Repairs

MKPP200003: MAR Tower Structural Repairs August 11, 2020
1. GENERAL:
1.1. The Contractor shall provide all management, tools, equipment, transportation, supervision and labor necessary for the repair and replacement of missing structural bracing in the MAR Tower (Bldg 14100) at Koke’e Air Station. The Contractor shall perform all work in accordance with this Statement of Work (SOW). The Contractor shall coordinate all work with the Contracting Officer and is subject to the work schedule as stated in this document.
1.2. ALL CHANGES to this statement of work must be approved by the Contracting Officer. Contractor shall not perform any work outside the scope of this contract.
1.3. Unless otherwise noted in writing by the Contracting Officer, the Contracting Officer is the only person with the authority to change, modify, deny or approve any part and has the sole authority for all aspects of contract, SOW’s, contract changes, approvals, payments and project changes.
2. STATEMENT OF WORK: This Statement of Work (SOW) is separated into two (2) Line Items and will be executed to the extent funds are available and approved by 611 CEN Project Manager (PM) and the Contracting Officer (CO).
2.1. Line Item #1: Contractor shall replace two (2) missing structural members and associated hardware.
One member is located directly adjacent to the 1st Floor Bath. The second member is located on the 2nd Floor in the MCS Room.
2.1.1. Contractor shall install members with material and hardware similar to adjacent structural components.
2.1.2. Components are Painted Steel L-Stock Angle Iron, 2” x 2” x ¼” (Contractor to verify dimensions), attached at each end by steel bolts with locking nuts.
2.1.3. 1st Component is located adjacent to the 1st Floor Bath. This unit requires a steel mounting plate fabricated and attached to the exterior wall column. This plate is secured with matching steel hardware. The structural member consists of two (2) pieces of Steel L-Stock attached at each end with four (4) sets of fasteners.
2.1.4. 2nd Component is located on the 2nd Floor in the MCS Room. This unit is one (1) piece of Steel L- Stock, mounted at each end with three (3) sets of steel hardware. A hole will need to be cut in the ceiling tile to allow passage of the structural member.
2.1.5. All materials installed will be painted to match existing and provide corrosion resistance.
2.1.6. All materials proposed shall be approved by 611 CEN PM or CO/COR prior to installation.
2.1.7. See Section 9.1 for photos.
2.2. Line Item #2: Contractor shall replace one (1) missing member in 1st Floor Bath. This component needs to pass through the ceiling and shower area. Shower will likely need to be relocated to accommodate the passage of the structural member.
2.2.1. Contractor shall demo shower to determine path for installation of new steel member. Save Shower Pan and Mixing Valve to reuse if possible.
2.2.2. Contractor shall cut hole in ceiling and wall to allow passage of structural component.
2.2.3. Contractor shall install member with material and hardware similar to adjacent structural components. Steel will likely pass in front of the window, similar to other windows in the 1st Floor area.
2.2.3.1. Components are Painted Steel L-Stock Angle Iron, 2 ½” x 2 ½” x 3/8” (Contractor to verify dimensions), attached at each end by steel bolts with locking nuts.
2.2.4. Contractor shall patch drywall around holes in wall and ceiling created by installation of steel.
2.2.5. Contractor shall frame in new shower area to match existing Shower Pan (if being reused).
Otherwise, shower should encompass no more than 36” x 36”.
2.2.6. Contractor shall modify Drain and Water Supply lines as necessary to accommodate new shower location.
2.2.7. Contractor shall install drywall and mud, tape, texture, and paint to match existing.
2.2.8. Contractor will install Shower Pan, Mixing Valve, Fiberglass Shower Surround, and Shower
Head.
2.2.9. All materials installed will be painted to match existing and provide corrosion resistance.
2.2.10. All materials proposed shall be approved by 611 CEN PM or CO/COR prior to installation.
2.2.11. See Section 9.2 for photos.
2.3. All materials demolished and scrap materials used/generated in this project must be removed and disposed of in accordance with Federal, State, and local policy.
3 SCHEDULING AND PHASING OF WORK:
3.1 Upon receipt of award notification for this project, the Contractor is to immediately commence with scheduling of all construction activities and provide the Contracting Officer with a schedule of work.
The construction schedule shall be provided to the contracting officer within ten (10) calendar days of award.
