FOR

Mobile SCBA Fill Trailer

STATEMENT OF WORK

FOR

Mobile SCBA Fill Trailer

1. DESCRIPTION OF SERVICES: The contractor shall provide all supplies, materials, tools, equipment, and qualified labor necessary to purchase and deliver a Mobile SCBA Fill Trailer IAW Salient Characteristics. Mobile SCBA Fill Trailer include the following options: a spare tire with cover, a 100ft air hose reel with hose, a lunette eye hitch, and two ASME air storage cylinders. The contractor shall comply with manufacturer’s recommendations and procedures. The service will be the sole responsibility of the contractor. An operational system shall be turned over to the Government. System shall have the ability to fill Self Contained Breathing Apparatus (SCBA) bottles at remote locations IAW NFPA 1901.

2. GENERAL INFORMATION:

2.1 Contractor delivery hours shall be during normal business days Monday through Friday from 0600-1500 hours unless prior written approval by the user and Contracting Officer. Standard federal holidays will be observed as non-work days.

2.2 Due to the operation use of building 218 there may be required immediate closure to all non-DoD personnel. These events are rare, and if required may result in additional time for the period of performance. Other events may be scheduled with short notice that require closure of the building to all visitors for a period of 1 to 4 hours. These non-emergency events will be scheduled with the Contractor at least 48 hours in advance.

2.3 Contractor shall not park for loading or unloading of equipment in front of high-bays. Due to the nature of building 218 full access into and out of the bays shall be left unimpeded at all times. Coordinate with the user for any times when large pieces of equipment may need to be delivered that would require use of the side or main doors.

3. QUALITY CONTROL. Contractor shall develop and maintain a quality program to ensure services are performed in accordance with the SOW.

3.1 QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance in accordance with the Statement of Work.

3.2 GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items for contractor’s failure to correct nonconforming services.

4. DELIVERY AND SITE INFORMATION

4.1 Access to Peterson Space Force Base is limited to personnel with active badge access. Contractor shall either have a badge access or shall submit all names to the 21 CONS Contracting Officer for badge access. All personnel shall be able to pass a simple background check and shall not have any outstanding warrants for arrest or be on previous felony list or sex offender registry.

4.2 The contractor and all deliveries shall utilize the commercial gate for base access. Contractor shall be aware that traffic and the gate can cause delays lasting up to hours and that peak traffic hours at the gate are 0600-0800 and 1530 onwards.

4.2.2 All commercial trucks must enter through Peterson SFB designated gates for inspection. In accordance with the Installation Peterson SFB entry procedures must adhere to delivery vehicle process requirements mandated by the Wing. Once at commercial entry gate, the driver must have a current pass/bill of lading.

4.2.3 Contractor at the Commercial Gate must be able to show:

4.2.3.1 Current Vehicle Registration

4.2.3.2 Current Proof of Insurance

4.2.3.3 Valid and current Driver's License for the driver

4.2.3.4 Valid and current Driver's License or other State Identification for any passenger(s) in the vehicle.

4.3 Gate Procedures:

4.3.1 Once at Commercial Gate, be prepared to exit the vehicle and allow for vehicle inspection. At this time you may ask for directions to your final destination.

4.3.2 If the final destination requires the driver to leave through Commercial Gate and re-enter the base through an alternate gate, the pass received will be sufficient documentation for entry into both gates, within a pre-established time directed by the Security Forces representative.

4.3.3 Arrival at the appropriate gate after the designated allotted time will result in non- admittance to the installation.

4.3.4 This procedure will occur each time a commercial truck requests access to the base.

4.3.5 Current Bill of Lading (BOL) to get delivery driver and the vehicle on base. BOL must include the AF installation name, date of anticipated delivery, and specific destination (including building number, street address, and point of contact information).

4.3.6 Depending on the current Force Protection Condition, a government representative may be required to sponsor those people in person at Pass & ID, West Gate Visitor Center.

4.3.7 The sponsor must state the guest's purpose and destination. It is the responsibility of the contractor to validate this need with their government representative prior to their arrival so as to coordinate the arrival time accordingly. Sponsors may request that a pass be applicable for multiple days if needed.

4.4 To sponsor 10 or more people, a typed list of those people must be submitted by the Government Acceptor in Word or Excel format, stating LAST NAME then FIRST NAME of each person. The sponsor must submit that list to Pass & ID at West Gate Visitor Center, off Airport and Powers Blvd., no fewer than three days prior to the anticipated arrival. Names listed must be complete names, (no nicknames), and correspond to the names on the state ID for each person.

4.5 Mission Essential Services: Contractors are not considered mission essential.

4.6 Prohibitions: Firearms may not be brought on base under any circumstances. Individuals with criminal records are not allowed on base. Contractor personnel are prohibited from sponsoring guests onto the installation.

5. DESCRIPTION OF WORK

5.1 Deliver, unpack and install/set-up Bauer TCOM Lite in designated space at Building 218, Peterson Fire Station 1. Provide minimum of 1 day of training/instruction to 2 instructors (as designated by the Installation Fire Chief) on the operations and maintenance of the equipment.
