the list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee List to the Visitor Control Center.

5.3. Visitor Control Center Documentation: Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification. If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.

5.4. Contract Performance: During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

5.5. Weapons, Firearms, and Ammunition: Contractor employees are prohibited from possessing weapons, firearms, and ammunition on themselves or within their contractor-owned or privately owned vehicle while on Hurlburt Field, FL.

5.6. Smoking, Eating, and Drinking: The contractor shall permit smoking, eating, and drinking only in 1st Special Operations Wing designated areas.

5.7. Driving License/On-base Requirements: The contractor and employees shall comply with base traffic regulations at all sites. The contractor shall ensure employees have a current and valid state/country driver’s license and government license for the type of vehicle being driven before allowing the employee to operate a government vehicle or personal vehicle on Hurlburt Field, Florida.

6. PROJECTED SCHEDULE:

6.1. Installation: The installation shall not last more than 14 calendar days from start of the project. The contractor shall uncrate/unpack all new items and perform required assembly in accordance with the manufacturer’s instructions. Contractor shall remove and dispose of packing material at an off base location.

6.2. Schedule: The contractor representative shall meet with the End User to review the installation drawings/design prior to installation. The contractor shall leave the area clean and free of all debris resulting from this installation. Before commencing work, the installation crew shall notify the Contracting Officer of the proposed schedule for approval. Additionally, the installation crew will bring the equipment/tools necessary to complete the job and will clean up at the end of each day and before leaving the designated job location.

7. MEASURING, CONVERSION AND GUARANTEE:

7.1. It shall be the complete responsibility of the contractor to obtain accurate dimensions at the job site and to examine all conditions as they relate to this installation. Skilled technicians shall perform the installation. All work shall be accomplished in a neat and professional manner.