3.2 A Pre-Construction (Pre-Con) meeting is required prior to construction. Some topics to be covered are:
Submittal Submissions, Site Access, and Approved Contractor Lay down Areas, Safety, and Period of Performance and Contractor questions. Pre-Con members include the Contracting Officer and Civil Engineer. Pre-Con shall be scheduled within 10 calendar days of award.
3.3 Notify 611 Civil Engineer Squadron (CES/CEN) at 907-552-1347, 766 SCONS Contracting Officer (CO) at 808-471-4333, and 611 Air Support Squadron Project Manager (ASUS/PM) at 907-552-5020 one week prior to project site visits, construction start and construction completion.
3.4 Notice to Proceed (NTP) shall be issued following a Pre-Con meeting scheduled by the Contracting Officer.
3.5 General Schedule:
3.5.1 Award: Begin scheduling construction activities.
3.5.2 Pre-Con Meeting and additional time for questions to be addressed: The NTP will be issued.
3.5.3 120 days (due to COVID-19) from issue of NTP: Contractor must begin construction activities.
3.5.4 Period of Performance: 60 Days.
4 STORAGE AND DISPOSAL OF CONTRACTOR MATERIALS:
4.1 The Contractor shall be responsible for the receipt, unloading, security and handling of all equipment and materials required for the completion of this work. The Contractor shall identify storage requirements and coordinate with the COR.
4.2 The Contractor shall secure all material and equipment at the end of each working day. No hazardous materials or waste may be left on site at the end of the work day. All material removed/demolished throughout the course of this project must be removed from site and be properly disposed of in accordance with in accordance with Federal, State and Local regulations.
4.3 All materials are the responsibility of the Contractor; Government will not reimburse contractor for stolen items or delivery related additional incurred costs (such as weather related materials delays).
4.4 The Contractor shall remove and dispose of all construction debris prior to final inspection.
5 CONTRACTOR FURNISHED ITEMS AND SERVICES:
5.1 The Contractor shall furnish all necessary supplies, services, and equipment that are required to perform this statement of work.
6 INSPECTIONS/QUALITY CONTROL:
6.1 The Contractor shall comply with the general terms of the overall contract with respect to construction quality control, inspections, and acceptance by the Government.
7 SAFETY:
7.1 The COR shall inspect the job site to insure safety standards are met. All contract workers must wear proper personal protection equipment (PPE) and comply with clean-up methods outlined in UFGS 02 83 13.00 10 Section 3.6.
7.2 The Project Engineer, COR, or the Contracting Officer may stop work at any time for a potential safety hazard. The Contracting Officer is the only personnel allowed to restart work in writing.
7.3 Due to the COVID-19 outbreak, Contractor may be asked for additional health, safety, and quarantine measures to be put in place for the wellbeing of all project and site personnel.
8 SUBMITTAL REQUIREMENTS:
8.1 GENERAL: Contractor shall submit a construction schedule and material specifications per AF Form
66 to the Contracting Officer for approval. Upon review by the Contracting Officer, the submittals along with any comments will be returned to the Contractor. If required, the Contractor shall modify the submittals per the Contracting Officer comments and resubmit within three (3) calendar days for final approvals. Upon final approval, the Contractor shall commence with the procurement and construction activity accordingly.
8.1.1 The construction schedule shall contain key milestones, critical path, and proposed start/completion dates and shall be considered as the project baseline schedule upon award.
8.1.2 The Contractor shall update this schedule as critical tasks are exceeded by more than 5 calendar days.
8.2 The Contractor shall have an established and maintained Quality Control and Accident Prevention program.
8.3 The Contractor shall submit Material Safety Data Sheets (MSDS) for all permanently installed materials prior to procurement.
8.4 The Contractor shall provide one (1) complete set of all documents:
8.4.1 SUBMITTALS: The Contractor shall submit materials the Contractor anticipates to install for
Contracting Officer approval. The submittals shall include salient characteristics and other pertinent standards. All project submittals will be accompanied by a completed AF Form 3000, Material Approval Submittal.
8.4.2 SUBCONTRACTING: The Contractor shall submit a fully executed SF 1413, Statement and Acknowledgment, upon award of each subcontract.
8.5 ACCEPTANCE AND APPROVALS: Acceptance and Approval shall be required from the COR and Contracting Officer on all submittals prior to procurement and installation.
